간호사의 간호업무 전산화 활용에 대한 조사연구 (A Survey on Nurses' Utilization of Computerization in Nursing Practice)

  • I have practiced this reserch for the purpose of proposition of basic data for amendment and addition of computer system after I surveyed the degree of abilities and satisfaction on the computerization in nursing\ practice. Subjects were collected from September 1 to September 30, 1998. Study subjects are 151 clinical nurses who are working at university hospital in Chonbuk area. Study instrument consists of 116 questionnaire which was developed by Kim H. K(1998). The data were analized number, mean ,t-test, ANOVA by using SPSS. The study results are as follows: 1. Most of the nurses who answered the questions were educated computerizing(96%). The abilities of computer program utilities were the most in inputting data(74%). The program nurses wanted computerizing education was wordprocess, dealing internet and statistics. 2. As for applying parts of computer system in nursing, the most nurses were Dr's order check (96%), treatment activity(91.4%), medication(89.4%), lab. test(87 .4%), transfering department(85.4%), transfering room(79.5%), diet(71.8%), discharge (70.2%), Barcord Sticker(70.2%) , reservation(62.2%) in order that were all conected order system mutually to other part. It showed that computer system was not applied for treating the original nursing work such as nursing record (13%), duty scheduling(6.0%) , nursing process(4.6%) , Q.I(1.3%), nursing research(1.3%), education(1.3%), 3. As for the benefit when computer system is applied for, the most respondence was promptness of work and convenience(90.6%) , exactness of work (82.8%), offering information exactily and efficiently(36.3%), offering good quality of nursing (6.0%) in order. 4. The degree of contentment on computerizing in nursing practice showed average 3.24%. The best marking item is that they are eager to participate in the computer education. Next, the automatic output of label and giving the number automatically is to prevent loss and mentioned items of patient registration from being changed(3.95) and to prevent mistakes possibly happening because of doctor's difficult order(3.85). 5. The problems which may happen to in nurses' utilization of computerization for the nursing practice showed average 3.18. The most problem is that the practice may stop because of the problem of computer itself(3.67), and the next, the indication of act can be imperfect(3.66) , manual training may overlap because of incomplete computerization (3.60), practical education for the computerizing is lack(3.41), and the computer literate nurses are lack(3.40) . 6. Study subject's contentment by age was significant difference(F=3.10,P=0.0119). Study subject's contentment by job posision was significant difference(F=6.001P=0.0034) I will propose the following according to the above results. The domain of original nursing practice is urgernt. Manual and indication of act should be made before long in computer obstacle. Nursing department should support the nurses so that they could receive the education needed for the nurses themselves.

장기요양보험제도의 도입에 따른 노인요양시설의 공간구조 변화에 관한 연구 (A Study on the Spatial Structure Change of Elderly Care Facility according to Introduction of Long-Term Care Insurance)

  • Purpose : Long-term Care Insurance sets up facility standard and installation standard of aged care facilities and decides the presence of minimum number of rooms and its size in care facilities by using systematic instruments. Therefore, most aged care facilities had renovation in expansion and reconstruction following the revised regulations and even facility space structure and space composition are continuously improving. The study is to determine the purpose and trend by comparing before and after space composition of facilities which followed the implementation of Long-term Care Insurance and also to suggest hierarchical space composition suitable for aged care facilities through derived problems and to provide basic materials to plan the most appropriate facility for the aged. Methods : J-graph based on Space Syntax Theory will be schematized through in-site facility survey and before and after facility floor plan. Space composition trend will be analyzed by comparing indexes through S3 program. Results : As a result of 5 cases analysis, the following results were found; the average of whole space depth is increasing due to the Extension, the number of volunteers is decreasing and rooms for geriatric care helpers are being created due to the geriatric care helper introduction duty. Also, there are solariums being created to improve the health of the aged and dispensaries are being placed on every floor with the increase of documentary work for geriatric care helpers. With the policy implementation, care facility space composition and structure are changing with facility standard and it was analyzed that facilities were mostly put under the control of limited number of people in care room and total ground area per person. It was also found that there is increase in care space integration through before and after comparison of whole integration value. This is considered as the important result not only in facility standard satisfaction, but also in care support of geriatric care helpers and the aged, its main users. Implication : In order for elderly care facilities to have quality improvement and to develop as suitable facilities for characteristics of the aged, independent environmental facility standard preparation of elderly care facilities is needed through mutual cooperation of construction fields with regulation and policy related researches.

복지용구 전문가 직무분석에 관한 연구 (A Study on the Job Analysis of Assistive Product Specialist)

  • 현재 우리나라는 고령화 사회로 노인들의 복지용구에 대한 수요와 필요성이 증가하고 있다. 그러나 노인들은 복지용구 급여 서비스에 대한 만족도가 낮은 편이며, 복지용구 전문가에 대한 필요성이 대두되고 있다. 이에, 복지용구 전문가에 대한 틀을 잡고 직무에 대해 명확하게 하는 것은 시급한 과제라고 할 수 있다. 따라서 본 연구에서는 복지용구 전문가에 대한 직무를 정의하고 및 직무를 분석하고자 하였다. 본 연구는 2015년 6월 1일부터 2016년 2월 10일까지 수행되었다. 문헌고찰과 현황분석 후, 전문가 자문회의를 통하여 복지용구 전문가에 대한 직무를 정의하고 복지용구 전문가의 책무, 수행작업, 작업요소들을 분석함으로써 복지용구 전문가 직무분석(안)을 도출하였다. 또한 도출된 복지용구 전문가 직무분석(안)에 대한 내용타당도 검증을 위하여 5명의 전문가 패널을 대상으로 델파이 설문 검증을 실시하였다. 연구결과, 복지용구 전문가의 직무로 5개의 책무, 12개의 수행작업, 28개의 작업요소가 도출되었다. 본 연구는 복지용구 전문가의 직무 정의 및 분석을 통하여 복지용구 전문가에 대한 체계와 기준을 만들고자 한 점에서 그 의의를 가진다고 할 수 있다. 본 연구를 통하여 복지용구 전문가의 전문성을 확보하고 복지용구 급여 서비스에 대한 질적 향상을 기대한다.

요양병원 간호사의 임종간호스트레스와 간호근무환경이 소진에 미치는 영향 (End of life care stress and Nursing Work Environment in Geriatric Hospitals Nurses affect burn out)

  • 본 연구는 요양병원 간호사의 임종간호스트레스, 간호근무환경과 소진 정도를 파악하고, 소진과의 관련요인을 분석하기 위한 서술적 상관관계 연구로, 2016년 10월 1일부터 10월 31일까지 B광역시에 있는 8곳의 요양병원 간호사 195명을 대상으로 임종간호스트레스, 간호근무환경, 소진에 관하여 구조화된 설문지로 자료를 수집하였고, SPSS win 21.0 프로그램을 이용하여 분석하였다. 간호사가 경험하는 대상자의 임종간호스트레스는 평균평점 $3.84{\pm}0.56$이며, 간호근무환경은 평균평점 $3.25{\pm}0.60$이며, 소진 정도는 평균평점 $2.93{\pm}0.52$로 나타났다. 대상자의 임종간호스트레스와 소진과는 상관관계를 보였고 (r=.206, p=.004), 간호근무환경과 소진의 상관관계는 역 상관관계를 보였으며 (r=-.431, p<.001), 소진에 가장 중요한 유의한 변수는 간호직 만족도 (${\beta}=-.302$)이었으며, 간호근무환경 (${\beta}=-.294$), 나이 (${\beta}=.286$), 근무형태 (${\beta}=-.17$), 임종간호스트레스 (${\beta}=.164$)순으로 나타났다. 전체 설명력은 41.2%였다. 따라서 요양병원 간호사의 소진을 최소화하기 위하여 간호사로서의 자긍심과 책임감을 가질 수 있는 지지적인 근무환경 조성 및 인센티브 제공, 효율적인 업무분담과 임종간호교육 프로그램 개발 및 스트레스를 완화할 수 있도록 행정적 차원이 필요하다고 사료된다.

치과위생사의 보수교육 실태 및 인식에 관한 연구 (A study on the state of inservice education for dental hygienists and their relevant awareness)

  • The purpose of this study was to examine the reality of inservice education provided to members of Korean Dental Hygienists Association, the state of relevant academic conferences, and the perception of the members about inservice education and academic conference. It's basically meant to help boost their participation in inservice education and their satisfaction with it, and to show some of the right directions for that. The subjects in this study were dental hygienists who attended a symposium on July 1, 2006. After a survey was conducted, the answer sheets from 489 participants were analyzed, and the findings of the study were as follows: 1. General hospitals and university hospitals made up the largest group(91.4%) that gave a monthly leave of absence, and the second largest group was dental hospitals(75.4%), followed by dental clinics(58.3%) and public dental clinics(48.0%). The most common closing time in dental clinics and dental hospitals was 5 p.m., and that was 12 p.m. in general hospitals and university hospitals. The dental hygienists in public dental clinics didn't work on Saturdays. By type of workplace, treatment was the most common duty for the dental hygienists in dental clinics and dental hospitals to perform, and those who worked at general hospitals, university hospitals and public health clinics were in charge of extensive range of jobs. 2. The rates of the dental hygienists who took that education stood at 94.9% in public dental clinics, 78.7% in dental hospitals and 75.3% in dental clinics, general hospitals and university hospitals. Regarding how many marks they got on an yearly basis, those who got eight marks or more made up the largest group(55.6%), followed by four marks or more(11.8%), six marks or more(3.4%), and two marks or more(1.5%). As for the usefulness of inservice education for their job performance, the largest number of the dental hygienists(40.8%) found it to be helpful, and the second greatest group(37.5%) considered its effectiveness to be so-so. The third largest group(8.4%) found it to be of great use, and the fourth biggest group(4.2%) considered it to be of no service. The fifth biggest group(l.3%) thought it was absolutely useless. By type of workplace, the workers in dental clinics, dental hospitals, general hospitals and university hospitals wanted the most to learn how to take care of clinical work(acquisition of up-to-date technology), and those in public health clinics hoped the most to learn about public dental health. By type of workplace, the workers in dental clinics had their sight set on self-development the most, and the dental hygienists in dental hospitals, general hospitals, university hospitals and public health clinics were most in pursuit of acquiring new knowledge. By type of workplace, the specific given conditions at work were most singled out by the dental clinic workers as the reason, and the dental hospital employees pointed out time constraints the most. The dental hygienists in general hospitals and university hospitals cited time constraints and financial burden the most, and the public health clinic personnels mentioned inaccessibility of a place for inservice education as the reason. 3. The public health clinic workers participated in academic conferences the most(90.8%), followed by the general and university hospital personnels(68.8%), dental hospital employees(65.6%) and dental clinic workers(65.5%). By type of workplace, the public health clinic workers(73.5%) expressed the most satisfaction, followed by the general and university hospital employees(67.7%), dental clinic workers(62.3%) and dental hospital personnels(54.1%). By type of workplace, the employees of dental clinics, dental hospitals, general hospitals and university hospitals preferred Saturdays, and the public health clinic workers had a liking for weekdays. As for a favored place, hotels were most preferred, followed by university hospitals, general hospitals, college lecture rooms, district halls and local public institutions. Hotels were most favored regardless of the type of workplace. 4. Regarding outlook on inservice education, they had the highest opinion on the facilities and given conditions of lecture rooms($3.41{\pm}0.83$), followed by the professionalism of lecturers($3.34{\pm}0.83$), procedures of receipt and attendance confirmation($3.34{\pm}0.83$) and class size($3.13{\pm}0.89$). On the contrary, they took the most dismal view of the inaccessibility of a place of inservice education($2.08{\pm}0.92$), followed by limited opportunity and limited date for that education($2.51{\pm}0.99$), extra financial burden($2.53{\pm}1.18$) and high tuition fee($2.57{\pm}0.96$).

대구.경북지역 학교급식 식재료 납품업체 생산직원의 위생관리 실태조사 (A Survey of Sanitation Management Practices of School Food Ingredients Manufacturing Company Workers in Daegu and Gyeongbuk Province)

  • 본 연구는 학교급식 식재료의 위생관리 실태를 파악하기 위하여 식재료 생산 가공업체의 생산직원 221명을 대상으로 위생지식을 조사하고, 위생관리 수행수준, 위생관리 현황을 5점 척도로 평가하였다. 생산직원의 위생관리 수행수준의 평균은 4.63/5.0점으로 높은 편이었으나, 위생 지식수준은 4.63/9.0점으로 낮았다. HACCP 인증업체 직원의 위생교육 횟수, 만족도, 위생관리 태도 등이 비인증업체에 비해 유의하게 높았고(p<0.05), HACCP 비인증업체에서의 시설설비에 대한 관리가 요구되었다. 또한 위생관리 수행영역에서 작업대와 작업장의 매일 청소 및 철저한 소독, 냉장고와 냉동실의 청결유지, 작업장의 온도확인, 작업 중 교차오염 방지, 작업 중 작업도구의 철저한 소독, 작업장 출입 시 신발소독 및 에어샤워, 작업복의 매일 세척의 항목은 HACCP 비인 증업체에서 유의하게 잘 지켜지지 않아 위생교육 및 평상시 작업관리 시 철저한 관리가 요구되는 항목이었다. 생산직원들이 학교급식 식재료를 생산하면서 가장 어려운 점은 위생관리와 납품시간 맞추기였고, 위생교육의 불만족한 이유는 일방적인 강의 때문이 가장 높았다. 따라서 생산 가공업체 에서는 교육대상자의 요구에 맞추어 위생교육 내용을 선정하고, 일방적인 강의형식의 교육보다는 활동자료나 교육매체를 활용한 보다 능동적이고 흥미로운 위생교육을 실시하여 교육효과를 최대한 높일 수 있도록 하여야 할 것이다.

향상된 장애물 극복형 실내 이·승강 전동휠체어의 설계 및 제작 (Design and Manufacture of Improved Obstacle-Overcoming type Indoor Moving and Lifting Electric Wheelchair)

  • 인구 고령화와 건강 복지에 대한 사회적 관심이 높아지면서 재활복지기를 포함한 실버 및 헬스케어 산업과 관련한 연구들이 활발하게 진행되고 있다. 본 연구에서는 기존에 1차적으로 설계·제작되었던 이·승강 전동휠체어를 사용자 측면에서 보다 더 안전성과 편의성을 갖도록 개선하고자 하였다. 사용자 체형을 고려한 헤드레스트 높이조절 기능의 시트 디자인, 사용성 증대를 위한 암레스트의 길이조절 및 95° 회전 기능, 사용자 탑승 편의성을 위한 바닥까지의 시트하강 및 운행 시 승강할 수 있는 기능, 캐스터 회전반경 및 안전성 증대를 위한 발판 추가 및 풋레스트 길이 조절 기능, 승·하강 시 흔들림 방지를 위한 프레임 및 승·하강 링크 설계 등을 개선시켰다. 또한 안전주행을 위한 컨트롤러 및 드라이브 부문과 사용자 편의성 측면의 기능들을 추가적으로 설계·제작하였다. 추가 기능을 보완하여 제작된 실내 전동휠체어의 동작 상태를 평가하기 위해 구동 실험을 수행하였다. 연속 주행시간, 회전반경, 승·하강 최대 하중, 최대 승강 높이, 소음레벨, 운행 보조센서 센싱 최소거리, 서버 및 앱 프로그램 상호연동 및 기기호환성, 듀티 사이클 최대 오차율의 성능실험을 수행하였다. 실험 결과, 더 개선하여 설계·제작한 전동휠체어는 시험 목표 값을 달성하였으며, 성공적으로 동작하는 것을 확인할 수 있었다. 향후 연구로는 개선하여 제작된 이·승강 전동휠체어에 대해 사용자 측면에서의 사용성 평가를 수행할 것이다.

민간경비원의 직무스트레스가 이직의사에 미치는 영향 (The Effect of Job Stress and Turnover Intention among the Employee of Private Security)

  • 본 연구는 민간경비원의 직무스트레스가 이직의사에 미치는 영향을 규명하는데 연구의 목적이 있으며, 2005년 현재 경찰청에 등록된 서울 및 수도권 소재 민간경비업체 중, 50명 이상 근무하고 있는 업체를 모집단으로 설정하였다. 선정된 경비업체의 민간경비원을 표집대상으로 설정한 후 유층집락무선표집법(stratified cluster random sampling)과 할당표집방법(quota sampling)을 병행 사용하여 표본을 추출하였다. 조사도구는 설문지를 사용하였으며, 타당도와 신뢰도 분석을 통해 검증을 실시한 후 사용하였다. 데이터는 각 목적에 맞게 부호화하여 입력시킨 후 사회과학통계 프로그램인 SPSS 12.0 version을 이용하여 전산처리 하였다. 이와 같은 방법과 절차에 따른 자료 분석을 통하여 본 연구에서 얻은 결론은 다음과 같다. 첫째, 민간경비원의 사회인구학적 특성에 따라 직무스트레스 및 이직의사의 차이를 검증한 결과 부분적으로 유의한 차이가 있는 것으로 나타났다. 둘째, 직무스트레스와 이직의사의 상관과계 분석 결과 직무환경과 이직의사에서 정적인 관계를 나타내었다. 셋째, 민간경비원의 직무스트레스가 이직의사에 미치는 영향을 검증한 결과 통계적으로 유의한 차이가 있는 것으로 나타났다.

대구.경북지역 산업간호사의 배치현황 및 제공실태에 관한 조사연구 (A Study on the Distribution of Industrial Nurses and Performance of Industrial Nursing Services in Taegue and Kyungpook area)

  • This study was attempted to find out the distribution of industrial nurses, analyze job performance by function and utilization state of medical dispensary among workers. The subjects for this study were 32 nurses working at industry located in Kyungpook and Taegu area. The data was collected through questionaire during the period of August 5-31, 1986, and analysed by the method of frequency and percentage. The following is the main findings of the study; 1. 72.2% of respondents was engaged in manufacturing industry, 33.3% in workplace whose regular workers was more than 2,000 workers. 67.7% in occupational health physicians was part-time system. 2. 93.1% of respondents was 20-29 years age group, 93.1% was graduates of junior nursing college, 96.6% was unmarried. 448% had 1-4 years of total working experiences. 3. For the motives which made them becomes industrial health nurse, 'good employment condition' was 62.1%. For the job satisfaction, 'moderate' was 586%. For the interest about the industrial health, 'moderate' was most frequent (58.6%). In the inservice education, 86.2% of the subjects was received education. 4. For the attitude of the dispensary and industrial nursing of employer, 'necessary' was most frequent (72.4%, 62.6%). 5. All establishment had dispensary facilities, 65.5% of them had independent dispensary. 6. In duty shift, 93.1% of respondents was working in one shift system. 41.4% of respondents was received from 250,000 won to 290,000 Won and 41.4% was belong to personnel section and 24.1% was direct controlled by general business section chief. 7. In the main health problem of their factories, 48.3% of respondents was work-environmental state, 24.1% was health education. 8. In the Dispensary budget, 60% of respondents was under 3,000 won per worker. 9. In the job performance rate by function, nursing service 73.1%, industrial health and nursing management 63.7%, environmental hygiene and safety management 54.5%, medical insurance 44.9%, welfare 38.4%. 10. Main health complaints among workers utilizing medical dispensary was 35.9% of respiratory system, 21.5% of gastro-intestinal system, 11.0% of skeletomuscular system.

의료사고의 손해배상과 위자료 산정 -한국소비자원 의료피해구제 사례들의 일별- (Assessment of Damages for Non-pecuniary Loss and Compensation for Damages in Medical Accidents - Overview for Cases of Medical Injury Relief in Korea Comsumer Agency -)

  • There are two opinions on the legal characteristics of damages for non-pecuniary loss, a private sanctions theory and complementary function of damages for non-pecuniary loss, briefly. There is a close connection between the legal characteristics and the function of damages for non-pecuniary loss. The functions of damages for non-pecuniary loss are consist of satisfaction, prevention( sanctions) and complementation. Several cases of medical injury relief reported to Korea Comsumer Agency are categorized as follows, 1) cases of death after having an operation, 2) diagnosed with disability after a medical accident, 3) extended damages happening related to delayed diagnosis, 4) et cetera(a plastic surgery, a treatment with oriental medicine), and the damages for non-pecuniary loss in respect to each cases are examined. In the case of occurring death or disability, Korea Comsumer Agency has set up guidelines for assessment of damages for non-pecuniary loss by classifying into major and collateral violation for a duty of care. Furthermore, the damages for non-pecuniary loss in the case of all sorts of cancers, are assessed in accordance with the degree of responsibility subsequent to dividing cancer into good and poor prognosis. When it comes to a complementary function of damages for non-pecuniary loss in the actual work, it is hard to assess the damages as it is difficult to objectify non-pecuniary loss, such as emotional distress. Though compensation for damages is major legal characteristics of consolation money, preventing a damage(private sanctions) through consolation for a victim or sanctions against an assailant also has great significance. Therefore, it is necessary to approach flexibly for mutual agreement by considering specialty( concrete facts) of individual issue thoroughly. If considering this aging society that limits the possible age for work to 60 years old, it is needed to have a complementary function of consolation money in mind not to make it less meaningful for victims due to small sum of consolation money.
