  • Objective : Patients treated with surgical clipping for anterior communicating artery (A-com) aneurysm often complain of anosmia, which can markedly impede their quality of life. We introduce a simple and useful technique to reduce postoperative olfactory dysfunction in A-com aneurysm surgery. Methods : We retrospectively reviewed the medical records of patients who underwent surgical clipping for unruptured aneurysm from 2011-2013 by the same senior attending physician. Since March 2012, olfactory protection using gelfoam and fibrin glue was applied in A-com aneurysm surgery. Therefore we categorized patients in two groups from this time-protected group and unprotected group. Results : Of the 63 enrolled patients, 16 patients showed postoperative olfactory dysfunction-including 8 anosmia patients (protected group : unprotected group=1 : 7) and 8 hyposmia patients (protected group : unprotected group=2 : 6). Thirty five patients who received olfactory protection during surgery showed a lower rate of anosmia (p=0.037, OR 10.516, 95% CI 1.159-95.449) and olfactory dysfunction (p=0.003, OR 8.693, 95% CI 2.138-35.356). Superior direction of the aneurysm was also associated with a risk of olfactory dysfunction (p=0.015, OR 5.535, 95% CI 1.390-22.039). Conclusion : Superior direction of aneurysm appears associated with postoperative olfactory dysfunction. Olfactory protection using gelfoam and fibrin glue could be a simple, safe, and useful method to preserve olfactory function during A-com aneurysm surgery.

  • Purpose: This study was conducted to identify factors that influence on self-determination for withdrawal of life-sustaining treatment in the community dwelling elderly. Methods: This study used a descriptive correlational design. A convenience sample of 201 elderly were recruited from a welfare center located in Seoul, Korea. Data collection was done from September 1 to September 18, 2015. Four survey questionnaires were used: Multidimensional Scale of Perceived Social Support (MSPSS), Life Satisfaction Index-Z (LSI-Z), Attitude on Do Not Resuscitate (DNR), and Self-determination for Withdrawing Life-sustaining Treatment. Data analysis was done by t-test, ANOVA, Pearson's correlation, and hierrarchical multiple regression. Results: Hierarchial multiple regression showed that the factors predicting the level of self-determination for withdrawing life-sustaining treatment were educational level, DNR experience in family, life satisfaction, and DNR attitude. These factors explained 44% of the self-determination for withdrawing life-sustaining treatment in community dwelling elderly. Conclusion: The results of this study suggest developing educational programs aiming at changing elderly's DNR attitude positive ways so that they can enhance self-determination for withdrawing life-sustaining treatment. Further study is needed with more elderly population in extended areas.

  • Purpose. This study investigated changes in attitudes toward elders in general and elders with dementia after students finished a gerontological nursing practicum. Methods. Questionnaires developed for Asian cultures were administered pre practicum, immediately post practicum, and at 8-months follow up to 31 senior students in a baccalaureate nursing program. The 1-week practicum occurred at two adult day care centers: a center for elders with dementia and a center for elders with stroke. Repeated measures ANOVA and Bonferroni correction procedures were used to analyze data. Results. Students' evaluation of elder vitality and flexibility increased significantly at post practicum, however this increase was not sustained at follow up. Score of generosity of elders, the only positively evaluated dimension for elders in general, improved partly at post practicum. Students evaluated flexibility and generosity of elders with dementia more negatively than general elders. All of the decreased attitudes at follow up were not significantly different from those at pre practicum. Conclusions. Students had more negative attitudes toward elders with dementia. Attitudes of students in direct contact with elders with dementia were improved through the practicum regarding generosity and flexibility. However the sustainability of the immediate effect was not observed at follow up.

  • Purpose: This study aimed to examine the elderly physical fitness test for elderly people living in urban areas and to evaluate the physical fitness level according to gender and age groups. Methods: The subjects were 180 elderly people aged 65 and over living in urban areas. The elderly people were classified into early old age, middle old age, and advanced old age groups. Five items of the physical fitness test for the elderly (arm curl, chair stand, 2 min step test, back scratch, and chair sit and reach). Results: In analyzing the average of each item by gender and age, a significant difference was found in the results of each item as age increased in all sports, but no difference was observed according to gender except dumbbell lifting. Conclusion: Although the physical fitness level of all ages decreased from early age to late age, the difference in physical fitness according to gender was not significant except the arm curl test. The results of this study can be used as basic data for a new "age-specific exercise program" for the elderly.

  • Purpose: This study examined the effects of individual reminiscence therapy on older adults' depression, morale, and the quality of life. Methods: The design was a single-group pre-test and post-test study. Subjects consisted of 31 older adults from two senior centers and a welfare center in Seoul. Individual reminiscence therapy was applied to study subjects four times, once a week for an hour at each time. Measurement tools were the Geriatric Depression Scale Short Form Korea (GDSSF-K) for depression, Mun Ae-ri's (1996) scale for morale, and Medical Outcomes Study Short Form 36 (SF-36) for the quality of life. Data was analyzed using descriptive statistics, paired t-test, and pearson correlation. Results: The application of individual reminiscence therapy reduced older adults' depression (t=-5.65, p=.000), and enhanced older adults' morale (t=4.65, p=.000). The application of individual reminiscence therapy improved older adults' quality of life (t=5.00, p=.000). Conclusion: Findings of the study suggest that individual reminiscence therapy may be applied as a nursing intervention that contributesto the improvement of older adults' quality of life, reduces their depression, and enhances their morale.

Methods: The research used a randomly assigned control group pretest-posttest design. The study participants were 34 senior students from the College of Nursing of S University. The study was conducted from March to April 2012. The biofeedback training conducted in this study consisted of eight sessions, each divided into two stages: 15 minutes each for breathing biofeedback training and electromyography (EMG) biofeedback training. The collected data were analyzed using SPSS version 19.0. Results: We found statistically significant decreases in perceived stress and EMG levels and an increase in academic resilience in the experimental group. Conclusion: The results showed that the biofeedback training for the nursing students was extremely effective in reducing their levels of perceived stress and EMG while increasing academic resilience. To clarify the effects of biofeedback training on the biological stress response, participants with a higher level of stress response should be recruited in future studies.
