Intralobar pulmonary sequestration communicating with the esophagus -1 cases report- (기관지 식도루를 동반한 내엽형 폐격리증 -1예 보고-)

  • We have experienced a case of intralobar pulmonary sequestration communicating with the esophagus. A 24 year old female patient was admitted of chronic cough related to food, purrlent sputum and left lower chest pain. Esophagography and bronchography revealed bronchoesophageal fistula and bronchiectasis with cystic lesion of the left lower lobe. There was multiple anomalous feeding vessels arinig from the intercostal arteries and no draining systemic vein on aortography. division of the bronchoesophageal fistula and left lower lobectomy was performed. Communication with the esophagus in rarely associated with intralobar pulmonary sequestration and esophagogram is useful method of diagnosis for this communication.

  • Pulmonary sequestration is an unusual congenital malformation characterized by the presence of nonfunctioning lung tissue which usually has no communication with the normal bronchial tree and receives its blood supply from an anomalous systemic artery. Extralobar form is a very rare congenital malformation. We have experienced a 54 year old female patient with a mass in the upper lobe complaining of cough and blood tinged sputum. A triangular shaped mass was located in the left upper lobe, medially. The arterial blood supply were from the thoracic aorta and the pulmonary artery but there was no the tracheobronchial communication. The venous drainage was through the pulmonary vein. The mass was confirmed as extralobar pulmonary sequestration associated with a pericardial defect.

  • Pulmonary sequestration is a complex anomaly involving the pulmonary parenchymal tissue and its vasculature. It presents as a cystic mass of nonfunctional lung tissue without communication with the tracheobronchial system. Usually, it receives blood supply from anomalous systemic vessels. Therefore, preoperative diagnosis of the pulmonary sequestration is difficult, especially when it is located in the abdomen and combined with congenital cystic adenomatoid malformation(CCAM). We encountered such a mass(CCAM type 2) detected prenatally by ultrasonography. It was a kidney bean shaped, pinkish mass straddling the thorax and abdomen on the right side. Because of the sonographic appearance, neuroblastoma was diagnosed preoperatively. The mass was completely extirpated without difficulty.

  • Pulmonary sequestration is an uncommon congenital pulmonary malformations characterized by presence of nonfunctioning lung tissue which receives its blood supply mostly from the anomalous systemic arteries. We have experienced a 30 year old male patient with a mediastinal mass complaining of intermittent chest pain, and the mass was histologically confirmed as extralobar pulmonary sequestration. The anomalous blood supply origined from the right pulmonary artery but there was no communication with the tracheobronchial tree. He underwent operation through midsternotomy incision for the purpose of concurrent resection of enlarged thymus noted on chest CT.

  • Pulmonary sequestration is a congenital malformation in which a mass of pulmonary tissue is detached from the normal lung and receives its blood supply from a systemic arterys from the thoracic aorta or the abdominal aorta, or occasionally from an intercostal artery. We have experienced the three cases of the intralobar pulmonary sequestration. In the first case a 5 year old male was admitted of productive cough and abdominal distension, and chest film showed pneumonic infiltration in RLL. Second case was 26 year old male patient complaining Rt. chest discomfort and hemoptysis and chest X-ray revealed infiltration in Rt. LLF. Third case was 26 year old male patient whose complaint was hemoptysis. Chest x-ray showed hazy density in Lt. lower lung field. In the all cases, the aortograms were performed and the confirmed diagnosis was intralobar pulmonary seqeustration.

  • Pulmonary sequestration is a very rare congenital malfomation, especially when it is located in the infradiaphragmatic retroperitoneal space. The patient was a 56-year-old man with no respiratory symptom but had some postprandial epigastric discomfort for several days. Plain chest x-ray revealed mild elevation of left diaphragm and abdominal CT scan showed large cystic retroperitoneal mass with some calcification. We performed the operation through the thorachoabdominal incision and the mass was proven to be a pulmonary sequestration. Therefore, we report the case with review of literatures.

  • In this study, we deduced the best estimation methodology for amount of carbon sequestration by vegetation, through the case study using the data obtainable from the environmental assessment procedure. Our results showed that the estimation methodology using the national vegetation map was the best for the strategic environmental assessment, whileas those using the vegetation growth equation were applicable for environmental impact assessment procedure. Furthermore, we found that the amount of carbon sequestration by farmland and/or grassland, not by vegetation, was not negligible. Therefore, we concluded that the area of farmland and/or grassland need to be taken into account during the landuse planning.

  • In this study, we investigated the effect of carbon sequestration and sink on the environmental impact assessment of a Bogeumjari Housing Project. Through the case study, we found that, if the project is implemented, the amount of carbon stock tends to decrease greatly while as the amount of the carbon emission tends to greatly increase. Furthermore, we found that the future land use should be planned in detail in order to maintain the soil carbon stock prior to development. Also, enlargement of undeveloped forest land area would be more efficient than that of newly planted area in terms of carbon sequestration.

Injectable Apatite for the Sequestration of Sr-90 in Groundwater

  • Laboratory column experiments were conducted to evaluate the feasibility of injectable apatite method for the sequestration of Sr-90 in groundwater. The columns were tested to evaluate the rate of citrate biodegradation, the amounts of apatite formed, and the treatability of strontium by the sediment and apatite. The results showed the decreases in citrate, calcium, and phosphate concentrations and the increases in alkalinity and citrate degradation products (acetate and formate) in the columns, suggesting that the citrate degradation and formation of calcium phosphate are occurring. Although the calcium and phosphate were not completely consumed within the columns, some amounts of apatite were formed and it showed an ability to treat strontium in groundwater. This study provides a fundamental understanding of reaction mechanisms for the injectable apatite sequestration method for Sr-90 removal.

Intralobar Pulmonary Sequestration: Report Of One Case (폐내엽형 격절부 1례 보고)

  • Intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derives its arterial blood supply through aberrant vessel directly of systemic circulation. Intralobar pulmonary sequestration Is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We experienced a case of pulmonary sequestration preoperatively confirmed. The patient was 17 year old female whose complaints were mild fever and profuse purulent sputum. Chest film showed a large thin walled cystic lesion with air-fluid level at the left lower posterior basal lung field. Aortogram revealed an aberrant artery originated from thoracic aorta just above the diaphragm and that drained via pulmonary vein into the left atrium. At time of operation, a large abscess cavity measuring 9x8x3 cm in dimension at the left lower lobe was noted. And the aberrant artery, measuring 0.5 cm in diameter and 2 cm in length, arising from thoracic aorta just above the diaphragm was noted. After division and ligation of the aberrant artery, a left lower lobectomy was performed and the patient`s postoperative course was uneventful.
