Clinical effect of Vitamin C, Vitamin E, lysozyme, carbazochrome complex medicine($IGATAN^{(R)}$) in periodontal disease : Double blind, randomized control study (중등도 치주질환에 대한 Vitamin C, Vitamin E, lysozyme, carbazochrome (이가탄$^{(R)}$) 복합제제의 임상 효과)

  • Purpose : The purpose of this study was to evaluate the clinical effect of Vitamin C, Vitamin E, lysozyme, carbazochrome complex medicine($IGATAN^{(R)}$) for periodontal disease. Material and Methods : The study was performed by double blinded, ramdomized method. Forty two subjects diagnosed as chronic incipient or moderate periodontitis at department of periodontology, Yonsei Dental Hospital were included in the study. This study was approved by Institutional Review Board, Yonsei University Hospital. All subjects received scaling at their first examination and second examination was scheduled after 2 weeks. At second examination, periodontal parameters such as plaque index(PI), gingival index(GI), probing depth(PD), bleeding on probing(BOP), gingival recession(GR) and clinical attachment level(CAL) were recorded(Baseline) with prescription of Vitamin C, Vitamin E, lysozyme, carbazochrome complex($IGATAN^{(R)}$) (Experimental group 23 subjects) or placebo medicine(Control group; 19 subjects). The subjects were recalled after 4 weeks for periodontal parameters measurement. Results : In the experimental groups, PI, GI, CAL and BOP scores were significantly reduced at 4 weeks compared to baseline. A statistically significant decrease in or and BOP scores were observed in the experimental group compared to the control group. Conclusion : It can be concluded that Vitamin C, Vitamin E, lysozyme, carbazochrome complex medicine($IGATAN^{(R)}$) have an effect in reducing gingival bleeding and improving periodontal inflammatory condition inchronic incipient- moderate periodontitis.

  • The performance of the turbofan engine, a medium scale civil aircraft which has been developing in Rep. of Korea, was analyzed and the control scheme for optimization the performance was studied. The dynamic and real-time linear simulation was performed in the previous study The result was that the fuel scedule of the step increase overshoot the limit temperature(3105 $^{\cire}R$) of the high pressure turbine and got small surge margine of the high pressure compressor. Therefore a control scheme such as the LQR(Linear Quadratic Regulator) was applied to optimizing the performance in this studies. The linear model was expected for designing controller and the real time linear model was developed to be closed to nonlinear simulation results. The system matrices were derived from sampling operating points in the scheduled range and then the least square method was applied to the interpolation between these sampling points, where each element of matrices was a function of the rotor speed. The control variables were the fuel flow and the low pressure compressor bleed air. The controlled linear model eliminated the inlet temperature overshoot of the high pressure turbine and obtained maximum surge margins within 0.55. The SFC was stabilized in the range of 0.355 to 0.43.

  • In order to maintain customer voltage within allowable limit($220{\pm}13V$), tap operation of SVR(step voltage regulator) installed in primary feeder could be carried out according to the scheduled delay time(30 sec) of SVR. However, the compensation of BESS(battery energy storage system) is being required because the customer voltages during the delay time of SVR have a difficultly to maintain within allowable limit when PV system is interconnected with primary feeder. Therefore, this paper presents modeling of SVR to regulate voltage with the LDC(line drop compensation) method and modeling of BESS to control active and reactive power bi-directionally. And also, this paper proposes the coordination control modeling between BESS and SVR in order to overcome voltage problems in distribution system. From the simulation results based on the modeling with the PSCAD/EMTDC, it is confirmed that proposed modeling is practical tool for voltage regulation analysis in distribution system.

A Study on Automatic Operation Control of Autonomous Ships (자율운항선박의 운항 자동제어 기초 연구)

  • In the era of the 4th Industrial Revolution, the interest in autonomous ship technology is increasing as high-tech technologies are being increasingly utilized throughout the industry. Therefore, we conducted a basic study on autonomous ships. In particular, a passenger ship model was produced and an autonomous navigation system was established by applying the ardupilot used for drones. The possibility of automatic control of the autonomous ship operations was confirmed by executing various voyage plans using the built model ship. In the performance test for maintaining the course the model ship could not follow the designated course straight and sailed up to 5.4 m away from the course while navigating in a zigzag (S-shape); however, after the parameters were modified, the deviation distance was reduced to a maximum of 1.8 m. In the turning performance test, the maximum diameter of the turning sphere was found to be approximately 9.3 m, but no significant change could be confirmed even after the parameters were modified. However, the results of our tests on slowing down the ship before arriving at the WP confirmed that the diameter of the turning sphere was reduced to a maximum of approximately 3.2 m. In order to evaluate the stopping performance, the last scheduled stopping position of all experiments was compared with the actual stopping position of the model ship and it was confirmed that the model ship stopped at a point at least 0.4 m and a maximum of 6.2 m away from the stopping position. In the future, improvement of course stability, turning performance, and stopping performance is required through modification and supplementation of various parameters. Moreover, a study on automatic berthing of the model ship through automatic control is planned.

The Effect of Epidural Low Dose Ketamine Plus Morphine on the Postoperative Pain Control (Morphine과 병용한 경막외 Low-dose Ketamine의 술후 통증에 대한 효과)

  • Background: Epidural morphine for postoperative pain control has a serious risk of respiratory depression and other side effects such as pruritus, nausea and urinary retention. In recent years, it is known that epidural administration of ketamine potentiates the effect of epidural morphine, and so decrease the side effects of epidural morphine. This study was performed to evaluate the analgesic efficacy of epidurally administered ketamine and whether this epidural administration can decrease the amount of epidural morphine. Methods: Sixty patients scheduled for the elective cesarean section were randomly selected. All patients were given subarachnoid injection of tetracaine 9 mg. Group I received epidural bolus injection of 0.15% bupivacaine 10 ml with morphine 2 mg followed by a continuous infusion of 0.125% bupivacaine 100 ml containing morphine 4 mg after peritoneum closure, and group II received the same method as group I except for the addition of epidural ketamine 30 mg. Analgesic effects were assessed using Numeric Rating Score (NRS) and Prince Henry Score (PHS). Also, the degree of satisfaction and the incidence of the side effects were observed. Results: Analgesic effects were significant in both groups after drug administration. But NRS and PHS were not significantly different between two groups at all times. The incidence of nausea and vomiting was 11 out of 30 in group I and 9 out of 30 in group II and the incidence of itching was 11 out of 30 in group I and 8 out of 30 in group II. Number of patients using additional analgesics were 2 and 1 in group I and II, respectively. Conclusions: Epidural ketamine did not potentiate the analgesic effect of epidural morphine and could not decrease the side effect of epidural morphine.

Post-operative Analgesic Effect of Epidural Midazolam Administered with Morphine (경막외강에 Morphine과 동반 투여한 Midazolam의 진통효과)

  • Background: Opioids and local anesthetics have been administered epidurally for the purpose of the postoperative analgesia. However opioids have a serious risk of respiratory depression and local anesthetics have the risks of hypotension, sensory block, or motor one. In recent years, reports of spinal administration of midazolam for acute postoperative pain control have appeared in the literature. This study was performed to observe the effect of epidural midazolam in patient-controlled analgesia (PCA) device. Methods: Forty-five patients scheduled for the elective total hysterectomy were randomly selected; epidurally take morphine only (group I, n=15), morphine plus 0.1% bupivacaine (group II, n=15), or morphine plus midazolam (group III, n=15). The visual analogue scale (VAS) at rest and with movement, the sedation score, the degree of the satisfaction, the total amounts of a morphine usage, and the incidence of the side effects were observed. Rusults: The VAS at rest of group II and III were decreased significantly than that of group I. The VAS with movement of group III was significantly decreased than that of group I and II. The sadation score and the cumulative dose of a morphine were statistically insignificant within groups. Conclusion: Epidural morphine plus midazolam was proven to be clinically effective in the post-operative pain control especially for the pain with movement, compared with epidural morphine only and morphine plus 0.1% bupivacaine.

  • To evaluate the morphologic changes of the renal arteries in the condition of high-salt diet, we scheduled the control group which fed routine animal diet added 0.06% of Sodium chloride, low-salt group which fed with 2.0% of sodium chloride, and high-Salt group which fed with 8.0% of sodium chloride. The experimental animals were sacrificed every two week until 20 weeks of final experimental week. The results obtained were as follows; 1. Slight intimal thickening of the renal arteries is observed from 16th experimental week and continued the end of the experiment in the rats of control group. 2. In low-salt group slight intimal thickening of the renal arteries is observed from 12th experimental week and continued to the end of the experiment. 3. In high-salt group the intimal thickening began from 6th experimental week and its degree was hasten with week, and provoked moderate to high degree of lesion at the end of the experiment. Medial proliferation and degeneration of the intima and media, though their quality is mild, also associated at the end of the experiment.

  • Business Process Management (BPM) System is a software system to support an efficient execution, control and management of business processes. The system automates complex business processes and manages them effectively to raise productivity. Traditional commercial systems mainly focus on automating processes and do not have methods for enhancing process performances and task performer's efficiency. Therefore, there is room for enhancement of task performers' productivities and efficiency of business processes. In this paper, we propose a new method of executing business processes more efficiently in that a whole process is scheduled considering the degree of participants' workload. The method allows managing the largest constraints among constituent resources of the process. This method is based on the DBR (Drum-Buffer-Rope) in TOC (Theory of Constraints) concepts. We first consider the differences between business process models and DBR application models, and then develop the modified drum, buffer and rope. This leads us to develop BP-DBR (Business Process-DBR) that can control the proper size of task performers' work list and arrival rate of process instances. Use of BP-DBR improves the efficiency of the whole process as well as participants' working condition. We then carry out a set of simulation experiments and compare the effectiveness of our approach with that of the scheduling techniques used in existing systems.

Descriptive Report on Pattern of Variation in Cancer Cases within Selected Ethnic Groups in Kamrup Urban District of Assam, 2009-2011

  • Background: The global burden of cancer is continuously increasing. According to recent report of the National Cancer Registry Programme (NCRP) on time trends it is estimated that future burden of cancer cases for India in 2020 will be 1,320,928. It is well known that knowledge of the incidence of cancer is a fundamental requirement of rational planning and monitoring of cancer control programs. It would help health planners to formulate public health policy if relevant ethnic groups were considered. North East-India alone contains over 160 Scheduled Tribes and 400 other sub-tribal communities and groups, whose cancer incidence rates are high compared to mainland India. As since no previous study was done focusing on ethnicity, the present investigation was performed. Materials and Methods: In this paper PBCR-Guwahati data on all cancer registrations from January 2009 to December 2011 for residents of the Kamrup Urban District, comprising an area of 261.8 sq. km with a total population of 900,518, including individual records with information on sex, age, ethnicity and cancer site are provided. Descriptive statistics including age adjusted rates (AARs) were taken as provided by NCRP. For comparison of proportional incidence ratios (PIR) the Student's t test was used, with p<0.05 considered as statistically significant. Results and Conclusions: Differences in leading sites of Kamrup Urban District since from the beginning of the PBCR-Guwahati were revealed among different ethnic groups by this study. The results should help policy makers to formulate different strategies to control the level of burden as well as for treatment planning. This study also suggests that age is an important factor of cancer among different ethnic populations as well as for overall population of Kamrup District of Assam.

The Effects of Preoperative Sprayed 10% Lidocaine on the Hemodynamic Response during Suspension Microlaryngeal Surgery (술 전 분무한 10% lidocaine이 현미경 하의 후두 미세 수술 시 혈역학적 반응에 미치는 영향)

  • Purpose : It is well known that suspension microlaryngeal surgery produces marked increases in arterial blood pressure and heart rate. In this study, we evaluated the effects of 10% lidocaine preoperatively sprayed for attenuation of the perioperative hemodynamic response during suspension microlaryngeal surgery. Materials and Methods : Fifty American Society of Anesthesiologists (ASA) class 1 patients scheduled for excision of a vocal polyp by suspension laryngoscopy were randomly divided into two groups (n=25 for each group). They were intubated without 10% lidocaine spray (control group) or given 1.5 mg/kg of 10% lidocaine sprayed onto the pharyngolaryngeal and intratracheal sites 90 sec prior to intubation (10% lidocaine group). Anesthesia was maintained using desflurane in $O_2/N_2O$ 50%. The arterial blood pressure and heart rate were measured at preinduction (T0), 1 min (T1), 3 min (T2), 5 min (T3) after tracheal intubation, and 1 min (T4), 3 min (T5), 5 min (T6) and 10 min (T7) after the suspension laryngoscopy. Results : In the 10% lidocaine group, the arterial blood pressure and heart rate at 1 (T1), 3 (T2) min after tracheal intubation and 1 (T4), and 3 (T5) min after suspension laryngoscopy were lower than the same measurements in the control group. Conclusion : 10% lidocaine sprayed onto the pharyngolaryngeal and intratracheal sites before intubation was an effective method for attenuation of the perioperative hemodynamic response during suspension microlaryngeal surgery.
