Effect of Minocycline on Activation of Glia and Nuclear Factor kappa B in an Animal Nerve Injury Model

  • Glial cells are activated in neuropathy and play a key role in hyperalgesia and allodynia. This study was performed to determine whether minocycline could attenuate heat hyperalgesia and mechanical allodynia, and how glial cell activation and nuclear factor kappa B (NF-kappaB) were regulated by minocycline in a model of chronic constriction of sciatic nerve (CCl). When minocycline (50 mg/kg, oral) was daily administered from 1 day before to 9 days after ligation, heat hyperalgesia and mechanical allodynia were attenuated. Furthermore, when minocycline treatment was initiated 1 or 3 days after ligation, attenuation of the hypersensitive behavior was still robust. However, the effect of attenuation was less when minocycline was started from day 5. In order to elucidate the mechanism of pain attenuation by minocycline, we examined the changes of glia and NF-kappaB, and found that attenuated hyperalgesia and allodynia by minocycline was accompanied by reduced microglial activation. Furthermore, the number of NF-kappaB immunoreactive cells increased after CCI treatment and this increase was attenuated by minocycline. We also observed translocation of NF-kappaB into the nuclei of activated glial cells. These results suggest that minocycline inhibits activation of glial cells and NF-kappaB, thereby attenuating the development of behavioral hypersensitivity to stimuli.

Anatomical variations of the tibial nerve and their clinical correlation

  • The tibial nerve is a branch of the sciatic nerve, which innervates the legs and feet. Anatomical variations of this nerve at the ankle are commonly found. The variation of the tibial nerve in its branching point and cross-sectional area (CSA) at the ankle is commonly related to clinical condition such as foot neuropathy. Knowledge of these variations can support the clinician in making appropriate clinical decisions. This review aims at providing knowledge on the anatomical variations of tibial nerve at ankle, as well as its clinical correlation. This review outlined the variation of the terminal branching point and CSA of the tibial nerve at the ankle in cadaveric and clinical studies.

생쥐 신경계의 미세구조에 미치는 Acrylamide의 영향 (The Effect of Acrylamide on the Ultrastructures of Nervous System of the Mouse)

  • Acrylamide의 신경독성에 대해 조사하기 위하여 초기운동실조증이 유도된 생쥐의 신경계에 대한 Acrylamide의 영향을 관찰한 결과, 중추 및 말초신경조직에 있어서의 손상이 보여 척수의 전반적 공포현상, 신경원 및 신경교의 세포손상 그리고 말초신경섬유의 퇴행현상들이 보였다. 또한 이들에 대한 미세구조적 관찰에 의하여 신경원에서의 비정상적 미토콘드리아 및 척수전근 섬유에서의 수초의 균혈화 현상등이 야기됨을 알았으며 좌골신경에서의 수초의 부분적 붕괴 및 축삭형질내의 퇴행현상들도 보였다. 위와 같은 실험결과는 Acrylamide로 인한 중독의 영향이 말초신경장애의 증상으로 나타나나 최초의 신경독성효과는 중추 및 말초신경계에 대해 거의 유사한 시기에 작용함을 시사한다.

신경독성에 의한 Schwann 세포의 형태적 변화 (Morphological changes of Schwann cells as neurotoxic responses)

  • 본 실험에서 초산납 투여로 인한 실험초기에 관찰할 수 있는 신경독성은 Schwann 세포의 종창이었다. 종창의 정도를 계측하기 위해 각 실험군 rat의 좌골신경을 횡단 조직표본으로 제작하여 Schwann세포의 두께와 핵의 장경을 측정 비교하였다. 즉 제1실험군은 30일간 0.5% 초산납이 함유된 음료수를 공급하였고 제2실험군은 제1군과 동일한 처리후 일반 실험실 조건에서 30일간 회복되게 하였으며 제3군은 대조군으로 정상 사육하였다. 초산납을 투여한 제1실험에서는 Schwann세포의 두께가 대조군보다 유의한 증가치를 보였는데, 제2실험군은 대조군과 유의한 차이가 없었다. Schwann세포핵의 직경 역시 제1군이 대조군보다 증가되었으나 유의한 차이는 없었다.

  • Aldose reductase(AR), a rate-limiting enzyme in the polyol pathway, has been demonstrated to cause the intracellular accumulation of sorbitol or galactitol and hence to play key roles not only in the cataract formation in the lens but also in the pathogenesis of diabetic complications such as neuropathy, retinopathy and nephropathy, etc. In a series of investigations to evaluate potential AR inhibitors from medicinal plants, we have shown that some hot water extracts exhibited a significant inhibition of a significant inhibition of bovine lens AR in vitro. Among active plants, the roots of Angelica dahuria (Umbelliferae) were shown to have relatively potent AR inhibitory activity. Systematic fractionation of the ether soluble fraction monitored by bioassay led to isolation of two furanocoumarins, byakangelicin(I) and ter-O-methyl byakangelicin( II), were identified as potential AR inhibitors, their $IC_{50}$ values being 6.2 M and 2.8 M, respectively.

A Case Report of a Drop Foot After Veno Venous Extracorporeal Membrane Oxygenation for a Patient Diagnosed COVID-19

  • Objective: To present a case study of a 69-year-old woman with COVID-19 who developed neurological complications due to Extracorporeal Membrane Oxygenation (ECMO) therapy and highlight the importance of daily neurological examinations and rehabilitation in the early detection and management of ECMO-related neurological complications in an isolation ICU. Design: A case report Methods: The patient received ECMO therapy, followed by neurological monitoring and rehabilitation in an isolation ICU. Daily neurological examinations were conducted to monitor the patient's neurological symptoms. Computed tomography was performed to confirm the presence of a hematoma in the left hamstring, which was identified as the cause of the neurological complication. Ultrasound-guided aspiration was immediately performed, and sciatic neuropathy predominantly involving the peroneal division was identified after aspiration. Results: Successful recovery was made possible by the early detection of neurological complications and rehabilitation in an isolation ICU. Although electrodiagnostic tests were not performed due to limited access to the isolation ICU, the appropriate intervention time could be determined through daily neurological examinations and rehabilitation, thereby minimizing neurological sequelae. Conclusions: ECMO-related neurological complications are well known, and their recognition in the ICU can be challenging. The presented case highlights the importance of daily neurological examinations and rehabilitation in the early detection and management of ECMO-related neurological complications in an isolation ICU, which can minimize neurological sequelae.

좌골신경통 백서모델에서 침치료의 효과: 체계적 문헌고찰 및 메타분석 (Effect of Acupuncture on Sciatica in Rat Models: Systematic Review and Meta-analysis)

  • Objectives While sciatic neuropathy is one of the common symptoms which have the lifetime incidence of 13~40%, still there is no consensus about the standardized and the most effective conservative treatment. In addition, the importance of systematic review and meta-analysis of preclinical are growing as they could suggest possible effective treatment strategy for future studies. Therefore, we conducted systematic review and meta-analysis to estimate analgesic effect of acupuncture on sciatica in rat models. Methods Systematic search was conducted for all controlled comparative preclinical trials which assessed analgesic effect of acupuncture in sciatica rat models. Database of PubMed, EMBASE, Web of Science, CNKI and 6 Korean databases were used. The primary outcome was pain, which is evaluated by stimulus behavior tests in rat models. We assessed the methodological quality with Systematic Review Centre for Laboratory Animal Experimentation's risk of bias tool. RevMan 5.3 was used for meta-analysis and subgroup analysis was conducted according to treatment site, acupuncture point, treatment period and frequency used in electroacupuncture. Results 14 studies were finally included following our inclusion criteria. The data from meta-analysis indicated that the acupuncture significantly improved the result values of behavior tests for pain evaluation, compared to no-treatment control group in animal models (standardized mean difference=4.43, 95% confidence interval 3.16 to 5.69, Z=6.84, p<0.00001; χ2=68.02, p<0.00001; I2=82%). The results of subgroup analysis indicate that acupuncture treatment of unilateral site, distal acupoints, longer treatment period and applying 2/100 Hz frequency in electroacupuncture could be more effective. Conclusions Systematic review and meta-analysis of animal studies are getting important for the future clinical studies and the improvement of heatlh care. Therefore the results of the study would provide evidence and better design for the forthcoming studies.
