Influence of External Scapular Stabilization on the Isometric Strength of Shoulder Flexor in the Sitting Position in Subjects with Scapular Winging

  • Purpose: The purpose of this study was to compare the influence of external scapular stabilization on the isometric strength of shoulder flexor muscle in subjects with and without scapular winging and conditions with and without external scapular stabilization. Methods: A total of 30 subjects with and without scapular winging were enrolled. Two groups were classified using clinical and diagnostic tests to divide the groups with and without scapular winging (15 with scapular winging 15 without scapular winging). The isometric strength of the shoulder flexor was measured using a tensiometer. The isometric strength was evaluated in the sitting position with and without an external scapular stabilization. The external scapular stabilization was provided with the examiner's hand to fix scapular winging during shoulder flexion. The changing value was calculated to determine the isometric strength difference between shoulder flexion with and without the external scapular stabilization in each group. The changing value between isometric strength of shoulder flexor with and without scapular winging was compared using the independent t-test. Statistical significance was set at 0.05. Results: In the group with scapular winging, the shoulder flexor isometric strength in the sitting position was greater with an external scapular stabilization than without an external scapular stabilization (p<0.05). There was no significant difference in the shoulder flexor isometric strength in the subjects without scapular winging between conditions with or without an external scapular stabilization (p>0.05). Conclusion: The external scapular stabilization in the individuals with scapular winging may increase shoulder flexor isometric strength in the sitting position.

  • Purpose: The purpose of this study was to compare the effects of passive scapular upward rotation and posterior tilt and active scapular posterior tilt on the muscle activity of the upper trapezius (UT), lower trapezius (LT), and serratus anterior (SA). Methods: Fifteen healthy subjects performed general arm elevation, arm elevation with passive scapular upward rotation and posterior tilt, and arm elevation with active scapular posterior tilt. For active scapular posterior tilt, the subjects were trained in this movement using visual biofeedback and a motion sensor. During each arm elevation condition, electromyography was used to measure the muscle activity of the UT, LT, and SA. The measured data were analyzed using a one-way repeated ANOVA. Results: LT muscle activity was significantly increased during arm elevation with active scapular posterior tilt compared to both general arm elevation and arm elevation with passive scapular upward rotation and posterior tilt (p < 0.05). SA muscle activity was greater during arm elevation with passive scapular upward rotation and posterior tilt than during general arm elevation (p < 0.05). There was no significant change in UT muscle activity among the tested arm elevation conditions (p > 0.05). Conclusion: Performing arm elevation with active scapular posterior tilt and performing arm elevation with passive scapular upward rotation and posterior tilt may be useful strategies for increasing muscle activation of the LT and SA, respectively.

The Effects of 4-Week Serratus Anterior Strengthening Exercise Program on the Scapular Position and Pain of the Neck and Interscapular Region

  • The purpose of this study was to investigate the effects of serratus anterior strengthening exercises on scapular position and Visual Analog Scale (VAS) pain measurements taken at the resting position in young adults with adducted scapular. The exercise program included stretching of the scapular retractor and strengthening of the serratus anterior muscle. We measured the distance from the midline of the thorax to the vertebral border of the scapular with a tape line (Superior Kibler), and the distance from the 7th cervical spinous process to the acromial angle with 3-dimension motion analysis system, to compare the resting scapular position before and after exercise. Fifteen subjects with adducted scapular were recruited to compare the resting scapular position and VAS. The distance from 7th cervical spine process to acromial angle of the scapular and VAS decreased significantly (p<.01) after exercise, while the distance from the midline of the thorax to vertebral border of the scapular increased (p<.05). The conclusion is that the serratus anterior exercise program altered the resting scapular position and decreased VAS.

Effects of Passive Scapular Stabilization on Upper Extremity Muscle Strength in Patients With Rotator Cuff Repair

  • Background: Scapular dyskinesis may cause not only rotator cuff (RC) tear but also weakness of the upper extremity, studies on scapular dyskinesis that may occur after RC repair is still lacking. Objects: To determine whether scapular dsykinesis was present in patients after arthroscopic RC repair and to investigate the influence of passive scapular stabilization on upper extremity strength. Methods: A total of 30 patients after RC repair participated in this study. To compare the scapula of the arthroscopic RC repair shoulder and the contralateral shoulder, the winged scapula (WS) was measured using a scapulometer and scapular dyskinesis was also classified by type. Fixed instruments for muscle strength measurements were used to measure upper extremity muscle strength differences depending on passive scapular stabilization position or natural scapular position. A chi-square test, an independent t-test and a 2-way mixed measures analysis of variance (ANOVA) was used as statistical analysis. In analyses, p < 0.05 was deemed to be statistically significant. Results: Postoperative shoulder had a significant association with scapular dyskinesis and the WS compared to the contralateral shoulder (F = 0.052, p < 0.01). Postoperative shoulder, muscle strength in the shoulder abduction (p < 0.01), elbow flexion (p < 0.01) and forearm supination (p < 0.05) were significantly greater in the scapular stabilization position than in the scapular natural position. Conclusion: Patients underwent arthroscopic RC repair had a significant association with scapular dyskinesis and muscle strength was improved by a passive scapular stabilization position, therefore scapular stabilization is important in rehabilitation program.

  • Purpose : To describes the important aspects of scapular movement and function used when applying PNF technique to the upper limb and scapular. Method : The scapular was a very important roles in the upper limb movement. This study summarizes the physiologic movement of scapular to the PNF upper extremity patterns or scapular patterns. Result : The shoulder joint has the most freedom of range of motion in the human body, composed of the glenohumeral joint, the subacromial joint, the acromioclavical joint, the sternoclavicular joint, the scapulothoracic joint, the costosternal joint, and the costovertebral joint. During upper limb movement, the scapular position change at the sternoclavicular joint and the acromioclavical joint. This concerted motion was characterized by scapulohumeral rhythm. In clinical situations, it is import to understand factors affect the scapulohumeral rhythm so that optimal evaluation and therapeutic intervention can be devised. Conclusions : The scapular movement depend on the proper and coordinated contraction of muscles. Physical therapists need to understand the normal scapular movement relationships of the scapulohumeral rhythm under different interventions for PNF techniques application.

Effects of the Additional Scapular Posterior Tilt Movement on Selective Muscle Activation of the Lower Trapezius during Prone Shoulder Extension

  • Background: Although the scapular posterior tilt movement could facilitate the lower trapezius (LT) muscle activity, no study identified the effects of the scapular posterior tilt movement on the selective activation of the LT muscle during prone shoulder extension. Objectives: To examine the influences of additional scapular posterior tilt on electromyography (EMG) of the upper trapezius (UT) and the LT muscles during prone shoulder extension. Design: Cross-sectional study. Methods: There were 15 asymptomatic male participants in this study who performed prone shoulder extension with and without scapular posterior tilt movements. For the scapular posterior tilt movements, participants performed visual biofeedback training for scapular movement using motion sensor. During the exercises, the EMG activity of the UT and LT was recorded using surface EMG system. Results: The EMG activity of the LT significantly increased during prone shoulder extension with scapular posterior tilt compared to that of general prone shoulder extension, whereas that of the UT was not significantly different between the two exercises. Moreover, scapular posterior tilt application significantly decreased UT/LT muscle activity ratio. Conclusion: Scapular posterior tilt movement may be emphasized during exercise when facilitating LT muscle activation.

Influence of Pre-Emptive Scapular Posterior Tilt on Scapular Muscle Activation and Scapulohumeral Movements during Shoulder Horizontal Abduction in the Prone Position

  • Background: Shoulder horizontal abduction in the prone position (SHAP) has been reported as an effective exercise to strengthen the lower trapezius. However, the effects of pre-emptive scapular posterior tilt on scapular muscle activity and scapulohumeral movements during SHAP have not been examined. Objectives: To examine the effect of the addition of scapular posterior tilt on muscle activity of the trapezius and posterior deltoid, and scapular posterior tilt and shoulder horizontal abduction, during SHAP. Design: Cross-sectional study. Methods: Fifteen healthy male subjects performed two types of SHAP: general and modified SHAP (SHAP combined with pre-emptive scapular posterior tilt). To perform modified SHAP, pre-emptive scapular posterior tilt training was performed prior to the modified SHAP. Muscle activity of the middle and lower trapezius and posterior deltoid, and the amount of scapular posterior tilt and shoulder horizontal abduction, were measured during two types of SHAP. Results: Muscle activity of the lower trapezius and scapular posterior tilt was significantly increased during the modified SHAP, while muscle activity of the posterior deltoid and the amount of shoulder horizontal abduction were significantly decreased. However, the middle trapezius muscle activity did not change during the modified SHAP. Conclusion: The SHAP with pre-emptive scapular posterior tilt can be useful to strengthen the lower trapezius.

Effect of the Abdominal Drawing-in Maneuver on the Scapular Stabilizer Muscle Activities and Scapular Winging During Push-up Plus Exercise in Subjects With Scapular Winging

  • Background: Scapular winging is a prominence of the entire scapular medial border, mainly caused by insufficient activity of the serratus anterior (SA) and imbalance of scapulothoracic muscles. Push-up plus (PUP) exercise has been commonly used to increase SA muscle activity. The facilitation of abdominal muscle may affect scapular muscle activity by myofascial connections. Thus, the sequential activation of the turnk muscles is suggested to facilitate the transition of proper force from upper limb and restore force couple of scapular muscles. The abdominal drawing-in maneuver (ADIM) has been effective in improving activation of the deep trunk muscles during movement. Objects: The aim of this study was to determine the effect of ADIM on the activity of the upper trapezius (UT), lower trapezius (LT), and SA during PUP exercises in subjects with scapular winging. Methods: Fourteen men with scapular winging (determined as a of distance between the scapular medial border and thoracic wall over 3 cm) volunteered for our study. The subjects performed the PUP exercise with and without ADIM. Surface electromyography was used to collect the electromyography data of the UT, LT, and SA. A scapulometer was used to measure the amount of scapular winging. Results: SA activity was significantly greater and scapular winging significantly lower during the PUP exercise with ADIM than during those without ADIM. Conclusion: PUP exercise with ADIM can be used as an beneficial method to improve SA activation and to reduce the amount of scapular winging in subjects with scapular winging.

어깨뼈 안정화운동과 등뼈관절가동술이 어깨뼈 기능이상이 있는 성인의 어깨 기능에 미치는 영향 (The Effect of Scapular Stabilization Exercise and Thoracic Joint Mobilization on the Scapular Function in Adults with Scapular Dysfunction)

  • 본 연구는 어깨뼈 기능이상이 있는 사무직원에 대해 어깨뼈 안정화 운동과 등뼈관절가동술을 시행한 후 어깨뼈의 위치와 움직임 및 어깨기능장애지수에 미치는 영향을 분석하기 위한 목적으로 실행되었다. 연구 대상자는 충남의 N대학에서 근무하는 사무직원 중에서 어깨뼈 기능이상 검사(Scapular Dyskinesis Test, SDT)를 하여 이에 해당되는 42명을 연구 대상자로, A그룹은 어깨뼈 안정화운동 적용군(N=14), B그룹은 등뼈관절가동술 적용군(N=14), 그리고 C그룹은 어깨뼈 안정화운동과 등뼈관절가동술을 결합한 실험군(N=14)으로 나누었다. 각 중재는 4주간 주 3회, 회당 25분씩 총 12회 실시하였다. 연구 결과, 어깨뼈 위치 변화의 그룹 간 비교에서는 C그룹이 다른 그룹에 비해 유의한 차이가 있었다(p<.05). 어깨뼈 움직임의 변화에서 위팔뼈 벌림 45°에서는 C그룹이 다른 그룹과 비교하여 그룹 내 및 그룹 간 비교에서 유의한 차이를 보였다(p<.001). 벌림 90°에서도 C그룹이 그룹 내 및 그룹 간 비교에서 유의한 차이를 보였다. 어깨기능장애지수(Shoulder pain and disability index, SPADI)에서는 C그룹이 다른 그룹에 비해 유의한 차이를 보였다(p<.05). 본 연구의 결과, 어꺠뼈 안정화운동과 등뼈관절가동술의 결합은 어깨뼈 기능이상이 있는 사무직원의 어깨뼈 위치와 움직임 그리고, 상지기능장애지수의 향상에 유의한 효과가 있었다.

견갑골 전후방향 검사 시 변형된 자세의 유용성 평가 (Effectiveness of the Modified Position of Scapular AP Projection)

  • Purpose: This study was to evaluate effectiveness of the modified position in scapular AP (anteriopo-sterior) projection. Materials and Methods: From June 2012 to July 2013, scapular AP projection was examined in 108 patients by use of conventional method and in 40 patients by use of modified method. We compared between conventional method and modified method following variables including sex, age, height, weight, and BMI. We measured and compared distance between lateral border of the scapular and the rib to confirm the separation of the scapular from the lung fields including the ribs. Results: The scapular AP projection was technically successful in all patients. There was no significant difference in patient characteristics including sex, age, height, weight, and BMI between conventional method and modified method. The mean distance between lateral border of the scapular and rib in conventional method and modified method was 15.09 mm and 26.33 mm, respectively. Modified method was significantly increased the distance between lateral border of the scapular and rib compared with conventional method (P < .01). Conclusion: The modified position in scapular AP projection was effective method to separate between the scapular and lung fields including the ribs compared with conventional position of scapular AP projection.
