Cohort Profile: Korean Tuberculosis and Post-Tuberculosis Cohort Constructed by Linking the Korean National Tuberculosis Surveillance System and National Health Information Database

  • We aimed to review the current data composition of the Korean Tuberculosis and Post-Tuberculosis Cohort, which was constructed by linking the Korean Tuberculosis Surveillance System (KNTSS; established and operated by the Korean Disease Control and Prevention Agency since 2000) and the National Health Information Database (NHID; established by the National Health Insurance Service in 2012). The following data were linked: KNTSS data pertaining to patients diagnosed with tuberculosis between 2011 and 2018, NHID data of patients with a history of tuberculosis and related diseases between 2006 and 2018, and data (obtained from the Statistics Korea database) on causes of death. Data from 300 117 tuberculosis patients (177 206 men and 122 911 women) were linked. The rate of treatment success for new cases was highest in 2015 (86.7%), with a gradual decrease thereafter. The treatment success rate for previously treated cases showed an increasing trend until 2014 (79.0%) and decreased thereafter. In total, 53 906 deaths were confirmed among tuberculosis patients included in the cohort. The Korean Tuberculosis and Post-Tuberculosis Cohort can be used to analyze different measurement variables in an integrated manner depending on the data source. Therefore, these cohort data can be used in future epidemiological studies and research on policy-effect analysis, treatment outcome analysis, and health-related behaviors such as treatment discontinuation.

Food habit and Nutrient Intake in Preschool Children -Daegu and its Environs- (지방의 일부 학령 전 아동의 식습관 및 영양소 섭취수준 -대구 및 인근 소도시 중심으로-)

  • This study was conducted to evaluate the food habits and nutrient intake of preschool children by investigating their food and snack consumption. To accomplish this, a food habit questionnaire and dietary intake survey was administered to 148 children between 2 and 6 years of age (average 4.8 years). According to the Kaup index, the underweight, normal, overweight and obesity levels of the subjects were 4.7, 60.1, 26.4, and 8.8%, respectively. In addition, the total energy intake met the EAR ($1433.94{\pm}369.70kcal$), whereas the protein, iron, phosphorus, vitamin B and vitamin C levels exceeded the EAR and the RI. Furthermore, the nutrient intake profile revealed that the mean caloric value of breakfasts consumed by preschool children in Daegu (403 kcal) was significantly (p<0.001) higher than that of preschool children in the Gyeongbuk area (271 kcal). Additionally, preschool children in Daegu ($382.96{\pm}255.79kcal$) usually consumed significantly (p<0.01) less food than those in the Gyeongbuk area ($413.82{\pm}275.43kcal$). The preferred snacks among subjects (in descending order) were milk, bread, fruit, potato or sweet potato and cookies. The mean score of the Food Habit was $3.72{\pm}0.35$ out of 5, although the score of preschool children in Daegu ($3.77{\pm}0.32$) was significantly (p<0.05) higher than that of children in the Gyeongbuk area ($3.65{\pm}0.32$). Taken together, these results indicate that there were significant differences in the Food Habit and nutrient intake level of children in Daegu and the Gyeongbuk area. Therefore, parents and caregivers should be aware of the importance of the nutritional status of preschool children and choose healthy snacks for them.

Service life evaluation of HPC with increasing surface chlorides from field data in different sea conditions

  • The penetrated chloride in concrete has different behavior with mix proportions and local exposure conditions, even in the same environments, so that it is very important to quantify surface chloride contents for durability design. As well known, the surface chloride content which is a key parameter like external loading in structural safety design increases with exposure period. In this study, concrete samples containing OPC (Ordinary Portland Cement), GGBFS (Ground Granulated Blast Furnace Slag), and FA (Fly Ash) had been exposed to submerged, tidal, and splash area for 5 years, then the surface chloride contents changing with exposure period were evaluated. The surface chloride contents were obtained from the chloride profile based on the Fick's 2nd Law, and the regression analysis for them was performed with exponential and square root function. After exposure period of 5 years in submerged and tidal area conditions, the surface chloride content of OPC concrete increased to 6.4 kg/m3 - 7.3 kg/m3, and the surface chloride content of GGBFS concrete was evaluated as 7.3 kg/m3 - 11.5 kg/m3. In the higher replacement ratio of GGBFS, the higher surface chloride contents were evaluated. The surface chloride content in FA concrete showed a range of 6.7 kg/m3 to 9.9 kg/m3, which was the intermediate level of OPC and GGBFS concrete. In the case of splash area, the surface chloride contents in all specimens were from 0.59 kg/m3 to 0.75 kg/m3, which was the lowest of all exposure conditions. Experimental constants available for durability design of chloride ingress were derived through regression analysis over exposure period. In the concrete with GGBFS replacement ratio of 50%, the increase rate of surface chloride contents decreased rapidly as the water to binder ratio increased.

Initial United Kingdom experience of endoscopic ultrasound-directed transgastric endoscopic retrograde cholangiopancreatography

  • Backgrounds/Aims: Gallstone disease is a recognized complication of bariatric surgery. Subsequent management of choledocholithiasis may be challenging due to altered anatomy which may include Roux-en-Y gastric bypass (RYGB). We conducted a retrospective service evaluation study to assess the safety and efficacy of endoscopic ultrasound-directed transgastric endoscopic retrograde cholangiopancreatography (EDGE) in patients with RYGB anatomy. Methods: All the patients who underwent EDGE for endoscopic retrograde cholangiopancreatography after RYGB at two tertiary care centers in the United Kingdom between January 2020 and October 2021 were included in the study. Clinical and demographic details were recorded for all patients. The primary outcome measures were technical and clinical success. Adverse events were recorded. Hot Axios lumen apposing metal stents measuring 20 mm in diameter and 10 mm in length were used in all the patients for creation of a gastro-gastric or gastro-jejunal fistula. Results: A total of 14 patients underwent EDGE during the study period. The majority of the patients were female (85.7%) and the mean age of patients was 65.8 ± 9.8 years. Technical success was achieved in all but one patient at the first attempt (92.8%) and clinical success was achieved in 100% of the patients. Complications arose in 3 patients with 1 patient experiencing persistent fistula and weight gain. Conclusions: In patients with RYGB anatomy, EDGE facilitated biliary access has a high rate of clinical success with an acceptable safety profile. Adverse events are uncommon and can be managed endoscopically.

Quality Characteristics of Haengbyeong using Dry Rice Flour added with Raw Apricot Juice and Steamed Apricot Juice (살구즙을 첨가한 건식 쌀가루 행병의 품질 특성 연구)

  • This study investigated the quality characteristics of Haengbyung using dry rice flour added with various amounts (0, 20, 40, 60%) of raw and steamed apricot juice. The moisture contents, color values, pH, texture and sensory evaluation(attribute difference and acceptance) for the samples were tested. The moisture content (p<0.001) and pH(p<0.001) of the Haengbyung significantly decreased with the increase of apricot juice. As for the color of the Haengbyung samples, the higher percentage of the apricot juice showed the lower L-value (p<0.001), and higher a-value(p<0.001) and b-value(p<0.001). Texture profile analysis of Haengbyung samples showed that hardness(p<0.001) and gumminess(p<0.001) increased, while adhesiveness(p<0.001) and cohesiveness(p<0.05) decreased with increasing amounts of apricot juice. An attribute difference test of Haengbyung samples showed that appearance(apricot color(p<0.001) and dryness(p<0.001)), flavor(sour(p<0.001), acerbic (p<0.001), and green fruit(p<0.001)), and taste(sour(p<0.001), acerbic(p<0.001), and astringent(p<0.05) as increased as apricot juice amounts increased. The control and Haengbyung sample with 20% of apricot juice (both raw and steamed) showed the highest acceptance in taste, texture, and overall acceptability in consumer ranking testing.

Modeling of Chloride Ingress in Reinforced Concrete Structures (철근 콘크리트 구조물의 염소이온 침투 모델)

  • The degradation of reinforced concrete (RC) structures due to physical and chemical attacks has been a major issue in construction engineering. Deterioration of RC structures by chloride attack followed by reinforcement corrosion is one of the serious problems. An objective of this study is to develop a form of mathematical model of chloride ingress into concrete. In order to overcome some limits of the previous approaches, a chloride ingress model, consisting of chloride solution intrusion through the capillary pore and chloride ion diffusion through the pore water, was proposed. Moreover, the variability of chloride ion diffusivity due to the degree of hydration of cement, relative humidity in pore, exposure condition, and variation of chloride binding, was considered in the model. In order to verify the proposed model, the results predicted by the proposed model were compared with analysis results of Life-365, a computer program for predicting the service life of reinforced concrete structures exposed to chlorides. In conclusion, the proposed model would be promising to predict the chloride ion profile and to estimate the service life of RC structures.

Numerical Study on the Strength Safety of High Pressure Gas Cylinder (고압가스 압력용기의 강도안전성에 관한 수치해석적 연구)

  • The strength safety of high pressure gas cylinder has been analyzed by using a finite element method. In this study, the internal gas pressures of a steel bombe include a service charging pressure of $9kg/cm^2$, high limit charging pressure of $18.6kg/cm^2$, high limit of safety valve operation pressure $24.5kg/cm^2$, and hydraulic testing pressure of $34.5kg/cm^2$. The computed FEM results indicate that the strength safety for a service charging pressure of $9kg/cm^2$ and high limit charging pressure of $18.6kg/cm^2$ is safe because the stress of a gas cylinder is within yield strength of steel. But the stress for a hydraulic testing pressure of $34.5kg/cm^2$ sufficiently exceeds the yield strength and remains under the tensile strength. If the hydraulic testing pressures frequently apply to the gas cylinder, the bombe may be fractured because a fatigue residual stress is accumulated on the lower round end plate due to a plastic deformation. The computed results show that the concentrated force in which is applied on a skirt zone does not affect to the lower round end plate, and the most weak zone of a bombe is a middle part of a lower round end plate between a bombe body and a skirt for a gas pressure. Thus, the FEM results show that the profile of a lower round end plate is an important design parameter of a high pressure gas cylinder.

Analysis of the relationship between lifestyle habits and glycosylated hemoglobin control based on data from a Health Management Plan

  • BACKGROUND/OBJECTIVES: Type 2 Diabetes mellitus (T2DM) is a hereditary disease that is also strongly dependent on environmental factors, lifestyles, and dietary habits. This study explored the relationship between lifestyle habits and glycosylated hemoglobin management in T2DM patients to provide empirical outcomes to improve T2DM management and patient health literacy. SUBJECTS/METHODS: This study enrolled 349 diabetic patients with more than 5 care visits to a Diabetes Mellitus care network under the Health Management Plan led by Taiwan Department of Health (DOH). Based on relevant literature, an Outpatient Record Form of Diabetes Mellitus Care was designed and lipid profile tests were conducted for data collection and analysis. RESULTS: When modeling the data, the results showed that the odds for HbA1c > 7.5% in T2DM patients duration over 10 years was 3.785 (P = 0.002) times that in patients with disease duration of fewer than 3 years. The odds of HbA1c > 7.5% in illiterate patients was 3.128 (P = 0.039) times that in patients with senior high school education or above. The odds of HbA1c > 7.5% in patients with other chronic illness was 2.207 (P = 0.019) times that in participants without chronic illness. Among 5 beneficial lifestyle habits, the odds of HbA1c > 7.5% in patients with 2 or 3 good habits were 3.243 (P = 0.003) and 3.424 (P = 0.001) times that in patients with more than 3 good habits, respectively. CONCLUSION: This empirical outcome shows that maintaining a good lifestyle improves T2DM management and patients' knowledge, motivation, and ability to use health information. Patients with longer disease duration, education, or good lifestyle habits had optimal HbA1c management than those in patients who did not. Thus, effective selfmanagement and precaution in daily life and improved health literacy of diabetic patients are necessary to increase the quality of T2DM care.

  • The Comprehensive Assistive Technology services delivery was devised to describe the full social and engineering context of assistive technology devices, systems, applications, and provision. It is based on a detailed decomposition of the four attributes: Person, Context, Activities, and Assistive Technology. The paper commences will a brief discussion of the different representations of the services delivery, including rental of devices, modification of manufacture goods and customized production and the use of engineering to supplement some elements of the description. The main contribution of the paper is the presentation of two case studies of different applications of the assistive technology services. These are the identification of accessibility barriers, the analysis and synthesis of assistive technology systems and the provision of a framework for identitying end-user requirements, providing assistive technology and assessing the associated outcomes. This case study involves the presentation of the concept of a personal assistive technology profile.

The Android-based Bluetooth Device Application Design and Implementation (안드로이드 기반의 블루투스 디바이스 응용 설계 및 구현)

  • Today, although most bluetooth hands-free devices within a vehicle provide telephone service functions such as voice communication, caller id display and SMS message display and so on, they do not provide a function that displays Internet-based text data. We need to develop a scheme that displays the internet-based text data including existing hands-free function because the request for using the Internet service is increasing within a vehicle recently. The proposed bluetooth device application includes advanced function such as SNS message arrival notification, the message display function and we chose Android as the implementation mobile platform giving consideration to the fact that most SNS applications operate on Android and the platform is easily embedded into small embedded device. Smartphone or tablet PC connected with the proposed bluetooth device is an Android-based device and we designed a form of Android app for the function implementation of the devices. When the audio-text gateway app receives SNS text data, it extracts title and sender information from the message header information in a form of text data and sends them via ACL (Asynchronous Connection-Oriented) link to the bluetooth device showing the data on the screen. Android-based bluetooth devices are not possible to play voice through speaker because the bluetooth hands-free or headset profile ported within Android platform normally only includes audio gateway's function. The proposed bluetooth device application, therefore, applies the streaming scheme that sends data via ACL link instead of the way that sending them via SCO (Synchronous Connection-Oriented) link.
