뇌혈관질환자에게 적용가능한 간호결과 분류체계의 타당성 검증 (Validation of the Nursing Outcomes Classification on Cerebrovascular Patients)

  • Purpose: The purpose of this study was to assess the importance and contribution of 9 nursing outcomes and their indicators that could be applied to cerebrovascular patients. Methods: Data were collected from 175 neurosurgical nurses working at two university affiliated hospitals and five secondary hospitals located in Gwang-ju. The Fehring method was used to estimate outcome content validity(OCV) and outcome sensitivity validity(OSV) of nursing outcomes and their indicators. Stepwise regression was used to evaluate relationship between outcome and its indicators. Results: The core outcomes identified by the OCV were Tissue Perfusion: Cerebral, Nutritional Status, Neurological Status, and Wound Healing: Primary Intention, whereas highly supportive outcomes identified by the OSV were Oral Health, Self-Care: ADL, and Nutritional Status. All the critical indicators selected for Fehring method were not included in stepwise regression model. By stepwise regression analysis, the indicators explained outcomes from 19% to 52% in importance and from 21% to 45% in contribution. Conclusion: This study identified core and supportive outcomes and their indicators which could be useful to assess the physical status of cerebrovascular patients. Further research is needed for the revision and development of nursing outcomes and their indicators at neurological nursing area.

간호사의 직무착근도와 이직의도의 관계에서 조직시민행동의 매개효과 (Mediation Effect of Organizational Citizenship Behavior between Job Embeddedness and Turnover Intention in Hospital Nurses)

  • Purpose: The purpose of the study was to identify the mediating effect of organizational citizenship behavior between job embeddedness and turnover intention in hospital nurses. Methods: Data were collected from 241 nurses in 3 tertiary hospitals and 2 secondary hospitals in Daegu and analyzed using descriptive statistics, Pearson correlations and structural equation model (SEM) with the PASW 18.0 and AMOS 18.0 program. Results: There were negative relationships between job embeddedness and turnover intention (r=-.53, p<.01), and organizational citizenship behavior and turnover intention (r=-.39, p<.01) but a positive relationship between job embeddedness and organizational citizenship behavior (r=.39, p<.01). There was a partial mediating effect of organizational citizenship behavior between job embeddedness and turnover intention (${\chi}^2=91.02$, p<.001). Conclusion: The results of this study indicate that organizational citizenship behavior has a mediating effect on the relationship between job embeddedness and turnover intention in hospital nurses, suggesting that enhancing organizational citizenship behavior is required. in order to promote job embeddedness and reduce turnover intention.

수술실 간호사의 조직사회화와 이직의도 (Organizational Socialization and Intention to Leave in Operating Room Nurses Working at Secondary General University Hospitals)

  • Purpose: To evaluate the relationship between organizational socialization and intention to leave, and to identify factors affecting on intention to leave in operating room nurses. Methods: This study used a descriptive design. Seventy operating room nurses recruited from three hospitals under the same management style, in-service educational system, and working conditions agreed to participate in this study, and completed self-administered questionnaires of the organizational socialization and the intention to leave questionnaire. Respondents were classified by career as advanced beginner, competent practitioner, proficient practitioner and expert practitioner by Benner's stages of nursing proficiency. Data were analyzed with t-test, ANOVA with multiple comparisons, Spearman correlation, and multiple regression analysis. Results: Nurses unsatisfied with the current in-service continuous education had lower organizational socialization and higher intention to leave. In advanced beginners, job performance was lowest and organizational commitment was highest. In expert practitioners, intention to leave was highest. Organizational commitment, interpersonal relationship, identity and burnout were associated with intention to leave. Mutual trust, burnout, and interpersonal relationship were predictors of intention to leave explained 20.8% of variance. Conclusion: The career ladder program to enhance nurses' organizational socialization and intention to stay should be established and expanded for well-experienced operating room nurses.

입원 환자의 건강정보문해력에 관한 연구 (Health Literacy of Inpatients at General Hospital)

  • Purpose: The purpose of this study was to investigate the health literacy of inpatients at general hospitals. Methods: The sample of this study was 157 inpatients from secondary general hospitals located in a J city during April 1st to April 15th, 2011. The linguistic health literacy was measured by Korean Health Literacy Assessment Tool (KHLAT). The functional health literacy was measured by Korean Functional Health Literacy Test (KFHLT). Results: The mean score of the linguistic health literacy was $36.03{\pm}16.33$ (percent of correct answer: 54.6%). The mean score of the functional health literacy was $9.62{\pm}4.75$ (percent of correct answer: 56.6%). The linguistic and functional health literacy were by age, education level, marital status, socioeconomic status and their health status. There was statistically a significant correlation between the linguistic health literacy and the functional health literacy (r=.75, p<.001). Conclusion: The health literacy of inpatients was strongly related to education level and age. There is a need to develop the educational materials for adjusting the health literacy level of inpatients.

  • 본 연구는 간호사의 간호업무수행과 이직의도의 관계에서 조직시민행동의 조절 및 매개효과를 파악하기 위해 수행되었다. 이를 위해 D지역 상급종합병원과 종합병원의 간호사 총 439명에게 자가보고식 설문을 실시하였으며, 자료분석은 PASW 18.0과 AMOS 18.0을 이용하였다. 조직시민행동의 조절효과를 검증하기 위해서는 위계적 회귀 (hierarchial regression analysis)를 실시하였고, 매개효과를 검증하기 위해서는 구조방정식 모델을 사용하였다. 연구결과, 조직시민행동은 간호업무수행과 이직의도와의 사이에서 매개효과가 나타났으나, 조절효과는 나타나지 않았다. 따라서 간호업무수행이 높은 임상 간호사의 이직률을 낮추기 위해서는 조직시민행동을 향상시키는 방안과 간호업무수행과 조직시민행동의 상호작용에 초점을 둔 연구가 필요하다.

병원 간호사의 폭력경험과 전문직 삶의 질과의 관계 (Relationship of Experience of Violence and Professional Quality of Life for Hospital Nurses')

  • Purpose: The purpose of this study was to identify the relation between violence experiences and the professional quality of life for hospital nurses. Methods: The participants for this study were 212 nurses in one general and three special hospitals located in the metropolitan area of Seoul, South Korea. Data gathered through October and November 2013 were analyzed using descriptive statistics and $x^2$ test. Results: Nurses experienced verbal violence, physical threats and physical violence more frequently from patients and their families rather than from doctors or peer nurses. Nurse's compassion satisfaction was low when nurses experienced violence from peer nurses. Burnout was high when nurses experienced violence from doctors, peer nurses, patients and their families. Secondary traumatic stress was affected by violence from patients and their families. The professional quality of life of nurses was associated with violence from doctors, peer nurses, patients and their families. Of the nurses, 69.3% answered that formation of a positive organizational culture would be the most effective measure for prevention of violence in hospitals. Conclusion: The formation of positive organizational culture, development of violence intervention policies and education are crucial to improve the professional quality of hospital nurses' life.

2차와 3차 병원에서 외상 치료의 적정성 비교 (Comparison of the Trauma Outcome Between Secondary and Tertiary Hospitals)

  • Background: This study compared the performance of trauma care in an urban and a suburban hospital before and after the enhancement of emergency and intensive care. Method: The medical records of patients who were admitted to the intensive care unit following trauma from 1994 to 1995 and from 2002 to 2003 were examined. The standardized W (Ws), the 95% confidence interval (CI) of the Ws, and the predicted survival rate (Ps) were calculated. During each period, each hospital's actual survival rate was compared with the 95% CI of the Ps according to the revised trauma score (RTS) and injury severity score (ISS). Spell out RTS and ISS. Result: From 1994 to 1995, 225 and 121 records from the urban and the suburban hospitals were reviewed, respectively. The 95% CI's of the Ws were -2.30 to 2.73 and -11.40 to -5.90, respectively. The actual survival rate of the suburban hospital was significantly lower than the predicted survival rate at all RTS. From 2002 to 2003, 315 and 268 records from the urban and the suburban hospitals were reviewed, respectively. The 95% CI's of the Ws was -3.56 to 0.24 and -3.73 to 0.26, respectively. There was no difference between the actual survival rate and the predicted survival rate. Conclusion: An enlargement of the capacities of emergency and intensive care may improve the performance of trauma care at a small suburban hospital.

  • 본 연구는 임상 간호사의 간호업무수행과 이직의도 간의 관계에서 직무착근도의 조절효과와 매개효과를 파악하기 위해 수행되었다. 대상자는 D시에 근무하는 200병상 이하의 3개의 병원 196명과 600병상 이상 2개의 종합병원 216명, 그리고 800병상 이상 3개의 상급종합병원에 근무하는 간호사 241명으로 총 653명이었다. 자료분석은 PASW 18.0과 AMOS 18.0을 이용하여 Pearson's correlation coefficient, 위계적 회귀(hierarchial regression analysis), SEM을 시행하였다. 연구결과, 직무착근도는 간호업무수행과 이직의도의 관계에서 매개효과가 있었으나 간호업무수행과 이직의도 사이에서 조절효과는 나타나지 않았다. 따라서 임상 간호사의 이직률을 낮추고 간호업무수행을 높이기 위해서는 직무착근도를 향상시키는 방안이 강구되어야 할 것으로 사료된다.

Pediatricians' perception of factors concerning the clinical application of blockchain technology to pediatric health care: a questionnaire survey

  • Background: Interest in digital medical information has increased because it allows doctors to easily access a patient's medical records and provide appropriate medical care. Blockchain technology ensures data safety, reliability, integrity, and transparency by distributing medical data to all users over a peer-to-peer network. This study attempted to assess pediatricians' thoughts and attitudes toward introducing blockchain technology into the medical field. Methods: This study used a questionnaire survey to examine the thoughts and attitudes of 30- to 60-year-old pediatricians regarding the introduction of blockchain technology into the medical field. Responses to each item were recorded on a scale ranging from 1 (never agree) to 7 (completely agree). Results: The scores for the intentions and expectations of using blockchain technology were 4.0 to 4.6. Pediatricians from tertiary hospitals responded more positively (4.5-4.9) to the idea of using blockchain technology for hospital work relative to the general population (4.3-4.7). However, pediatricians working in primary and secondary hospitals had a slightly negative view of the application of blockchain technology to hospital work (p=0.018). Conclusion: When introducing the medical records of related pediatric and adolescent patients using blockchain technology in the future, it would be better to conduct a pilot project that prioritizes pediatricians in tertiary hospitals. The cost, policy, and market participants' perceptions are essential factors to consider when introducing technology in the medical field.

Primary Arthroplasty for Unstable and Failed Intertrochanteric Fractures: Role of Multi-Planar Trochanteric Wiring Technique

  • Purpose: The primary objective of the current study is to demonstrate the trochanteric wiring technique. A secondary objective is to evaluate the clinico-radiological outcomes of use of the wiring technique during primary arthroplasty for treatment of unstable and failed intertrochanteric fractures. Materials and Methods: A prospective study including follow-up of 127 patients with unstable and failed intertrochanteric fractures who underwent primary hip arthroplasty using novel multi-planar trochanteric wiring was conducted. The average follow-up period was 17.8±4.7 months. Clinical assessment was performed using the Harris hip score (HHS). Radiographic evaluation was performed for assessment of union of the trochanter and any mechanical failure. P<0.05 was considered statistically significant. Results: At the latest follow-up, the mean HHS showed significant improvement from 79.9±1.8 (at three months) to 91.6±5.1 (P<0.05). In addition, no significant difference in the HHS was observed between male and female patients (P=0.29) and between fresh and failed intertrochanteric fractures (P=0.08). Union was achieved in all cases of fractured trochanter, except one. Wire breakage was observed in three patients. There were five cases of limb length discrepancy, three cases of lurch, and three cases of wire-related bursitis. There were no cases of dislocation or infection. Radiographs showed stable prosthesis in situ with no evidence of subsidence. Conclusion: Use of the proposed wiring technique was helpful in restoring the abductor level arm and multi-planar stability, which enabled better rehabilitation and resulted in excellent clinical and radiological outcomes with minimal risk of mechanical failure.
