A Case of Cockayne Syndrome with Focal Segmental Glomerulosclerosis (국소성 분절성 사구체 경화증(FSGS)을 동반한 Cockayne 증후군 1례)

  • Cockayne syndrome is a rare autosomal recessive disorder characterized by cachectic dwafism, mental retardation, loss of facial subcutaneous adipose tissue, microcephaly and photosensitive dermatitis. It is associated with renal abnormalities characterized by hyalinization of glomeruli, atrophy of tubules and interstitial fibrosis. To our knowledge, this is the first report of a case of Cockayne syndrome with FSGS in Korea. A 7-year old boy was admitted for evaluation of hypertension and proteinuria, which were detected 2 month ago. He was followed for short stature(<3 percentile), mental retardation(IQ 55), strabismus and dental caries since 3 years ago. He also showed microcephaly, a bird-like face and relatively large hands and feet. Laboratory findings showed decreased creatinine clearance($C_{Cr}$ 76.1 mL/min/$1.73m^2$) and proteinuria(1,548 mg/day). Renal biopsy demonstrated focal segmental glomerulosclerosis of the hilar type with large hyaline deposits, moderate tubular atrophy and interstitial fibrosis. His cardinal features, mental retardation, and renal biopsy findings were consistent with Cockayne syndrome. We report here a very rare case of Cockayne syndrome with FSGS presenting with proteinuria and hypertension.

  • K-SEC (Korean-Spoken English Corpus) is a kind of speech database that is being under construction by the authors of this paper This article discusses the needs of the K-SEC from various academic disciplines and industrial circles, and it introduces the characteristics of the K-SEC design, its catalogues and contents of the recorded database, exemplifying what are being considered from both Korean and English languages' phonetics and phonologies. The K-SEC can be marked as a beginning of a parallel speech corpus, and it is suggested that a similar corpus should be enlarged for the future advancements of the experimental phonetics and the speech information technology.

  • The paper introduces several studies of various aspects of Korean phonology and phonetics for speech recognition and synthesis. The phonological and phonetic studies presented in this paper are : i) For a study of segmental phonology, we made an annotated list of Korean allophones and their corresponding alphabetic symbols to type into computers. ii) For a study of segmental phonetics, we present some acoustic regulations in Korean consonants according to their phonological environment within a word. iii) For a study of prosodic phonology, we suggest the phonological functions of prosodic features and their acoustic cues. iv) For a study of prosodic phonetics, we present the characteristic patterns of accent and intonation in Korean. v) Finally, we suggest some ways of using this phonological and phonetic knowledge for possible improvement of speech recognition and synthesis.

Variables for Predicting Speech Acceptability of Children with Cochlear Implants (인공와우이식 아동 말용인도의 예측 변인)

  • Purposes: Speech acceptability means the subjective judgement of listeners regarding the naturalness and normality of the speech. The purpose of this study was to determine the predicting variables for speech acceptabilities of children with cochlear implants. Methods: Twenty seven children with CI participated. They had profound pre-lingual hearing loss without any additional disabilities. The mean of chronological ages was 8;9, and mean of age of implantation was 2;11. Speech samples of reading and spontaneous speech were recorded separately. Twenty college students who were not familiar to the speech of deaf children evaluated the speech acceptabilities using visual analog scale. 1 segmental (articulation) and 6 suprasegmental features (pitch, loudness, quality, resonance, intonation, and speaking rate) of speech were perceptually evaluated by 3 SLPs. Correlation and multiple regression analysis were performed to evaluate the predicting variables. Results: The means of speech acceptability for reading and spontaneous speech were 73.47 and 71.96, respectively. Speech acceptability of reading was predicated by the severity of intonation and articulation. Speech acceptability of spontaneous speech was predicated by the severity of intonation and loudness. Discussion and conclusion: Severity of intonation was the most effective variable to predict the speech acceptabilities of both reading and spontaneous speech. A further study would be necessary to generalize the result and to apply this result to intervention in clinical settings.

Congenital Esophageal Stenosis (선천성 식도 협착증)

  • Congenital esophageal stenosis (CES) is a narrowing of the esophageal lumen from birth. Three types of CES have been described; tracheobronchial remnants (TBR), membranous web (MW), and fibromuscular stenosis (FMS). We reviewed the clinical features and the surgical outcome of 14 patients, pathologically confirmed as CES. Nine patients had TBR, 3 FMS, and 2 MVV. The mean age at operation was 3.8 years. Five patients were boys and 9 girls. Four patients had other congenital anomalies. Segmental resection of the lesion and end to end anastomosis was utilized in all cases except one who underwent myotomy. The stenotic segment was located at the distal esophagus in all patients. There were 8 complications in 6 patients, but no mortality. The mean follow-up period was 68 months. There were no feeding problems but 3 patients had minor gastroesophageal reflux. Our result indicates that segmental resection and anastomosis is a satisfactory surgical procedure in the management of CES.

Clinical Analysis of Cases of Segmental Resection and Primary Anastomosis in Tracheal Stenosis (기관 절제 및 단단 문합술에 의한 기관 협착증의 치료)

  • Despite improvement in respiratory care, including use of low pressure and high volume cuffed tubes, tracheal stenosis remains a serious complication after a long-term tracheal intubation and tracheostomy. In such patients, tracheal resection and primary anastomosis is still considered ideal therapeutic modality. Between 1989 and 1997, we performed tracheal resections with end-to-end anastomosis on 14 patients with no operative mortality and some morbidity. Tracheal stenosis was caused by tracheostomy in nine patients, by endotracheal intubation in three patients and by thyroid carcinoma in two patients. The length of stenosis was various from 2cm to 4.5cm. All patient underwent segmental tracheal resection and primary anastomosis(14 patients) and additional procedures were cricoid cartilage reconstruction(2 patients), suprahyoid laryngeal release(3patients), carinal release technique(2 patients) and arytenoidectomy(2 patients). We have nine complications: granulona at anastomosis site in four patients, vocal cord palsy in two patients and restenosis, pneumonia, skin necrosis in each of those patients. The granuloma was removed by bronchoscopic forceps(4 patients). Vocal cord palsy was treated by arytenoidectorny(2 patients), restenosis by T-tube insertion, pneumonia by antibiotics and skin necrosis was treated by skin graft. We reviews our expenence of clinical features of tracheal stenosis and surgical treatment by tracheal one-to-end anastomosis with additional procedures to avoid postoperative complications for sucessful results.

NMR Relaxation Study of Segmental Motions in Polymer-n-Alkanes

  • $^{13}C$ spin-lattice relaxation times were measured for n-alkanes of moderate chain length, ranging from n-octane to n-dodecane, under the condition of proton broad-band decoupling within the temperature range of 248-318 K in order to gain some insight into basic features of segmental motions occurring in long chain ploymeric molecules. The NOE data showed that except for methyl carbon-13 dipole-dipole interactions between $^{13}C$ and directly bonded $^1H$ provide the major relaxation pathway, and we have analyzed the observed $T_1data$ on the basis of the internal rotational diffusion theory by Wallach and the conformational jump theory by London and Avitabile. The results show that the internal rotational diffusion constants about C-C bonds in the alkane backbone are all within the range of $10^9\;-10^10\;sec^{-1}$ in magnitude while the mean lifetimes for rotational isomers are all of the order of $10^{-11}\;-10^{-10}$ sec. Analysis by the L-A theory predicts that activation energies for conformational interconversion between gauche and trans form gradually increase as we move from the chain end toward the central C-C bond and they are within the range of 2-4 kcal/mol for all the compounds investigated.

A Feature-based Approach to American English Vowel Production by Korean Learners (한국 학습자들의 미국 영어 모음 발화에 대한 자질적 접근)

  • This study aims to examine Korean learners' production of American English vowel focused on feature analysis. Specifically, the present study adopts feature analysis so that vowel production is analyzed in terms of vowel features as well as overall segmental accuracy. To this end, 22 Korean college students participated in a production test which contained 11 English vowels /i, ɪ, eɪ, ɛ, æ, ɑ, oʊ, ɔ, ʊ, u, ʌ/. The results revealed that the degree of difficulty varied depending on features; the Korean participants showed higher accuracy for front/back features than for tongue height features and tense/lax features. In particular, the participants had more difficulty producing back vowels and non-high vowels than front vowels and high vowels with respect to tongue height features and lip rounding features. Among the individual vowels, /eɪ/ showed the highest accuracy in feature analysis. On the other hand, /ɑ, ɔ, ʌ/ showed low accuracy with respect to height features and lip rounding features, and high vowels /i, ʊ, u/ showed low accuracy with respect to tense/lax features. As for the correlation between the vowel features, tongue height features and lip rounding features are significantly correlated. Also, tongue height features and tense/lax features showed a strong correlation. Finally, pedagogical implications for teaching English vowels were further discussed based on the findings of the current study.

Case Study on the Countermeasure Methods and Collapsed Sources of Segmental Retaining Wall Considering Site Conditions (시공환경을 고려한 블록식 보강토옹벽의 붕괴요인 분석 및 대책방안 사례연구)

  • The geosynthetic reinforced segmental retaining walls(SRW) are improved that the disadvantage of existed retaining wall and the workability in field. Recently, the segmental retaining wall is replacing the exited wall because it is quickly advanced to using by the block in-situ. The use, therefore, is increasing. But, the trends of the large scaled construction was developed that the problems likely to crack and collapse, those are caused of careless in design and construction of SRW not considering about various surrounding conditions. In this study, the cause analysis on destructed SRW was carried out that based on the datum of measured displacement of walls, rainfall features and ground sounding conditions. Also, the analysis of the global slope stability was carried out on collapsed section and non-collapsed section using critical equilibrium method. For the rational stability and analysis of slope including SRW structure, the site conditions including situations of topography, ground and histories of construction and collapse etc should be considered. The rational countermeasure methods for non-collapsed and collapsed areas may be sustained as much as possible current state.

Characteristics of defect on segmental lining of TBM tunnel in operational subway (운용중인 국내 지하철 TBM터널의 세그먼트라이닝 결함특성 분석)

  • The precise inspection for safety and diagnosis (PISD) of tunnel has been conducted by the special act on safety control of public structures. However, the present assessment for the segmental lining of TBM tunnel has limitation such as: NDTs for integrity, segmental configuration for field inspection, and consideration for jacking system. Even if the number of TBM tunnel is less than 1% of enrolled facility in FMS, more attention to maintenance should be necessary due to its usage such as multi-use facility and national important facility. Compared to NATM tunnel, excavated by drilling and blasting and then installed lining by cast-in-place within 6~12 m, TBM tunnel is cut out ground by disk and cutter-bit and then assembled 7 pieces of precast segment, 1.2~1.4 m wide. Different features of design, construction, and maintenance should be considered to be more exact evaluation of TBM tunnel. The characteristics of defect is categorized and analyzed with 11 operational TBM tunnels in domestic subway. To be more comprehend various particular defects, foreign studies have been also adapted. Crack and leakage are categorized in 7 patterns. Breakage/spalling and corrosion are also grouped into 3 patterns. Patterned defects or damages are fed back in design, construction, and are useful guidelines for maintenance stage in future.
