Perceived Social Support and Morale of the Elderly Staying at Home (재가 노인의 사회적 지지와 사기)

  • Purpose: This study was done to identify the relationship between social support and morale in the elderly. Method: A structured questionnaire was carried out from April, 2003 to June, 2003 on 203 elderly. The data was analyzed with a SPSS program for descriptive statistics, Pearson's correlation coefficients, t-test, ANOVA, and stepwise multiple regression was done Result: The level of social support was moderate, and family support was the highest score. In types of support, appraisal support was the highest score. The level of morale was slightly lower than moderate, and the score of social support showed significantly positive correlation with morale. In general characteristics, several variables were significantly related to social support and morale. The most powerful predictor of morale was material support by family and the variance was 19.6%. A combination of material support by family, emotional support by relatives, level of satisfaction with pocket money, perceived health, level of intimacy with one's children, and material support by friends account for 43.3% of the variance in morale of the elderly. Conclusion: To increase the morale of the elderly, it is necessary to consolidate material support by family and relatives.

Development of a potential evaluation method for urban expansion using GIS and RS technologies (GIS와 RS를 이용한 도시확산 포텐셜 평가기법의 개발)

  • This study aims to develop a potential evaluation method for urban spatial expansion using remote sensing (RS) and geographic information system (GIS). A multi-criteria evaluation method with several criteria and their weighting values was introduced to evaluate the score and quantification of the potential surface around the existing cities. The six criteria with one geographic factor, slope, and five accessibility factors, time distance from center of the city, national road, interchange of expressway, a big city, and station, were defined for the potential. RS techniques were applied for classification of the actual urban expansion maps between two periods, and GIS functions were used for score of accessibility criteria with a distance decay function from geographic, road and several point maps, which was developed in this study. The new methodology was applied to a test area, Suwon, between 1986 and 1996. In order to optimize the six weighting values, this study made new findings to search the optimal combination of the weighting values from new methodology, weighted scenario method for intensity order (WSM), combined with intensity order and AHP method, including a trial and error method for sensitivity analysis to make the intensity order. The optimal combination of the weighting values by the new method generated the optimal potential surface, considering spatial trend of urban expansion in the test area.

The influence of learning style in understanding analogies and 2D animations in embryology course

  • Undergraduate students struggle to comprehend embryology because of its dynamic nature. Studies have recommended using a combination of teaching methods to match the student's learning style. But there has been no study to describe the effect of such teaching strategy over the different types of learners. In the present study, an attempt has been made to teach embryology using the combination of analogies and simple 2D animations made with Microsoft powerpoint software. The objective of the study is to estimate the difference in academic improvement and perception scale between the different types of learners after introducing analogies and 2D animation in a lecture environment. Based on Visual, Aural, Read/Write, and Kinesthetic (VARK) scoring system the learners were grouped into unimodal and multimodal learners. There was significant improvement in post-test score among the unimodal (P<0.001) and multimodal learners (P<0.001). When the post-test score was compared between the two groups, the multimodal learners performed better the unimodal learners (P=0.018). But there was no difference in the perception of animations and analogies and long-term assessment between the groups. The multimodal learners performed better than unimodal learners in short term recollection, but in long term retention of knowledge the varied learning style didn't influence its outcome.

Application of Giungoroen-Therapy through a Postoperative Delirium Case (수술 후 섬망 치험 1례를 통해 본 지언고론요법의 활용)

  • Objectives: The purpose of this study was to report the effectiveness of the combination of Giungoroen-therapy, as well as other Korean medicine therapy on postoperative delirium. Methods: The patient was diagnosed with postoperative delirium, that occurred after arthroscopic debridement two months before hospitalization. The patient received Giungoroen-therapy in addition to conventional Korean Medicine treatment, including acupuncture, herbal medicine, and moxibustion. The evaluation variables to check the effectiveness of the interventions, were the Korean Nursing Delirium Screening Scale (Korean Nu-DESC) and the Korean Version of the Delirium Rating Scale-Revised-98 (K-DRS-R-98). Results: During the hospitalization period, the Korean Nu-DESC score decreased from 5 to 1, and the K-DRS-R-98 score decreased from 36 to 23. The subjective expression of anxiety had been reduced. Conclusions: The combination of Giungoroen-therapy, as well as other Korean medicine therapy including acupuncture, moxibustion, and herbal medicine may be useful in alleviating postoperative delirium.

  • Object : This is a study of ischemic stroke patients designed for comparison Combination therapy of western medical treatment and oriental medical treatment. and western medical treatments alone. Methods : 45 patients were diagnosed by Br-MRI scan as having suffered ischemic stroke. They had entered Kyung-Hee Medical hospital within ten days of attack, between March 2011 and October 2012. Patients were divided into two groups; a group treated with Combination therapy of western medical treatment and oriental medical treatment and other group treated with Western medical treatments. Scandinavian stroke scale and Motricity Index score was checked at admission, 2 weeks or 3 weeks later to assess neurologic improvement and motor function recovery. Results : Comparing the Scandinavian stroke scale and Motricity Index score between baseline and 2 or 3 weeks later, the combination therapy group and western medical treatment group had improved but there was no significance. Conclusions : Combination therapy have more beneficial effect on acute stage of stroke.

Efficacy of Combination Treatment of Herbal Medicine for External Use and Western Medicine for Atopic Dermatitis: A Systematic Review and Meta Analysis (아토피 피부염에서 한약 외용제와 양약 병용 치료의 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives: This study was conducted to evaluate the effect of combination treatment of herbal medicine for external use and western medicine for atopic dermatitis (AD). Methods: We searched randomized controlled trials (RCTs) which assess the effect of combination treatment of herbal and western medicine for AD through 8 electronic databases from the start to December 2022. The data synthesis was conducted by using Review Manager (RevMan, ver.5.4.1) and Cochrane risk-of-bias tool was used to evaluate the risk of bias. Results: 13 RCTs were included. The combination treatment group had significantly higher total efficacy rate(p<0.0001) and lower SCORAD score (p<0.00001) than the western medicine treatment group. The adverse event rate was also significantly lower in the combination treatment than the western medicine treatment group (p<0.0001). But there was no significant difference in recurrence rate (p=0.09). Conclusion: This study demonstrates that the combination treatment of herbal and western medicine could be safe and effective for AD. However, due to limits of included studies such as high heterogeneity between the literature and unclear risk of bias, further studies are warranted.

Clinical Outcomes of Antiplatelets Combined with Statins in Patients with Ischemic Heart Disease (허혈성 심장질환 환자에서 스타틴과 병용하는 항혈소판제 종류에 따른 임상결과)

  • Background: Patients with cardiovascular risks are recommended to use statins and antiplatelet agents to prevent major cerebro-cardiovascular events (MACCE). Antiplatelet agents also possess anti-inflammatory and antioxidant effects, in addition to their inhibitory activity on platelets. The differences in clinical outcomes in ischemic heart disease (IHD) based on the type of antiplatelet therapy combined with statin treatment were investigated in this study. Methods: We conducted a retrospective cohort study using electronic medical records of IHD patients from January 2010 to December 2014 at Ajou University Hospital. Patients on combination therapy of antiplatelet drugs and statins were grouped based on antiplatelet drug types: clopidogrel, cilostazol, or sarpogrelate. Propensity score matching was applied to balance the baseline of the groups of clopidogrel vs. cilostazol and the groups of clopidogrel vs. sarpogrelate. The incidence and risk of MACCE as primary outcomes were assessed between the groups of antiplatelet drugs. Results: Among the approximately 128,500 patients with IHD, 1,049 patients had taken a combination therapy of statin and antiplatelet agents. The cohorts of patients administered clopidogrel, cilostazol, or sarpogrelate were 906, 79, and 64, respectively. The incidence of MACCE was not significantly different among the cohorts (p=0.58), and there were no differences between clopidogrel vs. cilostazol (p=0.72) or clopidogrel vs. sarpogrelate (p=1.00) after propensity score matching. Conclusion: There was no difference in the incidence of MACCE based on the type of antiplatelet drug (clopidogrel, cilostazol, or sarpogrelate) in combination with a statin in patients with IHD.

A Clinical Study on Patients with Traumatic Rupture of Intervertebral Disc Treated by Oriental Medicine Treatment in Combination with Soyeom Pharmacopuncture Therapy (추간판 외상성 파열 환자에 대한 소염약침 병행 치험 2례)

  • Objectives: This study is to observe the effect of oriental medicine treatment combined with Soyeom pharmacopuncture therapy on traumatic rupture of intervertebral disc patients caused by traffic accidents. Methods: The patients who have traumatic rupture in lumbar(L)-spine(case 1) and cervical(C)-spine(case 2) were treated by oriental medicine treatment in combination with Soyeom pharmacopunture therapy. VAS(visual analog scale), ROM(range of motion), and physical exam were checked on a daily basis while NDI(neck disability index) and ODI(Oswestry disability index) were examined for 3 times at 7 day interval. Results: 1. Fer 8-9 days from the admission day, the sole use of oriental medicine treatment did not make many improvements in the case of two patients' symptoms. After combining with Soyeom pharmacopuncture therapy, chief complaints were improved significantly. 2. In case 1, lumbago decreased from VAS 10 to VAS 5 and lumbar ROM got better than before. ODl score cropped from 42 to 27. 3. In case 2, left shoulder pain and nuchal pain lowered from VAS 10 to VAS 4, left upper limb numbness and weakness were improved, The patient showed nearly fun ROM. NDI score decreased from 26 to 19. Conclusions: Oriental medicine treatment in combination with Soyeom pharmacopuncture therapy is proved to be helpful to improve the symptoms of the traumatic rupture of intervertebral else patients caused by traffic accident.

Influencing Factors on Symptom Self Management in Patients with Chronic Mental Disorders (만성정신질환자의 증상 자가관리에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the factors influencing symptom self management (SSM) in the patient with a chronic mental disorder. Method: Data was collected by questionnaires from 204 chronic mental disorders in an outpatient clinic in a General Hospital and Public Mental Health Centers in Seoul and Kyunggi Province. The data was analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression, Results: The score of SSM showed a significantly positive correlation with the score of the level of self efficacy (SE) (r=.33, p=.00), social support (SS) (r=.27, p=.00), self care agency (SCA) (r=.36, p=.00) and daily living ability (DLA) (r=.34, p=.00). The score of DLA showed a significantly positive correlation with the score of level of SE (r=.46, p=.00), SS (r=.51, p=.00), and SCA (r=.52, p=.00), The most powerful predictor of SSM was SCA ($14\%$). A combination of SCA, DLA, and SS account for $20 \%$ of the variance in SSM. Conclusion: This study suggests that SCA, DLA, and SS are significant influencing factors on SSM in patients with chronic mental disorders.

Chronic Lateral Ankle Instability: Efficacy of Combination of Ankle Arthroscopic Exploration and Modified Brostrom Operation (족관절 외측 불안정성 재건 시 시행하는 탐험적 관절경의 유용성)

  • Purpose: To evaluate the efficacy of the arthroscopic exploration combined with modified Brostrom operation (MBO) for the treatment of chronic lateral ankle instability (CAI). Materials and Methods: Sixty patients who diagnosed a CAI were performed MBO. We divided to 2 groups, whether simultaneously inspected by arthroscopy (group B) or not (group A). The both group's results were compared according to American Orthopedic Foot and Ankle Society Ankle-hindfoot score (AOFAS), functional ankle score and visual analog scale (VAS) at preoperative and final follow-up period. Results: There were no significant differences of AOFAS, functional ankle score and VAS between both groups at final follow-up. However, in group A, 2 cases associated with medial ankle instability and syndesmotic injuries were did not diagnosed preoperatively, showed poor prognosis. In group B, one case had a permanent peroneal nerve symptom. The match rate of intra-articular lesions between preoperative diagnosis and postoperative arthroscopic diagnosis was 30% in group B. Conclusion: Combination of arthroscopic exploration and MBO is effective strategy for intra-operatively discrimination of intra-articular associated lesions for CAI.
