An Observation on the Incidence of Drowning Death in Korea (익사(溺死) ($WHO-E_{929},\;E_{934}$)의 역학적(疫學的) 관찰(觀察))

  • This paper describes the incidence and some epidemiological features of drowning accident out of a series of our study on the epidemiology of various accidents in Korea. By the daily press it is apparent from the frequent reporting of swimming accidents that the incidence would be high. In the rural areas, there are, at present, about 1,250 artificial lakes and farm ponds to be utlized for rice production. The reservoirs, irrigation ditches and riversides are also used for recreation. In most places facilities for aquatic activities is meager, and safety measures for the prevention of drowning is not sufficiently enforced. In the survey crude data on drownings were collected from the concerned governmental statistic books for the period 1955 to 1967 which were compiled not in a uniform way. Drownings were classfied into two categories, one is accidental drowning, E 929 and the other is due to cataclysm, E 934, according to the WHO international classification of diseases. Epidemiological variables in relation to drowning accident were obtained through qualitative analysis of informations from the popular news papers. The following summary may be drawn; 1. The average number of deaths due to accidental drowning totaled 1,088 annually and the mortality rate per 100,000 population was 3.4, The 42.0% of all drownings were rescued and the remaining were not saved. 2. The sex ratio (M/F) of the victims for all ages was 5 to 1, which had a wide range of difference among the age groups. The young ages less than 20 years occupied 68.0% of all deaths. 3. The percentage distribution of the causes of accidents revealed 31.9% for careless swimming, 45.5% for unskilled, 10.6% for swimmer's cramp and 6.0% for drunked. The distribution of places where accidents occurred showed 88.0% for rivers, water reservoirs, irrigation ditches and 12.0% for regular swimming pools. The seasonal distribution of cases indicated 85.0% of the total were seen during the summer months, June-August, and 50.0% of them occurred on Sundays, 4. The average annual deaths due to cataclysm were 402 and mortality rate per 100,000 population was 1.6, but the number of victims due to cataclysm varied greatly each year. 5. The accident cases due to cataclysm were classified into 60.0% for injuries, 40.0% for deaths. The 26.8% of all deaths were missing cases. 6. The deaths due to either accidental drowning or cataclysm totaled 1,490, and the death rate per 100,000 for the whole country was 5.0.

Nasal Anthropometry on Facial Computed Tomography Scans for Rhinoplasty in Koreans

  • Background Cephalometric analysis is essential for planning treatment in maxillofacial and aesthetic facial surgery. Although photometric analysis of the Korean nose has been attempted in the past, anthropometry of the deeper nasal structures in the same population based on computerized tomography (CT) has not been published. We therefore measured three anthropometric parameters of the nose on CT scans in our clinical series of patients. Methods We conducted the current retrospective study of a total of 100 patients (n=100) who underwent a CT-guided radiological measurement at our institution during a period ranging from January of 2008 to August of 2010. In these patients, we took three anthropometric measurements: the nasofrontal angle, the pyramidal angle, and the linear distance between the nasion and the tip of the nasal bone. Results The mean nasofrontal angle was $131.14^{\circ}$ in the male patients and $140.70^{\circ}$ in the female patients. The mean linear distance between the nasion and the tip of the nasal bone was 21.28 mm and 18.02 mm, respectively. The mean nasal pyramidal angle was $112.89^{\circ}$ and $103.25^{\circ}$ at the level of the nasal root, $117.49^{\circ}$ and $115.60^{\circ}$ at the middle level of the nasal bone, and $127.99^{\circ}$ and $125.04^{\circ}$ at the level of the tip of the nasal bone, respectively. Conclusions In conclusion, our data will be helpful in the preparation of silicone implants for augmentation and/or corrective rhinoplasty in ethnic Korean people.

Effect of Storage Condition of the Refined Palm Oil on its Heat Bleachability (탈산 팜유의 저장조건이 그의 고온 탈색도에 미치는 영향)

  • A series of tests ware conducted to find out whether continuous heat bleaching of the refined Malaysian plam oil stored in different conditions could reduce color of the finished oil in an actual plant situation. When the refined oil was stored in a stainless steel tank and was not abused by heat during 5 month storage period, heat bleaching followed by clay bleaching and deodorization resulted in a substantial reduction in color of the finished oil in comparison to conventional process (clay bleaching of the refined oil followed by deodorization) (2.6 vs 1.3 red in Lovibond color). However, when the refined oil was stored in a carbon steel tank and was highly abused by heat in the presence of iron picked up from the tank (6.53 ppm) during the same storage period, heat bleaching followed by clay bleaching and deodorization did not help reduce color of the finished oil in comparison to the conventional process (2.7 vs 2.8 red in Lovibond color). It was also shown that oxidation values were not good indices for heat bleachability. Heat bleaching caused slight increase in polymer content of the oil. However, trans isomers were not increased when the oil was heat bleached.

Chemical Effects on Head Loss across Containment Sump Strainer under Post-LOCA Environment (LOCA이후 환경에서 원자로건물집수조 여과기의 수두손실에 대한 화학적 영향)

  • A test apparatus has been fabricated to simulate chemical effect on head loss through a strainer in a pressurized water reactor (PWR) containment water pool after a loss of coolant accident (LOCA). Tests were conducted under condition of same ratio of strainer surface area to water volume between the test appratus and the containment sump. A series of tests have been performed to investigate the effects of spray, existence of calcium-silicate with tri-sodium phosphate (TSP), and composition of materials. The results showed that head loss across the chemical bed with even a small amount of calcium-silicate insulation instantaneously increased as soon as TSP was added to the test solution. Also, the head loss across the test screen is strongly affected by spray duration and is increased rapidly at the early stage, because of high dissolution and precipitation of aluminum and zinc. After passivation of aluminum and zinc by corrosion, the head loss increase is much slowed down and is mainly induced by materials such as calcium, silicon, and magnesium leached from NUKONTM and concrete. Furthermore, it is newly found that the spay buffer agent, tri-sodium phosphate, to form protective coating on the aluminum surface and reduce aluminum leaching is not effective for a large amount of aluminum and a long spray.

Changes in Taste Compound of Low Salt Fermented Pollack Tripe during Controlled Freezing Point Aging (저식염 창란젓의 저온숙성중 정미성분의 변화)

  • For the effective utilization of by-products from pollack, Theragra chalcogramma, processing, low salt fermented pollack tripe were produced and changes in taste compounds of low salt fermented pollack tripe during the controlled freezing point aging were investigated. No significant changes in contents of moisture, crude protein, crude lipid and ash were found during aging. The volatile base nitrogen of low salt fermented pollack tripe increased steadily as aging progressed. Contents of amino nitrogen and nitrogenous compounds of extract reached a peak in 50 days of aging, as 190.7 mg% and 2085.8 mg%, respectively. Inosine(HxR) of low salt fermented pollack tripe increased as controlled freezing point aging progressed for 60 days. 18 species of amino acid were detected in raw pollack tripe and law salt fermented pollack tripe aged for 50 days. The major amino acids were tyrosine, phenylalanine, histidine, arginine, lycine, leucine, aspartic acid and glutamic acid in raw pollack tripe. After 50 days aging, increase of a series of bitter taste amino acid such as methionine, valine and isoleucine were higher than other amino acids.

The Effect of Gamisungmagalguntang(加味升麻葛根湯) on Chronic Urticaria Identified as "Wind Heat" Pattern (풍열형(風熱型) 만성 두드러기에 대한 가미승마갈근탕(加味升麻葛根湯)의 치료효과)

  • Objective : This case series study was designed to assess the effect of Gamisungmagalguntang for chronic urticaria identified as "Wind-Heat" pattern by retrospective methods. Methods : We treated 19 patients for chronic urticaria with Gamisungmagalguntang(加味升麻葛根湯), who visited to care chronic urticaria at the department of Ophthalmology, Otolaryngology and Dermatology of Korean Medicine at Kyunghee University Korean Medical Center from 1st January 2013 to 31st August 2013. We analyzed information and body conditions of 19 patients. And by using Urticaria Activicy Score(UAS), frequency and number of wheals, itch severity and total score on a 4-point(0-3) scale were assessed in 19 patients. The change of each criterion and total scores between baseline and follow up were analyzed using paired t-test and Wilcoxon test(p<0.05). Results : 1. UAS improvement was statistically significant(p<0.001). Effects of symptom improvement were excellent(31.58%) that means symptom remission and good(42.10%) that means symptom improved over 50%. 2. Heat sensitivity as suspected provocation factor could be a marker for the Wind-Heat pattern urticaria. 3. Among the Wind-Heat pattern urticaria patients, normal digestion(52.63%) and no thirst(78.95%) were higher than abnormal conditions. Whereas about sweat item, abnormal conditions(57.89%) were higher than normal. That means abnormal sweat conditions could be a marker for a defense qi(衛氣) dysfunction of the Wind-Heat pattern urticaria. Conclusions : Our findings suggest that Gamisungmagalguntang could be effective on the Wind-Heat pattern urticaria.

Research on Fire Safety of Mortar-Containing Waste Tire Powders and Flame Retardant (폐타이어 분말과 난연제가 혼입된 모르타르의 화재안전에 관한 연구)

  • The purpose of this study is to determine how effectively waste tire recycled material mixed with flame retardant work in combating fire. As discovered in the previous study, waste tire mixed with cement mortar has more insulation capacity. However, this mortar is weak against fire. Therefore flame retardant, with a specific proportional mix, will be added to increase its fire prevention capacity. Tests will be made in accordance with ISO 5657 procedures for measuring fire ignition time, flame and shape variation of test pieces at the Building Material Test Institute. The test piece will be set up with horizontal levels having a constant radiation heat of $1{\sim}5W/cm^2$. Temperature transfers and increases from the surface into the interior. Combustible gases result due to pyrolysis, and regular contact is maintained between the fire source and the center of the test piece for assessment purposes. Ignition has not been occurred without adding retardant meaning that there is almost no possibility of ignition of waste tire particle. This fact can be considered as fire load to appreciate a volume of combustion materials. Flame is not occurred due to heat-absorbing effect by adding non-organic series retardant into waste tire particle. Conclusions have been summarized as follows; 1) Combustion of building material can be decreased by adding retardant to waste tire-mixing mortar. But compressive strength and insulation capacity of the material should be measured later. 2) Firing prevention and ignition are main points of building fire. Reasonable fire engineering assessment of interior material should be made for establishing effective disaster prevention system.

Recovery of Protein Hydrolysate from Hoki (Johnius belengeri) Frame with Tuna Pyloric Caeca Crude Enzyme and Its Functionalities (참치 유래 조효소를 이용한 민태(Johnius belengeri) Frame으로부터 단백질 가수분해물의 회수 및 그 기능성)

  • Enzymatic hydrolysis with tuna pyloric caeca crude enzyme(TPCCE) was performed to recover a protein hydrolysate from hoki frame, fish processing by-product. Optimum hydrolytic conditions were pH 10.0, temperature $50^{\circ}C$, and incubation time 12 hrs, and then the degree of hydrolysis was about 60%. The yield of the hydrolysate from hoki frame by enzymatic hydrolysis was approximately 77% on a dry weight basis. The prepared protein hydrolysates were also fractionated through a series of 30, 10, 5 and 1 kDa molecular weight cut-off (MWCO) membranes in order to investigate the effect of their functionalities according to the difference of their molecular size. As the result of studying functionalities of the hydrolysates, 1 K hydrolysate showed the highest solubility over all pHs, and 30 and 10 K hydrolysate showed more excellent emulsifying property and whippability than the other hydrolysates.

Prostaglandin $E_1$ Increases cGMP Levels in Beating Rabbit Atria: Lack of Effects of $PGE_1$-induced Cyclic Nucleotides on Secretory and Contractile Functions

  • Members of prostaglandin(PG) E-series elicit cellular effects mainly through adenylyl cyclase-cAMP signaling. The role of $PGE_2$-induced increase in cAMP has been shown to be compartmentalized in the cardiac myocytes: $PGE_2$-induced increase of cAMP is not involved in the control of cardiomyocytic contraction. The purpose of the present study was to define the effect of $PGE_1$ on the cGMP levels and the role of $PGE_1$ in the atrial secretory function. Experiments were performed in perfused beating rabbit atria and atrial contractile responses, cGMP and cAMP efflux, and atrial natriuretic peptide(ANP) secretion were measured. $PGE_1$ increased cGMP as well as cAMP efflux concentration in a concentration-dependent manner, however, no significant changes in atrial secretory responses were observed(with $1.0{\mu}M\;PGE_1$; for cGMP, $144.76{\pm}37.5%$, n=11 versus $-16.81{\pm}4.76%$, n=6, control, p<0.01; for cAMP, $187.60{\pm}41.52%$, n=11 versus $7.38{\pm}19.44%$, n=6, control, p<0.01). $PGE_1$ decreased atrial dynamics slightly but transiently, whereas $PGE_2$ showed similar effects but with lower potency. Isoproterenol increased atrial cAMP efflux(with 2.0 nM; $145.71{\pm}41.89$, n=5 versus $7.38{\pm}19.44%$, n=6, control, p<0.05) and mechanical dynamics and decreased ANP secretion. The $PGE_1$-induced increase in cGMP efflux showed a bell-shaped concentration-response curve. $PGE_1$-induced increase of cGMP efflux was not observed in the presence of L-NAME, an inhibitor of nitric oxide(NO) synthase, or ODQ, an inhibitor of NO-sensitive guanylyl cyclase. L-NAME and ODQ showed no significant effect on the $PGE_1$-induced transient decrease of atrial dynamics. These data indicate that $PGE_1$ increases cGMP levels via NO-soluble GC signaling in the cardiac atrium and also show that $PGE_1$-induced increases in cGMP and cAMP levels are not involved in the regulation of atrial secretory and contractile functions.

INFLUENCE OF NICKEL-TITANIUM SPREADER ON THE SEALING ABILITY IN LATERAL CONDENSATION TECHNIQUE (측방가압충전시 Nickel-Titanium spreader의 사용유무가 근관충전효과에 마치는 영향)

  • Lateral condensation with gutta-percha and sealer has been shown to provide an excellent apical seal; however, the lateral condensation technique has demonstrated less favorable apical leakage results in curved canals when compared with straight canals. Placement of endodontic spreaders to within 1 to 2mm of the root canal working length has been advocated for optimum gutta-percha obturation. Due to their stiffness, stainless-steel(SS) spreaders will often fail to achieve this position in curved canals. Newly marketed nickel-titanium(NT) spreaders may offer an advantage in this regard due to the increased flexibility of these instruments. The purpose of this study was to evaluate the effect of NT finger spreader on the sealing ability in lateral condensation technique, compared with conventional SS finger spreader. Twenty four standardized resin models simulating curved canals(30 degree) were randomly placed into 2 groups and instrumented to a #30 master apical file size with Ni-Ti Profile .04 taper series using step down technique. Each groups was obturated with standardized gutta-percha cone by standard lateral condensation technique using SS finger spreader, NT finger spreader. And then, each model was sectioned horizontally with microtome at 1, 2, 3, 4, 5mm levels from the apex. At each of 5 levels, ratio of the area of gutta-percha was obtained by calculating the area of gutta-percha to the total area of the canal. The data collected were then analyzed statistically using a t test for independent samples. The results as follows ; 1. The total mean ratio of area of gutta-percha was 89.20${\pm}$7.00(%) for SS spreader group. 92.20${\pm}$5.17(%) for NT spreader group. There was statistically significant difference between each group(p<0.05). 2. At 3mm level, the mean ratio of area of gutta-percha was 88.32${\pm}$5.41(%) for SS spreader group, 95.25${\pm}$2.60(%) for NT spreader group. There was statistically significant difference between each group(p<0.05). At 1,2,4mm levels, NT spreader group showed greater mean ratio of area of gutta-percha than SS spreader group, too. But there was no statistically significant difference. 3. At 5mm level, the mean ratio of area of gutta-percha was 91.83${\pm}$3.42(%) for SS spreader group, 87.91${\pm}$3.68(%) for NT spreader group. There was statistically significant difference between each group(p<0.05). This study concluded that the NT spreader demonstrated somewhat favorable apical sealing effect than SS spreader in prepared curved canals. The clinical use of NT spreaders may enhance our ability to create better apical seals in curved canals, but further studies in this area will help clarify some of the remaining areas with which practitioners are concerned, such as compaction forces exerted by NT spreaders.
