사상체질별 임상지표와 건강상태의 연관성 연구 - 여성을 중심으로 - (The Association between Symptom Evaluation Index and Health State according to Sasang Constitution in Women)

  • 1. Objectives: We aimed to know what are different in physiological and pathological symptoms associated with health state according to Sasang Constitution in women 2. Methods: We recruited 338 women between 10 and 80, excluding individuals who have physically or psychologically serious disease, or women in pregnancy among Korean population in Traditional Korea Clinics. We used CRF(Case Report Form, C-2009-002439) as measurement of physiological and pathological symptoms and SF-36 (Short Form-36) as measurement of health state We analyzed association between physiological and pathological symptoms and health state with Two-way ANOVA 3. Results: There are 9 different symptoms associated with health state, such as digestion state, uncomfortable problem in belly, indigestion in bad condition, abnormal sweat, night sweat, clean in urine, no problem in urine, feel heavy in chest according to Sasang constitution. 4. Conclusions: Physiological and pathological symptoms are associated with health state and some symptoms associated with health state are different according to Sasang Constitution in women.

담훈(痰暈)으로 변증하여 청훈화담탕(淸暈化痰湯)을 사용한 소뇌경색환자의 치험 1례 (Case Study of a Cerebellar Infarction Patient Diagnosed as Dam Hun with Korean Medicine treatment - Cheonghunhwadam-tang -)

  • Objectives: The aim of this study was to report the effect of Korean medicine treatment on a cerebellar infarction patient suffering from vertigo, ataxia, and nausea. Methods: We treated the patient with herbal medicine (Cheonghunhwadam-tang), acupuncture, and physical therapy. Clinical symptoms were assessed with a numeral rating scale (NRS), the Dizziness Handicap Inventory (DHI), and Study Short Form-36 (SF-36). Results: After 24 days of treatment, the vertigo, ataxia, and nausea symptoms were all improved, and the NRS, DHI, and SF-36 scores showed improvement. Conclusion: This case shows that Korean medicine treatments were effective in alleviating the symptoms of cerebellar infarction.

직접구 치료에 의해 호전된 척추수술후통증증후군 환자 5례 (Case Series of 5 Failed Back Surgery Syndrome Patients Who were Treated by Direct Moxibustion Therapy)

  • Objectives : To evaluate the effectiveness of direct moxibustion in patients with failed back surgery syndrome (FBSS). Methods : 5 patients (3 females and 2 males) with FBSS underwent direct moxibustion on ouch points (阿是穴) for 10 to 15 weeks (once a week). The effectiveness of the treatment was evaluated with NRS (numerical rating scale), ODI (Oswestry disability index), BDI (Beck depression inventory) score, SF-36 (short form 36 health survey) score and PSQI (Pittsburgh sleep quality index) before and after the treatment. Results : Within observation period, average value changed from 10.0 to 4.2 in NRS of pain, from 37.0 to 20.6 in ODI, from 20.6 to 12.0 in BDI score, 42.2 to 62.6 in SF-36 score, and from 8.6 to 5.2 in PSQI. Conclusions : Direct moxibustion on FBSS patients showed more or equivalent effectiveness when compared to conventional FBSS treatment modalities.

뇌졸중 환자의 일상적인 신체적 기능과 삶의 질 수준 (Daily Physical Functioning and Quality of life for Stroke)

  • Purpose: It remains controversial for the effect of daily functioning and quality of life on therapeutic exercise after stroke. The purpose of this study was to describe the effects on daily functioning and QOL. Methods: Outcome measures of daily functioning included, such as the Functional Independence Measure (FIM), Barthel index. Outcome measures of QOL included, such as Stroke Impact Scale(SIS) and the Medical Outcomes Study short-form 36-item questionnaire(SF-36). Results: 125 stroke patients were recruited, who were in or outpatients. The average age was 55.4 years. 64.8% were male. The mean Bathel index and FIM score was 63.7 and 87.5. The mean SIS score were higher in communication and mean SF-36 score were higher in physical pain. In/out patients are associated with SIS (communication, emotion) and SF-36(social function, energy or fatigue). Sex are associated with SF-36 (physical function). Other disease state are associated with SIS(hand function) and SF-36 (physical function). Paralysis portion are associated with SIS(communication, daily activity). Barthel index are associated with SIS(communication, mobility) and SF-36(social function, physical function, role limits due to emotional problems). Conclusion: These findings may provide the useful with rehabilitation professionals, who specilalized in the importance of QOL in designing treatment modalities.

퇴행성 슬관절염에서의 온침과 침의 효능 비교 연구 (Comparison of Warm-Needling and Acupuncture for Knee Osteoarthritis: A Randomized Controlled Trial)

  • 목적 : 본 연구는 퇴행성 슬관절염 환자의 통증을 줄이고 증상을 개선하는데 있어 현재 침구임상에서 활용되고 있는 온침이 침보다 더욱 유효한지를 알아보기 위하여 임상연구로 시행되었다. 방법 : 총 76명의 퇴행성 슬관절염 지원자가 연구에 참가하였으며, 이들 모두는 선정기준과 제외기준에 의하여 선발되었고 최종적으로 임상연구를 마친 지원자는 66명이었다. 지원자들은 무작위로 온침군과 침군 중에 할당이 되었다. 실험군(온침)에 속한 38명은 온침시술을, 대조군(침)에 속한 38명은 침시술을 받았으며, 8주 동안 총 16회의 시술을 환측의 무릎에 시행하였다. 주된 평가척도는 Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)이었고, 부수적 평가척도는 physical health score based on the 36-Item Short-Form Health Survey(SF-36), Patient Global Assessment이었다. 결과 : 실험군이 대조군에 비해서 WOMAC의 pain, function, total score와 Patient Global Assessment에서 월등한 감소를 나타내었으나 SF-36에서는 유의한 변화가 없었다. 결론 : 이번 연구의 결과를 통해 온침이 침에 비하여 퇴행성 슬관절염 환자의 통증 감소와 기능개선에 유의성있게 효과가 있음을 확인하였다.

Clinical results of conservative management in patients with full-thickness rotator cuff tear: a meta-analysis

  • Background: Several systematic reviews have reported on the conservative treatment of full-thickness rotator cuff tears; however, clinical results of this treatment still remain determined. Methods: PubMed, Cochrane Library, PEDro, and CINAHL databases were systematically searched for randomized clinical trials and observational studies. Two independent researchers reviewed a total of 2,981 articles, 28 of which met the criteria for inclusion in the study. Clinical outcome measures included Constant score, visual analog scale score for pain, range of motion, and short-form 36 questionnaires. The meta-analysis used a linear mixed model weighted with the variance of the estimate. Results: The meta-analysis showed a significant improvement after surgery. Pain score is 26.2 mm (1 month) to 26.4 mm (3 months), and 24.8 mm (12 months) (P<0.05); active abduction: 153.2° (2 months), 159.0° (6 months), 168.1° (12 months) (P<0.05); Constant score: 67.8 points (2 months) to 77.2 points (12 months) (P<0.05); short-form 36 questionnaires "vitality" section: 57.0 points (6 months) to 70.0 points (12 months) (P<0.05). Conclusions: Our data confirmed the effectiveness of conservative treatment in patients with full-thickness rotator cuff tears 12 months post-intervention. The results suggest that conservative treatment for patients with full-thickness rotator cuff tears should be the first line of treatment before considering surgery.

치료적 클라이밍 훈련이 뇌졸중 환자의 상지기능과 삶의 질에 미치는 영향 (Effects of Therapeutic Climbing Training on Upper Limb Function and Quality of Life in Stroke Patients)

  • 본 연구는 치료적 클라이밍 훈련이 뇌졸중 환자의 상지기능과 삶의 질에 미치는 효과를 알아보고, 임상적으로 중재될 수 있는지에 대한 가능성을 제시하는데 그 목적이 있다. 본 연구는 14 명의 뇌졸중 환자를 대상으로 하였으며, 7명의 치료적 클라이밍 훈련군과 7명의 대조군으로 지정하였다. 실험군은 일반적 물리치료와 작업치료를 시행하였고, 추가적으로 포츠담모델을 이용한 치료적 클라이밍 훈련을 6주 동안 주 3회, 총 30분 씩 훈련을 진행하였다. 대조군은 일반적 물리치료와 작업치료를 적용하였다. 상지기능은 Manual Function Test로 측정하였고, 삶의 질은 Short Form 36-item으로 측정하였다. 측정 결과, 그룹 내 시기에 따라 실험군의 상지기능은 유의한 차이가 있는 것으로 나타났다(p<.05). 삶의 질은 그룹 내 시기에 따라 실험군의 유의한 차이가 있었고, 각 그룹 간의 비교에서는 실험군이 대조군에 비하여 유의한 차이가 있었다(p<.05). 본 연구를 통해 뇌졸중 환자에게 포츠담 모델을 적용한 치료적 클라이밍 훈련이 상지기능과 SF-36점수를 증가시켜 삶의 질에 긍정적인 효과를 가져왔다. 따라서 향후 치료적 클라이밍을 이용한 훈련을 통해 뇌졸중 환자의 재활이나 임상 적용 중재에 있어서 가치가 있음을 확인할 수 있었다.

중국 인플루언서들의 K-pop 짧은 동영상 수용에 영향을 미치는 요인에 관한 연구 - 중국 '틱톡' 사용자를 중심으로 (A Study on the Factors Influencing the Acceptance of K-pop Short-form Video Created by Chinese Influencers - Focusing on Chinese TikTok Users)

  • 본 연구는 짧은 동영상 플랫폼 틱톡에서 중국인 인플루언서들이 한국 K-pop 노래와 댄스를 어떠한 방식으로 재창작하며, 어떠한 변형요소들이 틱톡 수용자들의 반응 - '좋아요수', '댓글수', '공유수'에 영향을 미치는지를 살펴보기 위해 틱톡 K-pop 커버영상 284개를 K-pop 원곡과 비교하여 이미지와 언어 유사성, 수용자 참여 유도 정도, 중국어로 가사/자막 번역된 정도, 유료광고 노출 여부를 분석하였고 추가로 영상길이, 인플루언서 명성 요인을 분석하여 변형요인과 함께 수용자 반응에 미치는 영향력을 분석하였다. 분석결과, 상대적으로 수동적인 수용자 반응으로 판단되는 '좋아요수'에는 인플루언서의 명성만 유의한 영향을 보였지만, 보다 능동적인 반응으로 판단되는 '댓글수'와 '공유수'에는 재창작으로 활용한 변형요소들이 유의미한 영향을 미친 것으로 나타났다. 수용자는 인플루언서가 이미지 측면에서 가수와 닮지 않다고 인식될수록 댓글을 더 많이 달아주며, 중국어로 번역된 가사나 자막보다 한국어로 표현한 것이 댓글과 공유를 더 많이 만들어내었다. 본 연구는 짧은 동영상을 활용한 K-pop 콘텐츠의 해외 확산에 있어 로컬 인플루언서들에 의해 재창작되는 K-pop 콘텐츠의 특성에 따른 수용자 반응을 구체적으로 분석함으로서, K-pop 확산을 위한 인플루언서들의 역할의 필요성을 확인하였다는 데 의미가 있다.

두통을 동반한 개방성 이관의 한의학적 치료 증례보고 (A Case Report of the Korean Medical Treatment for Patulous Eustachian Tube Patient with Headache)

  • This study was performed to report the case of Korean medical treatment for the Patulous Eustachian tube patient with headache. The patient was treated by acupuncture, herbal medicine, bee venom injection, needle-embedding therapy, chuna therapy, release of cranial base and sacro-occipital therapy. Korean medical treatment was administered during 3 weeks. The improvement of clinical symptoms was evaluated by VAS (visual analogue scale) and SF-36 (36-Item Short Form Healthy Survey Instrument). After treatment, most symptoms decreased, headache VAS score changed 10 to 6 and tinnitus VAS score changed 10 to 5. Also, most SF-36 scores increased. Our study suggested that Korean medical treatments are effective in the patient with Patulous Eustachian tube. And further studies are required to identify underlying mechanism of treatment.

Efficacy of Glycopyrrolate in Primary Hyperhidrosis Patients

  • Background: Primary hyperhidrosis is a disorder of excessive sweating, which shares several features with anxiety disorders and has a negative impact on a patient's quality of life. Oral glycopyrrolate is one of the treatments available. There are a few published studies on the use of glycopyrrolate given orally in the treatment of hyperhidrosis. Methods: Thies is study was a review of case notes in a series of 36 patients with primary hyperhidrosis. We made a comparison between the Keller's scale score of a pre-glycopyrrolate medication group and the Keller's scale score f a post-glycopyrrolate medication group. The Milanez de Campos score, Short Form_36 (SF-36) score, Beck Depression Inventory (BDI) score, Beck Anxiety Inventory (BAI) score, and autonomic nervous system (ANS) scale score were also compared between the two groups. Results: In the post-glycopyrrolate medication group, there were declines in Keller's scale, and Milanez de Campos scale score and BAI score (P < 0.001). In addition, there were increases in SF_36 score in the post-glycopyrrolate medication group (P = 0.03) However, no changes were seen in, BDI score and ANS score in the post-glycopyrrolate medication group (P < 0.001). Conclusions: Glycopyrrolate is an effective initial method of treating primary hyperhidrosis that, reduces anxiety and improve patients' quality of life.
