Objective: Anxiety has been shown to influence functional impairment in patients with attention deficit hyperactivity disorder (ADHD). This study aimed to compare functional impairment in subjects with and without adult ADHD and to investigate the associations among trait anxiety, functional impairment, and ADHD symptom severity. Moreover, the effects of ADHD symptom subtypes on trait anxiety and functional impairment were also examined. Methods: The sample included 209 adults between the ages of 20 and 31 years. Fifty-one adults received a diagnosis of ADHD, and an additional age, sex-matched group of 51 adults comprised the adult control. Participants were assessed with Conners' Adult ADHD Rating Scales (CAARS), the Beck Depression Inventory (BDI), the Spielberg Trait Anxiety Inventory (STAI-T), and the Sheehan Disability Scale (SDS). The relationships among ADHD severity, anxiety, and functional impairment were investigated using Pearson's correlation analysis. Subtypes of ADHD symptoms that predicted anxiety and functional impairment were investigated using regression analyses. Results: Adult ADHD patients significantly differed from normal control subjects according to BDI, STAI-T, and SDS assessment. Significant positive correlations were noted between ADHD severity, anxiety, and functional impairment. Multiple linear regression analysis confirmed anxiety as a mediator between functional impairment and ADHD CAARS symptom subscales. Conclusion: Patients with adult ADHD showed higher levels of anxiety, depression, and functional impairment. Additionally, ADHD symptoms and anxiety impacted subject functional impairment. Our results suggest that anxiety may be a strong mediator between ADHD severity and functional impairment.

Background: This study was conducted to investigate the effects of mechanical traction therapy on pain, symptom severity, and functional status in patients with carpal tunnel syndrome. Design: Randomized controlled trial study. Methods: The 24 participants in this study were randomly assigned to either the Mechanical traction therapy (MTT) group (n=12) or the placebo group (n=12). Both groups performed tendon and neural gliding exercise. Both groups underwent mechanical traction therapy with their forearms placed on a traction device. However, in the placebo group, the device was turned off, and a placebo treatment was administered without any mechanical stimulation. All the participants underwent 18 sessions of intervention (10 minutes, 3 days per week for 6 weeks). Both groups were assessed for pain using the numerical pain rating scale and for symptom severity and functional status using the Boston Carpal Tunnel Syndrome Questionnaire (BCTQ), which includes two additional scales: the Boston Symptom Severity Scale (BSSS) and the Boston Functional Status Scale (BFSS). Results: The muscle activation, pain, symptom severity, and functional status in the MTT group were significantly improved compared to the placebo group (p<0.05). Conclusion: The results of this study indicate that traction therapy combined with tendon and neural gliding exercise. are a beneficial method for improving pain, symptom severity, and functional status in patients with carpal tunnel syndrome.
Introduction: This research was performed to contract the attitude of dietary restriction and the psychological problems such as depressive mood and perceived stress and to investigate the relationship of these and obesity in women who visited the obesity clinic. Methods: During May 2001, sociodemographic variables, physical characteristics, Three Factor Eating Questionnaire(TFEQ), Symptom Check List-90-R(SCL-90-R) and Perceived Stress Scale were assessed from 150 female who visited the obesity clinics which were located at downtown, Seoul and the Hospital of Ajou University, Medical College. Hamilton depression rating scale(HDRS) was estimated by author. And then 116 female cases who filled up the questionnaire faithfully were included. Results: Obese group more than Body Mass Index(BMI) $25.0kg/m^2$ was 50% of the total subjects. BMI was increased as the age goes up(p<0.001). The frequency of unmarried cases in the under normal weight group was high rate of 48.8% while it in the obese group was 13.8%(p<0.001). There was no significant difference in the rate of smoking and alcohol drinking among subjects by BMI. There was no significant difference of TFEQ among subjects by BMI and the percent of body fat. Factor 2(r=0.27, p<0.01) and Factor 3(r=0.24, p<0.01) were significantly correlated with Global Severity Index(GSI). Only the paranoia scale among each estimated mean value of T scores of SCL-90-R by BMI was the significant difference between the overweight group and the obese group(p<0.05). T scores of scales of SCL-90-R were less than 50, but T scores of the under normal weight group and the obese group were higher than overweight group. GSI was significantly correlated with HDRS(r=0.75, p<0.01) and Perceived Stress Scale(r=0.32, p<0.01). Depressive mood in the obese group was significantly higher than non-obese group that HDRS was compared to two groups by the percent of body fat(p<0.05). Perceived Stress Scale was no significant correlation with BMI and the percent of body fat. All of the subject were in trouble of high stress. Stress affected dietary restriction owing that perceived stress had a relation with Factor 2(r=0.29, p<0.01) and Factor 3(r=0.37, p<0.01). Also, it affected psychological characteristics owing that perceived stress had a relation with the depression scale, GSI and HDRS(r=0.33, r=0.32, r=0.34, p<0.01). Conclusion: Obese women have more psychological difficulties including depression and high perceived stress, which closely related with the attitude of dietary restriction. Psychiatric intervention and aggressive assessment of psychological problems will be needed to the people who visit the obesity clinic in the future.

Objectives : The aim of this study was to investigate the factors including possibility of bipolar disorder that are related to intensity of suicidal idea. Methods : The study subjects consisted of 50 patients who did suicidal attempt and treated at the Inje University Haeundae Paik Hospital Emergency Room. All participants underwent psychiatric interview and underwent Columbia Suicide Severity Rating Scale(C-SSRS), Beck Depression Inventory(BDI), Korean Version of Mood Disorder Questionnaire(MDQ) and Bipolar Spectrum Diagnostic Scale(BSDS) to evaluate patient's suicide attempt, severity of depression and possibility of bipolar disorder. Results : Compared to non-high risk group, suicide high risk group showed significantly higher BDI(p<0.001) and intensity of ideation(IOI) in C-SSRS(p<0.001). Also intensity of ideation(IOI) was correlated with Mood Disorder Questionnaire(MDQ) bipolarity positive(p=0.033). Conclusions : The present study indicated that possibility of bipolar disorder plays a significant role in suicide attempters. Assessment of suicide ideation severity and possibility of bipolar disorder should be considered when suicide attempters come to emergency room.
Objectives : Amyotrophic Lateral Sclerosis(ALS) is a progressive disease that causes degeneration of the motor neurons of the brain and spinal cord. The purpose of this case study is to improvement of oriental mediacl treatment on ALS. Methods : The patients were treated by acupuncture, moxibustion, cupping therapy, herbal medication, physical treatment. To determine the effects of Oriental medical treatment, we evaluated weekly used by Korean version of Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised(K-ALSFRS-R), Amyotrophic Lateral Sclerosis Severity Scale(ALSSS). Results : Symptoms such as cervical and shoulder pain, knee pain, cold sweating, insomnia etc were improved after above treatment. But K-ALSFRS-R and ALSSS were no improvement after above treatment. Conclusions : The Oriental medical treatment is effective on local symptoms of ALS, but there were no functional improvement of ALS in this case study. It is necessary to have more examination about ALS.

Objective: The purpose of this study was to report the effectiveness of Korean medicine in the treatment of functional dyspepsia in a fibromyalgia patient with a history of long-term NSAIDs use. Methods: The patient was diagnosed as a Soeumin, one of the four constitution types in Korean medicine, and treated with herbal medicines such as Seungyangikgibuja-tang and Pyeongjinsunjeom-san. Acupuncture and moxibustion treatment were also performed. The severity of symptoms was assessed using the Numerical Rating Scale (NRS) of epigastric pain, daily oral intake changes, the Korean Gastrointestinal Symptom Rating Scale (KGSRS), the Gastrointestinal Symptom Score (GIS), and the Functional Dyspepsia-Quality of Life (FD-QOL) score. Results: After treatment for 48 days, the severity of epigastric pain decreased from NRS 8 to NRS 0, and daily oral intake was increased. The KGSRS score decreased from 55 to 43, the GIS score was reduced from 18 to 10, and the FD-QOL score also decreased from 69 to 55. Conclusions: This study suggests that Korean medical treatment could be an effective option for treating functional dyspepsia in fibromyalgia patients with a history of long-term NSAIDs use.

This paper outlines a systematic guideline for remanufacturing process using the Failure Mode and Effect Analysis (FMEA) method in order to estimate the reliability and quality of the remanufactured alternator. The method is just a tool to help, but the remanufacturer must determine the optimal remanufacturing process and specific inspection and production that will turn the alternator as-good-as new and place the product into the market with reliability and quality equal to a new product. FMEA is a method that is widely used in industry and has shown its value and effectiveness in the above remanufacturing case study. Actions taken often result in a lower severity, occurrence or detection rating. Redesign may result in lower severity and occurrence ratings while inserting validation controls and maintenance can reduce the detection rating. The revised ratings are recorded with the originals on the FMEA template form. After these corrective actions and revisions have been established, evaluation of the ranks can be repeated, until the redesign and control parameters comply with safety standards.

Objectives: The objective of this study was to investigate clinical and neuropsychological factors associated with treatment response and adverse events of atomoxetine in children with attention-deficit/hyperactivity disorder (ADHD) in Korea. Methods: Children with ADHD were recruited at the Department of Psychiatry of Asan Medical Center from April 2015 to April 2018. Diagnoses of ADHD and comorbid psychiatric disorders were confirmed with the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version. The subjects were subsequently treated with atomoxetine for 12 weeks and illness severity was scored using the ADHD Rating Scale, Clinical Global Impression-Severity scale (CGI-S) and/or Improvement scale (CGI-I), at pre- and post-treatment. They also completed the Advanced Test of Attention (ATA), while their caregivers completed the Korean Personality Rating Scale for Children (KPRC) at pre- and post-treatment. Independent t-test, Fisher's exact test, ${\chi}^2$ test, mixed between-within analysis of variance and correlation analysis were used for statistical analysis. Results: Sixty-five children with ADHD (mean age: $7.9{\pm}1.4years$, 57 boys) were enrolled, of which, 33 (50.8%) were treatment responders. Scores on the social dysfunction subscale of the KPRC (p=0.021) and commission errors on the visual ATA (p=0.036) at baseline were higher in treatment non-responders than in responders; however, the statistical significances disappeared after adjusting for multiple comparisons. Mood changes were also observed in 13 subjects (20.0%), and three of them discontinued atomoxetine due to this. Additionally, atomoxetine-emergent mood change was observed more frequently in girls (p=0.006), while the intelligence quotient (p=0.040) was higher in those subjects with mood changes than in those without. Conclusion: The results of our study suggest that clinical and neuropsychological factors could be associated with treatment response or adverse events of atomoxetine in children with ADHD. Further long-term studies with larger samples are needed.
Objective: The purpose of this study was to report the effectiveness of Korean medicine (Beewha-eum) on the treatment of non-cardiac chest pain (NCCP) in a patient with non-erosive reflux disease (NERD). Methods: The patient was diagnosed with a spleen-stomach weakness pattern identified by Korean medicine and was treated with herbal medicine (Beewha-eum). The severity of symptoms was assessed with a numerical rating scale (NRS) for chest pain, self-reported dyspepsia degree (%), Korean gastrointestinal symptom rating scale (KGSRS), gastrointestinal symptom score (GIS), and Functional Dyspepsia-Quality of Life(FD-QOL) score. Results: After Beewha-eum treatment, the severity of chest pain was decreased from NRS 8 to NRS 0 and the self-reported dyspepsia degree also decreased from 100% to 65%. The KGSRS score was decreased from 49 to 35, the GIS score was also decreased from 16 to 9, and the FD-QOL score was increased from 20 to 25. Conclusions: The study findings suggested that Korean medical treatment with Beewha-eum could be an effective option for treating NCCP in patients with NERD.

■ Objectives The purpose of this case is to report the effectiveness of Korean medicine in the treatment of irritable bowel syndrome with diarrhea. ■ Methods The patient was diagnosed as a Soeumin, one of the 4 constitution types in Korean medicine, and treated with herbal medicine, Osuyubujaijung-tang and acupuncture treatment. The severity of symptom was assessed using daily defecation frequency, Numerical Rating Scale (NRS) of abdominal pain, Gastrointestinal Symptom Rating Scale (GSRS) score and Irritable Bowel Syndrome - Quality of Life (IBS-QOL) score. ■ Results After treatment, the defecation frequency was reduced from 6 to 2 times a day, and the severity of abdominal pain was reduced from NRS 6 to NRS 2. Also, GSRS score was decreased from 18 to 4 and IBS-QOL score was improved from 70.59 to 81.18. ■ Conclusion This study suggested that Korean medical treatment could be an effective option for treating irritable bowel syndrome with diarrhea.
