The Effect of Parenting Efficacy, Parenting Stress on Self-efficacy and Daily Stress among Elementary School Children (부모의 양육효능감 및 양육스트레스가 학령기 아동의 자기 효능감과 일상 스트레스에 미치는 영향)

  • The purpose of this study was to examine the effects of parenting efficacy, parenting stress on elementary school children's self-efficacy and daily stress. The subjects were 355 children selected from 4 elementary schools and their mothers. Data was collected using the parenting efficacy index, parenting stress index, children's self-efficacy inventory and children's daily stress inventory. Data was statistically analyzed using the t-test, one way ANOVA(Duncan test), correlation analysis and multiple regression analysis. The study showed that there were some significant differences in children's self-efficacy according to the child's grade, gender, scholastic achievement, mother education, father education, and income. There were some significant differences in children's daily stress according to the child's grade, gender, and scholastic achievement. Correlation analysis indicated that the maternal parenting efficacy and children's self-efficacy, especially self-confidence had a significant positive correlation, and indicated a negative correlation with children's daily stress. Correlation analysis indicated that maternal parenting stress and children's self-efficacy had a significant negative correlation, and indicated a positive correlation with children's daily stress. It was also found that scholastic achievement, educational distress of the mother, father education were significant predictors of the children's self-efficacy, and healthy parenting ability, father education, stress of a difficult child, scholastic achievement, gender were all significant predictors of the children's daily stress.

Incidence of and Factors for Self-reported Fragility Fractures Among Middle-aged and Elderly Women in Rural Korea: An 11-Year Follow-up Study

  • Objectives: This community-based cohort study was performed to investigate the incidence of and factors related to self-reported fragility fractures among middle-aged and elderly women living in rural Korea. Methods: The osteoporosis cohort recruited 430 women 40 to 69 years old in 1999, and 396 of these women were followed over 11 years. In 1999, questionnaires from all participants assessed general characteristics, medical history, lifestyle, menstrual and reproductive characteristics, and bone mineral density. In 2010, self-reported fractures and the date, site, and cause of these fractures were recorded. Cox proportional hazards models were used to calculate hazard ratios (HRs). Results: Seventy-six participants among 3949.7 person-years experienced fragility fractures during the 11-year follow-up. The incidence of fragility fractures was 1924.2 per 100 000 person-years (95% confidence interval [CI], 1491.6 to 2356.8). In the multivariate model, low body mass index (HR, 2.66; 95% CI, 1.13 to 6.24), a parental history of osteoporosis (HR, 2.03; 95% CI, 1.18 to 3.49), and postmenopausal status (HR, 3.50; 95% CI, 1.05 to 11.67) were significantly related to fragility fracture. Conclusions: Fracture prevention programs are needed among postmenopausal, rural, Korean women with a low body mass index and parental history of osteoporosis Korea.

Sleep Quality Evaluation Using Self-Reported Questionnaires in Patients with Burning Mouth Syndrome

  • Purpose: Burning mouth syndrome (BMS) is ambiguous and enigmatic oral condition. Sleep disturbance is one of the most prevalent complaints of patients with chronic pain. The aim of this study was to estimate general sleep characteristics and propensity in patients with BMS. Methods: A total of thirty BMS patients and thirty healthy control subjects were investigated. Self-reported measures of sleep quality were conducted using two widely used methods; the Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS). Data were analyzed with one-way ANOVA, chi-square, Fisher's exact test, Kruskal-Wallis test, Holm method with 95% confidence interval and p<0.05 significant level. Results: BMS patients showed more poor sleepers than those in control subjects in both ESS and PSQI test. BMS patients also showed statistically significant poorer sleep quality compared with control subjects in both test. When BMS group were divided into three groups on the basis of numeric rating scale, the higher score subjects had, the more mean rank they had in the PSQI. Conclusions: BMS patients showed up poor sleep characteristics and propensity than control group, and they also showed the more severe the pain was, the worse the sleep quality was.

Demand Forecast of Tourists Based on Feasibility Rate -Focusing on installation of offshore cable car in Songdo, Busan- (실현율을 이용한 관광 수요 예측 - 부산 송도해상케이블카 설치를 사례를 중심으로 -)

  • Local governments are commercializing natural environment, one of tourist commodities, to ensure that the proceeds from sale of tourist commodities are returned to local residents(Han Yeong-joo, Lee Moo-yong, 2001). In Songdo beach, Busan, cable car dismantled in 1980s due to the run-down state of the facility is poised for restoration in 26 years and can be said to be of great value as tourist commodity of the region and necessitates the demand forecast. To overcome limitations of demand forecast in existing studies, the analysis was made based on feasibility rate(Gruber index, self-confidence index), the realizable predictive value, for the willingness-to-visit rate when forecasting the demand of visitors. The results of demand forecast suggested that number of visitors would range from approximately 550,684 persons to 1,514,416 persons when the target region for demand forecast was confined to Busan Metropolitan City, and was in the range between 1,013,740 persons and 2,854,340 persons when the target region was expanded to cover Busan, Ulsan, and Gyeongnam. Based on the results of this study, estimation of visitors and demand forecast for Songdo offshore cable car restoration which reflect characteristics of Songdo beach of Busan would provide important basis for proceeding with tourism industry development project.

A Structural Model on the Nursing Competencies of Nursing Simulation Learners (간호시뮬레이션 학습자의 간호역량에 관한 구조모형)

  • Purpose: The purpose of this study was to test a model of nursing competencies of nursing simulation learners. The conceptual model was based on the theory of Jeffries's simulaton theory. Methods: Data collection was conducted in October 2017 for 310 students from two nursing universities in Kyungbuk area for 20 days. Data analysis methods were covariance structure analysis using SPSS 21.0 and AMOS 22.0 statistical programs. Results: The hypothetical model was a good fit for the data. The model fit indices were comparative fit index=.97, normed fit index=.94, Tucker-Lewis Index=.97, root mean square error of approximation=.44, and standardized root mean square residual=.04. Teacher factors were directly related to simulation design characteristics, and it was confirmed that the curriculum, classroom operation and teaching method of the instructors were important factors. Learner factors were found to have a direct effect on nursing competence, self-confidence, and clinical performance that belong to nursing capacity. In particular, the results of this study indicate that the simulation design characteristics have a partial mediating effect on learner factors and clinical performance, and a complete mediating effect on learner factors and clinical judgment ability. Conclusion: In order to improve the learner's clinical performance and clinical judgment ability, it is necessary to conduct practical training through nursing simulation besides preparing the learner and the educator.

Relationships between Health Literacy, Knowledge and Self-Care Behaviors in Elderly Patients with Heart Failure (노인 심부전 환자의 건강 문해력에 따른 심부전 지식과 자가 간호)

  • Purpose: This study was to investigate the relationships between among health literacy, knowledge and self-care behavior in elderly patients with heart failure(HF). Methods: 166 patients (age ${\geq}65$ years) with HF were recruited in a cardiovascular center of an university affiliated hospital. The structured questionnaire included Health literacy, Dutch Heart Failure Knowledge Scale, European Heart Failure Self-care Behavior, Self-Care of Heart Failure Index. Data were analyzed by descriptive statistics, Chi-test, Pearson correlation analysis, t-test and ANCOVA using SPSS/WIN version 21.0. Results: Participants with the high level of health literacy were more likely to be younger (p=.001), men (p=.001), with more education (p<.001), and have a job (p=.004), and with a higher economic status (p=.005). The positive correlations between the level of health literacy, knowledge, and self-care behavior were confirmed (p<.001). Participants with the high level of health literacy showed higher level of knowledge, more self-care behavior for health maintenance, and confidence. Conclusion: This study shows that the level of health literacy may influence knowledge and self-care behavior in elderly patients with HF. In order to improve self-care behaviors in elderly patients, a strategic nursing approach based on the level of patients' health literacy needs to be considered.

The Study of Implicit Self-Esteem and Depression and Fashion Competency (암묵적 자존감 및 우울감과 패션능숙성에 관한 연구)

  • Previous research has investigated the influence of explicit self-esteem and depression. These studies did not examine the implicit self-esteem and depression that exist in the internal unconscious of humans that are not influenced by prejudice and social desirability. This study identifies that fashion competency (FC) encourages the implicit self-esteem and relieves the implicit depression. Implicit self-esteem and depression were measured by Implicit Association Test (IAT) utilizing reaction; FC was surveyed based on questionnaires. The data collected were analyzed using factor analysis. FC was then composed of three factors of fashion involvement, fashion innovativeness and self-confidence in fashion coordination. The results of comparing the index values that indicate IA SE (implicit association self-esteem) and IA DE (implicit association depression) of each FC group indicated that a higher FC results in a higher IA SE and lower IA DE; therefore, individuals with a higher fashion competence have a higher implicit self-esteem and a sense of well-being. The findings support previous studies in that the FC tends to be positively related to quality of life in young people.

Determinants of Poor Self-rated Health in Korean Adults With Diabetes

  • Objectives: Self-rated health is a measure of perceived health widely used in epidemiological studies. Our study investigated the determinants of poor self-rated health in middle-aged Korean adults with diabetes. Methods: A cross-sectional study was conducted based on the Health Examinees Study. A total of 9759 adults aged 40 to 69 years who reported having physician-diagnosed diabetes were analyzed with regard to a range of health determinants, including sociodemographic, lifestyle, psychosocial, and physical variables, in association with self-rated health status using multivariate logistic regression models. A p-value <0.05 was considered to indicate statistical significance. Results: We found that negative psychosocial conditions, including frequent stress events and severe distress according to the psychosocial well-being index, were most strongly associated with poor self-rated health (odds ratio $[OR]_{\text{Frequent stress events}}$, 5.40; 95% confidence interval [CI], 4.63 to 6.29; $OR_{\text{Severe distress}}$, 11.08; 95% CI, 8.77 to 14.00). Moreover, younger age and being underweight or obese were shown to be associated with poor self-rated health. Physical factors relating to participants' medical history of diabetes, such as a younger age at diagnosis, a longer duration of diabetes, insulin therapy, hemoglobin A1c levels of 6.5% or more, and comorbidities, were other correlates of poor reported health. Conclusions: Our findings suggest that, in addition to medical variables, unfavorable socioeconomic factors, and adverse lifestyle behaviors, younger age, being underweight or obese, and psychosocial stress could be distinc factors in predicting negative perceived health status in Korean adults with diabetes.

Agreement between Smoking Self-report and Urine Cotinine among Adolescents (청소년 흡연 자가보고와 요코티닌 검사간의 일치도)

  • Objectives : Cotinine, the major metabolite of nicotine, is a useful marker of exposure to tobacco smoke and self-reporting of smoking status is thought not to be reliable. This study aimed to evaluate the agreement between the smoking self-report among adolescents and the urinary cotinine test. Methods : The study subjects were 1226 middle and high school students in Hanam city, who were selected by stratified random sampling. The self-report about smoking behavior was compared with urine cotinine value measured with PBM $AccuSign^{\circledR}fi$ Nicotine(Princeton BioMeditech Corporation, USA). The percentage agreement, kappa and 95% confidence interval(CI) were calculated. Results : The overall percentage agreement was 88.6%, and those for boys, girls, middle school, general school and vocational school students were 87.3%, 90.1%, 93.7%, 85,5%, 90.7%, and 78.4%, respectively. The overall kappa index was 0.46(95% CI=0.39-0.54)for overall, .and those for boys, girls, middle school, general school and vocational school students were 0.56(95% CI=0.48-0.65), 0.20(95% CI=0.07-0.32), 0.21(95% CI=0.09-0.34), 0.55(95% CI=0.47-0.64), 0.42(95% CI=0.33-0.52), and 0.48(95% CI=0.36-0.60), respectively. Conclusion : The percentage agreement was relatively high but the kappa values very low for girls, and middle school students. Though the prevalence bias can be influenced by these results, the self-report was not a sufficient tool for the evaluation of adolescents' smoking status, especially in girls or middle school students.

Association between phytochemical index and metabolic syndrome

  • BACKGROUND/OBJECTIVES: Although previous experimental studies reported the health benefits of foods rich in phytochemicals, few epidemiologic studies have investigated the associations between phytochemicals and metabolic disorders. This study aimed to calculate a phytochemical index (PI) and examine its association with metabolic syndrome in the Korean population. SUBJECTS/METHODS: Data of Korean adults aged ≥ 19 years who participated in the 2008-2016 Korea National Health and Nutrition Examination Surveys were analyzed. The PI was calculated using 24-hour intake recall data regarding whole grains, vegetables, fruits, legumes, nuts and seeds, and soybeans and soy products. Demographic and lifestyle data were obtained using self-administered questionnaires. A multivariable logistic regression was performed to calculate the adjusted odds ratios (ORs) and 95% confidence intervals (CIs) for the prevalence of metabolic syndrome and its components according to PI quintiles. RESULTS: Overall, 31,319 adults were evaluated. Compared with men, women had a higher median PI level (9.96 vs. 13.63) and significantly higher caloric intake levels from most PI components (P < 0.05), except for soy products. After adjusting for multiple confounding variables, participants in the highest PI quintile had significantly lower prevalences of abdominal obesity (OR: 0.90, 95% CI: 0.81-0.99), hyperglycemia (OR: 0.83, 95% CI: 0.74-0.94), high blood pressure (OR: 0.82, 95% CI: 0.73-0.93), hypertriglyceridemia (OR: 0.84, 95% CI: 0.75-0.94), and metabolic syndrome (OR: 0.78, 95% CI: 0.69-0.88). CONCLUSIONS: Higher intakes of phytochemical-rich foods are associated with a lower prevalence of metabolic dysregulation and consequently, cardiometabolic diseases.
