  • Purpose: This article provides an overview of current knowledge on the impact of caregiving on the psychological and physical health of family caregivers of intensive care unit (ICU) survivors and suggestions for future research. Methods: Review of selected papers published in English between January 2000 and October 2015 reporting psychological and physical health outcomes in family caregivers of ICU survivors. Results: In family caregivers of ICU survivors followed up to five years after patients' discharge from an ICU, psychological symptoms, manifested as depression, anxiety and post-traumatic stress disorder, were highly prevalent. Poor self-care, sleep disturbances and fatigue were identified as common physical health problems in family caregivers. Studies to date are mainly descriptive; few interventions have targeted family caregivers. Further, studies that elicit unique needs of families from diverse cultures are lacking. Conclusion: Studies to date have described the impact of caregiving on the psychological and physical health in family caregivers of ICU survivors. Few studies have tested interventions to support unique needs in this population. Therefore, evidence for best strategies is lacking. Future research is needed to identify ICU caregivers at greatest risk for distress, time points to target interventions with maximal efficacy, needs of those from diverse cultures and test interventions to mitigate family caregivers' burden.

  • Purpose: Objectives of this study were to investigate the association between efficiency and quality of health care in Long-term Care Hospitals (LTCH) and determine related factors that would enable achievement of both high efficiency and high quality at the same time. Methods: Major data sources were the "2012 Korean Assessment of Propriety by Long-term Care Hospitals" obtained from the Health Insurance Review & Assessment Service. Cost variables were supplemented by a National Tax Service accounting document. First, data envelopment analysis was performed by generating efficiency scores for each LTCH. Second, matrix analysis was conducted to ascertain association between efficiency and quality. Lastly, kruskal-wallis and mann-whitney tests were conducted to identify related factors. Results: First, efficiency and quality of care are not in a relationship of trade-offs; thus, LTCH can be confident that high efficiency-high quality can be achieved. Second, LTCH with a large number of beds, longer tenure of medical personnel, and adequate levels of investment were more likely to have improved quality as well as efficiency. Conclusion: It is essential to enforce legal standards appropriate to the facilities, reduce turnover of nursing staff, and invest properly in human resources. These consequences will help LTCH to maintain the balance of high efficiency-high quality in the long-run.

  • Recent changes in the health care environment have directed increasing attention to the number and specialty mix of practicing physicians. A major concern identified in Korean health care system is the serious oversupply of specialists and a relative lack of primary care physicians. Currently only 21% of Korean physicians are primary care physicians(general practitioners and family physicians), and less than 10% of recent medical school graduates are choosing to enter primary care. More primary care physicians are needed to deal with major problems in the current health care system, such as cost and access. The infrastructure that relies on primary care physicians is needed to deliver cost-effective and efficient care. To achieve a better balance of primary care to non-primary care physicians. more medical students need to choose careers in one of the primary care specialties(family medicine. internal medicine and pediatrics). This paper suggests the necessity of reforming the Korean graduate medical education system, that is, establishing the path of training primary care physicians in internal medicine and pediatrics residency training programs.

  • Objectives: Individuals' self-reported unmet needs are an important indicator of heath care of their society. Using data from the Canadian Community Health Survey (CCHS) Cycle 1.2, we examine unmet needs for mental health care in Canada and discuss its implications to health education. Results: The most frequently reported reason for unmet needs was barriers to acceptability issues, which stem primarily from lack of knowledge of mental health, negative attitudes towards mental disorder, and mistrust of mental health professionals. Unmet needs for mental health care appeared an especially serious issue among the young. Also, individuals who reported unmet needs tended to have some ill health behavior such as illicit drug use that could be their coping mechanism. On the other hand, emotional and informational support factors have shown an important mitigating effect on unmet needs. Conclusions: These findings of unmet needs pose a major challenge to health education. To effectively address unmet mental health care needs, it is recommended that the focus of relevant policies and programs should be on enhancing a multi-level mental health education strategy including efforts on individual, environmental and professional level education.

The subjects were 831 persons of visiting health service workers and social welfare workers in Jeonnam Province who responded the mail questionnaire (85.5% respondse rate). The data were collected from 15th May to 30th June, 2004 using the structured questionnaire. It was composed of the socio-demographic characteristics, the content and frequency of services referred between health care and social welfare workers, and interdisciplinary perception. Data was analysed by SPSS for Windows 12.0. 374(45.0%) reported having experience of service cooperation between health care and social welfare workers for the last three month. The most service that health care workers requested to social welfare workers was to get information for the client, on the other hand the most service that social welfare workers requested to health care workers was to visit and treat the elderly. These service providers in rural area and good attitude toward the cooperation between health care and social welfare service showed more linkage. The findings of this study could provide the basic data for the development of efficient coordinating program of health and welfare.

Methods : Data were collected from 5 hospitals that have experienced the 2th Healthcare Accreditation in Busan, South Korea. A Likert 5-point scale was used to measure the research variables and a structured questionnaire was used. Finally, 206 valid cases were analyzed using SPSS win 18.0. For hypothesis testing, hierarchical multiple regression analysis were performed. Results : The perception of quality improvement in health care after the implementation of the Healthcare Accreditation was higher than the operational performance. In the customer orientation and job performance, the perception of quality improvement in health care and operational performance had a significant influence. Conclusions : It is necessary to use the Healthcare Accreditation as a virtuous cycle of management that can ensure enforcement of workplace regulations and improve the members'ability to provide high quality medical services.

  • Objectives: This study was conducted to assess dental health states of disabled people and analyze association between perception and awareness toward dental health and dental health status. Methods: The survey was performed from June 25 through October 30, 2004. A total of 548 disabled people participated in the study with details of 419 living in eight residential care centers located in Gyeongsangbuk-do and 129 children from a special school and two day-care centers. All subjects underwent oral examination and surveyed through a questionnaire. Parents of 129 children with disabilities were also surveyed through a separate questionnaire. Results: The dental caries experience rate was 82.1% of total 548 subjects. By age, those in their 20s experienced a high rate of dental caries with 87.5%. By educational level, those with a middle school education experienced a high dental caries rate with 91.8% (p<0.05). Of total subjects who experienced dental caries, 78.9% had experience in dental caries treatment. By age, those in their 10s showed a high rate of dental caries treatment with 87.4%(p<0.05). By educational level, those with a high school education showed a high rate of dental caries treatment with 87.7%(p<0.05). Those in residential care centers had a high rate of dental caries treatment with 82.1%, which is significantly higher than 68.8% of those who used day-care centers. A tooth extraction rate was 38.0% of total subjects. Those in their 40s had a higher rate of tooth extraction(p<0.01). Those in residental care centers had a significantly higher rate of extraction with 43.4%, compared with 20.2% of those in day-care centers. Of total subjects, 61.5% had plaque.
