Immune thrombocytopenia associated with sarcoidosis (면역성혈소판감소증을 동반한 사르코이드증)

  • Sarcoidosis is a systemic disease of unknown cause involving multiple organs and is characterized by noncaseating granuloma. Immune thrombocytopenia (ITP) is an autoimmune disease characterized by increased peripheral platelet destruction due to the presence of an antibody to the platelet and abnormal platelet production. There is no known pathogenesis that occurs concurrently with ITP and sarcoidosis. However, considering together of 2 known pathogenesis, abnormal immune response triggers either ITP or sarcoidosis. The disease that develops first stimulates secondary disease. After development of secondary disease, they stimulate each other. A few cases of ITP associated with sarcoidosis are well documented in English; however, the disease has rarely been reported in Korea. Here, we report on a case of ITP with sarcoidosis in a 29-year-old man. He suffered from easy bruising. The chest X-ray and the contrast-enhanced computed tomography scan showed bihilar lymphadenopathy and reticulonodular infiltrates. Bone marrow study and fluoroscopy-guided percutaneous needle biopsy were performed and the patient was diagnosed with sarcoidosis and ITP. He was put on 400 mg/kg of intravenous immunoglobulin for 5 days and administered oral steroids and further follow-up will be carried out. He has shown a good response without significant bleeding event. However, administration of more oral steroid and additional follow-up is required than for single disease, whether sarcoidosis or ITP.

Distribution Frequency of Pathogenic Bacteria Isolated from Cutaneus Leishmaniasis Lesions

  • Cutaneous leishmaniasis (CL) is a parasitic disease characterized by single or multiple ulcerations. Secondary bacterial infections are one of the complications that can increase the tissue destruction and the resulting scar. To better determine the incidence of real secondary bacterial infections in CL, we designed the current study. This was a cross-sectional study performed in Skin Diseases and Leishmaniasis Research Centre, Isfahan, Iran. A total of 1,255 patients with confirmed CL enrolled in the study. Sterile swaps were achieved for ulcer exudates and scraping was used for non-ulcerated lesions. All samples were transferred to tryptic soy broth medium. After 24 hr of incubation at $37^{\circ}C$ they were transferred to eosin methylene blue agar (EMB) and blood agar. Laboratory tests were used to determine the species of bacteria. Among 1,255 confirmed CL patients, 274 (21.8%) had positive cultures for secondary bacterial infections. The bacteria isolated from the lesions were Staphylococcus aureus in 190 cases (69.3%), coagulase negative Staphylococcus in 63 cases (23.0%), E. coli in 10 cases (3.6%), Proteus sp. in 6 cases (2.2%), and Klebsiella sp. in 5 cases (1.9%). The results show that the overall incidence of secondary bacterial infections in the lesions of CL was 21.8%, considerably high. The incidence of secondary bacterial infections was significantly higher in ulcerated lesions compared with non-ulcerated lesions.

Knowledge and Views of Secondary School Students in Kuala Lumpur on Cervical Cancer and its Prevention

  • Cervical cancer is one of the most frequent cancers in women worldwide. Persistent infection with a human papillomavirus (HPV) is the main cause for cervical cancer. Vaccination and Pap smear screening are the best methods for prevention of the disease. The objective of this cross-sectional study was to assess the knowledge and views of upper secondary school female students in Kuala Lumpur, Malaysia, toward prevention of cervical cancer. This study was conducted from April 2009 to September 2009 in 8 schools in Kuala Lumpur area using pre-tested and validated questionnaires. Results indicated that the respondents had low knowledge of cervical cancer and its prevention although the majority of students (80.4%) had heard about the disease. The level of knowledge of cervical cancr and its prevention was significantly higher among students from the science stream (p<0.001) compared to students from the art stream. Most students (69.3%) agreed to take the vaccination if the service was available in schools. A high percentage of students (82.2%) agreed that the vaccination should be compulsory to the students. In conclusion, most students had low knowledge of cervical cancer and its prevention but they had positive attitude toward vaccination and agreed that vaccination should be compulsory. Therefore, suitable educational programmes should be developed to improve the knowledge of secondary school students on the prevention of cervical cancer.

A Case of Unilateral Vocal Fold Paralysis Secondary to Sarcoidosis (사르코이드증에 의한 일측 성대 마비 1예)

  • Sarcoidosis is a multisystem granulomatous disease of unknown etiology. Vocal fold paralysis secondary to sarcoidosis is extremely rare but it can develop as a result of compressive lymphadenopathy, granulomatous infiltration, and neural involvement. We report the case of a 56-year-old woman who presented with unilateral vocal fold paralysis and enlarged supraclavicular lymph nodes. Computed tomography of the neck revealed multiple, enlarged, and matted lymph nodes at the cervical level of IV. An ultrasound-guided core needle biopsy of the lymph node was performed, and a histopathological diagnosis of sarcoidosis was made by validating the presence of noncaseating granuloma. After implementation of steroid therapy, the patient exhibited immediate recovery from vocal fold paralysis. Although an extremely rare disease, sarcoidosis should be included in the differential diagnosis of vocal fold paralysis. Accurate diagnosis and prompt steroid treatment may reduce the morbidity of patients with vocal fold paralysis secondary to sarcoidosis.

  • Recent studies showed that air pollution is a risk factor for hospitalization for chronic obstructive pulmonary disease (COPD). However, there is limited evidence to suggest which subpopulations are at higher risk from air pollution. This study was undertaken to examine the modifying effect of specific secondary diagnosis (including hypertension, diabetes, pneumonia, congestive heart failure) on the relationship between hospital admissions for COPD and ambient air pollutants concentrations. Hospital admissions for COPD and ambient air pollution data for Taipei were obtained for the period from 1999-2009. The relative risk of hospital admissions for COPD was estimated using a case-crossover approach. None of the secondary diagnosis we examined showed much evidence of effect modification.

Hyperplastic Autotransplanted Parathyroid Tissue Migrating into Fatty Tumor after Total Parathyroidectomy

  • Secondary hyperparathyroidism (SHPTH) occurs commonly in patients with end-stage renal disease (ESRD). Uncontrolled SHPTH is associated with complications of calcium deposition including calciphylaxis and elevated rates of cardiovascular morbidity. Current treatment recommendations for medically refractory disease include total parathyroidectomy, often with autotransplantation (TPTH+AT) of minced parathyroid gland. Surgical intervention is associated with a reduction in cardiovascular mortality. We report a case of a 56-year-old man with ESRD who developed SHPTH and underwent TPTH+AT of parathyroid tissue into the right brachioradialis muscle. Over the course of 7 years he developed a mass at the site of the autotransplanted gland as well as recurrent refractory hyperparathyroidism with increased forearm uptake noted on sestamibi scan. After excision of this mass, pathology demonstrated hyperplasia of the minced gland fragments which were embedded within a mass of fibroadipose tissue rather than the muscle tissue it was originally transplanted in.

  • In the event of a fatal infectious disease in the country, it is very important that the government respond quickly and prevent the secondary infection of the people to prevent the subsequent spread of damage. However, in order to detect infectious diseases in existing medical institutions, and to reach the KCDCP(Korea Centers for Disease Control and Prevention) a total of four steps must be taken. In this paper, we simplifies the existing reporting process using the open blockchain. In addition, not only infectious disease related organizations share infectious disease information on the blockchain, but also grant access to the blockchain to ordinary citizens. By sharing information quickly and transparently revealing the process, we can add credibility to the response to the outbreak and official announcements. The public can also build efficient next-generation defense systems by checking information on the blockchain to prevent secondary infections.

Klebsiella pneumoniae infection secondary to bovine viral diarrhea in two prematurely born calves

  • This paper describes the development of neurological signs of two prematurely born calves four days after birth. The pathological examination results indicated fibrinopurulent polyserositis, including meningoencephalitis with suppurative bronchopneumonia. Bovine viral diarrhea virus subtype 2a was detected in most of the internal organs, and the bacterial colonies cultured from the samples were identified as Klebsiella (K.) pneumoniae. Molecular analysis via multilocus sequence typing identified a different K. pneumoniae isolate in each calf-type 14 in calf A and type 65 in calf B. This is the first report identifying K. pneumoniae sequence types 14 and 65 in cattle.

  • Candida osteomyelitis affecting maxillofacial bones has been scantly documented in the literature. Infantile osteomyelitis is an uncommon and life-threatening disease. Candida osteomyelitis causes significant morbidity. The present report describes a case of a 9-month-old infant with infantile osteomyelitis secondary to candida infection. This report describes its presentation and the management of palatal fistula in an infant.

A survey of newcastle disease virus antibody titers on slaughtered broiler chicks (도축 육계에서 뉴캣슬형 바이러스에 대한 혈중항체가 조사)

  • Hemagglutination inhibition(HI) titers of Newcastle disease(ND) were measured to investigate the vaccination times on three different species of broiler chicks in Chonbuk province. Each 330 of Cobb, Ross and White-semi broiler chicks were selected from 11 broiler farms. The primary vaccine were sprayed in hatchery at one day old chicks. Secondary and tertiary vaccine were used by drinking water at 7 to 24 days old chicks. The ND antibody titer were measured by HI from each different species of broiler chicks at the marketing date. Total average HI titers of Cobbs vaccinated with primary ones, secondary and tertiary ones were recorded 1.86, 1.52 and 2.76, respectively. The antibody titers were shown to 2.22, 2.13, 3.07 in terms of vaccination of Ross broiler chicks. They were also 2.56, 2.65 and 2.78 in terms of vaccination of White-semi broiler chicks. The value HI titer were not statistically different of all treatments. The results of this experiment suggested that HI titer of sera is scored less than defensive value of ND antibody titer at more than two times of vaccination.
