Anomalous systemic arterial supply to lung without sequestration in an infant who has congenital heart disease : a case report (선천성 심장질환을 가진 영아에서 발견된 폐 격리증을 동반하지 않은 폐의 이상 체 동맥 기시 1례)

  • Systemic arterial supply from the aorta to the lung is a rare congenital anomaly within the spectrum of bronchopulmonary sequestration according to Pryce's terminology. We describe our experience of this anomaly in an infant with congenital cardiac disease confirmed by multidetector CT scan. We found a systemic arterial supply from the aorta to the right lower lobe of lung without right lower lobar pulmonary artery and bronchopulmonary sequestration. This combination of congenital anomaly is most rare form.

  • Bronchopulmonary sequestration is a rare congenital malformation of the lung, concerning about the abnormal feeding systemic artery, may happen a serious complication of heeding during operation if not recognized before operation. High index of suspicion of sequestration is the key to successful operative procedure with the aid of characteristic recurrent and long-standing symptoms, its location, and invasive or non-invasive diagnostic tools. We report 2 cases of intralobar type of bronchopulmonary sequestration and review 10 articles about the subject, totaling of 21 cases.

Surgical Experience of Pulmonary Sequestration -21 cases- (폐격리증 21례에 대한 수술 치험)

  • A total of 21 patients with pulmonary sequestration who had undergone surgical treatment were reviewed at the department of thoracic and cardiovascular surgery of catholic university medical college from May 1983 through May 1993. The pulmonary sequestration was intralobar in 19 patients, and extralobar in 2 patients. There were 17 woman and 4 man ranging of age from 17 months to 48 years with a mean age of 21.3 years. Clinical manifestations were varied from no symptom[3 cases to recurrent pneumonia[15 cases , and hemoptysis[3 cases . Preoperative diagnosis was correct in only 6 cases[29% of the 21 patients of pulmonary sequestration . The left lower lobe[11 cases is most often affected in intralobar sequestration. The aberrant artery in intralobar sequestration originated in 13 cases[68% of 19 cases from descending aorta. 15 of the 19 patients of intralobar type underwent lobectomy. The extralobar sequestration[2 cases were simply excised. There was no mortality and no complication.

  • Intralobar pulmonary sequestration is a rare congenital lung anomaly. It is defined as a portion of nonfunctioning lung parenchyma that receives its blood supply from an anomalous systemic artery. Patients often present with chronic or recurrent pneumonia. A chest radiograph may show a cystic lesion with air-fluid levels in the lung base. A high index of suspicion is needed for a diagnosis. Surgical removal of a symptomatic intralobar pulmonary sequestration is generally the treatment of choice. Identifying the aberrant artery is a difficult problem when resecting a pulmonary sequestration. The thoracic and abdominal aortas are the most common origins for the abnormal blood supply. However, arterial supply from the celiac artery is quite rare. We present a case of intralobar pulmonary sequestration with the blood supply originating from the celiac artery.

  • Anomalous systemic arterial supply to the normal basal segments with normal bronchial connection of the lung without sequestration is a rare anomaly. It was classified as a type of sequestration according to Pryce's terminology, but whether the term - one of the sequestration is appropriate or not, is controversial because of normal bronchial connection. We describe our experience with surgical treatments for anomalous arterial supply to the normal basal segments of the left lower lobe.

Leukocyte Sequestration and Free Radical-Mediated Lung Injury in Ovine Cardiopulmonary bypass Using Membrane Oxygenator (양에서 막형 산화기를 사용하여 심폐바이패스할 경우 백혈구격리 및 자유라디칼로 중재되는 폐손상)

  • Background: Complement activation with transpulmonary leukocyte sequestration is considered a main mediator leading to ischemia-reperfusion lung(I-R) injury. We studied the role of leukocytes in the formation of I-R injury in ovine cardiopulmonary bypass(CPB) model with a membrane oxygenator. Material and Method: Five sheep were used. CPB circuitry consisted of a roller pump(American Optical Corp., Greenwich, CT, USA) and a membrane oxygenator(UNIVOX-IC, Bentley, Baxter Health Corp, Irvine, CA, USA). The CPB time was fixed at 120 min. Ten minutes after the start of CPB, total CPB was established. Thereafter a total CPB of 100 min was performed, followed by another 10 min of partial CPB. The CPB was discontinued and the animals were fully recovered. For measuring left and right atrial leukocyte counts, blood samples were taken before thoracotomy, 5 min and 109 in after the start of CPB, and 30 min and 120 min after weaning. C3a was measured before thoracotomy, 109 min after the start of CPB, and 30 min and 120 min after weaning. Plasma malondialdehyde(MDA) was checked before thoracotomy, 109 min after the start of CPB, and 30 min after weaning. One to two grams of lung tissue were taken for water content measurement before thoracotomy, 109 min after the start of CPB, and 30 min after weaning. Lung biopsy specimens were examined by light and electron microscopy. Result: Of 5 animals, 4 survived the experimental procedures. Of these, 3 animals survived on a long-term basis. No significant differences in transpulmonary gradients of leukocyte were found and no significant complement activation was expressed by C3a levels. MDA level did not show significant changes related to lung reperfusion despite an increase after the start of CPB. On both light and electron microscopic examinations, mild to moderate acute lung change was observed. Interstitial edema, leakage of erythrocytes into the alveolar space and endothelial cell swelling were the main findings. Water content of the lung showed a slight increase after the start of CPB, but there was no statistical significance. Conclusion: These findings indicate that ischemia-repersusion lung injury may not be from complement activation-leukocyte sequestration but from another source of oxygen free radicals related to CPB.

  • Systemic arterialization of lung with/without sequestration (Sequestration/Anomalous Origin of Left Pulmonary Artery, AOLPA) is a rare form of congenital anomalous systemic arterial supply to the lungs. In this anomaly, the arterial supply of one or more arteries of the basal segments of the lower lobe derives from an aberrant vessel arising from the aorta. We report two adult cases of systemic arterialization of normal basal segments of left lower lobe lung with/without sequestration. The one (AOLPA) was treated by left lower basal segmentectomy and the other (Sequestration) by therapeutic angiographic embolization. Based on the favorable follow-up result in our patients, although lobectomy (segmentectomy) is the basic treatment modality, embolotherapy could also be a mode of treatment that could be selectively applied to elderly, infirm patients or high risk patients with poor pulmonary function.

Intralobar Pulmonary Sequestration A Case Report (내엽형 폐격리증 수술 1례 보고)

  • Pulmonary sequestration is an uncommon congenital malformation characterized by the presence of non-functioning lung tissues which receives its blood supply from an anomalous systemic artery instead of a pulmonary arterial branch. We present a case of intralobar pulmonary sequestration experienced lately. The patient was 7 years old girl with the complaints of chronic productive cough and right lower chest pain. Serial chest films showed a large cyst with or without a air-fluid level on the right lower lung field. Aortography revealed an aberrant artery originating from thoracic aorta just above the diaphragm and that drained into the right inferior pulmonary vein. During operation, a large abscess cavity measuring 6.5x5x5 cm in dimension at the right lower lobe was noted. And the two aberrant arteries, measuring 3 mm in diameter, arising from thoracic aorta 5 cm above the diaphragm was noted. After division and ligation of the aberrant arteries, right lower lobectomy was performed and the patient`s postoperative course was uneventful.

Surgical treatment of bilateral pulmonary sequestraion; -report of A case- (양축에 발생한 엽내형 폐격리증의 외과적 치료 -1례 보고-)

  • The pulmonary sequestration is a rare congenital malformation of the lung, concerning about the abnormal feeding systemic artery, may happen a serious complication of bleeding during operation if not recognized before operation. We experienced a case of bilateral intralobar pulmonary sequestration preoperatively confirmed by aortogram. An Aortogram demonstrated a anomalous systemic artery arising from thoracic aorta just above the diaphragm. The artery bifurcated and supplied areas of both right and left lower lobes. On the operative field, left lower lobectomy was done with devision and ligation of left branch of anomalous artery and triple ligation of remained branch of anomalous artery was done. Postoperative course was uneventful. She was discharged on postoperative seventeenth day.

  • A sequestrated Mass of ectopic non functioning pulmonary Tissue artery is an uncommon but clinically recognizable Entity. Pulmonary sequestrated, in general usage, designates an intralobar process intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derive its arterial blood supply through aberrant vessel directly of systemic circulation. As aberrant systemic vessel supplying the lung was reported by Hurber in 1777. We experienced a case of Intralobar pulmonary sequestration Pre-Operatively, confirm by Aortogram. The operative finding show that large Abscess cavity measuring 7x8 Well circumscribed, child fist sized Mass, and 4cm-length aberrant vessel arising from Descending aorta Just above the Diaphragm. The Anomalous systemic artery was ligatures & resection, and associated with left lower lobectomy was done. Post-Operative course was uneventful, and 7 days later discharged.
