건강보험 청구자료를 이용한 일반 질 지표로서의 위험도 표준화 재입원율 산출: 방법론적 탐색과 시사점 (Developing a Hospital-Wide All-Cause Risk-Standardized Readmission Measure Using Administrative Claims Data in Korea: Methodological Explorations and Implications)

  • Background: The purpose of this study was to propose a method for developing a measure of hospital-wide all-cause risk-standardized readmissions using administrative claims data in Korea and to discuss further considerations in the refinement and implementation of the readmission measure. Methods: By adapting the methodology of the United States Center for Medicare & Medicaid Services for creating a 30-day readmission measure, we developed a 6-step approach for generating a comparable measure using Korean datasets. Using the 2010 Korean National Health Insurance (NHI) claims data as the development dataset, hierarchical regression models were fitted to calculate a hospital-wide all-cause risk-standardized readmission measure. Six regression models were fitted to calculate the readmission rates of six clinical condition groups, respectively and a single, weighted, overall readmission rate was calculated from the readmission rates of these subgroups. Lastly, the case mix differences among hospitals were risk-adjusted using patient-level comorbidity variables. The model was validated using the 2009 NHI claims data as the validation dataset. Results: The unadjusted, hospital-wide all-cause readmission rate was 13.37%, and the adjusted risk-standardized rate was 10.90%, varying by hospital type. The highest risk-standardized readmission rate was in hospitals (11.43%), followed by general hospitals (9.40%) and tertiary hospitals (7.04%). Conclusion: The newly developed, hospital-wide all-cause readmission measure can be used in quality and performance evaluations of hospitals in Korea. Needed are further methodological refinements of the readmission measures and also strategies to implement the measure as a hospital performance indicator.

온라인쇼핑몰 신한복 스타일링에 관한 연구 (A Study on New-Hanbok Styling of Online Shopping Mall)

  • The purpose of this study is to analyze the characteristics of the New-Hanbok styling of online shopping mall, and to also suggest a solution to the problems of the New-Hanbok styling and develop a progressive plan. The research method was to search six keywords related to 'Hanbok' in the search portal 'Naver' and select 14 Hanbok brand companies. A total of 412 pictures of products for the model used on main screen were analyzed among 14 companies. The results of analyzing the New-Hanbok styling are as follows. First, the New-Hanbok styling showed the unstructured characteristics like unconventional arrangement after getting out of the fixed form of traditional Hanbok styling elements. Secondly, diverse images were represented as the hairstyle and makeup were highlighted as the elements of New-Hanbok styling. Thirdly, the new, fresh, trendy, and fashionable New-Hanbok styling was shown through the mix-and-match of traditional Korean-style accessories and fashion jewelries. However, regarding the New-Hanbok styling shown in online shopping mall, the overlapped items were especially found while the difference in material, pattern, and color required to overcome this problem was insufficient. It was lacking in the styling consistency for the establishment brand image while the awareness of the importance of accessory styling was insufficient. The brand competitiveness of the New-Hanbok could be secured by raising awareness on differentiation, consistency, and importance through the styling elements such as item composition, material, pattern, color, hairstyle, makeup, and accessory of brand.

Service life evaluation of HPC with increasing surface chlorides from field data in different sea conditions

  • The penetrated chloride in concrete has different behavior with mix proportions and local exposure conditions, even in the same environments, so that it is very important to quantify surface chloride contents for durability design. As well known, the surface chloride content which is a key parameter like external loading in structural safety design increases with exposure period. In this study, concrete samples containing OPC (Ordinary Portland Cement), GGBFS (Ground Granulated Blast Furnace Slag), and FA (Fly Ash) had been exposed to submerged, tidal, and splash area for 5 years, then the surface chloride contents changing with exposure period were evaluated. The surface chloride contents were obtained from the chloride profile based on the Fick's 2nd Law, and the regression analysis for them was performed with exponential and square root function. After exposure period of 5 years in submerged and tidal area conditions, the surface chloride content of OPC concrete increased to 6.4 kg/m3 - 7.3 kg/m3, and the surface chloride content of GGBFS concrete was evaluated as 7.3 kg/m3 - 11.5 kg/m3. In the higher replacement ratio of GGBFS, the higher surface chloride contents were evaluated. The surface chloride content in FA concrete showed a range of 6.7 kg/m3 to 9.9 kg/m3, which was the intermediate level of OPC and GGBFS concrete. In the case of splash area, the surface chloride contents in all specimens were from 0.59 kg/m3 to 0.75 kg/m3, which was the lowest of all exposure conditions. Experimental constants available for durability design of chloride ingress were derived through regression analysis over exposure period. In the concrete with GGBFS replacement ratio of 50%, the increase rate of surface chloride contents decreased rapidly as the water to binder ratio increased.

지능화시설의 통합운영관리를 위한 정의 및 구분에 관한 연구 (Definition and Division in Intelligent Service Facility for Integrating Management)

  • 스마트시티는 날로 복잡해지는 도시문제 해결 및 시민들의 편리하고 안전한 생활을 위한 도시차원의 방법론이자 미래 도시의 청사진으로 자리잡아가고 있다. 2008년 이후 한국 정부는 "유비쿼터스도시의 건설 등에 관한 법률"(이하 유시티법, 현재 스마트시티로 명칭 변경 및 법 개정 중)을 제정하여 스마트시티의 건설 및 관리에 관한 내용과 정부지원방안 등에 대하여 규정하고 있다. 또한 국토교통부장관은 스마트시티의 효율적인 건설을 위하여 유시티법에서 정한 사항을 고려하여 법에 따라 "유비쿼터스도시 종합계획"을 수립하고 있다. 그러나 스마트시티에 관한 개념은 정보화계획 측면과 도시계획 측면의 혼재로 인하여 통합관리 측면의 어려움은 물론 법규 정립의 미흡함으로 인해 실무단계에서도 다부처간의 협력적 계획의 설정 및 구현을 어렵게 하고 있다. 또한 스마트시티의 지능화시설은 다양한 정보 수집 및 표출 장비가 포함되어 있고 기술의 빠른 변화로 인해 정의 및 분류가 쉽지 않다. 이에 본 연구는 지능화시설의 통합관리운영 측면에서 필수적이라 할 수 있는 지능화시설의 법적 정의를 보완하고자 하였다. 지능화시설의 법적 정의의 미흡함을 파악하기 위하여 텍스트마이닝 기법을 이용해 상위 법 및 지자체들의 관련 조례를 분석 하였다. 또한, 스마트시티 관련 담당자 인터뷰를 통해 실무적 차원의 지능화시설의 설치 및 운영에 따른 실행적 차원의 문제점 및 법적 분류의 문제점을 파악하여 활용도 측면을 고려한 지능화시설의 분류방안을 제시하고자 하였다. 이는 차후 각 도시들이 5년 단위로 수립하는 유비쿼터스 도시계획을 수립 시 명확한 개념의 제공을 통해 다 부서간의 통합 활용을 위한 기반구축 등 관리 효율성 증대에 기여 할 것으로 기대한다.

병원 단위비용 결정요인에 관한 연구 (Analyses of the Efficiency in Hospital Management)

  • The objective of this study is to examine how to maximize the efficiency of hospital management by minimizing the unit cost of hospital operation. For this purpose, this paper proposes to develop a model of the profit maximization based on the cost minimization dictum using the statistical tools of arriving at the maximum likelihood values. The preliminary survey data are collected from the annual statistics and their analyses published by Korea Health Industry Development Institute and Korean Hospital Association. The maximum likelihood value statistical analyses are conducted from the information on the cost (function) of each of 36 hospitals selected by the random stratified sampling method according to the size and location (urban or rural) of hospitals. We believe that, although the size of sample is relatively small, because of the sampling method used and the high response rate, the power of estimation of the results of the statistical analyses of the sample hospitals is acceptable. The conceptual framework of analyses is adopted from the various models of the determinants of hospital costs used by the previous studies. According to this framework, the study postulates that the unit cost of hospital operation is determined by the size, scope of service, technology (production function) as measured by capacity utilization, labor capital ratio and labor input-mix variables, and by exogeneous variables. The variables to represent the above cost determinants are selected by using the step-wise regression so that only the statistically significant variables may be utilized in analyzing how these variables impact on the hospital unit cost. The results of the analyses show that the models of hospital cost determinants adopted are well chosen. The various models analyzed have the (goodness of fit) overall determination (R2) which all turned out to be significant, regardless of the variables put in to represent the cost determinants. Specifically, the size and scope of service, no matter how it is measured, i. e., number of admissions per bed, number of ambulatory visits per bed, adjusted inpatient days and adjusted outpatients, have overall effects of reducing the hospital unit costs as measured by the cost per admission, per inpatient day, or office visit implying the existence of the economy of scale in the hospital operation. Thirdly, the technology used in operating a hospital has turned out to have its ramifications on the hospital unit cost similar to those postulated in the static theory of the firm. For example, the capacity utilization as represented by the inpatient days per employee tuned out to have statistically significant negative impacts on the unit cost of hospital operation, while payroll expenses per inpatient cost has a positive effect. The input-mix of hospital operation, as represented by the ratio of the number of doctor, nurse or medical staff per general employee, supports the known thesis that the specialized manpower costs more than the general employees. The labor/capital ratio as represented by the employees per 100 beds is shown to have a positive effect on the cost as expected. As for the exogeneous variable's impacts on the cost, when this variable is represented by the percent of urban 100 population at the location where the hospital is located, the regression analysis shows that the hospitals located in the urban area have a higher cost than those in the rural area. Finally, the case study of the sample hospitals offers a specific information to hospital administrators about how they share in terms of the cost they are incurring in comparison to other hospitals. For example, if his/her hospital is of small size and located in a city, he/she can compare the various costs of his/her hospital operation with those of other similar hospitals. Therefore, he/she may be able to find the reasons why the cost of his/her hospital operation has a higher or lower cost than other similar hospitals in what factors of the hospital cost determinants.

  • 한국프랜차이즈산업은 1970년대 도입된 이후, 외식업, 도·소매업, 서비스업 등 광범위한 영역에 국민생활과 밀접한 지식기반서비스산업으로 고성장을 지속하고 있다. 이제 국내 프랜차이즈산업은 양적성장에서 질적 성장으로의 전환을 통한 새로운 도약을 준비해야 할 시기이다. 무엇보다도 중요한 것은 가맹본부와 가맹점 사업자 간의 신뢰구축을 위한 가맹본부와 가맹점사업자의 노력과 실천이다(Stanworth & Kaufmana, 1995). 가맹본부와 가맹점사업자간의 우호적이고 신뢰할 수 있는 관계(Relationship)야 말로 프랜차이즈에 대한 불신을 해소하고 건강한 프랜차이즈시스템을 유지할 수 있는 근간이기도 하다. 가맹사업을 하고자 하는 가맹본부는 프랜차이즈산업의 지속적인 성장과 발전을 위해서 가맹점창업의 성공에 많은 영향을 미치는 항목이 무엇인지를 파악해야 한다. 따라서, 가맹점 사업성과의 만족도와 가맹점 사업성공·실패에 미치는 가맹점의 선택속성이 무엇인지를 파악하고 업종별로 차이가 있다는 것을 밝히려는 연구의 목적이 있다. 본 연구는 가맹사업을 하고 있는 가맹본부나 예비 가맹본부 창업자들이 가맹점 모집뿐만 아니라 가맹점 지원시스템 구축력, 브랜드력, 가맹점의 상권 경쟁력, 교육·훈련 프로그램 수준, 가맹점사업자의 금융비용, 가맹점본부와 가맹점사업자와의 파트너쉽, 마케팅 믹스 관리능력(제품, 가격조건, 물류 및 배송서비스, 프로모션, 슈퍼바이징과 슈퍼바이저, 업무절차·과정, 물리적 증거)에 투자와 활동을 집중해야 가맹점의 성공가능성이 높아진다는 것을 시사하고 있다. 가맹점 예비창업자들이 우수한 가맹본부를 선택하기 위해 가장 크게 고려하는 사항은 가맹본부의 특성이다. 특히 가맹점에 대한 가맹본부의 지원은 가맹점의 사업성과와 만족도를 높이고 결과적으로 적극적인 추천이나 재 계약률을 높일 수 있다.

한국형 진단명기준환자군의 개발과 평가: 입원환자의 의료서비스 이용을 중심으로 (Development and Evaluation of Korean Diagnosis Related Groups: Medical service utilization of inpatients)

  • With expanded and extended coverage of the national medical insurance and fast growing health care expenditures, appropriateness of health service utilization and quality of care are concerns of both health care providers and insurers as well as patients. An accurate patient classification system is a basic tool for effective health care policies and efficient health services management. A classification system applicable to Korean medical information-Korean Diagnosis Related Groups (K-DRGs)-was developed based on the U.S. Refined DRGs, and the performance of the developed system was assessed in this study. In the process of the development, first the Korean coding systems for diagnoses and procedures were converted to the systems used in the definition of the U.S. Refined DRGs using the mapping tables formulated by physician panels. Then physician panels reviewed the group definition, and identified medical practice patterns different in two countries. The definition was modified for the differences in K-DRGs. The process resulted in 1,199 groups in the system. Several groups in Refined DRGs could not be differentiated in K-DRGs due to insufficient medical information, and several groups could not be defined due to procedures which were not practiced in Korea. However, the classification structure of Refined DRGs was retained in K-DRGs. The developed system was evaluated fur its performance in explaining variations in resource use as measured by charges and length of stay(LOS), for both all and non-extreme discharges. The data base used in this evaluation included 373,322 discharges which was a random sample of discharges reviewed and payed by the medical insurance during the five-month period from September 1990. The proportion of variance in resource use which was reduced by classifying patients into K-DRGs-r-square-was comparable to the performance of the U.S. Refined DRGs: .39 for charges and .25 for LOS for all discharges, and .53 for charges and .31 for LOS for non-extreme discharges. Another measure analyzed to assess the performance was the coefficient of variation of charges within individual K-DRGs. A total of 966 K-DRGs (87.7%) showed a coefficient below 100%, and the highest coefficient among K-DRGs with more than 30 discharges was 159%.

낙엽송 원주상 소경목의 원목성상과 방부 처리성 (Effects of Post Individualities on Treatability of Small Diameter Japanese Larch (Larix Leptolepis) with ACQ and CCA)

  • 국산목재시장에서 유통되는 낙엽송 원주상 소경목(직경 10 cm 정도)의 목재 특성(연륜 특성, 심 변재 분포, 할열 분포 등)이 ACQ와 CCA의 처리특성에 미치는 영향에 대하여 조사 연구한 결과, 다음과 같은 결론을 얻었다. 시장에 유통되는 낙엽송 원주상 소경목은 약 13~16, 17~20개의 연륜수를 갖는 빈도가 가장 높았으며, 2~3과 4~5 mm의 연륜폭을 갖는 원목의 빈도가 높은 것으로 나타났다. 원주상 소경목의 재면에 심, 변재가 혼재되어 나타나는 비율은 전체 공시목의 54%였으며 나머지는 심재만이 재면 노출된 것으로 밝혀졌다. 또한 원주상 소경목에 발생하는 할열 수는 재면에 1~6개, 횡단면에 1~4개의 빈도가 가장 높았으며, 할열의 크기는 재면에서 1~14 mm, 횡단면에서는 8~14 mm의 빈도가 가장 높았다. 가압처리 결과, 재면에 심 변재가 혼재되어 나타날 때에는 방사방향의 평균침투도는 ACQ에서 $4.3{\pm}4.19mm$, CCA에서 $8.5{\pm}4.98mm$로 침투도에 변이가 심한 것으로 나타났다. 그러나 심재의 경우에는 ACQ에서 1.3 mm, CCA에서 1.1 mm로 매우 작은 침투도를 나타내 두 약제 모두 산림청 고시 사용환경 H3기준에 미달되는 결과를 나타냈다. 심재에 대해 인사이징 처리하고 가압 처리 하였을 때 인사이징 부위의 접선방향 침투도는 ACQ의 경우 약 0.7 mm, CCA는 약 0.6 mm로 두 약제 모두 인사이징의 침투도 개선 효과가 매우 낮은 결과를 보였다. 한편 심재에서 연륜수가 많을수록 약제의 평균 흡수량은 감소 경향을 보였으며 연륜수와 평균 흡수량 간에 상관관계가 인정되었다(r=0.5345). 재면에 심재만이 노출된 경우에는 횡단면과 재면의 할열수 및 할렬크기와 흡수량사이에 상관계수가 각각 r=0.1301과 r=0.1802로 모두 상관관계가 없는 것으로 나타났다.

로보어드바이저 선정요인의 우선순위에 관한 연구: AHP를 이용한 사용자와 제공자의 차이분석 관점으로 (A Study on the Priority of RoboAdvisor Selection Factors: From the Perspective of Analyzing Differences between Users and Providers Using AHP)

  • 자산운용은 수많은 변수와 인간의 심리까지도 통찰해야 하는 복잡하고 어려운 분야이다. 따라서 전통적으로 전문가의 영역이었으며, 이러한 서비스를 받는 데 많은 비용이 들었다. 이러한 시장에 변화가 일어나고 있는데, 이른바 4차산업혁명으로 일컫는 디지털혁명이 그 동인이다. 그 중에서도 인공지능(Artificial Intelligence) 기술을 활용한 로보어드바이저(Robo-Advisor) 서비스는 그 백미이다. 편리한 접근성과 저렴한 비용으로 투자자문 서비스의 대중화가 가능하기 때문이다. 본 연구는 국내에서 서비스 사용자와 제공자의 로보어드바이저 선정 시, 어떤 요인들이 핵심적으로 중요한지, 또한 사용자와 제공자 집단 간의 선정요인에서 어떤 인식차이가 있는지를 밝히고자 하였다. 연구의 틀은 마케팅믹스 4C 모형을 기반으로 하였고, 모형의 설계와 분석은 델파이조사와 AHP를 활용하였다. 연구설계를 통해 4개의 주기준과 15개의 하부기준이 도출되었고, 연구의 발견내용은 다음과 같다. 첫째, 4개 주기준에서의 중요도는 양 집단 공히, 고객니즈> 고객편의> 고객비용> 고객소통 순으로 나타났다. 둘째, 15개의 하부기준들을 살펴보면 투자목적 Coverage, 투자성향 Coverage, 수수료 수준과 접근 편리성 요인이 가장 중요한 것으로 나타났다. 셋째, 집단 간을 비교하여 살펴보면 사용자 집단에서는 수수료 수준과 접근 편리성 요인이 가장 중요하게 나타났고, 제공자 집단에서는 투자목적 Coverage와 투자성향 Coverage 요인을 중요하게 인식하고 있는 것으로 나타났다. 본 연구는 실무적으로 유용한 시사점을 도출하였다. 먼저 로보어드바이저 서비스 확산을 위한 설계 시, 4개의 주기준과 15개 하부기준 가중치 차이에 따른 중요도 우선순위를 감안하여 사용자 지향적인 시스템 구축을 할 수 있는 근거를 마련하였다. 또한 집단 비교에서 나타난 각 하부기준의 우선순위 차이와, 가중치 차이가 큰 하부기준들에 대한 원인 파악, 제공자 집단 내의 전략/마케팅 담당과 시스템개발 담당 간 요인에 대한 인식 차이를 해소하기 위한 공감대 형성 등이 매우 중요함을 제시하였다. 학문적으로는 다수의 로보어드바이저 선정요인들을 도출함으로써 다양한 시각과 관점을 제시한 초기 연구라는 점에서 유의미하다. 본 연구의 발견점들을 통하여 국내에서도 성공적인 사용자 지향적 로보어드바이저 시스템을 구축하고 확산시켜 사용자들에게 도움을 줄 수 있기 기대한다.

GAP 인증인삼 현황과 4P 전략 (Situations of GAP certified ginseng and 4P's strategies)

  • Ginseng in Korea has not only the big production value but also till a good reputation from overseas in the name of 'Korea Ginseng'. Having spread rapidly nationwide from 2000 year, its production keeps on increasing but its consumption becomes lazy and its price is also falling down because of comsumer's concern about mainly overusing pesticide for ginseng. In order to cope with this problem, the government introduced the GAP certification system to ginseng in 2006 to reflect consumer's needs for food safety. This system will be a good opportunity to promote ginseng consumption dramatically. In this aspect it is very important to know how well this system is established and how ginseng farmers build marketing strategies to draw new wind in the market. This study was carried out to look over the GAP certified ginseng system and show its marketing strategies using 4P's(product, place, promotion and price). The main results are as follows. GAP ginseng system currently has some weaknesses such as lack of systematic certification management and after-service, nonrealistic certification fee and poor linkage from production to consumption. In the marketing mix strategies, product strategy suggests that the most desirable appearance be transplanted ginseng filled with branch roots and 4 to 5 year-ginseng, and it is necessary to choose multi-brand strategy divided for present into for self-sufficiency and family brand strategy by use if its brand enlarges to processed products in the future. In the place strategy, 3 stages like 'producer group' - 'GAP certified facility' - 'sales shop' are based as the physical marketing channel according to traceability, and connected with giant retail market and environment friendly stand, and if its sales volume enlarges, it should be considered the GAP ginseng specialized marketplace which is a type of chain store. In the promotion strategy, the promotion of government level is necessary at first and producer alliances require the promotion targeting at the group of women under 40 with differentiation from price, quality, and safety. In the price strategy, the early stage-high price strategy which sets 20~25% higher for self-sufficiency and 30~35% for present is desirable.
