Characteristics of Depression in the Aged People (노년기 우울증의 특징)

  • Some characteristics of depression in the aged were elucidated. Compared with the previous thoughts, much more attention was given to the biological aspects of depression. However, we have to face up to and solve a variety of enigmas about depression in the aged people. They are:locations and underlying mechanisms in the cerebrum responsible for depression;relationship of senile depression to the presenile one;possible difference(s) between depression with and without psychotic features;possible neurotransmitter differences between senile and presenile depressions. New insights into these areas could bring some lights in the under standing and treatment of depresseion in the aged.

A Study on the Burdens and Depressive Reactions on Families who Cared for Patients Suffering from Senile Dementia (치매노인을 돌보는 가족의 부담감과 우울반응에 관한 연구)

  • The purpose of this study was to investigate the relationship between the burden on families who live with an elderly person suffering from senile dementia, and the degree of their depression. There were 400 participants in this study, staying in the Seoul and Kyonggi areas from August 1, 1997 to February 28, 1998. Among the group, 100 participants took care of their patient at home, and another 300 participants left 100 patient at a day-care center, 100 sanatorium for senile dementia(asylum for helpless elderly people), 100 an infirmary for elderly people. Eventually 242 subjects out of the 400 were selected for the data analysis. The Zarit (1980) tool was employed to measure the degree of burden and Zung's(1965) “Self-Rating Depression Scale” was employed for the data analysis. The data was analyzed, and the percentage, t-test, ANOVA and Person's Correlation Coefficient were calculated. The results are as follows : 1. The average degree of burden that care-giving families felt was 49.13, which is somewhat high. 2. The average degree of depression that care -giving families felt was 51.95, which is relatively high. 3. The degree of burden was directly affected by the relation with the patient(F=2.48, P<.05), and the socio-economic status of the family(F=5.17, P<.05). Its also affected by the patient's educational status(F=2.17, P<.05). 4. The degree of depression of the family was significantly dependent on sex(t=-2.05, P<.05), age (F=2.99, P<.05), the relationship with the patient(F=3.65, P<.01), socio-economic status (F=7.74, P<.001), occupation(t=2.82, P<.01), health status(F=4.42, P<.01), and the place of residence(F=4.30, P<.01), The patient characteristics was significantly dependent on his/her educational status(F=3.85, P<.01), the period of suffering from senile dementia(F=2.47, P<.05), and smoking habit(F=6.17, P<.001). 5. The relationship between the degree of burden and that of depression reads r=0.43, which is statistically positive correlation in the high significant level. Upon analyzing the entire summation, most care-givers for elderly patients suffering from senile dementia lack time in caring for themselves. They also experience chronic fatigue and mental discomfort caused by the isolation from society, curtailment of certain activities, a sense of responsibility for their patients, and limits of their endurance in taking care of their patients over time. In conclusion, this study emphasizes the necessity for the following propositions : 1. In order to measure the degree of burden that Korean care-giving families undergo, a new tool must be developed on the basis of Korean culture. 2. An educational program based on the demands that care-giving families undergo must be developed, and its clinical effect also has to be examined.

Effects of Korean Medical Intensive Treatment Program on Geriatric Depression Disorder (노인성우울증에 대한 한방 집중치료 프로그램의 효과)

  • Objectives: As the elderly population grows and the prevalence of depression increases, the incidence of geriatric depression disorder will continue to increase. The effect of an intensive Korean medicine treatment program on geriatric depression disorder was studied. Methods: Geriatric depression disorder patients who received treatment over a period of three years were reviewed, with the geriatric depression score (GDS) used as the primary outcome. The intensive Korean medical treatment program applied to the geriatric depression disorder patients consisted of herbal medicine, acupuncture, pharmacopuncture, moxibustion, meditation, qigong exercises, and reminiscence therapy. Results: The GDS scores of patients who received the intensive Korean medical treatment program decreased from an average of 19.57 (${\pm}5.52$) to 15.76 (${\pm}6.99$) and there was a significant difference. Conclusions: When the intensive Korean medical treatment program was applied to geriatric depression disorder patients, the symptoms of depression improved. This treatment program will be helpful for improving symptoms in patients and can be applied by trained clinicians.

  • Recently, South Korea senile patients to undergo total knee arthoplasty surgery along with the aging of the population has increased. Hence, this descriptive investigational study is to identify the correlation among the variables including the pain, depression, social support, and the degree of life satisfaction in the senile patients with total knee arthroplasty, and to analyze the factors to affect the degree of life satisfaction. The study subjects were 115 the senile patients, who had been hospital in less than one year or visited the hospital on an out patient basis after getting surgery of total knee arthroplasty at the orthopedics of S hospital located in G Metropolitan City. Data collection period was from September 25, 2015 to October 31, 2015, they were collected using structured questionnaire with the researches describing the research objectives and methods.

A Study on risk factors for senile dementia (노인성치매 발생요인과 돌보는 가족원의 스트레스에 관한 조사 연구)

  • This study was conducted to investigate risk factors for senile dementia as well as care givers' stresses and thier needs for nursing care. It was done using a retrospective survey. A convenience sample or In senile dementia patients and l20 nor-mal elders in a rural area was used. The tools used in the study were the MMSE-K(Mini-Mental State Examination-Korea) for dementia screening test and a questionaire developed by the research team. Data were collected through home visits by Com-munity Health Practitioners. Data were analyzed using descriptive statistics, T-test, and Chi-square test. The findings are as follows : 1. There were significant differences in age, marital status, and religions between the two groups. 2. There was a significant difference in smoling behavior between the two groups. 3. There was a significant difference in past his-tory of cancer between groups. 4. There was a significant difference in past and present elderftmily relationship between the two groups. 5. There were significant differences in intellectual activities, assuming major role in family and seeking other's help in daily life troubles between the two groups. 6. There were significant differences in stress factors such as child problem, family conflict, health problem and illegal behavior between the two groups. 7. The major problems out by families in caring for dementia patient were catastrophic reactions, dirtiness, mood change, devouring and tremor. The most serious problems faced by families was dirtiness. with catastrophic reactions, sleep distrubance, changeableness, and a suspcio-usness following. The care givers expressed chronic fatigue, anxiety, tension, depression, disorder in daily life, shamefulness, blame from neighbours and guiltiness. 8. There is need for geriatric hospitals, nursing homes, burden sharing, and counselling or education for family care givers. A replicate study in the urban area is recommended to validate the findings of this study. To explore the impact of stress in life and ‘han’ on senile dementia, a qualitative study is recommended.

Observational Study of the Treatment of 116 Cases of High Risk Group for Dementia and Depression in Multiple Korean Medicine Clinic (치매 및 우울증 고위험군 116례의 한의 치료에 대한 관찰 연구)

  • In order to reduce the social and economic costs due to the increase in dementia patients, which is a representative senile disease in an aging society, it will be important to prevent dementia and to detect and treat early in high-risk groups. This study reported the results of treatment for the elderly with high risk of dementia and depression who received Korean medicine (KM) treatment in 9 KM clinics. Medical charts were surveyed on 116 patients with high risk of dementia and depression who received KM treatment at 9 KM clinics in Gangseo-gu from September 1, 2020 to December 31, 2020. The majority of the patients were female(76.72%), the average age was 71.66±7.18 years old, and the average education level was 8.96±3.91 years. The average treatment period was 61.47±10.30 days, the average number of treatments was 15.38±1.06 times, and both acupuncture and herbal medicine were administered. After receiving KM treatment, cognitive assessment scores such as MMSE-DS and MoCA-K, depression-related GDSSF-K score, dementia knowledge, attitude, and preventive behavior, blood debilitation scale, and GQOL-D score were improved compared to before. There was no specific adverse reaction except that one patient with elevated AST and ALT levels was observed. This study showed improvement in cognitive function, depression, dementia-related scale, and quality of life in high-risk groups for dementia and depression over 60 years old who received KM treatment. More systematic and large-scale planned clinical studies will be needed.

Effect of lumbar stabilizing exercise using the gym ball on psychosocial status in elderly women with chronic low back pain (짐볼을 이용한 요부안정화 운동이 만성요통 여성노인의 정신사회적 건강수준에 미치는 영향)

  • Objective : The purpose of this study was to analyze the effect on psychosocial status in elderly women with chronic low back pain after practicing lumbar stabilizing exercise using the gym ball, and to developed and activated on exercise program for healthy senile life. Methods : The subjects were 23 elderly women with chronic low back pain. Mean age of subjects was $67{\pm}13$ years. The lumbar stabilizing exercise using the gym ball was performed 3 times a week during the 8 weeks study period. The research tool were ODI(Oswestry Disability Index), VAS(Visual Analogue Scale), SES(Self-esteem Scale), GDSSF-K(Geriatric Depression Scale Short Form Korea Version), PSS (perceived stress scale), Life satisfaction, and MMSE-K. The analysis of data was processed by the statistical program SPSS 12.0 and paired t-test and frequency analysis were used to compare psychosocial status between the pretest and post tests. Result : The lumbar stabilizing exercise using the gym ball increased significantly SES and Life satisfaction of subjects, and ODI, VAS(trunk flexion & trunk extention), GDSSF-K, and PSS were decreased significantly(p<.05). While VAS(trunk rotation) were not significantly. Conclusion : The lumbar stabilizing exercise using the gym ball improved psychosocial status in elderly women with chronic low back pain.

  • These days as the average span of man's life increases, the patients of senile dementia also increase. In oriental medicine, the study of dementia developed during the Ming and Qing eras. So I investigated medical books on those eras, as a result, the following conclusion was drawn. 1. ZhangJingYue of the Ming era considered the cause of dementia to be emotional problems, he observed a delirium, abnormal actions, sweating loss and depression in the dementia patients and he prescribed 'BokManJeon', 'ChilBokYem' and 'DaeBoWonJeon'. 2. ChenShiZe of the Ming&Qing era considered the cause of dementia to be a dejection of liver energy, a weakness of stomach energy and phlegm, he described many symptoms and he prescribed 'SeSimTang' and 'ChukBoSunDan'. 3. WangQingRen of the Qing era considered amnesia caused by abnormal brain function, this fact is similar to Western Medicine.

  • Panax ginseng has often been suggested as a geriatric restorative. This study is a double blind crossover trial to test whether ginseng can improve function in subjects with the depression and reduced abilities associated with senescence. 49 screened subjects were given 1500 mg Korean Red ginseng and identical placebo each for ten days, with a washout period in between the dosages. Standard tests of mental function, neuromuscular reflexes, responsiveness, mood and well being were used. The subjects were somewhat better at the co-ordination and speed test ('number copying') but tests of concentration and memory ('Concentration', 'Paired Association', 'Digit Span' and 'Object Learning') gave equivocal results. There were small improvements in mood and well being ('Life Satisfaction' and 'General Health Questionnaire') while the subjects indicated increased energy, alertness and less sleep, but also less happiness on the daily analogue scales. The major result was a highly significant improvement in reactivity, speed and co-oridnation at the tapping test, and the visual, auditory and disjunctive reaction timer. These are the most objective and accurate tests used in this trial. It is therefore concluded that ginseng can increase function in senile individuals. This effect is most easily visualisable in objective psychophysical tests, rather than the more subjective memory and concentration tests.

  • The purpose of this study was to analyze the EEG according to the elderly's tendency to be addicted to smartphone games. We compared the effects of smartphone addiction on mental health such as brain waves, sleep problems and depression through comparative analysis of asymmetry and connectivity in quantitative EEG results. The study participants were two elderly people who were addicted to smartphone game and one elderly person who did not use smartphone (Ed- to confirm: only 3 participants?!). The participant's addiction tendency of smartphone was measured by using the smartphone addiction scale and EEG (QEEG) was used for EEG analysis. The results are as follows. First, the brain waves of elderly people and smartphone non-user elderly who showed symptoms of immersion and smartphone game showed a difference in asymmetry in both opening and closing anisles. Second, there were significant differences in the openness and the anxiety of the elderly who were immersed in the mobile phone and the elderly who did not use the smartphone. Through this, it is also meaningful to explore the relationship between senile cognitive impairment and smartphone use by exploring the effect of smartphone game use on brain cognitive function through comparison of EEG analysis.
