저소득층 노인의 의치만족도에 영향을 미치는 요인 (The influencing factors of denture satisfaction in the low income elderly people)

  • Objectives: The purpose of this study was to investigate the factors influencing on the denture satisfaction in the low income elderly people. Methods: The subjects were 143 elderly people from 60 to 75 years old wearing dentures and receiving consistent follow-up in the public health center in Busan. A self-reported questionnaire was filled out by the elderly people from February 1 to March 1, 2013. The questionnaire consisted of denture satisfaction, social variables, and psychological variables. Data were analyzed by ANOVA and multiple regression analysis using SPSS 18.0. The sociodemographic characteristics included gender, age, and chronic diseases. The recognition of oral health included pronunciation, denture maintenance, mastication ability, and education for denture care. The questionnaire was measured by Likert 5 scale. Results: Gender, chronic disease, denture care instructions, and the self-preception of the oral health after denture treatment were closely correlated with denture satisfaction(p<0.001). Age and the number of repairs were very important factor to denture satisfaction(p<0.05). Elderly women were more satisfied with denture than men and those who had no chronic diseases tended to be more satisfied with denture. Those who received oral care instructions were more satisfied with the denture than those who did not. The younger age group and no repairing prosthetic group tended to be more satisfied with the denture. Conclusions: It is important to provide the denture management services to the low income elderly when they demand the services. The national dental health policy must be focused on connection of the elderly people denture services with the public health center.

  • Objectives: The purpose of this study was to explore relationship among dft index, diet and oral health behavior, parental awareness in oral health care and social status. Methods: This study was conducted from 8 January, 2009 to 11 June, 2009 in Asan city and Seosan city in Chungnam area. 4 kindergratens, 3 preschools and 1 children language academy were randomly selected to research on dft index, diet and oral health behavior. cross-sectional study conducted among 561 valid samples out of 641 samples collected using the survey was and then followed by oral examination. Throughout the research, numerus SPSS 15.0 statistical techniques, T-test and ANOVA and Spearman correlation coefficients were used for analysis. Results: Key results of the study are as follows: Age was found to be statistically very significant to dft index(p=0.036). Age of mother, especially between 30 to 49, was found to be statistically significant to dft index rate(p=0.001). Occupation of father was also found to be statistically significant to dft index(p=0.036). High parental awareness in oral health care led to low dft index rate, which also was found to be statistically significant(p=0.036). Conclusions: In conclusion, as Spearman correlation coefficient demonstrates, occupation of father affects most on long-term healthy oral behavior followed by parental awareness in oral health care and age of mother.

자살시도 후 생존 퇴원한 환자에서 지역정신건강센터 등록 요인 분석 - 응급실 기반 자살시도자 관리시스템 자료 중심으로 - (Contributing Factors for the Registration Rates within Emergency Department Based Post-suicidal Care Program)

  • Purpose: This study was conducted to investigate the independent factors associated with the registration rate for the community-based post suicidal care program in the emergency department (ED). Methods: This prospective observational study was conducted between January and September 2015 at the academic ED in the tertiary urban hospital. The variables examined included gender, age, address, type of insurance, history of previous psychiatric disease, suicide methods, number of previous attempts, CES-D (The Center for Epidemiologic Studies-Depression Scale), and disposition at ED. Univariate and multivariate logistic regression analysis were conducted to identify factors affecting the registration rate for the community-based post suicidal care program. Results: Overall, 331 suicides were investigated, 61 (18.4%) of which were registered in the post-suicide care program. Factors such as a intervention by psychiatric physician (OR: 3.287, 95%; CI: 1.207-9.624) and levels of depression by CES-D score of 16-24 (OR: 3.635; CI: 1.055-12.526) were significantly correlated with registration for the program. Conclusion: The registration rate for the community-based post suicidal care program was influenced by frequent intervention by a psychiatric physician and levels of depression by CES-D score of 16-24.

산후관리서비스가 산모의 건강관련 삶의 질에 미치는 영향 (Effects of using Postpartum Care Services on the Health-Related Quality of Life in Women after birth)

  • Purposes: This study purposed to identify influence of postpartum care services on health-related quality of life in women after birth. Methodology: Korea Health Panel Survey 2009-2015 provided by the Korea Institute for Health and Social Affairs and National Health Insurance Service was used for the analysis. The health-related quality of life evaluate as a EuroQoL-5D(EQ-5D-3L, used the weight of the CDC) was used as dependent variables. Postpartum care services was used as independent variable. Demographic factors(education, economic activity, region, house income), health related variable(presence of chronic disease, self-rated health), birth related variable(birth-related problem, childbirth, pregnancy of advanced maternal age) used as covariates. Regression analysis was used. Findings: The rate of use of postpartum care services is increasing year by year. Postpartum care services and self-rated health positively influence on the health-related quality of life in women after birth and chronic disease and birth-related problem negatively influence on. Practical Implications: Therefore, it is necessary that the government's policy of the postpartum care service be expanded and systematized to increase accessibility on. There are rare studies on the health-related quality of life of women after childbirth, adjusted for birth-related variables. So this study has significance.

건강보험 청구자료를 이용한 진료 연속성이 당뇨 관련 예방 가능 입원에 미치는 영향 분석: 중·고령군을 중심으로 (Effects of Continuity of Care on Diabetes-Related Avoidable Hospitalizations among Middle- and Old-Aged Patients: Analysis of National Health Insurance Claims Data)

  • Background: Diabetes is known as one of the most important ambulatory care sensitive conditions. This study purposed to assess the status of continuity of care (COC) and diabetes-related avoidable hospitalizations (DRAHs) of a group of middle- and old-aged patients and to observe the relationship of the two elements by the two age groups. Methods: This study utilized the National Health Insurance Service's National Sample Cohort data and the subjects are diabetes patients of 45 and over, classified into two groups of 'middle-aged'(45-64 years) and 'old-aged'(${\geq}65years$) patients. The dependent variable was DRAHs, which was defined in accordance with the definition of the Organization for Economic Cooperation and Development "Health Care Quality Indicators" project. COC, as an independent variable, is measured by the COC index in this study. Two-part model (multi-variate and multi-level analyses) was utilized. Results: Factors associated with the status and the number of DRAHs differed by each age group. Meanwhile, the two-part model showed that higher COC was associated with a lower risk of preventable hospitalizations in both middle- and old-aged groups. Conclusion: Study findings can provide health policy insights and implications in order to strengthen the primary care system for further improvement of diabetes management, especially for middle- and old-aged groups.

Venous Thromboembolism Following Acute Ischemic Stroke: A Prospective Incidence Study

  • Background: A sians were known to have a relatively lower incidence of venous thromboembolism (VTE), and there is insufficient evidence to suggest a specific D-dimer threshold level for screening VTE in patients with acute stroke. Methods: We prospectively enrolled patients with acute ischemic stroke admitted to Jeju National University Hospital. The inclusion criteria were: 1) aged ${\geq}18$ years, 2) admission within seven days of symptom onset, and 3) an initial National Institute of Health Stroke Scale (NIHSS) score >1 for the affected lower limb. Ultrasound scans of the lower limbs and plasma D-dimer assays were performed on days 7-14 and 15-28 after stroke onset. Results: Of 285 patients admitted during the study period, 52 patients met inclusion criteria (mean age 74.5, male 40.4%, median initial NIHSS score 12, and unable to walk unassisted at discharge 76.9%). During 7-14 days, 23 of 52 patients (44.2%) had a D-dimer level above 1.57 mg/L, and 9.6% had a level above 5.50 mg/L. Proximal deep vein thrombosis (DVT) was detected in 3 patients (5.8%, 95% confidence Interval 1.2-16.0%) on ultrasound examination. All DVTs were found in elderly female patients with severe leg weakness. No patient was diagnosed with pulmonary embolism during the study period. Conclusion: The incidence of VTE seems to be very low among Korean patients with acute ischemic stroke. Advanced age, female sex, and severe leg weakness were important risk factors for developing DVT in this study.

Bronchoscopic Findings of Pulmonary Paragonimiasis

  • Background: Pulmonary paragonimiasis is a subacute to chronic inflammatory disease of the lung caused by lung flukes that result in prolonged inflammation and mechanical injury to the bronchi. However, there are few reports on the bronchoscopic findings of pulmonary paragonimiasis. This report describes the bronchoscopic findings of pulmonary paragonimiasis. Methods: The bronchosocpic findings of 30 patients (20 males, median age 50 years) with pulmonary paragonimiasis between May 1995 and December 2007 were reviewed retrospectively. Results: The diagnoses were based on a positive serologic test results for Paragonimus-specific antibodies in 13 patients (43%), or the detection of Paragonimus eggs in the sputum, bronchial washing fluid, or lung biopsy specimens in 17 patients (57%). The bronchoscopic examinations revealed endobronchial lesions in 17 patients (57%), which were located within the segmental bronchi in 10 patients (59%), lobar bronchi in 6 patients (35%) and main bronchi in 1 patient (6%). The bronchoscopic characteristics of endobronchial lesions were edematous swelling of the mucosa (16/17, 94%) and mucosal nodularity (4/17, 24%), accompanied by bronchial stenosis in 16 patients (94%). Paragonimus eggs were detected in the bronchial washing fluid of 9 out of the 17 patients with endobronchial lesions. The bronchial mucosal biopsy specimens showed evidence of chronic inflammation with eosinophilic infiltration in 6 out of 11 patients (55%). However, no adult fluke or ova were found in the bronchial tissue. Conclusion: Bronchial stenosis with mucosal changes including edematous swelling and mucosal nodularity is the most common bronchoscopic finding of pulmonary paragonimiasis.

중장년기 주요 구강건강 지표 (Major oral health indicators in mature and middle age)

  • Although the Korea National Health and Nutrition Examination Survey (KNHANES) calculates oral health functional restriction rate and chewing discomfort rate every year, these two indicators are not all indicators of oral health. Therefore, indicators are needed to subdivide by age group and cover dental caries, periodontal disease, remaining teeth, and oral care use. The purpose of this study is to identify the key indicators of oral health in mature and middle age. The average number of existing natural teeth, the rates of 20 or more natural teeth, complaints of chewing discomfort, oral examination, periodontal disease and dental caries, were analyzed using KNHANES VI (2013-2015) as a complex sample. In the age group between 40 and 64, there were 25.2 natural teeth remaining, 91.4% natural teeth retention rate, 23% chewing discomfort rate, 34.7% oral examination rate, 38.7% periodontal disease prevalence, 6.46 the number of caries experience teeth, 21.4% interdental brush usage rate.

미충족 치과의료 지속 현황과 관련요인 (Factors associated with the persistence of unmet dental care needs)

  • Objectives: The purpose of this study was to investigate effective predictive factors of the persistence of unmet dental care needs. Methods: Data were obtained from the Korea Health Panel studies of 2011 and 2015, and 4,406 subjects, aged 18 years or older, were included in this study. Of these subjects, those who persistently experienced unmet dental care needs over the three-year period were identified. Panel logistic regression analyses were conducted to identify socio-demographic and health-related factors associated with the persistence of unmet dental care needs in two groups, those aged between 18-64 years and over 65 years. Results: Approximately 12% of subjects showed a persistence in unmet dental care needs. Marital status, education level, household income, type of health insurance, and self-rated health status all significantly correlated with the persistence of unmet dental care needs in both age groups. Conclusions: Efforts should be made to identify factors related to the persistence of unmet dental care needs in order to improve patient accessibility to dental care services.

Quality of Life of Family Members Living with Cancer Patients

  • Background: Due to the rapid progress of industrialization, the expansion of the nuclear family, and an increase in women's social activities, the burden of care of cancer patients has increased, so that all family members are now involved in care. We compared the health-related quality of life (HRQOL) between members of families of cancer patients (hereafter, cancer families) and members of cancer-free families (non-cancer families). Materials and Methods: The data were from the Community Health Survey (2012). The study population included respondents at least 30 years of age. Data were adjusted for the following covariates: sex, age, education, marital status, household income, economic activity, household type, chronic disease, and perceived health status. Frequency analysis, analyses of variance, and multiple linear regression analysis were performed. Results: Among 163,495 respondents, 3,406 (2.1%) were part of a cancer family and 160,089 (97.9%) were part of a non-cancer family. Cancer families had lower EQ-5D scores than non-cancer families. However, by subgroup, the scores had significant association between cancer and non-cancer families only for females and for those who worked. Conclusions: There was a significant relationship between HRQOL scores and being a family member of a cancer patient. This indicates that the responsibility for care has been extended to the entire family, not only the primary caregiver.
