Effectiveness Evaluation of Scanogram Using Longbone Detector (Longbone 검출기를 이용한 Scanogram의 유효성 평가)

  • Scanogram is that combine several practical images into one image to observation. So it is an important consideration in many clinical situation such as iliac measurement, leg alignment measurement and Scoliosis. Currently, scanogram examinations are mainly conducted for children and elderly patients. In this study, in order to apply the longbone detector to children or elderly patients who are difficult to cooperate with, we compared the longbone detector from D equipment with the G equipment discovery 656 Puls equipment in reproducibility of images, distribution of irradiation dose, scattering dose, irradiation time and image acquisition time. D equipment took more than twice as much time as G equipment. The scattered dose generated about 50% more G equipment than D equipment. In the whole spine scanogram and the measurement length of the lower leg, D equipment was also measured longer than G equipment. However, both methods did not show much difference from the CT scanogram, so there was no problem in measurement. The height of the thyroid radiation dose of G equipment was produced more radiation than D equipment. However, the longbone detector deviated from the x-ray center line relative to the tube rotation method, and was measured lower by the directionality of the measuring instrument, so that the error could not be corrected. In the conclusion of study, using the longbone detector is excellent for applying to children or elderly patients to reduce scattering dose. However, using CR may be useful to normal patients. Because, the image quality may deteriorate due to an imbalance of dose difference in thickness depending on the body part. So, it is useful to using a compensation filter or tube rotation method when we take a whole spine scanogram.

  • The accurate measurement for a long bone have taken an important part in orthopedics surgical diseases when the length of extremities is different each other. However, it is true that various kinds of methods to search for the accurate measurement on used in medical centers. In this study, we measured the real length of long bone for CT Scanogram and F.F.D by 100cm and 200cm in the angle of inclination $0^{\circ},\;3^{\circ}$ and $7^{\circ}$. This method will be useful to diagnose and treat patients more accurately.

Juvenile Hemangioma Occurred in Distal Femoral Epiphysis (소아의 대퇴골 원위부 골단에 발생한 혈관종)

  • A hemangioma occurred in the bony epiphysis is extremly rare. A 5-year-old boy visited to our hospital with pain and flexion contracture on the right knee. MRI showed some lesions scattered in the epiphysis of the distal femur and the proximal tibia. Biopsy specimen from the distal femoral epiphysis revealed pathologic findings compatible with hemangioma. On 8 years follow-up, the lesion in the distal femoral epiphysis had been cured, and those in the proximal tibial epiphysis were spontaneously disappeared without surgery. The scanogram shows no leg length discrepancy and angular deformity. We reports a rare case of hemangioma occurred in the bony epiphysis with the results of 8 year follow-up with the review of literatures.

A COMPARISON OF SCANORA$\textregistered$ RADIOGRAPHY WITH WATERS' AND PANORAMIC VIEWS FOR THE DETECTION OF MUCOSAL THICKENING OF MAXILLARY SINUS (워터스, 파노라마 방사선사진과 Scanora$\textregistered$ 방사선사진의 상악동 점막비후 진단 결과의 비교)

  • The purpose of this study was to compare the diagnostic performance of Waters' and panoramic view; maxillary sinus posteroanterior and lateral scanography of Scanora/sup (R)/ for mucosal thickening of maxillary sinus as well as to identify the utility of Scanora/sup (R)/ for the detection of maxillary sinus disease. The assessment was done at 66 maxillary sinuses in 45 patients and the results were as follows ; 1. Estimation of presence or absence of mucosal thickening. The sensitivity, specificity, and positive and negative predictive value of maxillary sinus posteroanterior and lateral scanography were 0.865, 0.860, 0.921, and 0.805 respectively and slightly higher than those of Waters' and panoramic views, which were 0.832, 0.835, 0.903, and 0.728 respectively. However, paired t-test showed no significant differences in the diagnostic performance of the two pairs of imaging modalities. 2. Estimation of the types of mucosal thickening. The diagnostic accuracy for type I, II, III was 75.3% on Waters' and panoramic view; 77.9% on maxillary sinus posteroanterior and lateral scanography. It was higher on the latter ,but showed no significant differences from that on the former. 3. Reliability of interpretation. In itraobserver and interobserver agreement, both overall rates of agreement and kappa-value were slightly higher on maxillary sinus posteroanterior and lateral scanography than on Waters' and panoramic views. There was no significant differences between the two pairs of imaging modalities. These results suggested that scanogram is a useful diagnostic radiography as well as Waters' and panoramic views for detection of maxillary sinusitis.

The Clinical Results of Arthroscopic Modified Pull-Out Suture for Root Tear of Posterior Horn of Medial Meniscus (관절경하 내측 반월상 연골 후각부 뿌리 파열의 수정된 견인 봉합술후의 임상적 결과)

  • Purpose: This study is to evaluate the clinical results of pull-out suture for root tear of posterior horn of medial meniscus. Materials and Methods: Between March 2006 and February 2011, We studied 40 cases with the root tear of posterior horn of medial meniscus which follow up more than 1 years. Mean age was 49.5 years old. We excluded osteoarthritis or varus deformity patients more than 5 degrees. pull-out suture was performed to the patients with grade 0-2 of Kellgren and Lawrence classification. According to Outerbridge classification, evaluation of cartilage damage was performed during arthroscopy. Evaluation of clinical result was used the Lysholm score. Results: The mean pre-operation Lysholm score was 63.9 and post-operation score was 86.3. The complete failure rate was 3 of 40 cases (7.5%). Twenty of 40 cases (50%) showed cartilage lesions in weight bearing portion of medial femoral condyle which were 12 cases in grade 1, 5 cases in grade 2 and 3 cases in grade 3 according to the Outerbridge classification. Conclusion: In the treatment of pull-out suture for root tear of posterior horn of medial meniscus, exclusion of more than moderate arthritis or varus deformity is very importment. Pull-out suture seems to be a useful treatment of the root tear of posterior horn of medial meniscus in mild osteoarthritis or varus deformity of middle ages.
