An Evaluation of the Quantitative Risk of Plastic Process Manufacturing Industries by Means of the 4M Method

  • This study includes a case study among plastic process manufacturing companies, based on which, the currently used 4M method is applied in terms of machine, media, man, and management, to conduct quantitative risk evaluation, and thus to contribute to reducing human and material loss as well as preventing accidents in industrial fields. The result of this study is analyzed based on the 4M-risk assessment to find out the hazardous risk elements, and the quantitative evaluation made it predictable the value of risk(frequency $\times$ intensity) in such classified levels as serious risk, critical risk, and intolerable risk. Further, Among the businesses with hazardous risk elements and high frequency of industrial disaster, risk analysis was conducted for each process, and as a result, 38 cases among 76, including those of serious risk, critical risk, and intolerable risk, were improved, and the risk was reduced. Besides, it is thought that with the engineering approach with 4M-Risk Assessment, the attempt to improve safety level contributes to prevention of accidents.

Comparison Analysis on the Structural Stability of Concrete Median Barriers Using Commercial Software (상용 프로그림을 이용한 콘크리트 중앙분리대 방호울타리들의 구조적 안정성에 대한 비교 분석)

  • The concrete median barriers are the most popular safety appurtenances that can be installed on narrow medians and are effective in keeping uncontrolled vehicles from crossing into opposing lanes of traffic. It is necessary to install and maintain median barriers because it is very difficult to reserve enough room required for medians in KOREA. And concrete median barriers are accepted as the actual alternatives for median barriers, mostly because they require almost no maintenance even after serious collisions. Typical concrete median barriers are 810mm high and have 596mm high glare screens on top of them. But we have experienced a number of “climb” and “roll-over” accidents of heavy vehicles until now, and most of all, there have been some serious accidents caused by the part of broken glare screens. We have performed many computer simulations for the evaluation of the crashworthiness of concrete median barriers, and through the simulations we have tried to find a proper type of concrete median barrier.

  • 중대재해처벌법(이하 "동법" 병기) 시행 전후로 동법에 대응하기 위한 정부 지원과 학계 연구는 많이 진행되어 왔으나, 대부분은 사전 예방에 치중되어 재해 발생 이후 기업 운영상의 리스크 관리나 피해경감 등을 위한 연구가 부족하였다. 중대재해를 줄이기 위해서는 사전 예방이 우선이지만 재발방지 및 추가 피해 최소화 노력도 뒤따라야 한다. 따라서, 본 논문에서는 재난 및 업무중단성 사고에 대하여 예방, 대비, 대응 및 복구활동에 필요한 문서화된 관리체계를 규정하고 있는 재해경감활동관리체계(이하 "동 체계" 병기)를 활용하여 중대재해처벌법을 준수할 수 있는 방안을 연구하였다.

Design of ICT-based Agricultural Safety Monitoring System Models

  • Objective: This study carried out base research to build an agricultural safety monitoring system through ICT convergence to reduce safety accidents and enhance welfare in life in the agricultural field. Background: The functions and values of rural villages as the space of living are recognized anew, but occupational accident rate due to farm work accidents is on the rise each year. Therefore, the seriousness of such a problem emerges. The convergence technology combining ICT is recently applied to industries overall, and therefore better services are offered. However, studies on ICT convergence has not yet been applied to the agricultural safety field. Method: This study identified ICT convergence service technology trends and representative serious accident types mainly occurring in agricultural activities. This study defined the major factors of farm work accidents and ICT to solve those accident factors including the sensor technology, wired/wireless communication technology and location information service, and applied them to prototype PCB for the development of an agricultural safety monitoring system. Results: This study proposed an emergency monitoring system for farmers and a harmful environment monitoring system. The ICT technology to prevent farm work accidents can be summarized as sensing technology, ICT and network technology and user interface technology. This study also designed PCB module configuration and situation judgment algorithm as basic research for proposed monitoring system development. Conclusion: The ICT-based agricultural safety monitoring research proposed in this study is expected to become the basis to build a future real time monitoring system, and also is expected to contribute to social safety and welfare service improvement for farmers. Application: The ICT convergence farmer accident prevention system will make contributions to the prevention of serious farm work accidents.

The Criminal Liability of Physicians in the Case of Medical Accidents (의료사고에서의 형사책임 -원내감염사고의 해결을 향하여-)

  • Conventionally, there were few cases in which a medical accident became a criminal case in Japan. However, after the serious malpractice such as the Yokohama City University Hospital case, prosecutors came to be less hesitant to prosecute a malpractice. In a medical accident, the attribution of the responsibilities among medical personnel becomes one important element of the proof of negligence. The Supreme Court concluded that, when the attribution of the responsibilities is not established among medical personnel to confirm the identity of the patient like the Yokohama City University Hospital case, all the personnel who were involved bear the responsibilities to identify the patient. For serious cases which requireut most carefulness fortreatment such as the Saitama Medical University Hospital case, not only the chief physician in charge of the case concerned but also the director of the branch at the university hospital bear the responsibilities to confirm the treatment policy of the case. After the acquittal of the Ohno Hospital case, the voice demanding more prudent prosecution of malpractices has become stronger than before. Meanwhile, Ministry of Health, Labour and Welfare introduced Medical Accidents Investigation System for the prevention of medical accident and, has reinforced the third party inspection of medical accidents.

Comparative Analysis of Traffic Accident Severity of Two-Wheeled Vehicles Using XGBoost (XGBoost를 활용한 이륜자동차 교통사고 심각도 비교분석)

  • Emergence of the COVID 19 pandemic has resulted in a sharp increase in the number of two-wheeler vehicular traffic accidents, prompting the introduction of numerous efforts for their prevention. This study applied XGBoost to determine the factors that affect severity of two-wheeled vehicular traffic accidents, by examining data collected over the past 10 years and analyzing the influence of each factor. Among the total factors assessed, variables affecting the severity of traffic accidents were overwhelmingly high in cases of signal violations, followed by the age group of drivers (60s or older), factors pertaining only to the car, and cases of centerline infringement. Based on the research results, a reasonable legal reform plan was proposed to prevent serious traffic accidents and strengthen safety management of two-wheeled vehicles.   • As fatal accidents due to industrial accidents and deaths due to civil accidents were pointed out as social problems, the Act on Punishment of Serious Accidents Occurred in the Workplace was enacted to ensure the safety of citizens and to prevent serious accidents in advance. Effort is required. In this paper, we propose a distance prediction model in relation to the case where an operator is hit by heavy equipment such as a forklift. For the data, actual forklift trucks and workers roaming environments were directly captured by CCTV, and it was conducted based on the Euclidean distance. It is thought that it will be possible to learn YOLO-v4 by directly building a data-set at the industrial site, and then implement a model that predicts the distance and determines whether it is a dangerous situation, which can be used as basic data for a comprehensive risk situation judgment model.

Determination of Critical Hazard Factors in Vehicle-Mounted MEWP using Fuzzy-FMEA (퍼지-FMEA기법을 이용한 차량탑재형 고소작업대 사고의 주요 유해위험요소 위험우선순위 결정)

  • In this study, we aimed to identify the important hazard factors and determine their criticality in causing serious accidents in vehicle-mounted mobile elevated work platforms (MEWPs). Fuzzy failure modes and effects analysis (FMEA)was performed using accident data and a survey of experts. To determine the hazard factors, the accident data for the last 10 years were used and a questionnaire survey was designed. The questionnaire survey was sent to four experts in the field of occupational safety to determine the severity, occurrence, and detectability of serious accidents in MEWPs. Furthermore, objective RPN scores and risk priority were obtained using fuzzy FMEA. Finally, the criticality of hazard factors in descending order was found to be overloading, non-installation or defective installation of outriggers, breakage due to wire rope aging, and illegal remodeling of vehicle structures. The results were verified by comparing the occurrence data of serious disasters.

Study on the Application of the Serious Accidents Punishment Act in Construction Sites and Practical Improvement Measures for Legal Deficiencies

  • This study analyzed the practical issues in the Serious Accident Punishment Act (SAPA) for construction sites and proposed measures to improve provisions that are detached from reality. The Act aims to prevent major accidents across various industries, with a particular emphasis on the construction sector. While the Act's intention is commendable, several significant legal flaws have emerged during its implementation. These issues were critically analyzed and strategic recommendations were proposed. Key issues include ambiguous legal definitions, the differential impact of the law on small and large businesses, and the increased administrative and managerial burdens. This study proposed specific measures to enhance the clarity of legal terminology and technology, balance the responsibilities between small and large businesses, and streamline administrative processes. Additionally, methods for effectively revising the regulations and reasonably complying with them on-site were proposed. By addressing these issues, we aim to improve the effectiveness of the law, ensure fair application, and ultimately create a safer working environment in the construction industry. The recommendations are expected to serve as valuable resources for future revisions and improvements to SAPA and contribute to the broader goal of sustainable industrial safety and development.

A Study on the Link between the Severe Accident Punishment Act and ISO 45001 of SMEs (중소기업의 「중대재해처벌법」과 ISO 45001의 연계방안 연구)

  • Purpose: This study proposes a plan for linking the Severe Accident Punishment Act and the Occupational Health and Safety Management System so that SMEs can meet the requirements of the Severe Accident Punishment Act only by maintaining and implementing the Occupational Health and Safety Management System. Method: Conduct a theoretical approach, such as a literature review. First, as a theoretical approach, the safety and health management system of the Serious Accidents Punishment Act was examined, the requirements of ISO 45001 were analyzed, and the statistics of small and medium-sized enterprises (SMEs) from 5 to 49 to which the 2024 Serious Accidents Punishment Act was applied and the number of accident deaths were investigated. Result: No problems were found in linking with the Serious Accident Punishment Act when constructing Occupational health and safety management systems (ISO 45001). Conclusion: It is judged that the establishment of Occupational health and safety management systems (ISO 45001) and management of implementation performance will be able to satisfy the Act on the Punishment of Serious Disasters.
