적혈구(赤血球) Protoporphyrin과 철분대사(鐵分代謝)에 관(關)한 연구(硏究) (A Study on Red Cell Protoporphyrin Concentration and Iron Metabolism)

  • The relative state of human iron storage may be ascertained more reliably through determination of the serum iron, iron binding capacity, transferrin saturation and absorption of radioactive iron in conjunction with studies of red cell morphology than from the study of red cell morphology alone. Recent investigations have shown that there is an increase in red cell protoporphyrin concentration in iron deficiency anemia. The significance of the red cell protoporphyrin has been discussed greatly during the years since its discovery. Two of the main factors which appear to influence the amaunt of protoporphyrin are increased erythropoiesis and factors interfering with the utilization of iron in the synthesis of hemoglobin, and iron deficiency. Recently Heller et al. have described a simplified method for blood protoporphyrin assay and this technique could be used assess nutritional iron status, wherein even minor insufficiencies are detectable as increased protoporphyrin concentrations. Based on the evaluation of the relationship between nutritional iron status and red cell protoporphyrin as an index suitable for the detection of the iron deficiency is described in this paper. RESULTS 1. Hemoglobin Concentrations and Anthropometric Measurements. The mean and standard deviations of the various anthropometric measurements of different age and sex groups are shown in table 1. There measurements have been compared with the Korean Standard. In the absence of local standards for arm circumference and skin-fold thickness over triceps, they have been compared with the standard from Jelliffe. Table 2,3, and 4 give anthropometric measurements and frequency (%) of anemia in children surveyed. The mean height of the children studid was 10 to 20 percent; below the Korean Standard. The distribution of height below 80 percent of the Standard was 21.2 percent, however, among anemic group this percentage was 27.7 percent. In general, the mean weight of the children was 10 to 15 percent below the Korean Standard. The percentage of children with weight less than 80 percent of the Standard was about 35 percent. But in the anemic group of the children, this percentage was 44 percent. The mean arm circumference was about 15 percent lower than the Jelliffe's standard. 61.2 percent of the children had values of arm circumference below 80 percent of the standard. Children with low hemoglobin levels, this percentage was 80 percent. The mean skinfold thickness over the triceps of the children studied was about 25 Percent lower than the Jelliffe's standard and 61.2 percent of the children had the value less than 80 percent of the standard. Among anemic children, this percentage was 70.8%. As may be seen from table 5, the mean hemoglobin concentration of the total group was 11.3g/100ml. Hemoglobin concentration was less than 11.0g/100ml. in 65(36.5%) of the 178 children. The degree of anemia in most of these children was mild with a hemoglobin level of less than 8.0g/100ml. found in only one child. In general, the prevalence of anemia was high in female children than male and decreased its frequency with increasing age. Relatively close relationship was observed between hemoglobin level and anthrophometric measurements especially high between arm circumference and skinfold thickness and hemoglobin but very low in height and low in weight and hemoglobin level, estimated by chi-square value. II. Serum iron, Transferrin saturation (1) Serum iron, and transferrin saturation Serum iron, transferrin saturation and red cell protoporphyrin concentrations were estimated in sub-sample of 84 children from 1 to 6 years and 24 older children between 7 and 13 years of age. The findings are presented in table 6. The mean serum iron concentration of the total group was 59ug/100ml. However, the level incrased with age from 36.6ug/100ml. (1-3years) to 80.8ug/100ml. (7-13 years). 60 percent of these children had a serum iron level less than 50ug/10ml. in the 1-3 years age group and 31.4 percent for 4-6 years group. These contrast with the finding of 12.5 percent anemic children in the 7-13 years age group. The mean transferrin saturation for the total group was 18.1 percent and frequency of anemia by transferrin saturation was observed same pattern as serum iron concentration. (2) Red cell protoporphyrin concentrations. (a) Red cell protoporphrin levels of children: Red cell protoporphyrin and other biochemical data are shown in table 4. The mean concentration in red cell of all children was fround 46.3ug/100ml. RBC. and differences with age groups were observed; in the age group 1-3 years, the mean concentration was $59.5{\pm}32.14$ ug/100ml. RBC; 4-6 years $44.1{\pm}22.57$ ug/100ml. RBC. and 7-13 years, $39.0{\pm}13.56$ ug/100ml. RBC. (b) Normal protoporphyrin values in adults: It was observed that in 10 normal adult males studied here the level of protoporphyrin in red cell ranged from 18 to 54 ug/100ml. RBC. and the mean concentration was $47.5{\sim}14.47$ ug/100ml. RBC. Other biochemical determination made on the same subjects are presented in table 8. (c) Red tell protoporphyrin concentration of occupational blood donors: The results of analyses for red cell protoporphyrin as well as serum iron, transferrin saturation and hemoglobin in the 76 blood donors are presented in table 7 and 8. In this experiment, donors were selected at random, however, most of them bled repeatedly because of poor economic situation, I doubt. Table 9 shows the distribution of red cell protoporphyrin concentration and hemoglobin concentration of occupational donors. The mean hemoglobin value for the total was 11.9 g/100 ml. When iron deficiency anemia is defined as a transferrin saturation below 15%, prevalence of anemia was 47.4 percent and the mean serum iron was 27.1ug/100ml. and red cell protoporphyrin, 168.3ug/100ml. RBC. However, mean serum iron and protoporphyrin concentration of above 15% transferrin saturation were 11.6 ug/100 ml. and 58.8 ug/100 ml. RBC. respectively. The mean Protoporphyrin concentration of non-anemic (above 15% transferrin saturation) donors was slightly higher than the results of normal adult males.

정상식과 채식을 하는 여대생의 영양상태에 관한 연구 (The Nutritional Status of Female Collegian on Normal Diet and Lacto-Ovo-Vegetarian)

  • The purpose of this study is to discribe the nutritional status between female collegians on normal diet and lacto-ovo-vegetarian in the child bearing age. Physical examination, hematological parameters(hemoglobin, hematocrit, serum iron, serum albumin)and nutrient intake by dietary survey were carried out on each subject. Serum zinc, iron and copper concentrations were also determined. The results are summarized as follows : 1) Students on normal diet had higher values than lacto-ovo-vegetarian students in the height, weight, and skinfold thickness. 2) Hemoglobin, hematocrit, serum albumin values of the two groups showed no difference. But students on normal diet were higher in the serum irom and copper values than lacto-ovo-vegetarian students. 3) Serum zinc values differed significantly between the vegetarians and non vegetarians (Vegetarians') > non vegetarians') 4) On the nutrient intake of two groups, students on normal diet were higher than lacto-ovo-vegetarian students in the protein but students on normal diet were lower than lacto-ovo-vegetarian students in iron, vitamin A, $vitamin\;B_{2}$, vitamin C and crude fiber. 5) Intakes of calories and protein in the two groups were lower than RDA but Ca, vitamin A, $vitamin\;B_{1}$, $vitamin\;B_{2}$ niacin and vitamin C intakes of the two groups was higher than RDA. 6) Amounts of iron absorbed showed no difference in the two groups. But iron absorption rate was higher in students on normal diet than lacto-ovo-vegetarian students.

  • To study the effect of caffeine intake levels on iron metabolism, the iron utilization, iron contents of serum, liver, spleen, kidney, hemoglobin and hematocrit were compared in rats of different sex fed various levels of caffeine (3.5 and 7.0mg/100g body weight) for three weeks. There were no significant caffeine induced differences in feed intake, body weight gain but feed intake of male rats were significantly lower than that of female rats. Hemoglobin, hematocrit and iron contents of the serum were not significantly different between caffeine free and caffeine groups or male and female rats. Iron contents of kidney were decreased by elevation of injected caffeine levels rather than those of liver and spleen. caffeine male groups showed more increased uine volume, urinary and fecal excretions of iron than caffeine free or caffeine female groups. Apparent digestibility and retention of iron were significantly decreased by increment of injected caffeine levels. Male rate rats were more susceptible to injected caffeine on iron excretion than female rats. Current findings suggest that excessive caffeine consumption can affect iron excretion via urine and feces thereby decrease the utilization of iron, and have more significant effect on male than female rats.

  • Iron bound to heme appears to be more bioavailable than iron salts. A clinical study was performed to investigate the absorption efficiency of heme-iron and iron-salt products available. Heme-iron and nonheme-iron supplements have become available in Korea. We performed iron absorption studies to compare the absorption of heme-iron polypeptide (HIP) products made from digested hemoglobin, produced in Korea (HIPk) and imported from Japan (HIj), with that of iron salts. In the study, 80 subjects were divided into 5 groups (n=56): placebo group; 12 mg glucose, HIPk group; 12 mg iron as HIPk, HIj group; 12 mg iron as HIj, iron-salt group 1; 12 mg of iron as ferrous aminoacetate, and iron-salt group 2; 100 mg iron as ferrous aminoacetate. Changes in serum iron levels were measured at 3 and 5 hours post ingestion. Absorption of iron in HIPk was higher compared to HIj, iron-salt or placebo. There was a significant inverse correlation between low serum iron levels $(<80{\mu}g/dl)$ and iron absorption from HIPk. These results demonstrated that HIPk was more bioavailable, even taken with a meal, and would have potential advantages over iron salt or HIj as an iron supplement. Our results indicate that heme-iron absorption is regulated by iron status through a heme receptor, whereas iron-salt absorption is unregulated.

Nutirtional Status of Iron, Zinc and Copper of Preschool Children Residing in Low-Income Area of Seoul

  • This study was performed to investigate the biochemical status of iron, zinc and copper for 125 preschool children (Males : 69, Females : 56) residing in a low-income area of Seoul. The number of subjects aged, 3, 4, 5 and 6 were 19, 41, 41 and 24, respectively. The hemoglobin level of the children aged 3 was 11.8 g/dl and was lower thant hat of the other groups(p<0.05). Similar results were found for hematocrit and serum zinc. The percentage of children with an iron deficiency assessed by Hb(3-5) years : <11.0g/dl, 6 years: <11.5g/dl), Hct(3-6 years : 33%, 6 years : <35%), serum transferrin(<16%) and serum ferritin(<10ng/ml) were 4.3%, 9.5%, 8.2% and 17.7%, respectively. The mean serum zinc was 67.9$\mu\textrm{g}$/dl and urinary zinc was 0.1300mg/day. Low serum zinc (61.0$\mu\textrm{g}$/dl) occurred in 28.0% of the children. The mean serum copper was 110.5$\mu\textrm{g}$/dl and urinary copper was 0.0126mg/day. The prevalence of children with elevated serum copper($\geq$ 130$\mu\textrm{g}$/dl) was 54.8%, which was higher than 7.4%, the prevalence of low serum copper(<70$\mu\textrm{g}$/dl). Children with higher status, more weight, larger girth of chest, or larger midarm circumference showed higher values of Hb. The height and weight of children also showed a positive crrelation with serum zinc(P<0.001-P<0.05).

Effect of Vitamin C Supplementation on Iron Status of Adolescent Girls with Low Hemoglobin Levels

  • This study investigated the effects of vitamin C supplementation on the nutritional iron status of 31 adolescent girls, aged 12-15 years, with low hemoglobin levels. They were randomly divided into four groups, and for two groups daily150mg or 900mg of L-ascorbic acid(AsA) was given in three equal doses at three meals during 9 weeks. To another group daily 60mg iron as ferrous sulfate was given in the same way as AsA. The control group was given sugar placebo. Body iron status was monitored through the determination of Hb, Hct, MCHC, and serum ferritin concentrations. Dietary AsA and iron intakes were measured from food consumption surveys performed by 3-day 24-hour recalls. The amount of absorbed iron was estimated from the model of Monsen et al. The average amounts of food iron for four groups were 12.3- 15.0mg and 11.1 - 18.9mg at initial and at final period of the supplementation trial, respectively. The tentatively estimated amount of absorbed iron was significantly increased in the 900mg AsA and iron supplementing groups, but not in the 150mg AsA and placebo groups. Both Hb and MCHC were improved to above normal levels in all groups except the placebo group. Hct was elevated only in the AsA 900mg group whose Hct was relatively lower than the other groups. Serum ferritin concentrations of the four groups, which were as low as 8.50 - 14.39ng/mL on average at the intial periods, augmented significantly to 20.18ng/mL and 26.63ng/mL in the 900mg AsA and iron groups, respectively. Serum ferritin was not elevated in either the AsA 300mg group or the placebo group. The above data indicated that the daily supplementaion of 150mg AsA to the meals containing 12-15mg iron per day promoted Hb levels of adolescent girls with low Hb, and the 900mg AsA supplementing improved not only Hb level but also body iron store. A supplementation of 60mg iron per day appeared to be slightly more effective in improving the iron status in comparison to the 900mg AsA supplement. 어린이집을 이용하는 미취학 아동의 생화학적 분석에 의한 철분영양상태에 관한 연구 (A Study on the Iron Nutritional Status with Biochemical Parameters in Preschool Children)

  • The purpose of this study was to determine the iron status of preschool children in Ulsan, Korea. The study was con-ducted using 95 children aged 3 to 6 years by investigating the anthropometric indices and assessing the dietary intakes and biochemical analysis. A questionnaire for dietary intakes using 24-hr recall method were carried out by the mothers of the 95 subjects. And also a study was conducted to assess hematological and biochemical status of iron and prevalence of iron deficiency. The average height and weight were 105.4 $\pm$ 7.0 cm, 18.7 $\pm$ 3.2 kg in boys, 103.8 $\pm$ 6.7 cm, 17.6 $\pm$ 2.7 kg in girls. These were lower than the body growth standard values of Korean pediatrics. There was not a significant difference in both between boys and girls. For the daily nutrient intake, energy was 1201.5 $\pm$ 280.9 kcal (79.3$\%$RDA), protein was 63.8 $\pm$ 28.2 g (219.4$\%$RDA), iron was 11.9 $\pm$ 4.5 mg (133.2$\%$RDA). The iron nutritional status by hematological assay found that Hct was 34.7 $\pm$ 2.0 ($\%$), Hb was 12.0 $\pm$ 0.8 g/dl, RBC was 4.3 $\pm$ 0.3 ($10^6/{\mu}l$), MCV was 80.5 $\pm$ 3.0 fL, MCH was 27.8 $\pm$ 1.1 pg and MCHC was 34.6 $\pm$ 0.6 g/dl. The biochemical measurement of serum iron was 75.6 $\pm$ 30.3 ${\mu}g$/dl, TIBC was 320.3 $\pm$ 34.1 ${\mu}g$/dl, serum ferritin was 30.0 $\pm$ 14.8 ${\mu}g$/dl, Zinc-Protophor-phyrin (ZPP) was 32.7 $\pm$ 8.0 ${\mu}g$/dl, and ZPP/Heme was 71.1 $\pm$ 19.5 ($\mu$mole/mol heme). The prevalence with Hct, Hb, TS and serum ferritin less than cut-off value was $8.4\%,\;9.5\%,\;12.6\%$ and $4.4\%$ respectively. But the prevalence of iron deficiency estimated with ZPP and ZPP/Heme criteria were $25.3\%$ and $27.4\%$, and were higher than in case of any other indices. The prevalence of iron deficiency anemia ((low Hb (< 11.0 g/dl) and low serum ferritin (< 10 ${\mu}g$/L) or low TS (3-4 yews: < $12\%$, 5-6 years: < $14\%$)) was found in only one 3 year old girl. The prevalence of iron deficiency except Hct and Hb was the highest in 3 year group, but the prevalence by Hct and Hb was the highest in 5 year group. Iron deficiency and iron deficiency anemia do not seem to be a major public health problem in preschool children in Ulsan.

정신과 입원환자에서 항정신병 약물에 의한 급성 근긴장 이상증과 혈청 철 농도와의 관계 (The Relation of Antipsychotic Drug Induced-Acute Dystonia and Serum Iron Level)

  • 순천향대학 천안병원 신경정신과에 입원하여 항정신병 약물로 치료 받고 있는 78명의 환자 중 약물로 인한 급성 근긴장 이상증이 나타난 41명의 환자들을 대상군으로 하고 급성 근긴장 이상증이 나타나지 않은 37명의 환자를 대조군으로 하여 약물 투여전과 투여후 혈청 철의 농도를 측정하여 급성 근긴장 이상증과 혈청 철 농도와의 상관관계를 연구하여 다음과 같은 결과를 얻었다. 대상군에서 항정신병 약물을 투여하기 전 혈청 철의 농도는 $104.8{\pm}30.8{\mu}g/dl$, 항정신병 약물에 의한 급성 근긴장 이상증이 발생한 후 혈청 철의 농도는 $87.2{\pm}21.2{\mu}g/dl$이었으며 양 군간에 통게적으로 유의한 차이가 있었다. 대조군의 경우 항정신병 약물을 투여하기 전 혈청 철의 농도는 $92.5{\pm}34.5{\mu}g/dl$, 항정신병약물을 투여한 후 혈청 철의 농도는 $88.3{\pm}35.3{\mu}g/dl$이었으며 양군간에 통계적으로 유의한 차이가 없었다. 이상의 결과는 항정신병 약물을 투여하면 일부의 환자에서는 혈청 철의 농도를 감소시키고, 결국 정상적으로 철을 포함하고 있는 도파민 $D_2$ 수용체의 기능이 저하되어 급성 근긴장 이상증의 발생에 취약하게 된다는 것을 뒷받침 해 주는 소견이다.

Dietary Iron Intake and Body Iron Status of Myocardial Infarction Patients in Chunan Area

  • It has been known for some time that elevated body iron could be a risk factor for coronary heart disease. The present study was conducted to determine body iron status and dietary iron intake of patients with myocardial infarction(MI). Seventy five patients from the Chunam area with their first MI history within he past 2 months were recruited. The serum iron concentration, total iron binding capacity(TIBC) and percent transferrin saturation(TS) were selected as indicators of body iron status. Twenty four hour recall was conducted by trained interviewers to asses the dietary intake. Most women (91.3%) showed waist to hip ratio(W/H) greater than 0.85 while 17.3% of men were assessed to have a tendency of abdominal obesity(W/H>0.95). The average BMI of women was 25.80 and that of men was 23.98. The average diet intake of participants was below the recommended dietary allowances (RDA) for most nutrients. He average dietary iron intake was 10.03 mg/day for all subjects while women's iron intake was significantly lower than men's. However, a great proportion of participants (77%) showed a tendency to have normal iron status. About 9% of the participants were assessed as iron deficient and 14% had an iron overload. The mean serum iron concentration was 125 g/dl ranging from 13.3 to 280.6 g/dl. Iron intake from animal sources were significantly associated with body iron status (r=0.257, p=0.026) when TIBC was used as an iron status indicator. When iron status was assessed with TS, it was directly associated with iron intake from animal sources(r=0.278, p=0.05) for he subjects in the normal iron status group. He results of the present study showed that the nutrient intake of Mi patients in Chunan was not quite adequate while iron status was mostly in the normal range. Further studies are needed to investigated whether there is a possible difference in iron metabolism of the MI patients.

임신기간 중 철 섭취가 모체와 제대 혈청의 미량 무기질 농도에 미치는 영향 (Iron Intake During Pregnancy on Serum Concentrations of Trace Minerals in Mothers and Neonates)

  • It is a common clinical practice to recommend taking iron supplements for pregnant women during gestation. Although it is required to ensure adequate iron stores during pregnancy, there has been some debate over the interference effects of excessive iron load, because it is possible to compete in the transport in the intestine and placenta and in binding to serum proteins of other trace minerals. In this study, maternal and neonatal serum concentrations of Fe, Zn, Cu, Se, Cr, Mn, and Co were assessed along with maternal Fe intakes. A total of 124 pregnant women and their term neonates participated voluntarily in this research. The women were divided into one of the three groups {high Fe intake (HFI), median Fe intake (MFI), and low Fe intake (LFI)} by their total Fe intakes and one of the two groups (Anemic and Normal) by their Fe nutritional status. All the data were compared among the three groups and between the two groups also. Total Fe intakes of HFI, MFI, and LFI groups were 140.8 ${\pm}$ 76.1, 68.0 ${\pm}$ 11.2, and 30.2 ${\pm}$ 8.6 mg/day, respectively. Those of Anemic and Normal groups were 90.1 ${\pm}$ 74.8 and 86.6 ${\pm}$ 46.8 mg/day, respectively. Maternal Hb concentration and Hct were not significantly different among HFI, MFI, and LFI groups but those were significantly different between Anemic and Normal groups. However, neonatal Hb concentration was not significantly different among HFI, MFI, and LFI groups and between Anemic and Normal groups either. Maternal serum Fe concentrations of the three groups, HFI, MFI, and LFI, were similar but that of Anemic group was significantly lower compared to Normal group. However, there was no significant difference in neonatal serum Fe concentrations among the three groups and between the two groups either. Serum concentrations of the other trace minerals in both mothers and neonates were not significantly different among HFI, MFI, and LFI groups and between Anemic and Normal groups. In addition, in the maternal serum, Fe concentration was positively correlated to Zn and Se concentration, respectively. As for the neonatal serum, Fe concentration showed a positive correlation to Zn, Cu, Mn, Se, and Co concentration, respectively. No trace mineral concentration was found to correlate negatively to Fe concentration in both maternal and neonatal serum, The results in this study indicate that Fe intakes of pregnant women, even if it is considerably above the level of estimated average requirement (EAR), may not affect serum Fe concentration in both mothers and neonates. In addition it might not influence adversely on the availability of other trace minerals including Zn and Cu in both mothers and neonates.
