  • 치과의사는 자폐인의 치료시 자폐증의 정도와 의학적 관리가 필수적인 질환, 내과적 병력, 현재 환자가 복용하고 있는 약물등을 고려하여 이에 따라 적절한 행동조절 방법을 선택하여야 한다. 행동조절 방법의 선택시 술자가 선호하는 방법을 우선하기 보다는 환자의 상태와 치과치료시 환자와 보호자가 받는 스트레스를 최우선으로 고려하는 것이 필요하다. 환자의 현증에 대한 적절한 행동조절하의 치과치료도 중요하지만, 그보다 더 중요한 것은 환자가 거부감을 느끼지 않고 치과에 내원할수 있도록 훈련시키는 것과 지속적인 구강위생 관리이므로 보호자를 위한 구강위생 관리교육 및 주기적인 내원에 의한 관리가 필요하다고 생각된다.

생체전위를 이용한 중증 운동장애자들을 위한 컴퓨터 접근제어장치 설계 (Design of Computer Access Devices for Severly Motor-disability Using Bio-potentials)

  • In this paper, we describe implementation of a computer access device for the severly motor-disability. Many people with severe motor disabilities need an augmentative communication technology. Those who are totally paralyzed, or 'locked-in' cannot use conventional augmentative technologies, all of which require some measure of muscle control. The forehead is often the last site to suffer degradation in cases of severe disability and degenerative disease. For example, In ALS(Amyotrophic Lateral Sclerosis) and MD(Muscular dystrophy) the ocular motorneurons and ocular muscles are usually spared permitting at least gross eye movements, but not precise eye pointing. We use brain and body forehead bio-potentials in a novel way to generate multiple signals for computer control inputs. A bio-amplifier within this device separates the forehead signal into three frequency channels. The lowest channel is responsive to bio-potentials resulting from an eye motion, and second channel is the band pass derived between 0.5 and 45Hz, falling within the accepted Electroencephalographic(EEG) range. A digital processing station subdivides this region into eleven components frequency bands using FFT algorithm. The third channel is defined as an Electromyographic(EMG) signal. It responds to contractions of facial muscles and is well suited to discrete on/off switch closures, keyboard commands. These signals are transmitted to a PC that analyzes in a time series and a frequency region and discriminates user's intentions. That software graphically displays user's bio-potential signals in the real time, therefore user can see their own bio-potentials and control their physiological signals little by little after some training sessions. As a result, we confirmed the performance and availability of the developed system with experimental user's bio-potentials.

The Relationship between Hospital Selection by Employer and Disabilities in Occupational Accidents in Korea

  • Background; In the event of an industrial accident, the appropriate choice of hospital is important for worker health and prognosis. This study investigates whether the choice of hospital by the employer in the case of industrial accidents affects the prognosis of injured employees. Methods; Data from the 2018 Panel Study of Workers' Compensation Insurance in Korea were used in an unmatched case-controlled study. The exposure variable is "hospital selection by an employer," and the outcome variable is 'worker's disability." Odds ratios (ORs) were estimated by modified Poisson regression and adjusted for age, gender, underlying disease, injury severity, and workplace size and stratified by industrial classification. The group at increased risk was analyzed and stratified by age, gender, and area. Results; In the construction industry, hospital selection by the employer was significantly associated with increased risk of disability (adjusted OR 1.26; 95% confidence interval [CI]; 1.20-1.32) and severe disability (adjusted OR 1.38; 95% CI; 1.08-1.76) among the injured. Female and younger workers not living in the Seoul capital area were more at risk of disability and severe disability than those living in the Seoul capital area. Conclusions; Hospital selection by employers affects the prognosis of workers injured in an industrial accident. For protecting workers' health and safety, workplace emergency medical systems should be improved, and the selection of appropriate hospitals to supply treatment should be reviewed.

일본 장애아 보육시설의 시설설치기준에 관한 연구 (A Study on the Legal Regulations and Design Guidelines on the Child Care Centers for the Children with Disability in Japan)

  • A study on child care centers has been developed with a focus on normal children. Also the child care centers which take care of children with disability are rare. In Seoul, Korea, only 2% of children with disability are taken care of in child care centers. And even the disabled children at the child care centers are mostly mentally or emotionally disabled because the building, programs and services of the centers are restrictive to the children with severe physical disabilities. In Korea, it is not yet an obligation for child care centers to adopt Disability Accessibility Guidelines to make facilities accessible by the disabled. Also, Korea does not have specific design guidelines or legal standards. This study aims to review the legal standards and design guidelines which are applicable to child care centers for children with disability through a reference review. Japanese legal standards and references were collected and analyzed. As a result, we categorized the guidelines according to contents such as locations, areas, space organizations, nursing spaces, sanitary spaces, and doors and corridors. The goal of this study is to provide the basic information to develop domestic design guidelines to ensure that the child care centers are welcoming and usable for everyone possible.

성인 중증 지적장애에서의 문제행동에 대한 억간산가미(抑肝散加味) 치험 1례 보고 (A Case Report of Problem Behavior in Adult with Severe Intellectual Disability Treated with the Korean Herbal Medicine Yokukan-san-gami (Yigan-san-jiawei))

  • This case report presents a 31-year-old male intellectual disability patient with problem behavior such as food refusal and temper tantrums. He was treated with the Korean traditional herbal medicine (Yokukan-san-gami) continuously for 56 days. The effects of the treatment were measured by the frequency of his problem behavior and the score of the Korean version of Aberrant Behavior Checklist (ABC). After treatment, his challenging behavior became reduced, and his ABC score had been decreased by 24.6%. The findings from this case suggest that the Korean traditional herbal medicine (Yokukan-san) could be effective for challenging behavior in patients with intellectual disabilities.

Systematic review of the clinical and genetic aspects of Prader-Willi syndrome

  • Prader-Willi syndrome (PWS) is a complex multisystem genetic disorder that is caused by the lack of expression of paternally inherited imprinted genes on chromosome 15q11-q13. This syndrome has a characteristic phenotype including severe neonatal hypotonia, early-onset hyperphagia, development of morbid obesity, short stature, hypogonadism, learning disabilities, behavioral problems, and psychiatric problems. PWS is an example of a genetic condition caused by genomic imprinting. It can occur via 3 main mechanisms that lead to the absence of expression of paternally inherited genes in the 15q11.2-q13 region: paternal microdeletion, maternal uniparental disomy, and an imprinting defect. Over 99% of PWS cases can be diagnosed using DNA methylation analysis. Early diagnosis of PWS is important for effective long-term management. Growth hormone (GH) treatment improves the growth, physical phenotype, and body composition of patients with PWS. In recent years, GH treatment in infants has been shown to have beneficial effects on the growth and neurological development of patients diagnosed during infancy. There is a clear need for an integrated multidisciplinary approach to facilitate early diagnosis and optimize management to improve quality of life, prevent complications, and prolong life expectancy in patients with PWS.

근위축성측삭경화증 환자의 신체적 기능 상태 (Physical Function of Patients with Amyotrophic Lateral Sclerosis)

  • Purpose: This study was designed to present preliminary data for the development of appropriate nursing care system for the patients with ALS by analyzing their physical function. Method: The clinical data of 36 ALS patients, who visited ALS Clinic of H University Hospital in Seoul, were collected from January, 2006 to August, 2006. To determine the physical function, Norris ALS scale and Appel ALS Rating Scale were used. The data were analyzed by frequency, percentage, mean, standard deviation, range, t-test, ANOVA, using SPSS PC program. Results: The mean score of physical activity, muscle strength, upper extremity function, lower extremity function was 18.08, 27.72, 25.94, 25.19 respectively. There were significant differences in physical activity, muscle strength, and upper extremity function according to sender and comorbid disease(diabetes). Although sites of symptom onset were not statistically significant with all physical function, patients with bulbar onset showed relatively severe physical disabilities. Conclusion: The preliminary data on physical function of patients with ALS would be helpful for the development of ALS nursing guideline system.

발달장애아 부모의 재활적 의료 욕구도 (Rehabilitation Medical Desire of Parents with Developmental Disability Children)

  • This study is aiming for evaluating the factors influencing rehabilitation medical desire in parents (RMDP) with developmental disability children. This study was made with a randomized sample of 156 parents, who were given a questionnaire about how they feel medical needs for their children. The results of the study were as follows: The RMDP is significantly high in relatively the older aged parents group(p<.01). The RMDP is significantly high in severe disability group(p<.01). The RMDP is significantly high in the more spend group of monthly medical cost(p<.05). The group difference of RMDP by monthly income or movement type of disability is not significant(p>.05). It is thought that medical staff need to be actively involved in parents group with developmental disability children to improve motivation, who is relatively the younger than 35 years old or has mild to moderate disability degree of child.

Use of ADMSTM during sedation for dental treatment of an intellectually disabled patient: a case report

  • Dental treatment is often performed under general anesthesia or sedation when an intellectually disabled patient has a heightened fear of treatment or has difficulty cooperating. When it is impossible to control the patient due to the severity of intellectual disability, conscious sedation is not a viable option, and only deep sedation should be performed. Deep sedation is usually achieved by propofol infusion using the target controlled infusion (TCI) system, with deep sedation being achieved at a slightly lower concentration of propofol in disabled patients. In such cases, anesthesia depth monitoring using EEG, as with a Bispectral Index (BIS) monitor, can enable dental treatment under appropriate sedation depth. In the present case, we performed deep sedation for dental treatment on a 27-year-old female patient with mental retardation and severe dental phobia. During sedation, we used BIS and a newly developed Anesthetic Depth Monitor for Sedation (ADMS$^{TM}$), in addition to electrocardiography, pulse oximetry, blood pressure monitoring, and capnometry for patient safety. Oxygen was administered via nasal prong to prevent hypoxemia during sedation. The BIS and ADMS$^{TM}$ values were maintained at approximately 70, and dental treatment was successfully performed in approximately 30 min.

A newborn with developmental delay diagnosed with 4q35 deletion and 10p duplication

  • We report the case of an infant with a 4q35.1 deletion with 10p duplication. This mutation is rarely reported in the literature and has been found to have variable clinical findings, often including developmental delay. In this case, the condition was detected by chromosomal microarray analysis after initial manifestation of a feeding problem and developmental delay. Minor dysmorphic features with abnormal neurological examination led to further evaluation. The father's chromosome complement was 46, XY, t(4;10)(q35;p12.2). Parental balanced translocation can go unrecognized, because affected individuals are often phenotypically healthy until they have fertility issues such as recurrent miscarriages or children with severe congenital disorders. Genetic diagnoses help to establish a clear family genetic background that permits the development of clear treatment strategies. Prenatal counseling can also help to understand the possible risks associated with pregnancy or future child planning.
