견갑골 내측부 하방에 발생한 자루를 동반한 해면 혈관종 -증례 보고- (A Pedunculated Cavernous Hemangioma located beneath the Inferomedial Scapular Area -A Case Report-)

  • 저자들은 견갑골의 하내측연과 승모근의 외측연 사이에서 발생한 혈관종으로, 견갑골 밑으로 자라난 부위가 음향 그림자(acoustic shadowing)로 인해 초음파 검사상 확인할 수 없었으나 수술 시 견갑골 밑으로 연결된 긴 자루(peduncle)를 포함한 혈관종이 발견되어 수술적으로 완전 절제후 초음파 검사로 추시하여 관찰한 증례를 보고하고자 한다.

  • Proprioceptive neuromusculat facilitation(PNF), scapular patterns are very important for orthopaedic and neurologic patients. It is an essential treatment techniques for motor developmental disorder, CVA, cervical disk, frozen shoulder and pain control of cervical, shoulder girdle and upper extremity. Scapular patterns of PNF has 4 different type of pattern. each of them in combining of movement plane and functional movement. Biomechanically, most of PNF patterns are a concentric contraction with third-claw lever. But the movement pattern have a technique of combination of isotonic that should make a eccentric contraction with second-claw lever.

Botulinum toxin as adjunct therapy in surgical management of a periprosthetic scapular spine fracture: a case report

  • Six months after undergoing reverse shoulder arthroplasty (RSA) a 73-year-old woman sustained a periprosthetic scapular spine fracture following a fall. She was treated with open reduction and internal fixation (ORIF), followed by botulinum toxin injection into the deltoid muscle to temporarily minimize strain at the fracture. Fracture union was achieved by 3 months, with excellent clinical function more than 1 year following fracture fixation and full resolution of deltoid function. Scapular spine fracture following RSA can be treated with ORIF and temporary deltoid paralysis using botulinum toxin in the immediate postoperative period to safely support fracture healing.

견갑골 체부 골절의 수술적 치료 (Surgical Treatment of the Fracture of the Scapular Body)

  • 목적: 견갑골 체부 골절의 수술적 치료 결과를 분석하고자 하고자 한다. 대상 및 방법: 7명의 환자를 대상으로 수술적 치료를 시행하였다. 견갑골 외측 경계가 100% 전위된 경우, 관절와가 $25^{\circ}$ 각변형 되거나 1 cm 이상 내측 전위된 경우, 견갑골 체부와 쇄골 골절이 동반된 경우를 수술적 치료의 대상으로 삼았다. 남자가 5명 여자가 2명, 평균 나이는 49(40~58)세, 평균 추시는 11(6~24)개월 이였다. 두 명은 다발성 늑골 골절과 척추 골절을 동반하였다. 기능적 평가를 위해 UCLA score 와 Korean shoulder score, 술 후 주관적인 만족도를 이용하였다. 결과: UCLA score는 평균29(17~33)점 이었고 평균 Korean shoulder score는 86(63~94)점이었다. 환자의 주관적인 만족도는 10점 만점에 7.7(4~9)이었다. 합병증으로 정복된 골절의 소실 없는 나사못 이완이 1예에서 관찰되었으며 관절와로 돌출된 나사못으로 1예에서는 교환하여야 하였다. 결론: 심하게 전위된 견갑골 체부골절에서는 견관절 및 견갑흉곽관절의 생역학적 이상으로 인한 견관절의 운동 장애를 예방하기 위해 수술적 치료가 도움이 될 것으로 판단된다.

견갑골 두께의 전산화 단층 촬영을 이용한 기하학적 모델 (Geometric Assessment of Scapular Thickness by Computed Tomography)

  • 목적: 견갑골의 수술적 치료에 있어서 해부학적 및 형태학적인 이해는 매우 중요하다고 할 수 있다. 전산화 단층촬영을 이용하여 견갑골의 정확한 두께를 측정하고, 기하학적인 형태를 재구성함으로써 견갑골의 정확한 해부학적 및 형태학적인 정보를 제공하고자 하였다. 대상 및 방법: 총 51개의 해부용 시체에서 적출한 102개의 견갑골을 대상으로 하였으며, 일반적인 견갑골의 형태를 얻기 위해 8개의 평균적인 길이와 2개의 각을 측정하였다. 또한 견갑골의 두께를 측정하기 위해 각각의 견갑골에서 동일한 위치의 시상면을 얻고, 각각의 시상면에서 견갑골의 두께를 전산화 단층 촬영을 이용하여 측정하였다. 견갑골의 두께에 대한 실측치를 토대로 평균값을 계산하였으며, 성별에 따른 견갑골 두께의 차이를 Student t-test 를 이용하여 비교하였다. 결과: 견갑골 관절와의 평균 두께는 22.4 mm, 외측연의 평균 두께는 13.83 mm, 내측연의 평균 두께는 4.44 mm, 견갑골 체부의 중간 1/3 위치의 평균 두께는 2.06 mm 로 측정되었으며, 남성의 견갑골이 여성의 견갑골 보다 더 두껍게 측정되었다. 측정된 실측치를 참고로 하여 견갑골들의 두께에 대한 3차원적인 기하학적인 모델을 재구성하였다. 결론: 측정 수치의 분석 결과 견갑골의 두께가 가장 두꺼운 부분은 관절와 부위 및 외측연 부위이며, 반대로 두께가 가장 얇은 부분은 견갑골 체부의 중간 1/3 지점이 견갑골에서 가장 얇은 부위로 나타났다.

  • In general, reverse shoulder arthroplasty revision is performed using the same implant for both the humerus and glenoid components. However, the authors of the present case used different implants from what was used previously for treating instability with scapular notching and glenoid aseptic loosening and report the case.

유방암 절제술과 견관절 수술을 한 여성의 견관절 관절가동범위, 통증수준, 기능수준, 견갑골의 자세 비교 (Comparison of Shoulder Range of Motion, Pain, Function, Scapular Position Between Breast Cancer Surgery and Shoulder Surgery Female Patients)

  • This study aimed to compare the characteristics of breast cancer surgery and shoulder surgery patients on the shoulder range of motion (ROM), degree of pain and dysfunction, and scapular position. This study was carried out with a total of 90 women: a breast cancer surgery group (BS, $n_1=30$), a shoulder surgery group (SS, $n_2=30$) and a control group ($n_3=30$). Shoulder ROM, the Quadruple Visual Analogue Scale (QVAS), the Shoulder Pain and Disability Index (SPADI), and the Scapular Index (SI) were used to assess shoulder function. Statistical analyses were performed using a one-way analysis of variance, crosstab test, and independent sample t-test. Post-hoc testing was carried out with Bonferroni test. There were significant differences in shoulder ROM when the BS and the SS were compared with the control group. However, there was no significant difference in ROM between the BS and SS. Furthermore, there was a significant difference in shoulder pain between both surgery groups, and there was greater shoulder dysfunction in the SS than in the BS. There was also a significant difference in upper extremity posture when the BS and the SS were compared to the control group. Finally, there was no significant difference in upper extremity posture between the BS and the SS. This study compared shoulder ROM, pain, dysfunction, and upper extremity postures between the BS and SS. While there were no significant differences in shoulder ROM, pain, and upper extremity posture between both surgery groups, the level of dysfunction was found to be significantly different. Therefore, health professionals managing for breast cancer surgery or shoulder surgery patients should consider these outcomes.

Comparison of the Effects of Closed Kinetic Chain Exercise and Open Kinetic Chain Exercise According to the Shoulder Flexion Angle on Muscle Activation of Serratus Anterior and Upper Trapezius Muscles During Scapular Protraction

  • Background: Methods for exercising serratus anterior (SA) and upper trapezius (UT) muscles are important for the recovery of patients with various shoulder disorders, yet the efficacy of closed or open kinetic chain exercises have not yet been evaluated. Objects: The purpose of this study was to compare the activation of the SA and UT muscles during scapular protraction considering both closed and open kinetic chain exercises. Methods: Thirty subjects were randomly divided into experimental groups (closed kinetic chain exercise) and control groups (open kinetic chain exercise) in which scapular protraction was performed at $90^{\circ}$ or $125^{\circ}$ shoulder flexion. Electromyographic activity data were collected from the SA and UT muscles per position and exercise method. Results: Separate mixed 2-way analysis of variance showed significant differences in the activation of the SA (F1,28=6.447, p=.017) and the UT (F1,28=35.450, p=.001) muscles between the groups at $90^{\circ}$ and $125^{\circ}$ shoulder flexion. Also, the SA/UT ratio measures at $90^{\circ}$ and $125^{\circ}$ shoulder flexion significantly differed between the groups (F1,28=15.457, p=.001). That is, the closed chain exercise was more effective than open chain exercise for strengthening the SA muscle and controlling the UT muscle, $125^{\circ}$ of shoulder joint was more effective than $90^{\circ}$. Conclusion: The findings suggest that scapular protraction with shoulder $125^{\circ}$ flexion at the closed kinetic chain exercise may be more effective in increasing SA muscle activation and decreasing UT muscle activation as well as increasing the SA/UT ratio than open kinetic chain exercise.

Effect of Craniocervical Flexion on Muscle Activities of Scapula Upward Rotator Muscle During Push-Up Plus Exercise in Subject With Winging of Scapula

  • The aim of this study to investigate the effects of craniocervical flexion on muscle activities of scapular upward rotators during push-up plus exercise in subjects with winging scapula. Eighteen males with scapular winging were recruited, and each subject performed knee push-up plus and other exercises, in two conditions (craniocervical flexion vs. natural head positions). A surface electromyography (EMG) was used to measure upper trapezius (UT), serratus anterior (SA), and lower trapezius (LT) muscle activity. A paired t-test was used to determine the statistical significance between the different condition with/without applying of craniocervical flexion. UT EMG activity significantly decreased and SA EMG activity significantly increased during knee push-up plus involving the craniocervical flexion compared to the natural head position. However, no significant differences (p>.05) were found in the activity of the LT muscle. The UT/SA ratios with and without craniocervical flexion showed a significant difference (p<.05). These results showed that the knee push-up plus other exercises performed with craniocervical flexion could strengthen the serratus anterior muscle and minimize the activity of the UT muscle.
