Bronchogenic Carcinoma with Multiple Subcutaneous Metastasis: one case report (다발성 피하조직 전이를 일으킨 기관지폐암 1례 보고)

  • Bronchogenic carcinoma with subcutaneous metastasis is rare, and the incidence ranged from 1.0 to 3.0 %. Therefore metastatic nodules are incidental findings in some instances. The distribution in order of frequency Is as follows: chest wall, scapular region, abdominal wall, course of spine, upper extremities and scalp etc, and mos. of nodules are between 0.5-2.0 cm in diameter. In this report, we present one case of a 54 year old male having bronchogenic carcinoma (squamous cell type) with multiple subcutaneous metastases. The metastatic nodules were located in subcutaneous tissue of right posterior chest wall ($4{\times}5$ cm), frontal scalp area (two; $2{\times}3$ cm, $0.5{\times}0.5$ cm), left scapular region ($1{\times}1$ cm) and left iliac crest ara ($1{\times}11$ cm).

New insights into pathways of the accessory nerve and transverse cervical artery for distal selective accessory nerve blockade

  • Background: The aim of this study was to clarify the topographical relationship between the accessory nerve (AN) and transverse cervical artery (TCA) to provide safe and convenient injection points for AN blockade. Methods: This study included 21 and 30 shoulders of 14 embalmed Korean adult cadavers and 15 patients, respectively, for dissection and ultrasound (US) examination. Results: The courses of the TCA and AN in the scapular region were classified into four types based on their positional relationships. Type A indicated the nerve that was medial to the artery and ran parallel without changing its location (38%). In type B (38%), the nerve was lateral to the artery and ran parallel without changing its location. In type C (19%), the nerve or artery traversed each other only once during the whole course. In type D (5%), the nerve or artery traversed each other more than twice forming a twist. At the levels of lines I-IV, the nerve was relatively close to the artery (approximately 10 mm). TCAs were observed in all specimens around the superior angle of the scapula at the level of line II, whereas they were not found below line VI. In US images of the patients, the TCA was commonly observed at the level of line II (93.3%) where all ANs and TCAs were observed in cadaveric dissection. Conclusions: The results expand the current knowledge of the relation between the AN and TCA, and provide helpful information for selective diagnostic nerve blocks in the scapular region.

PRELAMINATED FREE FLAP FOR THE RECONSTRUCTION OF MAXILLARY DEFECTS (전판상화 유리조직판을 이용한 상악결손 재건)

  • Background In contrast to defects of the mandible and mouth floor region, in the defect of maxilla, the availability of firmly attached oral and nasal mucosal linings is needed. In addition to it, in consider of operation field, operating convenience, and esthetics, reconstruction using prelaminated flap is strongly recommended. Therefore we consider the prelaminated flap through the cases that is reconstructed using prelaminated forearm flap and prelaminated scapular flap. Patients and Methods From 2001 to 2008, in OMFS SNUDH, there were 6 cases that had reconstruction using prelaminated forearm free flap and other 3 cases that had reconstruction using prelaminated scapular flap of maxilla. The average age of patients that were reconstructed using prelaminated forearm free flap was 47.5 years, the average prelaminated period (after $1^{st}$ operation ${\sim}$ until $2^{nd}$ operation) was 51.8 days and the average follow-up period after $2^{nd}$ operation was 35.3 months. As well, the average age of patients that were reconstructed using prelaminated scapular free flap was 37 years, the average prelaminated period (after $1^{st}$ operation ${\sim}$ until $2^{nd}$ operation) was 57 days and the average follow-up period after $2^{nd}$ operation was 42.3 months. Results Except 1 case that were reconstructed using prelaminated scapular flap, we could get firmly attached oral and nasal stable skin(mucosal like) lining, more adequate thickness flap than any other flap and improved esthetic and functional results in the other 8 cases that were reconstructed using prelaminated flap. The complications of the prelaminated forearm flap cases were inconvenient swallowing, sputum, limitation of mouth opening and difficult mastication. It came from flap shrinkage of the flap in some aspect, as well as other combined operations such as mass resection or RND. The difficult point of the reconstruction of prelaminated scapular flap was the possibility of vascular damage at preparation of flap in $2^{nd}$ surgery. The damage could cause the failure of the prelaminated scapular flap. And the skin-lining of the prelaminated flap had limitations, so it is needed to study about the cultured oral epithelium-lining flap instead of the skin-lining flap. Conclusion We considered about advantages, complications and notable things of prelaminated flap through maxillary reconstruction cases using prelaminated forearm flap and prelaminated scapular flap so far. Furthermore, we should go on studying for functional reconstruction of prelaminated fasciomucosal flap using cultured oral epithelium.

The Clinical Study on the Relationship of Headache and Back shapes (두통(頭痛)과 배부체형(背部體形)과의 상관성에 대한 임상적 고찰)

  • This study was to investigate the relationship of headache and back shape using the Moire interferometer and Questionnaire investigations. In this study the subjects consisted of 317 pupils[168 boys(53%), 149girls(47%)] attending S high school in Sungnamsi, Kyoungkido in 2002, and their ages ranged from sixteen to eighteen. The results are following. 1. In questionnaire investigation, we observed that the ratio of headache was more than 70% and the ratio of girls are more than that of boys. The inclination for ages was not shown. 2. In moire topography, the remainder values of scapular region were from 0.5 cm to 1.1 cm in boys, and from 0.45 cm to 1 cm in girls. The remainder values of gluteal region were from 0.2 cm to 0.8 cm in boys, and from 0.4 cm to 0.6 cm in girls. The reminder values of the vertical lines of cervical and buttock region were from 0.71 cm to 1.51 cm in boys, and from 0.96 cm to 1.43 cm in girls. More frequent findings of reminder value of the vertical lines of cervical and buttock region were observed that the vertical lines of cervical region were inclined to left than the vertical lines of buttock 3. Through the Pearson's Correlation analysis of headache and Moire topography, we found the close relationship of headache and the remainder values of scapular region(Pearson correlation coefficient : 0.116, P<0.05) and gluteal region(Pearson correlation coefficient : 0.153, P<0.01). But the relevances to headache and the remainder values of neck lines, sacral lines and neck-sacral lines were not found.

Back and Chest Pain Related to Scalenus Medius Muscle (중사각근과 관련된 배부통과 흉통에 관한 연구)

  • One hundred and four patients who complained of chest pain or back pain in the region between scapular were studied. In most cases, anatomical location of pain was in the rhomboid or serratus anterior muscle. Hyperactivity of dorsal scapular nerve or long thoracic nerve which innervate those muscles was thought to be responsible for the pain. The hyperactivity of the nerves may be due to the spasm of the scalenus medius muscle which the nerves meet during their course to the rhomboid or serratus anterior muscles. Therefore, spasmolytic treatment including trigger point injection, physical therapy, laser therapy, or NSAIDs may be effective for the treatment of chest pain or back pain.

Correlation between Forward Head Posture, Round Shoulder Posture, and Muscle Activity during the Shoulder Flexion and Abduction Task

  • Purpose: This study was to investigate the effect of forward head posture (FHP) and round shoulder posture (RSP) on changes in muscle activities according to shoulder flexion and abduction tasks. Methods: Twenty-two male subjects with no history of neurological, musculoskeletal surgery or injuries, or pain in the spine region within the previous 3-month periods were recruited for this study. Craniovertebral angle (CVA) and Scapula Index were measured before performing 90° abduction and flexion tasks holding a 3kg dumbbell. Muscle activities were measured during the tasks. All measurements except height of the acromion were carried out in a sitting position at the height of the subject's knee angle of 90 degrees, and two tasks were randomly performed with the arm that the subject mainly use to throw the ball. The abduction and flexion angles were checked by the examiner using a goniometer beside the subject. Results: Correlation coefficient analysis between Scapular Index and upper trapezius muscle activity during shoulder abduction task showed significant positive correlation. No significant correlation was observed between CVA, Scapular Index, and other muscle activities. Conclusion: FHP showed increased muscle activation, making it difficult to change muscle activity under lower loads, and RSP was correlated with UT activation in shoulder abduction. Therefore, in the RSP, the loaded shoulder abduction is considered a potential risk factor for increasing shoulder muscle tension. This paper proposes an approach to treating RSP before FHP.

Photosensitization due to the ingestion of Japanese millet (Echinochloa crusgalli) in a Holstein calf (피삼에 의한 송아지 광과민증 1례)

  • Photosensitization occurred in a 4-month old Holstein calf soon after going onto the pasture lush with green Japanese millet(Echinochloa crusgalli). Skin lesions were restricted to the unpigmented white area of skin. They were most pronounced on the dorsum of the body, diminishing in degree down the sides and were absent from the ventral part. The demarcation between lesions and normal skin was clearcut. There were edema, exudation and sloughing of affected skin on the left gluteal region, and erythema, edema and scab on right scapular region. Interception of the light, discontinuance of Japanese millet ingestion, and the administration of antihistamine and penicillin made the calf rapid recovery. This disease was considered photosensitization due to chlorphyll in ingested Japanese millet.

  • This study has been aimed at figuring out the fact that four kinds of body type grouped according to the tilting of scapular of upper extremity and ilium of low extremity showed the mobility increase for joint parts and the limitation. When we make a thrapy plan which should be comprehensive and general in preventative management and therapy. For the sake of patients who have spinal disabilities including low back pains, it is required for many joints distributing among upper and lower extremity to have hypermobility and hypomobility based on regulation. So, this study was conducted to establish a standard for the effective pattern of hypermobility or hypomobility. I hope that the result of this study can help support the practics of physical therapy as a part of scientic method. Especially I think it will be able to be conductive to those who major in physical therapy on the arthrogenic region.

  • To investigate the effect of neck collars for the protection of self-mutilation, attainable sites of the body in dogs with collars and durability of collars were examined for 7 days. The collars used in the present experiment were hand made plastic basket collar, IASIS$\^$R/ ( heat moldable plastics), commerical plastic and cardboard collars The hand made plastic basket collar was made of basket which could buy at any supermarkets. The dogs with hand made plastic basket collar and IASIS$\^$R/ which was cast from the neck to the scapular region could lick only on digits of the forelimb. However, the dogs with commercial plastic and cardboard collars could lick not only the digits of forelimb but femur and digits of the hindlimb. It was suggested that hand made plastic basket collar was superior to the other collars in easiness of putting, durability, protection of self mutilation and economy.

Infiltrative Lipoma in a Dog (개에서의 침윤성 지방종)

  • A 7 year-old male Maltese dog was referred to Seoul National University Veterinary Medical Teaching Hospital with a recurrent mass in the right scapular region. The patient had no pains associated with the mass. On aspiration cytology, the neoplastic mass is composed of clusters of mature adipocytes with large lipid vacuoles, intermixed with frequent microcapillary vessels which is characteristic of lipoma. At surgery, the mass was infiltrated into the surrounding muscles and therefore the complete resection of the mass was difficult. Histologically, the mass was diagnosed as infiltrative lipoma. The patient has no recurrent signs 8 months after surgery.
