Congenital midline cervical cleft: An easily misdiagnosed disease

  • Congenital midline cervical cleft is a rare congenital disease. The disease is often misdiagnosed as a branchial cleft deformity, thyroglossal duct cyst, or other skin diseases. It has the following characteristics: skin defect at the midline of the anterior neck, a skin tag at the upper end of the lesion, and a blind sinus tract at the caudal aspect with or without mucoid discharge. Treatment is usually for aesthetic purposes; therefore, early surgical en bloc resection with Z-plasty or W-plasty is recommended to reduce recurrence and scar formation.

  • The mammalian skin contains hair follicles, which are epidermal appendages that undergo periodic cycles and exhibit mini-organ features, such as discrete stem cell compartments and different cellular components. Wound-induced hair follicle neogenesis (WIHN) is the remarkable ability to regenerate hair follicles after large-scale wounding and occurs in several adult mammals. WIHN is comparable to embryonic hair follicle development in its processes. Researchers are beginning to identify the stem cells that, in response to wounding, develop into neogenic hair follicles, as well as to understand the functions of immune cells, mesenchymal cells, and several signaling pathways that are essential for this process. WIHN represents a promising therapeutic approach to the reprogramming of cellular states for promoting hair follicle regeneration and preventing scar formation. In the scope of this review, we investigate the contribution of several cell types and molecular mechanisms to WIHN.

  • Rupture of the pectoralis major muscle typically occurs in the young, active male. Acute management of these injuries is recommended; however, what if the patient presents with a chronic tear of the pectoralis major? Physical exams and magnetic resonance imaging can help identify the injury and guide the physician with a plan for management. Nonoperative management is feasible, but is recommended for elderly, low-demand patients whose functional goals are minimal. Repair of chronic tears should be reserved for younger, healthier patients with high functional demands. Although operative management provides better functional outcomes, operative treatment of chronic pectoralis tears can be challenging. Tendon retraction, poor tendinous substance and quality of tissue, muscle atrophy, scar formation, and altered anatomy make direct repairs complicated, often necessitating auto- or allograft use. We review the various graft options and fixation methods that can be used when treating patients with chronic pectoralis major tears.

Noticeable localized corrosion of solid boric acid on 304 stainless steel

  • With the aim to determine the potential corrosion effects of solid boric acid (BA) on light water reactors or other BA-involved equipment, the corrosion behaviors of solid BA on 304 stainless steel (SS) at different temperatures were investigated. Upon comparing the corrosion behaviors of solid BA at different temperatures, significant localized corrosion was observed on 304 SS surfaces at 150 ℃ following 90-day. This localized corrosion exhibited a characteristic pattern of scattered corrosion craters including B-containing Cr-rich oxides. These oxides were found to originate within micro-cracks, gradually evolving into scar-like protrusions within the craters. The proposed corrosion mechanisms entail the interactions between solid BA and chromium oxides/hydroxides, leading to the formation of B-containing Cr-rich oxides. Our findings offer insights into potential corrosion incidents and protective strategies for industries dealing with solid BA.

PRODUCTION OF TRANSFORMING GROWTH FACTOR-${\beta}_1$ IN HUMAN FIBROBLASTS INDUCED WITH BACTERIAL TOXINS (세균 독소를 작용시킨 섬유아 세포에서 Transforming Growth Factor-${\beta}_1$의 생성)

  • TGF-${\beta}_1$ is a potent chemotactic factor for inflammatory cells and fibroblasts. It also stimulates the celluar source and components of extracellular matrix and the production of proteinase inhibitors. Collectively, these biologic activities lead to the accumulation and stabilization of the nascent matrix, which is vital to wound healing. The objective of this study is to investigate production of TGF-${\beta}_1$ in vitro fibroblast culture in the presence of Staphylococcus enterotoxin B(SEB) and/or lipopolysaccharide(LPS) and to elucidate the role of TGF-${\beta}_1$ which may be responsible for wound healing. The fibroblasts were originated from facial dermis and hypertrophic scar in 26 year-old male patient. In the presence of LPS($0.01{\mu}g$, $0.1{\mu}g$, $1.0{\mu}g$), SEB($0.01{\mu}g$, $0.1{\mu}g$, $1.0{\mu}g$) respectively, cells($5{\times}10^3ml$) were cultivated in vitro. At 1, 3, and 5 days after incubation, cells were counted. Also, cells($2.5{\times}10^5ml$) were cultivated in EMEM with LPS(0.01, 0.1 and $1.0{\mu}g$), SEB(0.01, 0.1 and $1.0{\mu}g$) respectively and LPS($0.1{\mu}g$) and SEB($0.1{\mu}g$) in combination for 24, 48, and 72 hours respectively. Culture supernatants were harvested at 1, 2, and 3 days after incubation period and triplicate culture supernatants were pooled and TGF-${\beta}_1$ was assayed in duplicate. The results were as follows. 1. In facial dermal fibroblast induced with SEB and LPS respectively or in combination, the suppression of cell proliferation occurred very significantly at 1 day after incubation, compared with the control. In SEB exposure, the production of TGF-${\beta}_1$ was decreased very significantly at 1 day after incubation, compared with the control. However, in LPS, SEB and LPS exposure, the production of TGF-${\beta}_1$ was increased very significantly at 1 day after incubation, compared with the control. 2. In hypertrophic scar fibroblast induced with SEB and LPS respectively or in combination, the suppression of cell proliferation did not occur at 1 day after incubation, compared with the control. In SEB and LPS exposure in combination, the production of TGF-${\beta}_1$ was increased very significantly at 1 day after incubation, compared with the control. However, the production of TGF-${\beta}_1$ did not occur in SEB and LPS exposure respectively. In conclusion, the concentration of bacterial toxins and the incubation period correlated with cell proliferation and production of TGF-${\beta}_1$ very significantly and both fibroblasts have different phenotype each other in this regard. This data suggest that the significant production of TGF-${\beta}_1$ may develope abnormal wound healing associated with tissue fibroproliferative disorder, such as hypertrophic scar and keloid formation.

Synthese and Anti-wear Properties of Diol Derivatives Containing Dithiophosphate Group-effect on Main Alkyl Chain and Side Alkyl Chain (Dithiophosphate Group을 함유한 디올유도체의 합성 및 내마모성-말단 알킬기 및 몸체 알킬기의 탄소사슬에 따른 영향)

  • Three types of bis[3-(dialkyloxylphosphorothionyl) thio-2-methylpropanyloxy]alkane (BAPA) drived from alkane diol were synthesized. The principal chain of each BAPA had a different carbon number, i.e., 6, 9, and 11. The three types of synthesized BAPA were compared to zinc dialkyl dithiophosphates (ZDDPs) in terms of abrasion resistance. A four-ball test was conducted to evaluate the anti-abrasion performance of the synthesized BAPA according to the length of the principal carbon chain. Each product was added to an additive at a concentration of 1% of the base oil weight, and the wear scar diameter (WSD) was measured as 0.472, 0.459, and 0.480 mm, respectively. Among the BAPA compounds, dialkyl dithiophosphoric acid (DDP), which is the side chain of bis[methacryloyloxy] nonane (BMOO9), was synthesized by varying the carbon number, i.e., 4, 8, and 12, and subsequently the 4-ball test was carried out. The WSD was determined as 0.537, 0.459, and 0.531 mm, respectively. As a result, it was found that when a side chain is short, a thin film is formed. In contrast, a long side chain hindered the formation of a film, and hence the best result was achieved when the carbon number was 8. As for the ZDDPs, the WSD was determined to be 0.563 mm, when measured under the same conditions. The measurements confirm that the synthesized BAPA compounds are superior to the ZDDPs as abrasion resistance additives.

Scar Formation of the Chronic ACL Rupture (만성 전방 십자 인대 파열의 반흔 형성)

  • Purpose : To analyze changes of the anterior translation, MRI findings and associated injuries at scarring of the torn ACL in the chronic ACL rupture. Materials & Method : From Dec. 1996 to May 2000, 19 patients who were diagnosed as the chronic ACL rupture were studied. We analyzed KT-2000 side to side difference of maximal manual anterior displacement(MMAD), MRI findings and associated injuries. Results : There was statistically significant difference in the average MMAD between the 7 cases$(37\%)$ with scar formation$(2.78{\pm}2.41mm)$ and the 12 cases without scarring$(5.75{\pm}2.52mm)$. The chronic ACL rupture without scarring had more meniscal injuries$(67\%)$ than with scarring$(28\%)$. MRI showed that relatively straight bands toward expected insertion site with single large fragment(5 cases) and continuous band with focal angulation(2 cases). Conclusion : If relatively straight bands toward expected insertion site with single large fragment or continuous band with focal angulation on MR imaging is showed in the patient without significant anterior translation at arthrometer, the possibility of the scar formation of the torn ACL should be considered. We think that the chronic ACL rupture with scarring had less meniscal injuries than without scarring will give additional information on the natural history of ACL injuries.

These result supports the mechanism that the h hydrated silica debris is the reason for low friction coefficient in humid environment. Second e evidence of the role of Si rich oxide debris could be found in the triOO-chemical reactions d during initial stage of triho-test. When the Si concen$\sigma$ation was less than 5 at.%, initial t transient period of high friction coefficient was commonly observed. Mter the transient period, m the friction coefficient becomes lower with increasing contact cycles. The initial $\sigma$ansient p peri여 becomes shorter and the starting and maximum friction coefficients in $\sigma$ansient 야,riod d decreased with increasing Si concentration. Composition of the debris on the wear scar s surface was analyzed by Auger spe따'Oscopy at v뼈ous stages in the initial transient period. W We observed that when the friction coefficient increased in earlier stage of the $\sigma$'ansient p period, iron and oxygen was observed in the debris. However, decrease in the 당iction c coefficient in the later stage of the transient period was associated with the formation of s silicon rich oxide debris. This result also supports the friction mechanism of Si-DLC films t that the formation of Si rich oxide debris results in low friction coefficient in ambient a atmosphere. atmosphere.

The Study of Blister Caused by Cupping Therapy (부항 시술에 의해 형성된 수포에 관한 고찰)

  • Objectives : The aims of this study were to evaluate a blister caused by cupping. Methods : We searched relevant case reports, survey, and review articles using databases of online bibliography. Results : 1. The fluid in the blister caused by cupping therapy is normal substance by laboratory analysis. The fluid has no signs of infection in the culture, Gram stain, or tissue biopsy 2. In histological finding, the blister caused by cupping therapy is made by dermo-epidermal seperation at subcellular level. Suction blistering was neither inflammatory nor autolysis activation of lysosomal hydrolases. 3. Blistering times directly, related to suction pressure. Suction blister formation time is accelerated in older subjects compared with younger individuals and higher temperature was more susceptable to the blister compared with lower temperature. The flexor aspect of forearm is a easy site for suction blister formation compared with leg and abdominal site. 4. Blister caused by cupping therapy is treated by regular and judicious changes of sterile dressing over several weeks. The vesicles healed well and left no visible scar. Conclusions : Blister caused by cupping therapy is artificially controlled by doctor's therapeutic purpose. Blister is not histologically injurious to health and the blister is a natural concomitant after cupping therapy.

Application of Human Dermal Fibroblast and Keratinocyte on Allogenic Dermis(AlloDerm®) (동종진피에 사람진피 섬유모세포와 각질세포를 적용한 인공피부의 실험적 제작)

  • Purpose: Large skin defect by various causes, should be covered by autologous skin graft. But, the donor site of autologous skin graft is limited and leaves permanent donor scar and contracture. There have been our trial to engineer artificial skin using allogenic dermis (AlloDerm) with basement membrane. Methods: Dermal and epidermal layer were separated by immersing in dipase solution for 30 minutes, and the separated layers were treated with 0.05% trypsin for 10 minutes. And then each layer was cultivated to fibroblasts and keratinocytes on a culture medium. Fibroblasts were first penetrated into basement membrane of allogenic dermis facing down, then allogenic dermis was flipped over to face up and keratinocytes were transplanted to allogenic dermis. Results: Observing artificial skin fabricated in vitro, we found following: 1) The artificial skin opened in air for 5 days formed epidermal layer. In dermal layer, fibroblast was distributed evenly among all. 2) The artificial skin opened in air for 30 days formed thicker and thicker, and it formed basement membrane, spinous and granular layers. PAS stain to confirm existence of basement membrane showed positive reaction. 3) Cytokeratin 10 stain to confirm the formation of epidermal layer showed positive reaction. 4) The formation of thick keratin, lamellar body and desmosome similar to human skin were observed in result of an electron micrograph. Conclusion: As a result of research, the structure seen in normal skin such as rete ridge, is found in reproduced artificial skin. This type of artificial skin can be used as a useful model for investigating skin disease and for clinical application also.
