Junctional rhythm with severe hypotension following infiltration of lidocaine containing epinephrine during dental surgery

  • We experienced an unusual case of accelerated junctional rhythm with severe hypotension after infiltration of lidocaine containing epinephrine during dental surgery under general anesthesia. The patient's electrocardiogram exhibited retrograde P-waves following the QRS complex, which could be misinterpreted as ST-segment depression. As a temporary measure, administration of ephedrine restored the patient's blood pressure to normal levels. The importance of this case lies in its demonstration of an unexpected and serious side effect of commonly used epinephrine infiltration. This case also highlights the need for accurate interpretation of the electrocardiogram and comprehensive understanding of best practices for patient management.

  • Calcaneal or calcaneovalgus deformity can occur after surgical treatment of equinus or eguinovarus deformity in cerebral palsy patient. It is a serious complication and the results of many conservative and operative treatments are reported unsatisfactory. We experienced one case of both calcaneovalgus deformity following operative treatment of diplegic equinovarus deformity in a spastic cerebral palsy patient and report about the result of the treatment.

Emergency Quadrido-Bentall Procedure for Aortic Rupture in a Patient with Behcet's Disease

  • Cardiovascular involvement in cases of Behcet's disease is a rare but life-threatening condition, and prosthetic valve detachment is a frequent and serious complication attributable to Behcet's disease following the surgical repair of aortic regurgitation. We report the case of a patient with Behcet's disease presenting with contained aortic rupture around the aortic root. The patient had previously undergone aortic valve surgery three times due to recurrent prosthetic valve detachment. An emergency operation was performed, consisting of aortic root replacement (ARR) using a composite valved conduit and the replacement of the hemiarch. ARR may be an appropriate surgical option for patients with Behcet's disease in order to prevent recurrence of the disease.

  • As laparoscopic surgery becomes more popular, various complications following the laparoscope are also increasing. Nerve injury following the laparoscope is an infrequent but serious complication for both the doctor and patient. A 30-year old female patient suffered severe burning pain of the left buttock, inguinal area, external genitalia and inner side of vagina following laparoscopic surgery for ovarian mass. We successfully treated this patient with ilioinguinal, iliohypogastric nerve block in combination with epidural blocks.

장애인 치과 치료시 발생한 이물질 흡인 (Foreign Body Aspiration during Dental Treatment in the Disabled Patient)

  • There are a lot of dental emergency situation. Especially on disabled or pediatric patient with negative behavior, accidental aspiration or ingestion of foreign body (ex: dental instrument) is one of the common accidents in dental procedure. Dentists and dental hygienists must have knowledge about the precaution and be ready to deal with foreign body aspiration during dental treatment, especially on the disabled. This is a case of an accident during the dental treatment of 14-year-old female patient with cerebral palsy. During scaling, prolonged retained primary tooth which had resorbed roots was fell into left bronchi. So we tried to remove the crown by endotracheal approach. Most ingested foreign bodies pass through the gastrointestinal tract spontaneously, but some of them need endoscopic or surgical removal. Moreover aspiration into broncho-trachea can be more serious events and must be treated as emergency situation.

  • 42 dentally handicapped patients who require general dental treatments but behavioral managements had been an obstacle to proper treatments were chosen and the author conducted dental treatment under outpatient general anesthesia. Followings are the conclusions 1. The average time for the treatment procedure was 2 hours and 1 minute and the average of 9.52 teeth were treated for individual patient indicating its high efficiency. 2. The average time for the recovery was 3 hours and 14 minutes and none of the patients showed any specific serious complications. 3. Treatments under outpatient general anesthesia can be a proper patient management method for the dentally handicapped patients if the patient selection is proper.

기관에 생긴 선양낭포암 치험 1례 (A Case Report of Adenoid Cystic Carcinoma in Trachea)

  • Adenoid cystic carcinoma of the tracea is rare, but is a very serious critical life-threatening disease.Nearly all the lesions of the trachea are presented as obstructive lesions. Bronchoscopic examination including chest CT, tomogram and air tracheogram are essential for the further definition of these lesions. This is a case report of adenoid cystic carcinoma in a 47 years old male patient. The tumor was located in cervical trachea with wide base and obstructing the lumen almost completely. The patient`s symptom was productive cough and hoarseness for 4 months. The tumor was resected and End-to-End anastomosed. The tumor was confirmed to be adenoid cystic carcinoma histopathologically. The need for removal of tracheal tumor whether complete or incomplete, is clear enough regardless of the histology of the tumor. This patient was treated post-operatively with radiation.

소음인(少陰人) 오치(誤治)로 병발(倂發)한 태음인(太陰人) 환자(患者)의 양독발반(陽毒發斑)에 대한 증례보고(症例報告) (A case of Yangdokbalban(陽毒發斑) of Taeumin patient mistreated with Soumin)

  • The Yangdokbalban(陽毒發斑) of Taeumin(太陰人) is classed as Gansuyoilliyoilbyung(肝受熱裏熱病). This is a kind of dermatitis caused by Ganyoilpaejo(肝熱肺燥) what was risen from disorder of Simyok(心慾). In the treatment, we should temper the Simyok and recover Hosanjigy(呼散之氣) of Lung by Cheongganjoyoil(淸肝燥熱). In this study, it is experienced that Yangdokbalban of Taeumin patient mistreated with Soyangin, got sick by Korean medical treatment, and also it came to improve the psychosomatic disorder(心身症) generally. The patient was 52 years old, female. She entered our hospital for Yangdokbalban, including serious itching, insomnia, chest discomfort, dyspepsia, pantalgia, and she had neutropenia in the blood test. We classified this as Taeumin-liyoiljung(太陰人 裏熱證) and prescribed Galgunhaegitang(葛根解肌湯) and Chungsimyoinjahabopaewontang(淸心蓮子合補肺元湯) in according to the principle of Cheongganjoyoil. In the result, we had the general improvement of Yangdokbalban and psychosomatic disorder.

Laparoscopic Gastrectomy and Transvaginal Specimen Extraction in a Morbidly Obese Patient with Gastric Cancer

  • Laparoscopic gastrectomy for cancer has some significant postoperative benefits over open surgery with similar oncologic outcomes. This procedure is more popular in the Far East countries where obesity is not a serious public health problem. In the Western countries, laparoscopic gastrectomy for cancer is not a common procedure, yet obesity is more common. Herein, we aimed to demonstrate the feasibility of laparoscopic gastrectomy for advanced gastric cancer in a morbidly obese patient. Additionally, we used natural orifice specimen extraction as an option to decrease wound-related complications, which are more prevalent in morbidly obese patients. In this case, we performed a fully laparoscopic subtotal gastrectomy with lymph node dissection and Roux-en-Y gastrojejunostomy with the specimen extracted through the vagina. To the best of our knowledge, this was the first report of a natural orifice surgery in a morbidly obese patient with gastric cancer.

Aortic Root Reimplantation in a Patient Who Underwent an Arterial Switch Operation

  • Neo-aortic insufficiency associated with root enlargement following an arterial switch operation is a serious late complication. To achieve successful surgical correction of this condition, multiple factors should be considered, including the individual patient's anatomy, the challenging nature of the redo procedure, and the patient's young age. However, limited publications have described the use of valve-sparing techniques for the treatment of neo-aortic insufficiency associated with root enlargement following an arterial switch operation. Herein, we report our recent experience of a valve-sparing aortic root procedure with ascending aorta and hemiarch replacement despite the presence of a discrepancy in leaflet size and nearby severe adhesions.
