Clinical Study of Tracheocutaneous Fistula (기관피부누공에 대한 임상적 고찰)

  • The tracheocutaneous fistula(TCF) may develop infrequently as a complication after tracheostomy. Prolonged tracheostomy tube dependence increases the risk of TCF developing, and in growth of stratified squamous epithelium lines the furrow connecting the tracheal mucosa and the skin, accounting for persistence of the fistulous tract. Such fistulas are a nuisance and create nursing and social problems including poor hygiene, aspiration, difficulty with speech, and depletion of pulmonary reserve. Surgical closure has generally been successful by primary closure, fistulectomy with primary closure, and closure by secondary intention following excision of the tracheocutaneous fistula. No large series compares the efficacy of these techniques and each has its own merits. Recent literature has purposed to minimizing complications. For ten years, from January 1985 to December 1994, the authors experienced 25 cases of TCF which were analyzed in respect to incidence and interval of cannulation, duration between decanulation and fistular closure, precedent disease, closure methods, and complications of TCF repair.

ORBITAL SUBPERIOSTEAL ABSCESS SECONDARY TO ODONTOGENIC INFECTION OF LOWER MOLAR : A CASE REPORT (하악대구치의 치성감염으로 유발된 안와골막하농양의 치험례)

  • A 61-years old man with diabetes mellitus(DM) was admitted to our hospital, complaining of progressive right periorbital swelling, headache and toothache on the right lower second molar. On the first visit, moderate swelling was noticed from the right periorbital region with exophthalmos and subconjunctival effusion. Intraorally, right lower second molar had a severe periodontal disease and fistular formation on its distal area. From 3 days after hospitalization, the visual acuity of his right eye was gradually worsen and we performed CT scan. CT scan demonstrated an inflammatory change at the right orbit with subperiosteal abscess at the inferior orbital wall, which was extended from the right infratemporal, parapharyngeal and internal pterygoid space. Patient was treated by mean of intraoral(right upper vestibular and retromolar) and extraoral(infraorbital) incision and drainage, massive anti-therapy and DM control. The patient improved gradually and finally was discharged from the hospital, but his visual loss of right side was not recovered.

Frailty and its related Factors in Vulnerable Elderly Population by Age Groups (취약계층 노인의 연령별 허약정도와 관련 요인)

  • Purpose: This study aimed to investigate factors affecting frailty by age groups among vulnerable elders in Korea. Methods: In this secondary analysis, data were collected from records for 22,868 elders registered in the Visiting Health Management program of Public Health Centers in 2012. Health behaviors, clinically diagnosed disease, frailty, depression and cognitive condition were assessed. Data were analyzed using stepwise regression to determine the associated factors of frailty by age group. Results: Alcohol consumption, physical activity, number of diseases, DM, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors significantly associated with frailty among the elders aged 65~74 (F=135.66, p <.001). Alcohol consumption, physical activity, CVA, arthritis, urinary incontinence, depression and cognitive condition were found to be factors associated with frailty in the elders aged 75~84 (F=245.40, p <.001). Physical activity, CVA, arthritis, depression and cognitive condition were factors associated with frailty in the elders over 85 years of age (F=96.48, p <.001). Conclusion: The findings show that frailty of elders and associated factors were different by age group, and common factors affecting frailty were physical activity, CVA, arthritis, depression and cognitive condition. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to age group.

Report of two cases of osteopetrosis with maxillary osteomyelitis in siblings (상악 골수염을 동반한 골화석증의 증례보고)

  • Osteopetrosis is a rare bone disease characterized by systemic osteosclerosis due to an osteoclast dysfunction that decreases bone resorption. This report demonstrates two cases of adult osteopetrosis with secondary osteomyelitis of the maxilla, in siblings who are 43-year old female and 55-year old, male respectively. The common radiographic features of these cases were increased radiopacity in skull, rib and vertebra. The radiographic features that differed between these two cases were the osteosclerotic pattern of the jaw bones, that is, diffuse patterns in the female case, while the male case showed nodular patterns that were confined to the root apices. The diagnosis of osteopetrosis may be complicated due to the varying degree of osteosclerosis on panoramic radiograph. Additional radiographs such as the chest and skull radiograph may be helpful for the diagnosis of osteopetrosis.

  • Bronchial stump aspergillosis(BSA) is an infrequent disease developing on the bronchial stump after lobectomy with the silk as a suture material. The silk induces inflammatory reaction, granuloma formation, secondary infection of the bronchial stump and leads to BSA. If a patient complains of cough, hemoptysis, expectoration of suture material several months or years after lobectomy with the silk, BSA should be considered as one of causative mechanisms. Bronchoscopic identification of silk thread with aspergillosis and its removal is necessary. We report a case of BSA in a 59-year-old man with review of the literature.

  • Background : Airway obstruction due to blood clot occurs unusually but in a variety of clinical settings. Initial efforts for removal of the endobronchial blood clot involve flexible bronchoscopic evaluation with saline lavage and suctioning and then forceps extraction. If unsuccessful, further options include rigid bronchoscopy, Fogarty catheter dislogement of the clot, and topical thrombolytic agents. The several successful uses of endobronchial streptokinase or urokinase to dissolve an endobronchial blood clot have been previously reported, but not yet in Korea. Herein we describe a 51-year old man with superior vena cava thrombosis secondary to Behcet's disease who experienced life threatening airway obstruction after hemoptysis due to a large organized blood clot in left main bronchus. Urokinase(260,000 U), injected through a fiberoptic bronchoscope, totally dissolved the clot. No complications occurred.

Medicinal potential of Panax ginseng and its ginsenosides in atopic dermatitis treatment

  • Atopic dermatitis (AD) is a chronic and relapsing inflammatory disease that affects 1%-20% of people worldwide. Despite affecting many people, AD current treatments, such as corticosteroids and calcineurin inhibitors, have not only harmful secondary effects but are also often ineffective. Therefore, natural nontoxic compounds are on high demand for developing new effective AD treatments. Panax ginseng Meyer has been used traditionally for its promising healing and restorative properties to treat many diseases including skin disorders, reason why in this review we want to explore the research performed with AD and P. ginseng as well as determining its potential for new drug development. Previous researches have shown that P. ginseng has positive effects in AD patients such as lower eczema area and severity index, transepidermal water loss, and immunoglobulin E levels and better quality of sleep. In vivo animal models, as well, have shown positive results to P. ginseng and derived ginsenosides, such as the decrease of transepidermal water loss, immunoglobulin E levels in serum, allergy-related cytokines, and downregulation of NF-κB, MAPK, and Ikaros pathways. All of these previous data suggest that P. ginseng and its derived ginsenosides are undoubtedly a nontoxic effective option to treat AD.

One Case of Patient Diagnosed as the Carvenous Angioma (해면상혈관종 의증(疑症) 환자 경과 1례 보고)

  • The carvenous angioma is the secondary common disease which occupies about 8-15% of most central nervous systemic disorders following to the arterio-venous malformation. According to the damaged functional area, the patients experience some sort of motor weakness, seizure or hemorrhage. Nevertheless, general operations and radiologic methods are not considerable in those clinical cases mostly. Typicaliy, the conservative treatments are mainly recommendable choice as the other clinical options have variable side effects like post operative complications. in this case, a 73 year-old male diagnosed as the carvenous angioma, his motor weakness resulted from carvenous angioma has improved with oriental medical treatments. So, the authors report this case with a brief review of related lieratures.

  • Nephrotic syndrome is a common chronic disease in childhood. Patients with nephrotic syndrome are at an increased risk of bacterial Infections due to immunological changes secondary to proteinuria. Primary bacterial peritonitis is one of the most serious and common infectious complications. The rate of peritonitis is, 2-6% and overwhelming infection still carries a mortality rate of 1.5%. We experienced a rare case of nephrotic syndrome complicated with severe peritonitis and Peritoneal empyema in a 10-year old girl after 2 months of medical neglect by parents. Here we emphasize thf: importance of early detection and treatment of peritoneal infection in nephrotic syndrome.

  • Stroke patients have reduced aerobic capacity. Therefore, intensive structured exercise programs are needed. We report the case of a patient with stroke and cardiac disease who underwent early inpatient cardiac rehabilitation (CR). A 38-year-old male patient with atrial fibrillation, heart failure, and cerebral infarction underwent a symptom-limited exercise tolerance test (ETT) without any problems on day 45 after admission. He completed a 2-week inpatient program and an 8-week home-based CR program. Follow-up ETT showed increased exercise capacity. The present case might be the first to report a safely performed CR program in a patient with stroke and cardiac comorbidity in Korea. Systematic guidance is needed for post-stroke patients to receive safe and effective CR for the secondary prevention of stroke and cardiovascular risk.
