A Case of Buccal Pilomatricoma Removed Through Intraoral Incision (구강내 절개로 제거한 협부 모기질종 1예)

  • Pilomatricoma(or calcifying epithelioma) is a not common benign solitary tumor originated from outer root sheath cell of hair follicle or hair follicle of sebaceous glands. The tumor usually presents as an asymptomatic, hard, superficial located, and skin colored to reddish blue cutaneous mass. Most of the tumors are less than 10mm in diameter and adherent to the skin. Recently, 48-year-old man presented with cheek mass. The tumor was 2.6cm sized and located at the subcutaneous layer of cheek on CT scan. The tumor was clearly removed via transoral approach with buccal incision leaving no wound on face. The mass was confirmed as pilomatricoma on pathologic examination. Herein, we report our experience with literature review.

Three Cases of Cervical Branchial Remnants (경부 새성기형 3례)

  • Purpose: Failure of proper migration, fusion, or maturation of the branchial apparatus components results in a variety of congenital defects. Of these, cartilaginous rests are infrequent, while branchial cysts and sinuses are more common, relatively. The purpose of this study is to examine the clinical and pathological features of rare cervical branchial remnants in order to provide basis for its correct diagnosis and treatment. Methods: We report three cases of cervical branchial remnants which were treated in our hospital from December 2004 to December 2009. These cases were examined their clinical features, histologic findings and treatments. The patients had been operated with simple excision, excision of the combined components and preoperative antiboitics. Results: A retrospective review produced 2 cases of the cervical branchial remnants and 1 case of the cervical chondrocutaneous branchial remnant. All cases were on the left side of the neck, and anterior to the sternocleidomastoid muscle. Histopathological examination showed that fistula & sinus were lined with stratified squamous epithelium, additionally, they were consisted of a cutaneous envelope containing sebaceous glands, hair follicles, various amounts of adipose tissue, and elastic fibers. And, One case revealed containing hyaline cartilage. No patient developed complications or reccurences. Conclusion: The authors recommend simple surgical excision of the remnants when discharge, infection, or cosmetic problem occur. Finally, these lesions do not have fistulous tracts or connections with important, deeper organs, and so can be safely transected at the level of the superficial musculature.

  • MFD (Make up Film Deterioration) is a gradual deterioration of applied make up and is a common problem experienced by most foundation users. Our investigation revealed that for 64% of all make up users MFD is their greatest consern is using foundations. Known that the primary cause of MFD is sebum secretion. We observed that the length of time prior to onset of MFD in people who produce high level of sebum varies significantly from person to person. This suggests that other factors besides quantity of sebum production can affect MFD. Control over this factor would, we believe, be key to developing longer-lasting makeups. We studied the relationship between MFD and skin surface conditions. Our study revealed that furrows on the skin surface affect MFD significantly. Sebum reaches the skin surface from sebaceous glands and flows along furrow on the skin. If there are many deep furrows, it takes longer for sebum to overflow. But if the furrows are few or shallow, sebum quickly overflows and spreads over the skin surface where it can degrade the make up film. Therefore even when the volume of sebum produced is the same, the rate of MFD will be different depending on the number and shape of the furrows. A longer-lasting foundation could be produced by matching personal skin condition, but this would be very difficult because individual variations in texture are very large. Therefore we approached the problem by attempting to impose sebum resistance in under make up and foundation. We have developed two new materials and make up products based on our theory. A new fluoroalkyl acrylate-methacrylates copolymer designed for incorporation in under make up is extremely sebum resistant and sweat proof. Another new acrylate polymer designed for inclusion in foundation absorbs sebum and changes to a solid. Usage tests confirm it is possible to reduce MFD by using under make up and foundation which incorporate our new materials to cover where skin furrows are few or shallow

Two Cases of Supernumerary Nipple (과다 유두(Supernumerary Nipple) 2례)

  • Purpose: Supernumerary nipple or polythelia is one of the developmental anomalies occurring at the embryonic stage and this anomaly usually arises from the milk line. While this atypical feature is determined during early developmental stage, it may not come out obviously or become troublesome until puberty or lactation. Moreover, sometimes it is confused with a pigmented nevus. Methods: Case 1, a 18-year-old woman with intramammary supernumerary breast consisted of another nipple with middle sized areola on the right lower breast was admitted for a $2.8{\times}3.1\;cm$-sized mass on the right breast which was starting appeared 1 year earlier. The preliminary cytological examination of the material obtained by needle aspiration biopsy from the mass was revealed by fibroadenoma with no malignant change. The patient had the surgical excision of the mass and accessory breast. Case 2, a 16 year-old woman admitted for intra-areolar polythelia of the left breast, even she doesn't have any family history of polythelia. Since she wanted surgical correction of her atypical nipple for aesthetic and psychological reasons, we reconstructed the areola using transposition flaps in an S-plasty design. Results: Case 1, the excised supernumerary nipple showed following histological features. In the superficial layer, an acanthotic and hyperpigmented epithelium with elongated rete ridges was found. In the dermis, there were follicles with hairs surrounded by hypertrophic sebaceous glands. In the deepest portion, abundant secretory glomerules and excretory ducts of apocrine gland type were observed. Case 2, follow-up visits 3 months after the procedure showed a satisfactory result with good shape and projection of the nipple. Conclusion: We report two cases of aberrant mammary tissue who underwent surgical correction, including complete breast (with nipple, areola, and glandular tissue) and intra-areolar polythelia according to the Kajava's classification, and the results were satisfactory.

Anti-inflammatory activity of Eurya persicifolia Gagnep. extract in Propionibacterium acnes-induced inflammatory signaling by regulation of NF-κB activity (Propionibacterium acnes에 의한 염증반응에서 Eurya persicifolia Gagnep. 추출물의 억제효과)

  • Acne is a chronic inflammatory disease outbreak in the sebaceous glands within the hair follicle. The proliferation of Propionibacterium acnes (P. acnes) causes monocytes to stimulate secretion of inflammatory cytokines. A number of studies proposed the inhibitory effects of P. acnes-mediated inflammation by several natural extracts. However, studies on the effect of Eurya persicifolia Gagnep. (E. persicifolia) extracts on the inflammatory responses by P. acnes have not been explored yet. In this study, we investigated the anti-inflammatory effect of E. persicifolia extract in the inflammatory reactions induced by P. acnes. We found that E. persicifolia extract successfully diminished the expression levels of inflammatory mediators such as IL-$1{\beta}$, IL-6, TNF-${\alpha}$, and iNOS in P. acnes-activated mouse macrophage RAW 264.7 cells. We found that the immunosuppressive effect of E. persicifolia extract in the P. acnes-activated inflammatory signaling is mediated by the regulation of NF-${\kappa}B$ transcriptional activation, which is a key regulator of inflammatory cytokine expression. Our results suggest that E. persicifolia extract held potentials for the treatment of P. acnes by suppressing NF-${\kappa}B$ signaling pathways.
