타석은 타액선에 발견되는 흔한 병리 상태 중 하나이다. 타석의 형성과 관련된 기전은 아직까지 명확하게 밝혀지지 않았다. 본 연구에서는 악하선의 만성타액선염을 앓고 있는 58세 여성 환자로부터 얻은 타석을 전자현미경 및 에너지분산 X선 분광법을 통해 화학적인 조성과 미세형상을 분석하였다. 전자현미경 평가에서 고도로 광화된 무정형 핵이 층판의 동심원상 구조로 둘러싸인 형태가 관찰되었으나, 이물질, 유기물질 또는 미생물의 소견은 확인되지 않았다. 에너지분산 X선 분광 분석에서 중심부 핵은 Ca, O 및 P로만 구성되어 있었고, C도 중심부 근처에서 높은 함량으로 검출되었다. 이러한 결과는 타석의 무기물 조성은 주로 수산화인회석 결정으로 되어 있고 중심핵 주변으로 무기-유기 구조가 형성되어 있어, 악하선관 내 타석의 핵이 무기결정핵의 성장을 촉진하는 타액선염에 의해 이차적으로 형성되었을 가능성을 제시한다.
발암물질인 NDMA 17m/kg을 경구 투여했을 때 흰쥐설 von Ebner선과 설 점액선의 glycoconjugates에 미치는 영향을 prelectin 조직화학법인 PAS반응, AB pH 2.5 염색법, AB pH 0.4 염색법, AB pH 2.5 -PAS 염색법, AF pH 1.7-AB pH 2.5 염색법 및 HID-AB pH 2.5 염색법으로 연구하였다. NDMA 투여군의 설 von Ebner선의 형태적인 주된 변화는 장액선포의 위축 및 파괴, 장액세포의 세포질 과립의 감소 및 소실, 세포질의 공포변성 그리고 도관상피세포의 점액화였으며 NDMA 투여후 12시간군부터 72시간군까지 현저하였다. NDMA 투여군의 설점액선의 주된 형태적 변화는 점액선포의 확장, 융합, 파괴 및 점액세포의 세포질의 과립 소실 및 공포변성, 도관상피의 점액화 등이었으며 NDMA 투여후 12시간군부터 72시간군까지 현저하였다. 설 von Ebner선의 neutral glycoconjugates가 대조군에 비해 NDMA 투여후 12시간군과 72시간군에서 현저히 감소되었으며 NDMA 투여후 96시간군후에 회복되는 경향을 나타내었다. 대조군에서 나타나지 않던 acidic glycoconju-gates가 NDMA 투여후 6시간군부터 48시간군, 그리고 120시간군에서 약간의 장액세포들에서 나타났다. 설 점액선의 neutral glycoconjugates와 acidic glycocon-jugates가 대조군에 비해 NDMA 투여후 3시간군, 24시간군, 48시간군에서 다같이 현저히 감소되었으며, 후자는 NDMA 투여후 72시간군에 아주 현저히 감소하였다. Aci-dic glycoconjugates중 sulfated glycoconjugates는 대조군에 비해 NDMA 투여후 감소하는 경향을 나타내며 NDMA 투여후 72시간군에 가장 현저한 감소를 나타내는데 비해 sialic glycoconjugates는 NDMA 투여후 3시간군, 12시간 군 및 48시간군에서 증가하는 경향을 나타내었다.

Epidermal growth factor is a single-chain polypeptide consisting of 53 amino acids and has a potent mitogenic activity that stimulates proliferation of various normal and neoplastic cells through the interaction with its specific receptor(epidermal growth factor receptor, EGFR). Pleomorphic adenoma is the most common salivary benign tumor and histologically, it contains the epithelial cell, the myo-epithelial cell and mesenchymal ingredient, which is various aspect. Adenoid cystic carcinoma is an infiltrative malignant salivary gland tumor with three different histological patterns: cribriform, tubular or solid. The tumor cell structure composed of modified myoepithelial cell, and basaloid cell. In this study, we used an immunohistochemical technique to investigate the expression of EGF in 6 specimens of adenoid cystic carcinoma and 10 specimens of pleomorphic adenoma taken from patients treated at Dept. of Oral and Maxillofacial Surgery, Dankook University. The results were as follows. 1. In pleomorphic adenoma, ductal structure and scattered spindle cells in hyalinized stroma, disclosing myxoid stroma and hyalin, cartilage formation were observed. Immunohistologically, weak EGF expression in ductal structure and negative in stromal area were observed. 2. Cribriform type of adenoid cystic carcinoma showed numerous pseudocyst surrounded by dark small neoplastic cells in the back-ground of fibrous connective tissue and moderate EGF expression of dark cells adjacent to pseudo lumen in cribriform pattern, while weak expression in other most cells. 3. Tubular type of adenoid cystic carcinoma showed numerous ductal pattern surrounded by two layered neoplastic cells in the back-ground of fibrous connective tissue and strong EGF expression in luminal cells of ductal structure, while weak expression in outer cells. From the results obtained, we suggest that EGF is mainly biosynthesized in cells forming duct like structures of tubulo-ductal type or cribriform adenoid cystic carcinoma and it may play a role, as a cell mitogen in adenoid cystic carcinoma growth.
The purpose of this study was to investigate the effects of irradiation on the striated duct cells of the rat submandibular gland ductal tissues which control the characteristics of saliva. For this study, the experimental group was composed of 36 irradiated Sprague Dawley strain rats divided into 8 subgroups 1 hour, 2 hours, 3 hours, 6 hours, 12 hours, 24 hours, 48 hours, 72 hours after irradiation. 4 non-irradiated rats were used as the control group. The experimental animals were singly irradiated with a dose of 18Gy gamma ray to their head and neck region by the Co-6- teletherapy unit and sacrificed after each experimental duration. The specimens were examined with a light microscope with an H-E stain and with a trans- mission electron microscope. The results of this study were as follows. In the light micrograph, a severe atrophic change occurred in the striated duct cells at 2hours after irradiation and gradual recovery occurred from 6 hours after irradiation. 2. The nuclear chromosomes of the striated duct cells were changed granular at 2 hours after irradiation. Recovery was observed at 6 hours after irradiation. Nuclear bodies were also observed from 3 hours after irradiation. 3. The mitochondria of the striated duct cells had indistinct cristae at 2 hours after irradiation, and were degenerated or swollen at 3 hours after irradiation. They recovered, however, from 6 hours, with an increasing number at 48 hours and a regular arrangement was observed at 72 hours after irradiation. 4. The microvilli showed atrophic changes at 2 hours after irradiation and were almost lost at 3 hours after irradiation. They were observed again from 48 hours after irradiation. 5. The rough endoplasmic reticulum and golgi body were not apparent at 1 hour after irradiation and were dilated with degeneration 2 hours after, but intact rough endoplasmic reticulum were observed from 3 hours after irradiation and developed well at 24 hours after irradiation. By the result of this study, showing a mild change in the functional morphology of the salivary striated duct cells immediately following irradiation, it is considered that the many complications which occur after radiation therapy, will disappear in time with the histological and the functional recovery of the glandular tissues.
Myoepithelioma는 타액선에 발생하는 드문 질환이다. Myoepithelioma는 소아 및 청소년보다 성인에서 호발한다. 구개부의 종창을 주소로 8세 여환이 본원으로 내원하였다. 환아의 나이와 상대적으로 큰 종양의 크기를 고려하여, 전신 마취 하에 구개부 점막 조직을 보존하는 보존적인 외과적 절제술이 시행되었다. 수술 부위의 치유가 적절히 일어났으며, 40개월의 관찰기간 동안 재발되지 않았다. 주변의 구개부 조직을 보존하는 최소 침습적 제거술은 소아에서 발생되는 myoepithelioma의 치료시 유용할 것으로 사료되었다.

Background Functional restoration of the facial expression is necessary after facial nerve resection to treat head and neck tumors. This study was conducted to evaluate the functional outcomes of patients who underwent facial nerve cable grafting immediately after tumor resection. Methods Patients who underwent cable grafting from April 2007 to August 2011 were reviewed, in which a harvested branch of the sural nerve was grafted onto each facial nerve division. Twelve patients underwent facial nerve cable grafting after radical parotidectomy, total parotidectomy, or schwannoma resection, and the functional facial expression of each patient was evaluated using the Facial Nerve Grading Scale 2.0. The results were analyzed according to patient age, follow-up duration, and the use of postoperative radiation therapy. Results Among the 12 patients who were evaluated, the mean follow-up duration was 21.8 months, the mean age at the time of surgery was 42.8 years, and the mean facial expression score was 14.6 points, indicating moderate dysfunction. Facial expression scores were not influenced by age at the time of surgery, follow-up duration, or the use of postoperative radiation therapy. Conclusions The results of this study indicate that facial nerve cable grafting using the sural nerve can restore facial expression. Although patients were provided with appropriate treatment, the survival rate for salivary gland cancer was poor. We conclude that immediate facial nerve reconstruction is a worthwhile procedure that improves quality of life by allowing the recovery of facial expression, even in patients who are older or may require radiation therapy.
Background and Objectives: Adenoid cystic carcinoma (ACC) is a unique tumor characterized by frequent and delayed distant metastasis (DM) with uncommon regional lymph node metastasis. We evaluated the factors affecting DM of ACC and survival after appearance of DM. Materials and Methods: Medical records, radiographs and pathologic slides were reviewed for 94 patients from 1979 through 2001. Results: DM of ACC occurred in 46 patients, and developed more frequently in patients with tumors of the solid histologic subtype than in patients with tubular or cribriform subtypes. DM occurred less frequently in the sinonasal tract, and development of DM was not affected by tumor stage. Disease-specific 5- and 10-year survival rates were 88% and 72% for patients without DM, respectively and 76% and 48% for those with DM(p=0.02). Regarding the site of DM and its impact on outcomes, 30 patients had lung metastasis alone, 5 patients bone metastasis alone and 6 patients developed both lung and bone metastasis. Median survivals after appearance of DM among patients with isolated lung metastases and those with bone metastases with or without lung involvement were 54 and 21 months, respectively (p=0.04). Conclusions: Development of DM in ACC is predicted by solid histologic subtype, and major salivary gland or oral/pharyngeal rather than sinonasal primary site. Those patients with bone involvement with our without lung metastases had worse outcomes than those with pulmonary metastasis only.
Background: Mucous retention cyst refers to a cyst made by expansion due to the blockage of the salivary gland near the maxillary sinus, and it is surrounded by epithelial cells. Most of them are small; therefore, they cannot be found well and are frequently with antral polyp. The aim of this study was to evaluate the clinical prognosis of sinus bone graft in patients with mucous retention cyst. Methods: This study was performed retrospectively on 23 patients who had sinus bone graft. Group 1 was 8 patients (10 sinuses) who had a mucous retention cyst, and group 2 was 15 patients (17 sinuses) who had no pathologic history about the maxillary sinus. For these patients, sinus bone graft was performed using the lateral approach technique. The total 51 implants were placed 6.22 weeks on the average after sinus bone graft. Sinus membrane perforation during operation, postoperative complications, marginal bone loss after restorative function, implant success rate, and survival rate were analyzed. Results: There was no complication in group 1, and there were three complications in group 2. In group 2, two cases of implants failed. The types of postoperative complications consisted of two minor infections and one wound dehiscence. Two implants of total 51 implants were removed, and the survival rate of implants was 96.08 % (group 1 100 %, group 2 93.5 %). The total success rate of implants was 92.2 % (group 1 95 %, group 2 90.3 %). Conclusions: The clinical prognosis was not affected by the presence of mucous retention cyst.

Von Ebner's glands (vEG) are minor salivary glands associated with circumvallate and foliate papilla. The secretions of vEG consist of microenvironment of the taste buds in the circumvallate and foliate papillae, and thus saliva from vEG plays a role in the perception of taste. The $Ca^{2+}$ signaling system in rat vEG acinar cell was examined using the $Ca^{2+}$-sensitive fluorescent indicator Fura-2. Agonist-induced increase in intracellular $Ca^{2+}\;([Ca^{2+}]_i)$ was stimulated by carbachol (CCh) and substance P (SP), but not by norepinephrine (NE), and recovered to control levels by their receptor antagonists dose-dependently. The effects were also observed in $Ca^{2+}$-free medium, suggesting mobilization from intracellular $Ca^{2+}$ store. These results in the vEG acinar cell indicate that 1) $[Ca^{2+}]_i$ is at least regulated by muscarinic and neurokininergic (NK1) receptors; 2) the increases in $[Ca^{2+}])i$ activated by CCh and SP are mainly mediated by discharge of cytosolic calcium pool.
Kimura씨 병은 원인이 알려지지 않은 만성 염증성 질환으로 주로 동양의 젊은 남자에게 호발되는 드문 질환이다. 주로 두경부의 피하 조직이나 타액선에 무통성의 종괴로 발생하며, 주변의 국소 림프 병증을 동반하기도 한다. 말초 혈액에 호산구 증가증과 혈청 IgE 증가를 나타내고, 조직학적으로 이환된 림프절의 과증식된 배중심과 호산구 등의 염증세포 침윤의 병리 소견을 보인다. 치료 방법으로는 스테로이드 치료, 방사선 치료 및 수술적 치료 등이 있으며 예후는 양호하다. 저자들은 28세 남자 환자의 좌측 상완 원위부 내측에 발생한 종괴를 변연부 절제술을 시행하여, 병리 조직학적으로 Kimura씨 병으로 확진한 1예를 문헌 고찰과 함께 보고하는 바이다.
