A Study on Activities of Daily Living and Perceived Health Status of Disabled Persons at Home in Rural Areas (농촌 지역 재가 장애인의 일상생활 수행능력과 지각된 건강 상태)

  • Purpose: This study was attempted to provide basic data required to develop community-base rehabilitation program for disabled persons at home by investigating their characteristics. ADL, and perceived health status. Method: The subjects were 146 disabled persons at home in rural areas. Data were collected from January 22 to January 31, 2004. Data were analyzed using frequencies, percentages, means, standard deviations, minimums, maximums, $\chi^2$-test, Fisher's exact test, t-test, one-way ANOVA, Scheffe test, Contingency coefficient, Pearson's correlation, Stepwise multiple regression. Results: The average score of ADL was $5.51{\pm}1.92$: sphincter control $(5.85{\pm}2.10)$, communication $(5.66{\pm}2.02)$, transfers $(5.67{\pm}2.21)$, self care $(5.44{\pm}2.05)$, social cognition $(5.33{\pm}2.08)$, and locomotion $(4.85{\pm}2.32)$. This means that the respondents were in the state of dependence. The main items requiring others' help were 'bathing' (57.7%) and 'going up and down stairs' (51.1%) ADL was significantly different according to age, sender, occupation, medical fee payer, type, severity and duration of disability, BMI, and alcohol drinking. Among the respondents, 82.8% perceived that their health status was bad. Main factors influencing the perceived health status of disability were age and ADL$(R^2=.343)$. Conclusion: Perception of health status among the disabled is related to their characteristics and ADL, so it is necessary to develop community-base rehabilitation programs in order to improve ADL and the perception of health status.

A New Disability-related Health Care Needs Assessment Tool for Persons With Brain Disorders

  • Objectives: This study aimed to develop a health needs assessment (HNA) tool for persons with brain disorders and to assess the unmet needs of persons with brain disorders using the developed tool. Methods: The authors used consensus methods to develop a HNA tool. Using a randomized stratified systematic sampling method adjusted for sex, age, and districts, 57 registered persons (27 severe and 30 mild cases) with brain disorders dwelling in Seoul, South Korea were chosen and medical specialists investigated all of the subjects with the developed tools. Results: The HNA tool for brain disorders we developed included four categories: 1) medical interventions and operations, 2) assistive devices, 3) rehabilitation therapy, and 4) regular follow-up. This study also found that 71.9% of the subjects did not receive appropriate medical care, which implies that the severity of their disability is likely to be exacerbated and permanent, and the loss irrecoverable. Conclusions: Our results showed that the HNA tool for persons with brain disorders based on unmet needs defined by physicians can be a useful method for evaluating the appropriateness and necessity of medical services offered to the disabled, and it can serve as the norm for providing health care services for disabled persons. Further studies should be undertaken to increase validity and reliability of the tool. Fundamental research investigating the factors generating or affecting the unmet needs is necessary; its results could serve as basis for developing policies to eliminate or alleviate these factors.

Clinical Characteristics of Female Panic Disorder Patients with Abortion History (유산 경험이 있는 여성 공황장애 환자의 임상적 특성)

  • Objectives The objective of this study is to investigate differences in clinical characteristics between female panic disorder (PD) patients with abortion history (PD+A) and without abortion history (PD-A). Methods We examined data from 341 female patients diagnosed with PD. We divided the patients with PD into PD+A (82 patients) and PD-A (259 patients) to compare demographic and clinical characteristics. The following instruments were applied : stress coping strategies, NEO-neuroticism, the Anxiety Sensitivity Index-Revised (ASI-R), the Albany Panic and Phobia Questionnaire (APPQ), the Beck Depression Inventory, the Beck Anxiety Inventory (BAI) and the Sheehan Disability Scale. Results Compared to the PD-A, the PD+A group showed no significant difference in coping strategies. However, significantly higher scores in neuroticism, the ASI-R, the APPQ and the BAI were observed. In terms of health-related disability, the PD+A group did not show significant difference. Conclusions Our results suggest that the PD+A group may differ from the PD-A group in trait markers such as neuroticism and anxiety sensitivity, and abortion history may be associated with panic-related symptom severity. Our study suggests that further consideration is needed on such clinical characteristics in PD patients with abortion history.

THE RESULT OF THE DENTAL BEHAVIOR TREATMENT FOR THE PATIENTS WITH THE DEVELOPMENTAL DISORDER IN SEOUL DENTAL HOSPITAL FOR DISABLED (서울시장애인치과병원의 발달장애아동에 대한 치과행동치료 결과)

  • The developmental disorder(intellectual disorder(ID) and autism spectrum disorder(ASD)) can severely impair a patient's ability to communicate and socialize. So they require physical management techniques, pharmacologic agents, and general anesthesia more than the normal at the dental clinic. The behavior therapy is a kind of the psychotherapy and is applied to the patient with behavioral problems. Seoul Dental Hospital for Disabled(SDHD) set up the Dental Behavior Clinic for the patients with developmental disorder and treated 32 patients with ID or ASD, blind. 18 patients were treated according to the ordinary plans, 14 patients stopped the treatment by different reasons. The various results of the treatment were produced by the kind or severity of the diseases, age and cooperation of the patients and the caregivers. Especially, the behavior therapist helped to figure out the characteristics of the patients and to make individualized treatment plans. In the future, it will be necessary to treat more patients and to make the dental behavior therapy objectify. And through the dental behavior therapy, it will be expected that the patients will become more cooperative to the dental clinic and can receive regular check-up peacefully, reducing the frequency of the general anesthesia.

Family Functions Characterized by the Industrial Accident Disabled (산재장애인의 특성에 따른 가족기능정도)

  • Purpose: This study was attempted to provide the basic data in order to develop a supportive system for the family by investigation of the general and disability-related features of the industrial accident disabled and their family function. Method: Three hundred and forty six industrial accident disabled were randomly selected among those who resided in the Seoul, Incheon, and Gyeonggi areas, and who were confirmed as disabled according to its severity from the first to fourteenth grade from January, 1998 to December, 2000. The data were collected for one month from August 1 to August 31, 2001. Each study team consisted of two members out of 20 researchers and they interviewed the disabled individually by visiting their homes. Collected data were analyzed using frequency, percentages, t-test, ANOVA, and Correlation, etc. Results: The average score for family function was 102.60. The adaptability represented the highest score among the concepts dependent on the family function. The concepts were cohesion, adaptability and communication, which scored as 33.59, 35.38, 33.34, respectively. The family function was significantly different in education, marital status and subjective health status. In particular, the family function was excellent from the industrial accident disabled who had graduated from middle school, was married, and felt healthy. Depression and quality of life were other factors, which were also correlated significantly with the family function. The lower the degree of depression and the better the quality of life, the better the family function. Conclusion: The industrial accident disabled who had been in poor health or faced with difficulties in daily activities even after the therapy for the disability was completed. In particular, their family always had a great burden to care for the disabled and to take financial responsibility. Therefore, the rehabilitation program for the disabled should be family oriented.

  • Aplastic anemia (AA) is a blood dyscrasia characterized by hypocelluar bone marrow and peripheral blood pancytopenia. Symptomatology depends on the severity of pancytopenia. Patients with AA are susceptible to infection because of agranulocytosis. Hemorrhage caused by thrombocytopenia can be fetal to these patients. Therefore dental procedure potentially can cause serious complications and should be applied with caution. A 6-year-old boy with moderate aplastic anemia was referred to treat dental caries. General dental procedure including resin filling, preformed crown, pulpectomy and extraction was performed under sedation. Combination of modalities such as platelet transfusion, oral hygiene instruction, and dental prophylaxis was also implemented. In this report, the dental and medical management of a patient with AA was presented. Interdisciplinary care should be administered to patients with AA.

  • Objective : The aim of this study was to examine Differences of overall characteristics depending on Symptom Severity in Acute Stroke Patient Method : We studied hospitalized patients within 10days after their ictus who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2011. We compared the general characteristics of acute stroke patient according to Scandinavian Stroke Scale score Result : The patient who had mild severity show significant difference for FBS, PP2, Homocysteine, Exercise, Tongue color, Mouth dryness, Chest discomfort, Constipation, Stool hardness. Conclusion : The above result contribute to predict severity of stroke symtoms according to risk factor and general condition of patients. Also, After occurrence of stroke, We will can block worsening of symptoms progression. Further studies will be needed to observation of follow up studies about progression of stroke among acute stroke patients with a serious disability.

Health Promoting Behaviors and Quality of Life of Korean Women with Arthritis (여성 관절염 환자의 건강증진과 삶의 질)

  • Factors related to health promotion activities and quality of life in Korean women with arthritis have not been clearly identified. Predictors of health promotion might be identified that will enhance the well - being of this group. Accordingly, the findings of the study will contribute additional information about the relationship between health promotion and quality of life and will add to the research on quality of life of individuals with a leading cause of disability--arthritis. The purpose of the study was to examine the relationship of selected background factors (years of illness, perceived severity of illness, uncertainty in illness), perceived self- efficacy, and health promoting behaviors to the quality of life of Korean women with arthritis. A cross - sectional descriptive design was used in this study to investigate relationships among the variables of interest. The sample was composed of 96 women who had arhtrits and visited large university hospital in Seoul for regular check up or pre-scription of medication. The purpose of a descriptive correlational design was to determine the absence or presence of relationships among variables that were measurable (Polit & Hungler, 1981, p.147). The design of this study was appropriate because it yielded answers to the research questions and hypotheses regarding the relationships among the model variables. the Questionnaire contained demographic information, translated Mishel Uncertainty in illness Scale-Community form (MUIS-C) (Mishel, 1987), translated and modified Disease Course Graphic Scale(DCGS) which was developed by Braden (1990), translated Sherer. et al.’s General Self-Efficacy Scale (1982), The Health -Promoting Lifestyle Profile (HPLP), developed by Walker, Sechrist, and Fender (1987) and traslated to Korean by Ha, and quality of life was measured by Face Scale (Andrew, 1976). Several steps of verification for the translation process were carefully conducted. Data analysis included descriptive correlational statistics and multiple regression techniques. Health promotion was the only contributor to pre-dict quality of life. Results showed that enabling cognitive perceptual factor (self-efficacy) mediates the disruptive force (uncertainty in ill-ness) on achieving a health promoting self- help behavior. The findings of this study also indicated that illness - related variable of severity of illness was mediated by health promotion, which buffered it's impact on quality of life.

Comparative Study of Lumbar Magnetic Resonance Imaging and Myelography in Young Soldiers with Herniated Lumbar Disc

  • Objective : This study was undertaken to compare the diagnostic performances of magnetic resonance imaging (MRI), MR myelography (MRM) and myelography in young soldiers with a herniated lumbar disc (HLD). Methods : Sixty-five male soldiers with HLD comprised the study cohort. A visual analogue scale for low back pain (VAS-LBP), VAS for leg radiating pain (VAS-LP), and Oswestry disability index (ODI) were applied. Lumbar MR, MRM, and myelographic findings were checked and evaluated by four independent radiologists, respectively. Each radiologist was asked to score (1 to 5) the degree of disc protrusion and nerve root compression using modified grading systems devised by the North American Spine Society and Pfirrmann and the physical examination rules for conscription in the Republic of Korea. Correlated coefficients between clinical and radiological factors were calculated. Interpretational reproducibility between MRI and myelography by four bases were calculated and compared. Results : Mean patient age was $20.5{\pm}1.1$. Mean VAS-LBP and VAS-LP were $6.7{\pm}1.6$ and $7.4{\pm}1.7$, respectively. Mean ODI was $48.0{\pm}16.2%$. Mean MRI, MRM, and myelography scores were $3.3{\pm}0.9$, $3.5{\pm}1.0$, and $3.9{\pm}1.1$, respectively. All scores of diagnostic performances were significantly correlated (p < 0.05). However, none of these scores reflected the severity of patients' symptoms. There was no statistical difference of interpretational reproducibility between MRI and myelography. Conclusion : Although MRI and myelography are based on different principles, they produce similar interpretational reproducibility in young soldiers with a HLD. However, these modalities do not reflect the severity of symptoms.

Relevant on oral health, oral health beliefs and practices of dental care in some elementary students (일부 고학년 초등학생들의 구강건강신념 및 치과진료에 따른 구강건강 행위의 관련성)

  • Objectives : This study is based on oral health knowledge and oral health belief, oral health behaviors are processed of an elementary school oral health education. Methods : Questionnaire survey was carried out targeting the elementary grades 5,6, in Nowon region. The data collected with T-test and pearson correlation analysis results were as follows: Results : 1. Correct answers of oral health knowledge is not required to treat dental decay in children's showed 5 grade students 17 persons 11.3%, 6 grade students 10 persons 6.8%. 2. 2 times of a day brush strokes showed 5 grade students85 persons56.6% and 6 grade students 79 persons 53.7%. 3. Students who have received dental care, oral health beliefs of the seriousness of the average $10.80{\pm}3.94$ and showed, Students who have never received dental care in the severity of oral health beliefs appear to the average $9.16{\pm}3.15$ were significantly different. 4. Elementary students' oral health beliefs and health of the sensitivity of the severity and disability - increasing the motivation to increase susceptibility showed a positive correlation, negative correlation between benefit and importance was the. Conclusions : In this study, elementary school students learn proper oral health knowledge will be required to be properly trained, improving oral health, oral health education beliefs lead to action would be to help.
