We evaluated a self-inflatable osmotic tissue expander for its utility in creating sufficient soft tissue elongation for primary closure after bone grafting. Six patients with alveolar defects who required vertical augmentation of >6 mm before implant placement were enrolled. All had more than three prior surgeries, and flap advancement for primary coverage was restricted by severely fibrosed scars. Expanders were inserted beneath the flap and fixed with a screw. After 4 weeks, expander removal and bone grafting were performed simultaneously. A vertical block autograft and guided bone regeneration and distraction osteogenesis were performed. Expansion was sufficient to cover the grafted area without additional periosteal incision. Complications included mucosal perforation and displacement of the expander. All augmentation procedures healed uneventfully and the osseous implants were successfully placed. The tissue expander may facilitate primary closure by increasing soft tissue volume. In our experience, this device is effective, rapid, and minimally invasive, especially in fibrous scar tissue.

본 연구에서는 포하수 혹은 팽창기를 열매체로한 스크루우팽창기의 회전자회 전각에 의한 회전자간체적변화, 흡입구의 면적변화, 팽창기내에서의 누설통로에서의 임계등의 기하학적 특성과 흡입구와 누설통로에서의 임계유속을 고헌한 해석프로그램 을 개발하여 회전자회전수 흡입열매체의 건도, 흡입.토출압력비 그리고 팽창기내부에 서의 누설등의 효과에 대한 해석을 행하여 스크루우 팽창기의 동력발생기로서의 실용 성을 검토하였다.

Rapid palatal expansion(RPE) with the tooth-born appliance is not sufficient to apply to the patients with periodontal problem or insufficient tooth anchorage, and it leads to tipping of the anchorage teeth and increasing teeth mobility and root resorption. To avoid these disadvantages, we present the case using palatal screws and custommade palatal expander. A 23-year-old patient underwent surgically assisted rapid maxillary expansion with the Hyrax expansion using 4 tent screws. The study models were used to measure the pre-/-post surgical width of the anterior and posterior dental arches with a digital sliding caliper. In the result, the custom-made palatal expander with 4 tent screws is suitable for delivering a force to the mid-palatal suture expansion. And it is low cost, small sized and simply applied. The results indicated that maxillary expansion with the custom-made palatal anchorage device is predictable and stable technique without significant complications in patients.

구순구개열은 악안면의 선천기형중 발생빈도가 가장 높은 것으로 알려져 있으며,출생시부터 성장이 완료되는 시기까지 일관된 치료계획에 의한 종합적인 진료체계가 필수적이다. 구순구개열이 존재할 경우 구개부의 조기 봉합후의 반흔조직 형성으로 인한 치열궁의 협착 또는 중안모의 함몰 경향과 함께 상악 측절치의 선천적 결손 또는 기형치, 전치의 회전과 경사 등이 수반되므로 교정치료에 의한 기능적, 심미적인 치열의 회복이 필요하게 된다. 구순구개열 환자의 교정치료에 있어서 상악치열의 정상적인 배열과 구치부 반대교합의 치료를 위해 상악골 확대가 종종 필요하며, 이를 위하여 Hyrax appliance, Quad-helix 및 Fan-type expansion screw, Jointed fan type expander 등 다양한 장치가 활용될 수 있다. 심하지 않은 구순구개열 환자의 교정치료에서 상악골 확대를 위한 장치의 선택은 환자의 치열궁 형태, 연령, 구치부와 전치부의 치열궁 폭경 등 다양한 요인이 고려되어야 하며, 치료 후 악궁형태의 보정에 특별한 주의를 기울여야 할 것이다.
