The Determinants of Undiagnosed Hypertension Among Indonesian Adults: A Cross-sectional Study Based on the 2014-2015 Indonesia Family Life Survey

  • Objectives: This study investigated the determinants of undiagnosed hypertension among Indonesian adults. Methods: This study involved an analysis of secondary data from the 2014 Indonesia Family Life Survey (IFLS) on 5914 Indonesian adults (≥40 years). The determinant variables examined in this cross-sectional study were education level, monthly per capita expenditures (PCE), whether the participant experienced headaches in the morning, and other general health variables. The outcome variable was undiagnosed hypertension, which was defined as participants with hypertension who had not received a hypertension diagnosis from a health professional and had never been prescribed medication for treating hypertension. The data were analyzed using logistic regression. Results: A total of 3322 participants (56.2%) were found to have undiagnosed hypertension. The odds ratios (ORs) and 95% confidence intervals (CIs) of undiagnosed hypertension were significantly higher among those who completed primary school or lower (OR, 1.60; 95% CI, 1.29 to 1.98), had low monthly PCE (OR, 1.28; 95% CI, 1.13 to 1.43), did not report experiencing headaches in the morning (OR, 1.97; 95% CI, 1.76 to 2.21), and reported a general health status of healthy (OR, 2.05; 95% CI, 1.82 to 2.30) than those who had a higher education level, had high monthly PCE, experienced headaches in the morning, and were unhealthy. Conclusions: Education level, monthly PCE, the experience of headaches in the morning, and general health status were associated with undiagnosed hypertension. The monitoring system for detecting undiagnosed hypertension cases must be strengthened. Health promotion is also necessary to reduce the prevalence of undiagnosed hypertension.

  • Page kidney refers to the phenomenon of hypertension secondary to long-standing compression of renal parenchyma caused by renal subcapsular collection. The most common cause of renal subcapsular collection is a hematoma which usually occurs after a history of blunt trauma. A 42-year-old female patient who received botulinum toxin injection in her back during chiropractic care was admitted to the emergency room with sudden bilateral flank pain and hypertension. The computed tomography (CT) images demonstrated the presence of bilateral subcapsular renal hematoma. The patient was treated conservatively and recovered well. The follow up CT images showed markedly resolved bilateral hematoma.

기관지 폐 이형성증을 가진 극소 저체중 출생아에서 폐고혈압증 (Pulmonary Hypertension Secondary to Bronchopulmonary Dysplasia in Very Low Birth Weight Infants (<1,500 g))

  • 목적: 극소 저체중 출생아에서 폐고혈압증은 기관지 폐 이형성증의 대표적인 심폐관련 합병증으로 높은 사망률과 합병증과 연관되어 있으나, 이에 대한 연구는 드문 실정이다. 이에 본 저자들은 기관지 폐 이형성증을 가진 극소 저체중 출생아에서 폐고혈압증의 유병률, 경과 및 치료뿐 아니라 예후 및 이와 관련된 인자들에 대해 확인해 보고자 하였다. 방법: 2000년 1월부터 2007년 7월까지 성균관대학교 의과대학 삼성서울병원 신생아 집중치료실에 입원한 출생 체중 1,500g 미만 환아 865명 중 28일 이상 산소 치료를 필요로 하였던220명을 대상으로 하였다. 심장 초음파 검사상 삼첨판 역류 속도 3 m/s 이상이 확인되면서, 우심실 비대 혹은 심실 중격이 평형함이 관찰되는 경우를 폐고혈압으로 진단하였으며, 이들을 대조군과 비교하였다. 환아들의 의무기록을 후향적으로 분석하여, 인구학적, 주산기 인자, 합병증, 사망률을 비교하였으며, 폐고혈압증을 가진 환아 중 사망군과 생존군으로 나누어 이들의 특성 및 관련인자를 분석하였다. 결과: 기관지 폐 이형성증을 가진 VLBWI 218명 중 14명(6%)에서 폐고혈압증이 발생하였으며, 모두 중등도 이상의 기관지 폐 이형성증에서 발생하였다. 폐고혈압증은 평균 생후 133일(40-224일)에 진단 받았으며, 중증 기관지 폐 이형성증, 부당 경량아, 외부 환아는 폐고혈압증 발생과 관련된 인자이다. 기관지 폐 이형성증에서 발생한 폐고혈압증의 사망률은 57%이며, 중증 기관지 폐 이형성증은 사망의 중요한 원인이다. 생존한 환아에서 회복되는데 까지 걸리는 시간은 평균 3개월이었다 결론: 극소 저체중 출생아에서 기관지 폐 이형성증과 관련된 폐고혈압증은 높은 사망률 및 합병증을 보이며, 이들의 발생 및 예후는 기관지 폐 이형성증 정도와 연관성이 있다. 이들의 조기진단을 위해 중등도 이상의 기관지 폐 이형성증 환아에서 정기적인 선별검사 및 퇴원 이후에도 장기간 지속적인 추적 관찰이 필요하다.

  • 신성 고혈압은 소아기 이차성 고혈압의 흔한 원인이며, 그 중 신동맥 협착은 치료 가능한 고혈압의 가장 흔한 원인이다. 신혈관성 고혈압 치료로 약물치료에 반응하지 않는 경우 혹은 초기 단계부터 치료목적으로 경피적 혈관 성형술과 신장동맥 스텐트 등의 중재적 혈관 성형술이나 수술요법이 시행할 수 있다. 죽상 경화형 협착, 다발성의 협착, 미세동맥류, 그리고 협착 부위가 혈관 성형술이나 stent로 교정되기 어려운 경우에는 부분적 혹은 완전 신장동맥 색전술이 시도될 수 있다. 저자들은 신장동맥 색전술을 이용한 renal ablation요법으로 고혈압의 호전을 보인 신혈관 고혈압증 1례를 경험하였기에 보고하는 바이다.

Endothelial-specific deletion of Ets-1 attenuates Angiotensin II-induced cardiac fibrosis via suppression of endothelial-to-mesenchymal transition

  • Cardiac fibrosis is a common feature in chronic hypertension patients with advanced heart failure, and endothelial-to-mesenchymal transition (EndMT) is known to promote Angiotensin II (Ang II)-mediated cardiac fibrosis. Previous studies have suggested a potential role for the transcription factor, ETS-1, in Ang II-mediated cardiac remodeling, however the mechanism are not well defined. In this study, we found that mice with endothelial Ets-1 deletion showed reduced cardiac fibrosis and hypertrophy following Ang II infusion. The reduced cardiac fibrosis was accompanied by decreased expression of fibrotic matrix genes, reduced EndMT with decreased Snail, Slug, Twist, and ZEB1 expression, as well as reduced cardiac hypertrophy and expression of hypertrophy-associated genes was observed. In vitro studies using cultured H5V cells further confirmed that ETS-1 knockdown inhibited $TGF-{\beta}1$-induced EndMT. This study revealed that deletion of endothelial Ets-1 attenuated Ang II-induced cardiac fibrosis via inhibition of EndMT, indicating an important ETS-1 function in mediating EndMT. Inhibition of ETS-1 could be a potential therapeutic strategy for treatment of heart failure secondary to chronic hypertension.

Portal Hypertension of a Delayed Onset Following Liver Abscesses in a 12-Month-Old Infant: A Case Report and Review of the Literature

  • We report a 12-month-old female infant who had a history of neonatal sepsis with liver micro-abscesses that resolved with intravenous antibiotics during neonatal period. During her neonatal admission period, no umbilical vein catheter was inserted. Also, she did not undergo any abdominal surgeries or had a postnatal history of necrotizing enterocolitis. However, the child developed upper gastrointestinal bleeding in form of hematemesis and melena secondary to esophageal varices at the age of 12 months with an extra-hepatic portal vein obstruction with cavernous transformation and portal hypertension subsequently. The child underwent a successful endoscopic injection sclerotherapy. She is now 20-month-old and has portal hypertension but otherwise asymptomatic. We are proposing the possibility of a delayed-onset portal hypertension as a complication of liver abscess and neonatal sepsis.

폐동맥 고혈압을 동반한 후천성 심질환의 술후 폐동맥압의 변화에 대한 연구 (A study of postoperative pulmonary arterial pressure change in the acquired heart disease with pulmonary hypertension)

  • To see the change of pulmonary arterial pressure after mitral valve replacement, postoperative cardiac catheterization and echocardiographies were performed in 12 patients of mitral valvular disease with pulmonary hypertension[systolic pulmonary arterial pressure>50 mm Hg]. The mean follow-up duration was 35.4[range: 15-47] months per patient. The following results were obtained. 1] Preoperative systolic pulmonary arterial pressure value of 66.17\ulcorner10.73mmHg decreased significantly to 29.17\ulcorner6.86mmHg postoperatively[p<0.01]. 2] Preoperative Pp/Ps value of 0.67\ulcorner0.13 decreased significantly to 0.28\ulcorner0.06 postoperatively[p<0.01]. 3] Preoperative PAWP value of 29.00\ulcorner4.02mmHg decreased significantly to 9.92\ulcorner4.27 mmHg postoperatively[p<0.01]. 4] Preoperative LAD value of 5.58\ulcorner1.20cm decreased significantly to 4.37\ulcorner0.67cm postoperatively [p<0.01]. In conclusion, pulmonary arterial hypertension secondary to mitral valvular disease could be reduced to normal range after successful mitral valve replacement.

Pulmonary hypertension in infants with bronchopulmonary dysplasia

  • An increase in the number of preterm infants and a decrease in the gestational age at birth have resulted in an increase in the number of patients with significant bronchopulmonary dysplasia (BPD) and secondary pulmonary hypertension (PH). PH contributes significantly to the high morbidity and mortality in the BPD patients. Therefore, regular monitoring for PH by using echocardiography and B-type natriuretic peptide (BNP) or N-terminal-proBNP must be conducted in the BPD patients with greater than moderate degree to prevent PH and to ensure early treatment if PH is present. In the BPD patients with significant PH, multi-modality treatment, including treatment for correcting an underlying disease, oxygen supply, use of diverse selective pulmonary vasodilators (inhaled nitric oxide, inhaled prostacyclins, sildenafil, and endothelin-receptor antagonist) and other methods, is mandatory.

Occipital Intradiploic Epidermoid Cyst with Intracranial Hypertension

  • Intradiploic epidermoid cysts of the cranium are rare, benign and slow growing lesion. However, these cysts may cause neurologic deficits due to mass effect. Intradiploic epidermoid cysts are thought to be derived from persistent ectodermal remnants present in the cranial bones during embryogenic development. Here, we report a case of an occipital intradiploic epidermoid cyst with posterior fossa extension. The patient developed intracranial hypertension associated with hydrocephalus due to aqueductal stenosis, and venous congestion secondary to compression of the torcular and the transverse sinus. The imaging studies included a computerized tomography[CT] venogram, which is essential for determining the pathophysiological mechanism of the clinical spectrum and for surgical planning. Near total removal was accomplished and the postoperative course was uneventful. Postoperative imaging studies demonstrated a reversal of the neural structures, but no patency of the sinus, which was presumably indicative of prolonged sinus stenosis. The patient gradually improved and was discharged free of symptoms.

Hypertensive crisis following mRNA COVID-19 vaccination in adolescents: two case reports

  • In response to the global coronavirus disease 2019 (COVID-19) pandemic, vaccines were developed and approved quickly. However, numerous cardiovascular adverse events have been reported. We present two adolescent cases who developed a hypertensive crisis following NT162b2 mRNA COVID-19 vaccination. Patient 1 was an 18-year-old male and his systolic blood pressure was 230 mmHg one day after the second vaccine. He was obese. No secondary cause of hypertension other than the vaccine was identified. Patient 2 was an 18-year-old male who complained with palpitation after the first vaccine. His blood pressure was 178/109 mmHg. He had autosomal dominant polycystic kidney disease. Both were treated with continuous infusion of labetalol followed by losartan, and blood pressure was controlled. Patient 2 received second vaccination and his blood pressure did not rise. It is warranted to measure blood pressure in adolescents at high risk of hypertension after NT162b2 mRNA COVID-19 vaccination.
