간호중재분류(NIC)에 근거한 간호중재수행분석 II -한방병동과 일반병동 간호사를 중심으로- (Nursing Interventions Classification(NIC) Use in Korea : Oriental Medicine Hospitals and General Hospitals)

  • The purposes of this research were to identify nursing interventions performed by Korean nurses and to compare the interventions performed by nurses working in the oriental medicine hospitals and with those performed by nurses working in the general hospitals. The samples consisted of 144 Korean nurses working in three hospitals, 70 nurses working in the oriental medicine hospitals and 74 nurses working in the general hospitals. The Nursing Interventions Classification (NIC) Use Questionnaire developed by the Iowa Intervention Project team was translated to Korean and verified using the method of back-translation. The questionnaire consists of 433 intervention labels and definition. Thirteen interventions were used at least daily by nurses working in the oriental medicine hospitals, while twenty-one interventions were used at least daily by nurses working in the general hospitals. The most frequently used interventions by nurses working in the oriental medicine hospitals were Documentation, Shift Report Vital Signs Monitoring, Pressure Ulcer Prevention, Positioning, Fall Prevention, Exercise Promotion, Intravenous (IV) Therapy, Pressure Ulcer care, and Bed Rest Care in that crder. For nurses working in the general hospitals the most frequent intervention was Analgesic Administration, followed by the interventions of Medication Administration : Parenteral and Intravenous Therapy (IV) Therapy, Documentation, Intravenous(IV) Insertion, Shift Report, Fall Prevention, Vital Signs Monitoring, Medication Adnninistraction : and, Fluid Monitoring, and Medication Maragement in that order. The interventions performed least often by nurses working in the oriental medicine hospitals were Hemodialysis Therapy and Bleeding Reduction : Antepartum Uterus, while the interventions performed least often by nurses working in the general hospitals were Rape Trauma Treatment and Contact Lens Care. The nurses working in the oriental medicine hospitals performed the interventions in the Physiological : Complex domain significantly more often than the nurses working in the general hospitals, while the nurses working in the general hospitals performed the intervention in the Behavior domain significantly more often than the nurses working in the oriental medicine hospitals. This study suggests that further study will be needed to developed and validate more interventions sensitive to Korean culture.

한국 간호사의 건강연구에서 야간근무 노출 평가에 대한 고찰 (Working time patterns and health among Korean nurses: a literature review)

  • Objective: Shift work including night work is known to be harmful to the health of nurses. This study was conducted in order to systematically review the academic literature on the health effects of working time patterns, including shift work or night work, among Korean nurses and propose a research direction for the future. Methods: We searched online academic databases to find relevant papers in domestic and international journals using the keyword terms 'shift work', 'night work', 'night shift', 'work shift', 'nurse', and 'Korea', and reviewed a total of 36 articles. Results: As a result of this literature review, more than half of the articles were found to investigate the health effects between shiftwork and non-shiftwork nurses. A few studies considered the shift type(n=6), frequency of night shift(n=4), and shiftwork duration(n=3) as working time patterns. These studies focused mainly on mental health(n=18) or sleep(n=15). Conclusions: Based on the review results obtained from this study, it is necessary to assess working time patterns of Korean nurses using a variety of variables including night-work-related factors, and various health outcomes should be examined considering the confounder.

교대근무 간호사의 직무스트레스와 스트레스반응과의 관계에서 수면의 질의 매개효과 (Mediating Effects of Sleep Quality on the Relationship between Job Stress and Stress Response of Shift-Working Nurses)

  • Purpose: The aim of this research was to identify the mediating effect of sleep quality on the relationship between job stress and stress response of shift-working nurses. Methods: The participants in this study were 150 nurses who had more than 6 months of shift-working experience. A survey was conducted from May to June 2017 in a hospital in Gyeonggi-do, South Korea. The data were collected through self-report questionnaires. The collected data were analyzed using SAS 9.4 program. The analysis was based on Baron and Kenny's model to examine the mediating effects of sleep quality on the relationship between job stress and stress response of shift-working nurses. Results: In the first and second steps, job stress was identified as a significant predictor of sleep quality (β= .29, p= .001) and stress response (β = .24, p= .004). In the third step, sleep (B = .55, p< .001) was observed as a significant predictor of stress response, and the direct relationship between job stress and stress response was not significant (β= .07, p= .291). Sleep quality was found to exhibit complete mediating effect on the relationship between job stress and stress response. Conclusion: The results of this study suggest that strategies for managing the stress of shift-working nurses should include effective ways to ensure sleep quality.

교대근무 간호사의 효율적인 근무형태 개발을 위한 실태조사 (A Study for Developing the Effective Working Pattern for Nurses in Shift Work)

  • Purpose: This study was conducted to provide baseline data to develop work patterns for nurses that are suitable to Korean hospitals. Shift work patterns and nurses' preferences for working condition were examined. Methods: Questionnaires for nurse departments and nurses were mailed to 478 hospitals from June 1 to July 30, 2012. Nurse departments of 174 hospitals and 1,409 nurses responded to the questionnaire. Data were analyzed using SPSS statistical program. Results: Of the nurses, 89.4% were working three shifts. Of the hospitals, 17.3% had implemented fixed night work. Although 80% of nurses preferred fixed time work, only 24.9% of hospitals had fixed working hours. Of the nurses, 57% agreed with night work as fixed but potential night nurses accounted for only 17% of the respondents. Sixty-three percent of the nurses agreed on short time work during busy hours. There were preference differences according to age, marital status and clinical career. Conclusion: As many nurses reported a preference for various flexible working patterns rather than three shift work, there is a need for new decisions on hospital policy to ensure improved working conditions for nurses and enhanced job security. Legal requirements also need to be considered for various work patterns.

교대근무간호사의 욕구만족, 소명의식, 직무스트레스가 재직의도에 미치는 영향 (The Effect of Shift-Working Nurses' Need Satisfaction, Calling and Vocation, and Job Stress on Retention Intention)

  • Purpose: This descriptive study aimed to identify shift nurses' retention intention, need satisfaction, calling and vocation, and job stress, as well as the effects of these factors on retention intention. Methods: The participants of this study were 193 nurses working shifts with more than three months' clinical experience at general hospitals. Data were collected using a structured questionnaire from February 1 to 28, 2023. Nurses' Retention Index and Nurse Needs Satisfaction Scale, Korean version of the Calling and Vocational Questionnaire, Korean Occupational Stress Scale were used. Data were analyzed using SPSS/WIN 23.0 program. Independent t-test, one-way ANOVA, Pearson's correlation analysis, and hierarchical multiple regression analysis were conducted for statistical analysis. Results: Regarding nurses' shift work, the mean scores for retention intention, needs satisfaction, calling and vocation, and job stress were 32.12±9.04, 94.87±14.74, 26.95±6.84, and 66.12±10.34. Moreover, the nurses' retention intention differed significantly by age, education level, religion, marital status, clinical experience, satisfaction with wage, and job satisfaction. Furthermore, retention intention was positively correlated with needs satisfaction and calling and vocation and negatively correlated with job stress. Needs satisfaction, and calling and vocation as well as education level of bachelor's degree, job satisfaction had positive effects on shift work nurses' retention intention. Job dissatisfaction had relatively negative effects on retention intention compared to having neutral job satisfaction. Conclusion: It is necessary to develop programs that promote needs satisfaction, calling and vocation, and job satisfaction to improve nurses' retention intention in relation to shift work; moreover, institutional support for acquiring a bachelor's degree or higher is needed.

어린 두 자녀를 둔 3교대 간호사의 양육 갈등 경험: 현상학적 연구 (Experience of Conflict in Three Shift Nurses Rearing more than Two Kids: Phenomenological Study)

  • Purpose: To understand the essentials of rearing conflict experience by three shift nurses in advanced general hospitals. Methods: The design was a qualitative research of phenomenology. Participants were 7 shift nurses working in advanced general hospitals who were rearing young children. Data were collected individually through in-depth interview on their life experiences. Data were analyzed by Colaizzi's phenomenological methodology. Results: Eighteen themes were drawn from 256 meaningful experiences and these themes were integrated to six theme clusters. The most influencing themes were 'Regret that I cannot satisfy even the slightest wish', 'Fail to care for kids', and 'Mutual feeling to care giver between appreciation and inconvenience'. Other themes were as follows: 'Body and mind are broken', 'The need for a three-shift system to support nurses who are rearing children', 'Doing my best for work and child rearing'. Conclusion: The nature of three-shift nurses working in advanced hospital and caring kids is explained as 'lives with conflict' between work and home. This study suggests it is necessary to establish a 24-hour care center for 3-shift nurses to keep working while rearing their children.

Sleep Deprivation and Fatigue among Nurses Working Consecutive Night Shifts: A Prospective Observational Study

  • Purpose: This study aimed to identify changes in sleep patterns and fatigue levels during consecutive night shifts among shift nurses and to determine the association between sleep parameters and increased fatigue levels during work. Methods: This prospective observational study employing ecological momentary assessments was conducted using data collected from 98 shift nurses working in Korean hospitals between June 2019 and February 2021. The sleep patterns were recorded using actigraphy. The participants reported their fatigue levels at the beginning and end of each night shift in real time via a mobile link. Linear mixed models were used for the analysis. Results: Nurses spent significantly less time in bed and had shorter sleep durations during consecutive night shifts than on off-duty days, whereas their wake times after sleep onset were much longer on off-duty days than on on-duty days. Fatigue levels were higher on the second and third night-shift days than on the first night-shift days. A shorter time spent in bed and asleep was associated with a greater increase in fatigue levels at the end of the shift than at the beginning. Conclusion: Nurses experience significant sleep deprivation during consecutive night shifts compared with off-duty days, and this sleep shortage is associated with a considerable increase in fatigue levels at the end of shifts. Nurse managers and administrators must ensure sufficient intershift recovery time during consecutive night shifts to increase the time spent in bed and sleeping.

여성 간호사의 근무형태 선호도에 따른 조직몰입 및 이직의도 비교연구 (A Comparative Study on Organizational Commitment and Turnover Intention according to the Type of Shift Work Preferred by Female Nurses)

  • 본 연구는 여성 간호사가 선호하는 근무형태에 따른 조직몰입도와 이직의도를 비교하여 여성 간호사의 근무 환경을 개선하고자 수행되었다. 본 연구는 서술적 조사연구로서 7개 의료기관에서 3교대 근무중인 여성 간호사 465명에게 온라인 설문조사를 통해 자료수집을 하였다. 수집된 자료는 SPSS WIN 24.0를 이용하여 χ2 test, two sample independent t-test, pearson correlation, MANCOVA로 분석한 결과 고정근무 선호집단의 연령이 많을수록 조직몰입도가 높았으며, 이직경험이 있는 경우 이직의도가 높았다. 그리고 고정근무 선호집단이 교대근무 선호집단 보다 조직몰입도가 낮았으며 이직의도가 높았다. 고정근무를 희망하는 여성 간호사들의 조직몰입도를 높이고, 이직을 줄이기 위해서는 다양한 근무형태를 개발하여 여성 간호사의 근무환경을 개선하기 위한 노력이 필요하다.

일 병원의 순환교대근무와 주간고정근무 여성 간호사의 근무형태에 따른 정신신체 증상 (Psychosomatic Symptoms of Rotating Shift and Daytime Working Nurses in a General Hospital)

  • 연구목적 일 종합병원에 근무하는 여성 간호사 중 순환교대자와 주간고정근무자 두 군 간의 정신신체 증상의 차이를 조사하였다. 방 법 일 종합병원에서 근무하는 여성 간호사의 인구학적 정보를 수집하여, 두통의 강도(VAS), 두통의 유형, 두통의 빈도를 알기위한 질문지와 우울척도(BDI), 전반적 수면평가지(GSAQ)를 사용하였다. 통계분석은 Mann-Whitney test와 ${\chi}^2$ test를 사용하였다. 결 과 전체 대상군 84명 중 순환군이 37명, 주간군이 47명이었다. 순환군이 주간군보다 젊고 미혼이 많았고 근무연수가 적었다. 순환군이 주간군보다 두통, 불면, 과다 주간졸리움증, 우울, 불안과 같은 정신신체 증상이 통계학적으로 유의하게 많았다. 결 론 종합병원의 순환교대근무 여성간호사가 주간근무자보다 더 많은 두통, 불면, 과다 주간졸리움증, 우울, 불안을 보였다.

고정근무간호사와 교대근무간호사의 건강상태와 직무만족도에 관한 연구 (A Study of Health Status and Job Satisfaction of Nurses on Day Shift and Nurses on Night Shift)

  • Purpose: This study is conducted to compare and analyse the health status and job satisfaction of nurses. Method: The subjects were a group of 90 nurses on fixed day nurse and the other group of 100nurses on night nurse. The collected data were analysed by using the SPSS WIN(10.0) program for descriptive statistics, t-test, ${\chi}^2-test$, ANOVA, and Pearson correlation coefficient. Result: There was a significant difference according to years in services. In nurses on day shift, there was a inverse correlation between the health status and job satisfaction. And in nurses on day shift, job satisfaction levels differed significantly depending on personality, satisfaction to pay, and current health status. In nurses on night shift, nurses' job satisfaction levels differed significantly depending satisfaction to pay. Conclusion: It was confirmed that health of nurses is greatly affected by shift work. so, it is very important to maintain and promote their health status in nurses on night shift. We must find out the factors that greatly affect health of nurses, and continuously make an effort to improve the working environment.
