배구: 견관절 손상과 재활 (Volleyball: Shoulder Injuries and Rehabilitation)

  • Volleyball has become one of the world's most popular participatory sports in recent years. There are many dynamic skills and movements needed to play the game. As a result, many acute and overuse injuries to the shoulder may occur. This article addresses the common injuries and rehabilitation recommendations for the shoulder injuries in the volleyball players. The glenohumeral instability, primary or secondary impingement, internal impingement, labral injuries, and suprascapular nerve lesions are common problems in volleyball players. A basic knowledge of the biomechanics and volleyball maneuvers (blocking, serving, and spiking) can help in the development of appropriate trainging and reha- bilitation protocols. Special emphasis must be placed on the knowledge of muscular patterns involved in serving and spiking, because they typically require a strenuous unilateral action of the dominant shoulder. It is therefore impera- tive to include adequate stretching and muscular training programme for the prevention, as well as for therapy, of shoulder pain in volleyball players.

  • Overhead athletes에서 특히 자주 볼 수 있는 불안정성 SLAP 병변, Internal impingement, 회전근 개 병변 등의 질환은 개별적으로 나타나는 것이 아니라 여러 질환이 동반되어 나타나므로 진단이나 치료에 어려움이 많다. 어깨 관절 손상의 성공적인 치료 및 운동으로의 복귀를 위하여는 각각의 운동의 생역학적인 측면과 개개의 질환의 증상 및 정확한 진단, 병리기전에 대한 확실한 이해가 선행되어야한다.

Upper extremity musculoskeletal pain during rehabilitation in persons with spinal cord injuries using manual wheelchairs

  • Objective: The purpose of this study was to investigate the relationship between physical features, strength, function, and upper extremity musculoskeletal pain during rehabilitation of manual wheelchair users with spinal cord injuries. Design: Cross-sectional study. Methods: The degree and frequency of upper extremity musculoskeletal pain were measured in persons with spinal cord injuries using manual wheelchairs with the use of questionnaires. The pain scores of the hand, wrist, and shoulder joints were calculated by multiplying the seriousness and frequency of pain. We collected data on the manual muscle test, Spinal Cord Independent Measure-III, and the Body Mass Index. Statistical analysis was performed by descriptive analysis and Pearson's correlation analysis. Results: A total of 47 patients participated in this study and the neurological level of the injuries ranged from C2 to S5. Pain in the shoulder joints was the most common in persons with tetraplegia and paraplegia. Pain was experienced as mild to moderate, and occurred one or more times a week. Of the 32 persons with paraplegia, the most common area of complaint was the shoulder. Of the 15 persons with paraplegia, the shoulder joints were the most common site of pain. The independence levels of the persons with spinal cord injuries were highly correlated to muscle strength levels (p<0.05). Conclusions: This study investigated upper extremity musculoskeletal pain during rehabilitation of manual wheelchair users with spinal cord injuries and the relationship between physical features, strength, and function. In most persons with spinal cord injuries, pain and frequency of shoulder joints were high and pain levels were also related to functional levels.

사고와 관련된 급성 외상성 어깨 손상의 진단 (Diagnosis for Acute Traumatic Shoulder Injuries)

  • 목적: 자동차 사고 또는 산업재해와 관련된 견관절 부상은 최근 현저하게 증가하고 있어 진단 기준을 제시하는 것이 목적이다. 대상 및 방법: 그 중 사고와의 인과 관계 및 기왕증 부분에서 가장 논란이 되고 있는 견관절 손상은 회전근 개 파열과 상부 관절와 순 전후방 파열 (SLAP 병변)이다. 회전근 개 병변이나 SLAP 병변은 질환의 특성상 퇴행성 변화와 관계되어 자연 발생적으로 병변이 발생할 수 있으며, 손상 정도와 임상 증상간의 불일치 등으로 사고 관여도 판정이 애매모호한 경우가 많다. 결과: 이에 견주관절학회 보험위원회는 국내 견관절 전문의들을 대상으로 설문 조사를 실시하여, 전문가들의 회전근 개 손상 및 SLAP 병변과 교통사고와의 인과관계를 판단하는 기준과 실정 등을 조사하였다. 결론: 회전근 개 손상과 SLAP 병변의 발생 원인 및 자연 경과를 생체역학, 병태생리학적 접근을 통하여 고찰해 보고, 회전근 개 손상과 사고와의 관련성 판단에 관한 연구 결과들을 참고하여 급성 견관절 손상과 사고와의 인과관계 판단에 도움이 되는 진단 기준을 본 종설을 통해 제시하고자 한다.

American Football Injuries In Korea

  • American football is currently played in 29 Universities in Korea, It is a contact sport where injuries are inevitable. This study was undertaken to determine the incidence and the anatomical distribution of injury based on evaluation of Yonsei University American football team. A total 74 injuries was reported among 77 players Knee, ankle and hand were vulnerable to injury. Operations were performed on $9(12.2\%)$ of the injuries. Catastrophic injuries can occur in this sport, but none has happened in Yonsei University team during the 20 years for which the sports has been practised. The frequency of injuries and their profile resemble those in US-based studies so it is suggested that the precautions and preventive measures recommended in the USA should be applied and followed in Korea.

Management of Ulnar Collateral Ligament Injuries in Overhead Athletes

  • Ulnar collateral ligament injuries of the elbow are frequent among overhead athletes. The incidence of ulnar collateral ligament reconstructions (UCLRs) in high-level players has increased dramatically over the past decade, but the optimal technique of UCLR is controversial. Surgeons need to manage the patients' expectations appropriately when considering the mode of treatment. This article reviews current studies on the management of ulnar collateral ligament injuries, particularly in overhead athletes.

Treatment of Acute Acromioclavicular Joint Injuries Using AO Hook Locking Plate

  • Background: To evaluate clinical and radiological outcome using AO hook locking plate in acute acromioclavicular joint injuries. Methods: This study was based on patients with Rockwood type 3 or 5 acromioclavicular joint injuries who received surgery with AO hook locking plate from June 2008 until June 2009. Among the 22 patients, 19 of them were male and 3 were female, the mean age was $44.4{\pm}15.57$ years (20-72 years) and follow-up period was $15.5{\pm}3.90$ months (12-23 months). Preoperatively, postoperatively, and at the final follow-up after the plate removal, both coracoclavicular distances were measured from the anteroposterior radiograph. Also, the Shoulder Rating Scale of the University of California at Los Angeles scores (UCLA scores), the American Shoulder and Elbow Surgeons scores (ASES scores), Constant scores, and the Korean Shoulder Society scores (KSS scores) were measured at the final followup to evaluate the function of the shoulder joint. Results: At the time of injury, the mean coracoclavicular distance of the injured side was $17.69{\pm}4.23mm$ (9.57-27.82 mm) and the unaffected side was $7.55{\pm}2.20mm$ (3.24-13.05 mm). The mean coracoclavicular distance measured postoperatively and at the final follow-up was $6.87{\pm}2.34mm$ (4.07-14.13 mm) and $8.47{\pm}2.96mm$ (4.37-17.48 mm), respectively. The mean UCLA, ASES, Constant, and KSS scores measured in the final follow-up were $33.5{\pm}1.30$ (31-35), $90.8{\pm}8.36$ (72-100), $78.6{\pm}8.80$ (62-100), and $94.4{\pm}5.08$ (84-100) each. Conclusions: From this short-term research, the surgical treatment using AO hook locking plates in acute acromioclavicular joint injuries is clinically and radiographically satisfying and considered as a useful treatment method.

목맴 후 발생한 양측 팔신경얼기손상: 증례보고 (Bilateral Brachial Plexopathy Following an Attempted Hanging: A Case Report)

  • The brachial plexus is a network of nerves that provides movement and feeling to the shoulder, arm and hand. The majority of acute brachial plexus injuries occur when the plexus is stretched violently or torn. This happens as result of the shoulder being pressed down forcefully while the head is pushed up and away from that shoulder. Such injuries frequently result from automobile or motor-cycle accidents or from falls and usually affect one side. Nerve injuries vary in severity from a mild stretching of the nerve to a tearing of the nerve root away from the spinal cord. We experienced a 50-year-old woman with weakness in both upper extremities after an attempted hanging. A consecutive workup revealed bilateral brachial plexus injuries. Six months after the incident, she had fully recovered. This is a very rare case of bilateral brachial plexus injuries after an attempted hanging.

진동을 동반한 슬링 운동이 어깨 손상 환자의 관절가동범위, 근력, 통증, 기능장애 수준에 미치는 영향 (Effects of Sling Exercise With Vibration on Range of Motion, Muscle Strength, Pain, Disability in Patients With Shoulder Injuries)

  • Background: Sling exercises are frequently used for the rehabilitation process of patients with shoulder joint injuries, but research on the significant frequency intensity and appropriate treatment duration for sling exercises with local vibration stimulation is lacking. Objects: The aim of this study was to investigate the effects of sling exercise with vibration on shoulder range of motion (ROM), muscle strength, pain, and dysfunction in patients with a medical diagnosis of shoulder joint injury. Methods: Twenty-two patients were randomly assigned to the experiment and control groups. Six sling exercises with and without 50 Hz vibrations were applied in the experiment and control groups, respectively. Each exercise consisted of 3 sets of 5 repetitions performed for 6 weeks. The assessment tools used included shoulder joint range of motion, muscle strength, pain level, and shoulder pain and disability index for functional disability. We conducted re-evaluations before and 3 and 6 weeks after intervention. The changes in the measurement variables were analyzed and compared between the two groups. Results: The ROM of the external rotation of the shoulder joint had a significant interaction between the group and the measurement point (F=3.652, p<.05). In both groups, we found a significant increase in external rotation angle between the measurement points (p<.05). The flexor strength of the shoulder joint significant interaction between the group and the measurement point (F=4.247, p<.05). Both the experiment (p<.01) and control groups (p<.05) showed a significant increase in shoulder flexor strength at the measurement points. After 6 weeks of the interventions, both the groups showed significantly improved VAS (p<.01), SPADI (p<.01), and orthopedic tests (p<.01). However, there was no significant difference between the group and the measurement point in terms of the clinical outcomes observed. Conclusion: The sling exercise with local vibration of 50 Hz affected the external rotation of the shoulder range of motion and improved shoulder flexor strength in the patients with shoulder injuries. Therefore, we propose the use of the sling exercise intervention with vibration in the exercise rehabilitation of patients with shoulder joint injuries.
