Objectives : As there was no sufficient research done on Sasang (four harms: 氣[qi], 血[blood], 痰[phlegm], and 鬱[congestion]), which serves as a precept in treatment of miscellaneous diseases, in contrast with 'Yang is always teeming whereas Yin is always scarce' and 'Sanghwa-ron', which are Judangye's major theories, I have come to work on it. I expect that with this, we can understand Judangye's medical theory and therapy for a variety of diseases. Methods : To begin with, I take a look at the definition of Sasang. And then, I make selections of theories and therapy related to Sasang from Gyeokchiyeoron(格致餘論) and Geumgweguhyeon(金匱鉤玄), which are Judangye's writings. My study follows the order of energy, blood, phlegm, and congestion. Results : Through Gyeokchiyeoron, I have learned more about Judangye's theory on how energy, blood, phlegm, and congestion cause diseases. And as for therapy, I have tapped into Geumgweguhyeon to use sagunja-tang(四君子湯) for drained energy, samul-tang(四物湯) for drained blood, ijin-tang(二陳湯) for phlegm, and wolguk-hwan(越鞠丸) for congestion, thus verifying the originality of Judangye's theory. Conclusions : "Judangye for miscellaneous diseases" was confirmed through his treatments for energy, blood, phlegm, and congestion. And his idea of Yang-eum(養陰 'nurturing yin') is now reflected in therapy for miscellaneous diseases, now serving as a study that provides a comprehensive understanding of Judangye's medical theories.
Objectives: The purpose of this study is to report the effect of Korean medicine treatments on a chronic pelvic inflammatory disease (CPID). Methods: We treated 2 cases of chronic pelvic inflammatory disease patients with Korean herbal medicine, acupuncture and moxibustion. One patient is a 50 years old woman, who suffered from uterine bleeding, lower abdominal pain, right pelvic pain. She was treated with Gwichuligyeong-tang-gami and Samul-tang-gami. Other patient is a 40 year old woman, who appealed for pain such as pelvic pain, lower abdominal pain, urinary frequancy, dysuria. She was treated with Cheongpochugeo-tang. Both patients are taken acupuncture at Junggeuk (CV3), Gwanwon (CV4), Chagung (CA111), Sameumgyo (SP6), Eumneungcheon (SP9) and moxified at Gwanwon (CV4). Results: After treatments, their symptoms were remarkably improved. Conclusions: This study suggests that Korean medicine treatments have significant effect on reducing symptoms of chronic pelvic inflammatory disease.
Objectives : The purpose of this study is to report one case of Korean medical treatment of patient with developmental abnormality by cerebral palsy whose main symptoms are difference in left and right leg length, foot size and construction of right ankle. Methods : We did retrospective study for one developmental abnormality patient who had been treated by herbal medicine(Samul-tang, Yookmijihwang-tang hap Yijin-tang + Sungjang-hwan), acupuncture, and moxibustion for more than one year. The effects of treatment were evaluated by measurement of leg length, foot size and dorsiflexion ROM. Results : After treatment, length and foot size difference between both leg, ROM difference between both ankle were almost reduced. According to the ratio of the right to the left, leg length was improved from 0.91 to 0.98 and foot size was improved from 0.93 to 0.98. The difference in left and right ankle dorsiflexion ROM was improved from 55° to 27.5°. Conclusions : This clinical case study has demonstrated that Korean medicine may be an safe and effective option for treating developmental abnormality.
This study was performed to investigate the effects of very low calorie diet (VLCD) using newly meal replacements that contain the wild grass extracts based on Samul-tang ingredients on weight reduction and health in the obese adult women ($BMI{\geq}25\;kg/^2$) for four weeks. Seventy five women participated in this experiment. Subjects were randomly classified three groups: 1) General Diet group (GD group, n=25) consumed 3 regular meals within 600 kcal/day, 2) Meal replacements group (MR group, n=25) consumed 1 regular meal and 2 meal replacements within 600 kcal/day, 3) Herbal Meal replacements group (HMR group, n=25) consumed 1 regular meal and 2 meal replacements within 600 kcal/day. Anthropometric measurements, body composition, biochemical measurements and body symptoms were assessed before (the initial) and after (the 4th week) the study. Anthropometry measurements such as weight, waist and hip circumference, and BMI and body composition such as body fat percent, fat mass significantly decreased in all groups after diet intervention. Anthropometric measurements and body composition of the ]lim group significantly decreased more than those of GD and MR groups. Serum Total cholesterol was significantly decreased in all groups. However, there was no significant difference among three groups during the experimental period. HMR group had significantly less felt a pain than GD and MR groups in body symptoms such as anemia, powerlessness, vomiting, constipation and dryness of skin during the experimental period. Therefore, very low calorie diet (VLCD) using meal replacements that contain the wild grass extracts based on Samul-tang ingredients was very effective on weight reduction and health in the obese adult women.
Objectives : The water extract of Samul-tang (SMT) has traditionally been used for treatment of ischemic heart and brain damage in oriental medicine. However, little is known about the mechanism by which the water extract of SMT rescues cells from these damages. Methods: This study was designed to investigate the protective mechanisms of SMT on oxidative stress-induced toxicity in H9c2 cardiomyoblast cells. Treatment with $H_2O_2$ markedly induced death of H9c2 cardiomyoblast cells in a dose-dependent manner. Results: The characteristics of H20z-induced death of H9c2 showed apparent apoptotic features such as DNA fragmentation and morphological change. However, SMT significantly reduced both H202-induced cell death and morphological change. The decrease of Bc-2 expression by High were inhibited by SMT. In addition, the increase of Bax expression was also inhibited by SMT. The cotreatment of SMT and $H_2O_2$ in H9c2 cells also induced the phosphorylation of ERK in a time-dependent manner. Moreover, PD98059, a specific inhibitor of ERK1/2 attenuated the protective effects of SMT on $H_2O_2-induced$ toxicity in H9c2 cardiomyoblast cells. These results suggest that both ERK1/2 signaling pathways play important roles in the protective effects of SMT on $H_2O_2-induced$ apoptotic death of H9c2 cells. Also, the expression profile of proteins in $H_2O_2$ cardiomyoblast cells were screened by using two-dimensional (2-D) gel electrophoresis. Among 300 spots resolved in 2-D gels, the comparison of control versus apoptosis cells revealed that signal intensity of 17 spots increased and 11 spots decreased. Conclusions: Taken together, this study suggests that the protectiw effects of the water extract of SMT against oxidative damages may be mediated by the modulation of Bc1-2 and Bax expression via the regulation of the ERK signaling pathway.
Objectives: The aim of this study is to describe the effects of traditional Korean medicine treatment on a patient with an intracerebral hemorrhage (ICH) at the left basal ganglia, thalamus, and posterior limb of the internal capsule. Methods: The patient was treated with acupuncture, moxibustion, Hyangsayukgunja-tang, and Samul-tang-gami. The effect of these treatments were evaluated using the manual muscle test (MMT), modified Barthel index (MBI), and National Institutes of Health Stroke Scale (NIHSS). Results: After the treatments, the patient's status improved, as shown by his MMT grade, which increased from 1/1 to 4/4. In addition, the patient's MBI score improved from 0 to 77, and his NIHSS score declined from 16 to 2. In addition, the patient's Levin tube and Foley catheter were removed. Conclusion: The results suggest that traditional Korean medicine may be effective in the treatment of patients with ICH.
1. 한의학(韓醫學) 고전(古典)에서 사경(斜頸)과 동일한 병명은 없으나 낙침(落枕)(항강증(項强症)), 요두(搖頭), 근급(筋急), 근축(筋縮), 계, 기통(氣痛) 등의 범주에서 찾아볼 수 있다. 2. 후천성(後天性)으로 발생하는 경련성(痙攣性) 사경(斜頸)에 한의학적(韓醫學的) 치료(治療)가 유효함을 입증할 수 있었고, 근맥(筋脈), 경락(經絡), 기혈(氣血), 심비(心脾)의 다양한 측면에서 진단되어야 하며 특히 순기(順氣), 이기해울(理氣解鬱), 서근(舒筋), 양혈보심(養血補心)하는 치료(治療)가 중요함을 알 수 있다. 3. 약물(藥物)치료 뿐만이 아니라 추나요법(推拿療法), 운동요법(運動療法) 및 물리치료(物理治療) 등이 동시에 이루어져야 효과적으로 치료(治療)됨을 알 수 있다. 향후 경련성(痙攣性) 사경(斜頸)을 위와 같은 한의학적(韓醫學的)인 이론에 근거하여 치료한다면 대부분의 임상 실제에서 유효할 것으로 사료된다.
Objectives This study aimed to evaluate the effects of Integrative treatment of traditional Korean medicine (TKM) on 7 hospitalized patients after hip fracture surgery, and to identify significant herbs and co-prescribed herbs by using network analysis and association rule mining. Methods A retrospective chart review of the 7 hospitalized patients treated for postoperative hip fractures between January and December 2021 was performed. All TKM treatments for the patients were identified and Wilcoxon signed-rank test was performed to compare hip pain and mobility on admission and discharge. We visualized the network of herbal medicines and complications. By using network analysis, we also identified the significant herbs (high centrality of degree, eigenvector, and sub-graph). Co-prescription patterns for the hip fracture patients were further analyzed by association rule mining. Results We found that TKM treatment significantly relieved hip pain and improved mobility. Accompanying symptoms reported by the patients were general weakness, anorexia, dizziness, delirium, edema, sputum, sore throat, cough, rhinorrhea, and chills. Herbs composed of Sagunja-tang and Samul-tang showed high centralities and high associations with other herbs. In addition, Gupan, Nokyong, Yukjongyong, Useul, and Hyunhosaek were identified as important herbs for postoperative hip fracture patients. Conclusions This study provides evidence for clinical TKM use as an effective postoperative treatment for pain relief and improvement of mobility in patients with hip fractures. In addition, herbs that can be considered in the treatment of patients after hip fracture surgery were identified through network analysis and association rule mining.
Objectives : This study is to report effectiveness of Korean medicine monotherapy on an acute urticaria patient. Methods : A 26-year-old female acute urticaria patient visited Dong-Eui university Korean medical hospital at November 10th, 2020. The patient received Korean monotherapy including herbal medicine and aroma cream. The treatment continued for about 1 month until December 4th, 2020. Urticaria activity score(UAS), visual analogue scale(VAS), photographs were used to assess the symptoms. Results : After 20 days of medication, the symptoms were disappeared. UAS score was dropped 6 to 0 and VAS score dropped 9 to 0. About two years later, there was no recurrence. Conclusions : These results suggests that acute urticaria which accompanied by a moderate degree of pruritus can be sufficiently managed by Korean medicine therapy.
목 적: 본 연구는 갱년기 우울증 모델에서 사물탕가향부자(SGH)의 항우울 효능 및 면역반응에 대한 실험적 유의성을 확인함으로써 임상적인 응용에 기초자료로 사용하기 위하여 시행되었다. SGH가 반복적인 스트레스를 가한 난소적출 흰쥐에서 항우울행동 효과와 estradiol level 및 anti-inflammatory cytokine인 IL-4에 미치는 영향을 연구하고자 하였다. 방 법: 난소적출 흰쥐에 2주간 반복적인 스트레스를 주고 동시에 SGH(200 and 400 mg/kg/day)를 경구 투여한 후 행동검사인 Elevated Plus Maze(EPM)를 통한 우울행동과 혈청 estradiol, IL-4의 변화를 측정하였고, 또한 뇌 내 Locus coeruleus(LC)와 Paraventricular Nucleus(PVN)에서 IL-4의 변화를 측정하였다. 결 과: 1. EPM에서 SGH 400 mg 투여군은 대조군에 비해서 open arms에 머무는 시간이 현저히 증가하였고, closed arms에 머무는 시간이 현저히 감소하였다. 2. EPM에서 SGH 투여군은 대조군에 비해서 open arms와 closed arms를 교차하는 횟수가 증가하였으나, 통계적인 유의성은 없었다. 3. Estradiol 측정에서 SGH를 투여한 후 estradiol 수준이 현저하게 증가하였다. 4. IL-4 측정에서 SGH를 투여한 후 혈청 IL-4 수준이 현저하게 증가하였다. 5. 뇌에서의 IL-4 면역반응은 대조군에서 감소하였으나, SGH를 투여한 후 LC와 PVN에서 IL-4 수준의 유의성 있는 변화는 나타나지 않았다. 결 론: 이상의 결과로 사물탕가향부자는 난소적출 흰쥐의 항우울행동과 면역조절에 유효함을 알 수 있었다.
