조혈모세포이식 (Hematopoietic stem cell transplantation : overview for general pediatrician)

  • Hematopoietic stem cell transplantation (HSCT) has expanded and evolved substantially in the last decades to treat various malignant and nonmalignant diseases. However, the conditioning regimen can lead to transplantation related death by major organ dysfunction, severe infection and bleeding. In the allogeneic setting, graft versus host disease may also develop, making post-transplant management complex. To overcome these problems, new stem cell sources, stem cell mobilizing agents and new skills, nonmyeloablative stem cell transplantation including reduced intensity stem cell transplantation has been introduced in clinical practice, but problems remained so far. Recipients of stem cell transplant may be severely immunocompromised for many months after transplantation. Furthermore, long-term complications (endocrine, metabolic, relapse, second malignancies, etc) can develop. Pediatrician is open called on to participate in the evaluation and consideration of patients for possible transplant and long-term follow-up of HSCT patients. This review is intended as a basic overview of HSCT relevant to general pediatrician.

Differential Subsampling with Cartesian Ordering Contrast-Enhanced Magnetic Resonance Angiography for the Preoperative Assessment of Anterolateral Thigh Flap

  • Objective: To investigate the clinical application of differential subsampling with Cartesian ordering (DISCO) contrast-enhanced (CE) magnetic resonance angiography for anterolateral thigh (ALT) flap transplantation, using operative findings as a reference. Materials and Methods: Thirty patients (21 males and nine females; mean age ± standard deviation, 45.5 ± 15.6 years) who were scheduled to undergo reconstruction with ALT flaps between June 2020 and June 2021 were included in the prospective study. Before ALT flap transplantation, patients were scanned using CE-DISCO imaging. All acquired DISCO images of the 60 lower limbs (both sides from each patient) were analyzed using maximum intensity projection and volume rendering methods. Two experienced radiologists were employed to examine the patterns of the lateral circumflex femoral artery (LCFA), its branches, and perforators and their skin termini, which were compared with the operative findings. Results: Using CE-DISCO, the patterns of the LCFA and its branches were clearly identified in all patients. Four different origins of the LCFA were found among the 60 blood vessels: type I (44/60, 73.3%), type II (6/60, 10.0%), type III (8/60, 13.3%), and type IV (2/60, 3.3%). Owing to a lack of perforators entering the skin, two patients did not undergo ALT flap transplantation. For the remaining 28 patients, the ALT flaps in 26 patients were successfully operated without flap reselection during the operation, while the remaining two patients underwent other surgical procedures due to the thin diameter of the perforator or injury of the perforator during the operation. The success rate of flap transplantation was 92.8% (26/28). All transplanted flaps exhibited good blood supply and achieved primary healing without infection or delayed healing. Conclusion: CE-DISCO imaging can be an effective method for preoperative perforator imaging before ALT flap transplantation.

조혈모세포이식 후 생착 실패나 재발한 소아환자에서 2차 이식의 의의 (Second allogeneic hematopoietic stem cell transplantation in children to overcome graft failure or relapse after initial transplant)

  • 목 적 : 조혈모세포이식은 혈액암뿐만 아니라, 유전 질환, 면역질환에서 완치의 방법으로 널리 사용되고 있다. 그러나 생착 실패나 재발로 인해 조혈모세포이식이 실패하는 경우가 종종 있고, 이때는 장기 생존이 거의 불가능한 것으로 알려져 있으며, 치료방법도 정립되지 않았다. 이에 본 저자들은 1차 이식이 실패한 경우 치료법으로써 2차 이식의 의의에 대해 알아보고자 하였다. 방 법 : 1991년 5월부터 2004년 12월까지 전남대학교병원 소아과에서 조혈모세포이식을 시행한 115례 중에서 생착 실패나 재발로 2차 이식을 시행한 비혈액암 8례(재생불량성빈혈 7례, 부신백질이영양증 1례)와 혈액암 7례(급성골수성백혈병 3례, 급성림프구성백혈병 2례, 만성골수성백혈병 1례, 골수이형성증후군 1례), 총 15례의 의무기록지를 바탕으로 치료 방법, 합병증, 치료성과 등에 대해 분석 하였다. 결 과 : 비혈액암의 경우 2례는 일차성 생착 실패, 5례는 후기 이식편 거부, 1례는 Fanconi 빈혈로 1차 이식 후 급성골수성백혈병으로 전환되어 2차 이식을 시행하였다. 1차 이식 후 이식편기능 부전까지 기간은 중앙값 130.5일(범위, 59-279일)이었고, 1차와 2차 이식 사이의 간격은 중앙값 348일(범위, 86-1,875일)이었다. 전처치는 7례에서 cyclophosphamide를 기본으로 하였고, fludarabine을 3례에서, 방사선 조사를 2례에서 사용하였다. 이식원은 6례에서 조직형 일치 형제간 말초조혈모세포를, 1례에서 조직형 일치 형제간 골수를, 1례에서 제대혈을 사용하였고, 제대혈을 사용한 1례를 제외한 7례는 2차 이식의 공여자가 1차 이식 시와 동일하였다. 급성 이식대숙주병은 1례에서 Grade IV로 발생하였다. 총 8례 중 2례가 사망하였고, 1례는 생착 실패하였으나 생존하여, 5년 Kaplan-Meier(K-M)전체생존율은 75.0%, 무병생존율은 62.5%이었다. 혈액암의 경우 모두 재발로 인해 2차 이식을 시행하였다. 1차 이식 후 재발까지 기간은 중앙값 174일(범위, 90-1,474일)이었고, 1차 이식과 2차 이식 사이의 간격은 중앙값 319일(범위, 178-1,715일)이었다. 전처치는 5례에서 fludarabine, busulfan, antithymocyte globulin 병합요법을, 1례는 busulfan, ara-C, idarubicine 병합요법을, 나머지 1례는 melphalan, busulfan 병합요법을 사용하였다. 이식원은 5례는 1차 이식과 동일한 공여자의 말초조혈모세포였고, 나머지 2례는 제대혈을 사용하였다. grade II 이상의 급성 이식대숙주병은 2례에서 발생하였다. 사망한 5례 중 4례는 재발로, 나머지 1례는 이식관련 합병증으로 사망하였다. 2년 K-M 전체생존율과 무병생존율은 각각 28.6%이었다. 결 론 : 조혈모세포이식을 시행하고 이식편 거부나 재발이 된 경우 2차 이식은 일부 환자에서 장기 생존을 가능하게 하는 방법이 될 수 있다. 비혈액암 질환, 특히 재생불량성빈혈에서 생착 실패를 보이는 경우 2차 이식을 시행하는 것을 추천할 수 있지만, 적절한 전처치와 이식원에 관한 연구가 더 필요하겠고, 혈액암 질환에서 재발한 경우에는 소수에서만 장기 생존이 가능하므로 더욱 효과적인 항백혈병 치료가 필요할 것으로 사료되었다.

신장이식 면역억제요법 가이드라인 개발을 위한 권고안 적용 범위 및 핵심질문 선정 (Formulation of the Scope and Key Questions of the Guideline Recommendations for Immunosuppressive Treatment in Kidney Transplantation)

  • Background: Although a growing number of guidelines and clinical researches are available for immunosuppressive treatment of post-transplantation, there is no clinical practice guideline for the care of kidney transplant recipients in Korea. Selection of a researchable question is the most important step in conducting qualified guideline development. Thus, we aimed to formulate key questions for Korean guideline to aid clinical decision-making for immunosuppressive treatment. Methods: Based on previous published guidelines review, a first survey was constructed with 29 questions in the range of immunosuppressive treatments. The experts were asked to rate the clinical importance of the question using a 5-point Likert scale. The questions reached 60% or more from the first survey and additional new questions were included in the second survey. In analyzing the responses to items rated on the 9-point scale, consensus agreement on each question was defined as 75% or more of experts rating 7 to 9. Results: In the first survey, 50 experts were included. Among the 29 questions, 27 were derived to get 60% or more importance and 3 new questions were additionally identified. Through the second survey, 9 questions were selected that experts reached consensus on 75% and over of the options. Finally, we developed key questions using PICO (patient, intervention, comparison, and outcome) methodology. Conclusion: The experts reached a high level of consensus on many of key questions in the survey. Final key questions provide direction for developing clinical practice guideline in the immunosuppressive treatment of transplantation.

치근단 완성된 치아의 자가이식 (Autogenous transplantation of tooth with complete root formation)

  • Purpose: Autogenous transplantation of teeth can be defined as transplantation of teeth from one site to another in the same individual, involving transfer of impacted or erupted teeth into extraction sites or surgically prepared sockets". Successful autogenous transplantation of teeth depends upon a complex variety of factors. Such factors include damage to the periodontal ligament of the donor tooth, residual bone height of the recipient site, extra-oral time of tooth during surgery. Schwartz and Andreasen previously reported that autogenous transplantation of teeth with incomplete root formation demonstrated higher success rate than that of teeth with complete root formation. Gault and Mejare yielded similar rate of successful autogenous transplantation both in teeth with complete root formation and in teeth with incomplete root formation when appropriate cases were selected. This case report was aimed at the clinical and radiographic view in autogenous transplantation of teeth with complete root formation. Materials and Methods: Patients who presented to the department of periodontics, Chonnam National University Hospital underwent autogenous transplantation of teeth. One patient had vertical root fracture in a upper right second molar and upper left third molar was transplanted. And another patient who needed orthodontic treatment had residual root due to caries on upper right first premolar. Upper right premolar was extracted and lower right second premolar was transplanted. Six months later, orthodontic force was applied. Results: 7 months or 11/2 year later, each patient had clinically shallow pocket depth and normal tooth mobility. Root resorption and bone loss were not observed in radiograph and function was maintained successfully. Conclusion: Autogenous transplantation is considered as a predictive procedure when it is performed for the appropriate indication and when maintenance is achieved through regular radiographic taking and follow-up.

HY 항원 불일치 췌도 이식에 의한 면역 관용의 유도 (Immune Tolerance in Murine Islet Transplantation Across HY Disparity)

  • Background: Minor histocompatibility HY antigen, as a transplantation antigen, has been known to cause graft rejection in MHC (major histocompatibility complex) matched donor-recipient. The aim of our study is to investigate the role of male antigen (HY) disparity on MHC matched pancreatic islet transplantation and to examine the mechanism of the immune reaction. Methods: Pancreatic islets were isolated and purified by collagen digestion followed by Ficoll gradient. The isolated islets of male C57BL6/J were transplanted underneath the kidney capsule of syngeneic female mice rendered diabetic with streptozotocine. Blood glucose was monitored for the rejection of engrafted islets. After certain period of time, tail to flank skin transplantation was performed either on mouse transplanted with HY mismatched islets or on sham treated mouse. The rejection was monitored by scoring gross pathology of the engrafted skin. Results: HY mismatched islets survived more than 300 days in 14 out of 15 mice. The acceptance of second party graft (male B6 islets) and the rejection of third party graft (male BALB/c islets) in these mice suggested the tolerance to islets with HY disparity. B6 Skin with HY disparity was rejected on day $25{\pm}7$. However, HY mismatched skin transplanted on the mice tolerated to HY mismatched islets survived more than 240 days. Tetramer staining in these mice indicated the CTL recognizing MHC Db/Uty was not deleted or anergized. Conclusion: The islet transplantation across HY disparity induced tolerance to HY antigen in C57BL6 mouse, which in turn induced tolerance to HY mismatched skin, which otherwise would be rejected within 25 days. The MHC tetramer staining suggested the underlying mechanisms would not be clonal deletion or anergy.

Pneumocystis Pneumonia after Kidney Transplantation in Children

  • Pneumocystis pneumonia (PCP) is a rare disease in healthy people but a potentially fatal opportunistic infection by Pneumocystis jirovecii in immunocompromised patients with organ transplantation. We present three cases of PCP after kidney transplantation in pediatric patients. First case was a 4-year-old boy diagnosed with Denys-Drash syndrome and received living-donor kidney transplantation from his mother at age of 1. Second case was a 19-year-old male, with polycystic kidney disease, who received kidney transplantation from his mother at the age of 18. Third case was a 19-year-old female with chronic kidney disease of unknown etiology, who received kidney transplantation from her father at age of 15. These three patients who were on immunosuppressive therapy and completed of routine PCP prophylaxis for 6 months had presented with cough and dyspnea more than 1 year after transplantation. Chest x-ray all showed diffuse haziness of both lung fields, and bronchoalveolar lavage from bronchoscopy revealed Pneumocystisjirovecii infection. All patients showed clinical resolution with intravenous trimethoprim-sulfamethoxazole (TMP-SMX) therapy for at least 3 weeks and had continued secondary prophylaxis for another 6-12 months. This report suggests that clinicians should have suspicion for the possibilities of opportunistic infection such as PCP after kidney transplantation in children.

Analysis of factors involved in brain-death donor processing for face transplantation in Korea: How much time is available from brain death to transplantation?

  • Background Face transplantation has naturally evolved from reconstructive procedures. However, few institutions perform face transplantations, because it is time-consuming and it is necessary to justify non-vital organ transplantation. We investigated the process of organ donation from brain-dead patients and the possibility of incorporating face transplantation into the donation process. Methods A retrospective review was performed of 1,074 brain-dead patients from January 2015 to December 2016 in Korea. We analyzed the time intervals from admission to brain death decisions (first, second, and final), the causes of brain death, and the state of the transplanted organs. Results The patient base (n=1,074) was composed of 747 males and 327 females. The average period between admission to the first brain death decision was 8.5 days (${\pm}15.3$). The average time intervals between the first brain death decision and medical confirmation using electroencephalography and between the first brain death decision and the final determination of brain death were 16 hours 58 minutes (${\pm}14hours$ 50 minutes) and 22 hours 57 minutes (${\pm}16hours$ 16 minutes), respectively. The most common cause of brain death was cerebral hemorrhage/stroke (42.3%), followed by hypoxia (30.1%), and head trauma (25.2%). Conclusions When face transplantation is performed, the transplantation team has 22 hours 57 minutes on average to prepare after the first brain death decision. The cause of brain death was head trauma in approximately one-fourth of cases. Although head trauma does not always imply facial trauma, surgeons should be aware that the facial tissue may be compromised in such cases.

Factors Associated with Lung Function Recovery at the First Year after Lung Transplantation

  • Purpose: Post-operative pulmonary function is an important prognostic factor for lung transplantation. The purpose of this study was to identify factors affecting recovery of forced expiratory volume in 1 second (FEV1) at the first year after lung transplantation. Materials and Methods: We retrospectively reviewed the medical records of lung transplantation patients between October 2012 and June 2016. Patients who survived for longer than one year and who underwent pulmonary function test at the first year of lung transplantation were enrolled. Patients were divided into two groups according to whether they recovered to a normal range of FEV1 (FEV1 ${\geq}80%$ of predicted value vs. <80%). We compared the two groups and analyzed factors associated with lung function recovery. Results: Fifty-eight patients were enrolled in this study: 28 patients (48%) recovered to a FEV1 ${\geq}80%$ of the predicted value, whereas 30 patients (52%) did not. Younger recipients [odds ratio (OR), 0.92; 95% confidence interval (CI), 0.87-0.98; p=0.010], longer duration of mechanical ventilator use after surgery (OR, 1.14; 95% CI, 1.03-1.26; p=0.015), and high-grade primary graft dysfunction (OR, 8.08; 95% CI, 1.67-39.18; p=0.009) were identified as independent risk factors associated with a lack of full recovery of lung function at 1 year after lung transplantation. Conclusion: Immediate postoperative status may be associated with recovery of lung function after lung transplantation.
