토끼 귀에 발생한 초기 창상 반흔에 베라파밀이 VEGF의 발현 및 세포자멸사에 미치는 영향 (Effect of verapamil on VEGF expression and apoptosis in early wound scarring of the rabbit ear)

  • Purpose: Excessive scarring in the forms of keloid and hypertrophic scar could be a consquence of the accumulation of granulation tissue cells due to aberrant control of apoptosis. Verapamil retard extracelluar matrix production and inhibits VEGF production in primary cultured keloid fibroblast. The object of this study was effect of verapamil on VEGF expression and apoptosis in early wound scarring of the rabbit ear. Methods: Full thickness wounds were created on the ventral side of 6 New Zealand rabbits's ear. 16 days after initial wounding verapamil and saline were injected each scars and scars were harvested 1 week, 2 weeks, 4 weeks later. The wounds were stained with hematoxylin and eosin, TUNEL stain, immunohistochemical stain for VEGF and calculated scar elevation index. Results: Histologic analaysis demonstrated significant reduction in inflammation, vascularity and improvement in dermal collagen organization in experimental group. In TUNEL staining apotosis positive cells were increased and immunohistochemial staining of VEGF demonstrated significant reduction of VEGF expression in experimental group. No significant difference was noted in scar elevation index between two groups. Conclusion: This study suggest that intralesional injection of verapamil on early wound scarring of the rabbit ear decreased VEGF production and increased apoptosis and have a benefit on the pathophysiology of scar formation.

CONTROL OF SCARRING IN ADULT WOUNDS USING ANTISENSE TRANSFORMING GROWTH FACTOR-$\beta$ OLIGODEOXYNUCLEOTIDES

  • Although synthetic antisense oligodeoxynucleotides (ODNs) have been used to dissect gene function in vitro, technical difficulties of targeted delivery prevented the use of this approach for investigating the effect of gene products in vivo. Here we report the use of local delivery of antisense transforming growth factor-${\beta}$l (TGF-${\beta}$1) oligonucleotides to decrease the fibrosis in the skin wound. Adult wounds heal with scar-tissue formation, whereas fetal wounds heal without scarring and with a lesser inflammatory and cytokine response. We reasoned that strategy emptying antisense TGF-${\beta}$1 ODNs complementary to TGF-${\beta}$1 mRNA might decrease the scarring in dermal wound of mouse. To evaluate this concept, we tested the effects of antisense ODNs targeted to TGF-${\beta}$1 mRNA by topical application of the chemical on the skin wound. Phosphorothioate antisense ODNs was employed to retard their degradation. When antisense TGF-${\beta}$1 ODNs were applied into the wound site, there was a maked reduction of scar compared with control wound site, These effects of antisense TGF-${\beta}$1 ODNs on the scar formation were associated with decreased expression of TGF-${\beta}$1 gene. However sense TGF-${\beta}$l ODNs had no effect on expression of TGF-${\beta}$1 gene. Also, control wounds healed with excessive fibrosis, whereas the antisense treated wounds healed with less fibrosis. In conclusion, our results indicate that antisense TGF-${\beta}$1 ODNs could be used for amelioating scar formation during wound healing.

Correlation between dermal thickness and scar formation in female patients after thyroidectomy

  • Background: Minimizing scarring has long been a challenge in plastic surgery. Factors affecting scar formation are well known, but the effect of some patient-specific factors such as dermal thickness remains unverified. Management of factors predictive of scarring can improve postoperative patient satisfaction and scar treatment. Methods: For 3 years, we used ultrasonography to measure dermal thickness in female patients who had undergone thyroidectomy for cancer at our hospital. We confirmed the influence of dermal thickness on hypertrophic scar formation and the Patient and Observer Scar Assessment Scale scar score 6 months after surgery. Results: There was a positive correlation between dermal thickness and scar score (p<0.05), and dermal thickness appears to be a cause of hypertrophic scar formation (p<0.05). Conclusion: Thick dermis was found to cause poor scar formation and hypertrophic scarring. Prediction of factors that can influence scar formation can be used to educate patients before surgery and can help in scar management and improvement in patient satisfaction.

  • Objectives : The purpose of this study is to present the effect of subcision, microneedle therapy system and herbal peeling therapy which is treatment of Korean medicine to atrophic acne scar patients. Methods : We administered subcision, microneedle therapy system and herbal peeling therapy to atrophic acne scar patients and tested the results by Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Results : After the treatment, all of the patients showed improvements in Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Qualitative Global Acne Scarring Grading System of patients changed from grade 2.71±0.71 to grade 1.14±0.86 along with the change of Korean Acne Grading System from grade 1.57±0.57 to grade 1.00±0.00. Conclusions : Subcision, microneedle therapy system and herbal peeling therapy may be effective in the treatment of moderate to severe acne scars, and could be applied in clinical practice.

Transtherapy for moderate to severe acne scar: a study of 2 cases

  • Objectives: The purpose of this study is to present effect of transtherapy which is treatment of Korean Medicine to acne scar patients. Methods: We used transtherapy to moderate and severe acne scar patients and evaluated by a Photo evaluation with 10-point score and Qualitative Global Acne Scarring Grading System. Results: After transtherapy treatment, mean of Qualitative Global Acne Scarring Grading System of patients changed from $3.5{\pm}0.71$ to $1.5{\pm}0.71$. Acne scar showed an improvement at least 8 up to 9 and mean of their acne scar improvement scores was $8.33{\pm}0.52$ on photo evaluation with 10-point score. Conclusions: This paper shows that transtherapy in the treatment of moderate to severe acne scars is beneficial for promoting skin regeneration.

Effective Lateral Canthal Lengthening with Triangular Rotation Flap

  • In Korea, lateral canthoplasty, along with medial epicanthoplasty, has become popular over the past years to widen the horizontal length of the palpebral fissure. However, the effect of the surgery differs greatly depending on the shape and structure of the eyes. If over-widened, complications such as eversion, scarring, and conjunctival exposure may occur. Thus, the author of this study suggests a more effective and safe method for lateral canthal lengthening that causes minimal complications. A total of 236 patients underwent lateral canthoplasty between July 2007 and December 2015. For each patient, a triangular flap 4-5 mm away from the lateral canthus was elevated and rotated 45 degrees laterally while the continuity of the lower eyelid gray line was maintained. A new lateral canthus was created by fixating the rotation flap to the lateral orbital rim with minimal skin trimming and tension-free sutures, preventing relapse and maintaining a triangular shape. In more than 95% of cases, effective and satisfactory extension was achieved. On average, a 3 mm extension of the lateral canthus was achieved. There were minor complications such as wound dehiscence, webbing, and scarring, which were easily corrected. The author not only extended the lateral canthus 3-4 mm laterally but also maintained the continuity of the gray line on the lower lid as a more natural-looking triangular shape, while minimizing complications such as webbing and conjunctival exposure.

선택적 Cyclooxygenase-2 저해제 국소 도포가 토끼 귀의 창상반흔에 미치는 영향 (The Effect of a Topical Selective Cyclooxygenase-2 Inhibitor on Skin-Wound Scarring of the Rabbit Ear)

  • Purpose: The inflammatory phase is considered an integral part of adult wound healing, but fetal wound healing studies have shown scarless healing results in the absence of the inflammation process. The COX-2 pathway is an essential component of inflammation. The purpose of this study is to identify the effect of a topical selective COX-2 inhibitor on inflammation in rabbit skin wound healing and scarring. Methods: Full-thickness wounds were made on 6 New Zealand rabbits' ears. Topical 5% celecoxib + vehicle (experimental tissue) and vehicle only (controlled tissue) were applied daily for 14d on each side of the ears. Scar samples were harvested at 2 wks, 4 wks, and 8 wks after the wounding. Each sample was stained with hematoxylin and eosin and the Masson's trichrome stain to evaluate inflammation and scar formation. Results: Histological analysis demonstrated a significant reduction of inflammation, neovascularization, and scar elevation in the experimental tissue as compared to the control. Additionally, experimental tissue exhibited faster improvement of collagen organization similar to that of normal tissue. Conclusion: This study suggests that the topical application of a selective COX-2 inhibitor on a rabbit ear wound resulted in decreased inflammation and had a positive effect on the reduction of scar formation.

A case of alopecia areata after rabies vaccination: unreported adverse effect?

  • Alopecia areata (AA) is an autoimmune-related disorder characterized by non-scarring hair loss in children. We report the case of a child who had AA after the fifth dose of rabies vaccine and summarized various potential mechanisms of vaccination induced AA. This case indicates that rabies vaccine might be a predisposition of AA by causing immune dysregulation.

지오멤브레인 상의 모래의 전단거동에 관한 연구 (A Study on the Shear Behavior of Sands on the Geomembranes)

  • The shear behavior of any interface is a function of the fundamental properties of both materials at the interface. This study aimed at investigating the effect of planar surface roughness on the stress-horizontal displacement curve at theinterfaces composed of various geomembrane textures and granular materials. In addition, the extent of surfacialscarring on smooth geomembranes against granular materials during shearing induced by plowing effect was studied. It wasobserved that the displacements required to achieve peak and residual interface resistance, and the stress-displacementcurve at the interface vary greatly with the surface roughness of geomembrane. Quantification of surface roughnessvariations on smooth geomembrane due to plowing effect showed that the surfacial scarring during shearing by the soilparticles is directly related to both the normal stress and the angularity of the soil particles at the interface. The findingsof this study can be used to provide the useful information for the design and selection of counterface materials.

EFFICACY AND BIOLOGICAL ACTIVITIES OF A NEW ANTI-AGING AGENT OBTAINED FROM ARECA CATECHU

  • Inhibitory effects of the new material obtained from Areca catechu seed (CC-516) according to a special process, and its applicability to the skin as a cosmetic raw material in terms of its efficacy were presented. Areca catechu extract out of 150 medicinal plants, exhibited high inhibitory effect on the porcine pancreatic elastase ($IC_{50}$ : $40.8{\mu}$g/ml). It also had an inhibitory effect on the human leukocyte elastase ($IC_{50}$ : 48.1$\mu$g/ml), hyaluronidase ($IC_{50}$ : $416{\mu}$g/ml), antioxidative activity ($IC_{50}$ : $45.4\mu$g/ml) and free radical scavenging activity ($SC_{50}$ : $10.2{\mu}$g/ml). The cream contained 3% of CC-516 improved skin hydration above 16.5%. Especially, the skin elasticity increases more than 35% and skin wrinkles decreased more than 23%. The CC-516 was designed to be utilized in cosmetology. The cream containing 3% of this product has not only protecting effect on the skin mechanical properties provided by the collagen and the elastin in the derm but also restructuring effect of scarring or aging tissue.
