이하선과 상경부에 동시 발생한 외투세포 림프종 1예 (A Case of Mantle Cell Lymphoma Involving the Parotid Gland and Upper Neck)

  • The Mantle cell lymphoma(MCL) is a relatively rare subtype of malignant lymphoma characterized by chromosomal translocation t(11 ; 14)(q13 ; q32), positive response for CD5, and nuclear cyclin D1. It is account for an estimated 3-6% of all non-Hodgkin's lymphoma. The involvement of extra-nodal site is not uncommon, whereas salivary glands are rarely affected. It is more commonly occurred in men and old age and approximately 75% of cases are diagnosed with advanced stage. It is usually characterized by an aggressive clinical course, and the prognosis is poorer than other type of head and neck lymphoma. We recently encountered a 69-year-old female with mass in parotid tail and upper neck, and it was diagnosed as mantle cell lymphoma. We report the unique case with a review of literature.

경부전이를 동반하여 이하선에 발생한 기저세포선암 1예 (A Case of Basal Cell Adenocarcinoma with Neck Metastasis in the Parotid Gland)

  • Basal cell adenocarcinoma is a extremely rare low grade malignancy of the salivary gland. It is composed of basaloid cells with dark cell, light cells and pale cytoplasm which shows infiltrative growth pattern into adjacent glandular parenchyme. The pathophysiology of basal cell adenocarcinoma is not well known. First hypothesis is a malignant transformation of the monomorphic adenoma and second one is a de novo origin. Metastasis to the regional lymph nodes is rare. Recently, we have experienced a case of basal cell adenocarcinoma with neck metastasis after previous superficial parotidectomy. For this reason we report this rare case discussed with references.

호산구 증다증을 동반하지 않은 기무라병 1예 (A Case of Kimura's Disease Without Eosinophilia)

  • Kimura's disease is a rare disease of unknown etiology, commonly presenting with slow-growing head and neck subcutaneous nodules. It primarily involves the head and neck region, presenting as deep subcutaneous masses and is often accompanied by regional lymphadenopathy and salivary gland involvement. Clinically it is often confused with a parotid tumor or lymph node metastasis. It is difficult to diagnose before surgery, and fine needle aspiration cytology has only limited value. Even though this disease has not shown any malignant transformation, it is often difficult to cope with because of its high recurrence rate. Surgery, steroids, and radiotherapy have been used widely as the first-line recommendation, but none of them is standard procedure until now because of high recurrence rates. The recurrence of the disease reported up to 62%. We recently experienced a case of Kimura's disease, not accompanying peripheral eosinophilia, on the parotid gland treated by surgical resection in an 82-year-old woman with polycythemia vera. Here, we report this case with a review of the literature.

  • Epithelial-myoepithelial carcinoma(EMC) is a rare, low grade malignant tumor of the salivary glands. The EMC has a distinctive histological appearance comprising ductal structures with an inner epithelial cell component and an outer layer of myoepithelial cells which show plump clear cytoplasm. The cytologic features of the EMC have been rarely described. A correct cytological diagnosis to this rare tumor is difficult with high false negative rate. We report a case of EMC in which fine needle aspiration cytologic findings were misinterpreted as a pleomorphic adenoma.

  • Sialolithiasis is the formation of calcareous concretions within ductal system of a major or minor salivary gland. They are formed by deposition of calcium salts around a central nidus which may consist of desquamated epithelial cells, bacteria, foreign bodies, or products of bacterial decomposition. An 11-year-old boy complained of pain during meals and intermittent mild swelling in the right submandibular region. Although it was not detected in true occlusal radiograph, panoramic radiograph showed a round radiopaque mass 3mm in diameter. Computed tomography(CT) was taken for locating the stone and 3-dimensional reconstruction was performed. Under general anesthesia, sialoadenectomy was done through extraoral approach. Diagnosis of submandibular sialolithiasis using high-resolution CT with reconstructions was helpful for surgical decisions, namely radical removal of the submandibular gland and its duct.

  • Running title: A plunging ranula extended into parapharyngeal space Ranulas are lesion of sublingual gland origin, which occur in the floor of the mouth. Most ranulas, whether simple or plunging, are pseudocysts without and epithelial lining and ranulas have higher levels of salivary amylase and protein content. They can be classified into two types based on their extent: simple ranulas, confined to the sublingual space and plunging ranula which extend into adjacent spaces. Plunging ranula requires differential diagnosis with other lesions (neuroma, monomorphic adenoma, hemangioma, lipoma, dermoid cyst, lateral cervical cyst). The patient was diagnosed as plunging ranula. We experienced 17 years old male, visited to our department, who complain Rt. cervical swelling and in MRI view, this lesion involved sublingual, submadibular, parapharyngeal, skull base. We experienced a rare case of plunging ranula, extended into parapharyngeal space. We report the case.

가토 악하선이 조영제 주입후 방사선 및 조직학적 연구 (Radiographic and Histologic Study After Infusion of Contrast Media into Rabbit Submandibular Gland)

  • 50 submandibular glands of rabbits were examined historadiologically after infusion with normal and over volumes of physiologic saline and 5 radiographic contrast media. The results were as follows: 1. All water soluble contrast media showed similar radiographic contrasts and absorbed about 5 minutes after infusion except Telebrix 30 which took 30 minutes in both normal and overfilled glands. 2. Lipid soluble medium, Lipiodol UF had excellent radiographic contrast and could be seen on the radiograms even after 24 hours after infusion. 3. Salivary glands infused with physiologic saline didn't show any histologic changes except slight duct dilation right after infusion. 4. Telebrix 30 caused mild to moderate duct dilation and inflammation at immediate & 24 hours after infusion which was more severe in overfilled glands. At 7 days after infusion, there were mild to moderate fibrosis of the gland and areas of necrosis was seen in overfilled glands. 5. Hypaque 60% showed similar histologic reactions to Telebrix 30 except more severe tissue destruction at 7 days after infusion. 6. Urografin 60% showed mildest histologic changes among the media used in the study. 7. Biliscopin had mild duct dilation which returned to normal after 7 days but there were moderate inflammation and tissue necrosis at that time. 8. Lipiodol UF showed severe duct dilation with numerous vacuoles and there were tissue fibrosis at 7 days after infusion but no tissue necrosis was seen.

귀밑샘의 상피-근육상피암종의 세침흡인 세포소견 -2예 보고- (Fine-Needle Aspiration Cytology of Epithelial-Myoepithelial Carcinoma of Parotid Gland -A Report of Two Cases -)

  • Epithelial-myoepithelial carcinoma (EMC) is a rare low grade malignant tumor of the salivary glands and it shows a characteristic biphasic population of epithelium and myoepithelium. It shows various cytologic and histologic features, so making an exact diagnosis is difficult. We report here on two cases of epithelial-myoepithelial carcinoma arising from the parotid gland and we compare the cytologic findings of the aspirated samples with the histologic findings of the tumors. We think the finding of mixed pattern of large, clear myoepithelia and small epithelia is the most valuable finding in the diagnosis of EMC.

  • Fine needle aspiration is usually performed as a preoperative cytologic evaluation in salivary gland tumors, and complications of the lesion after fine needle aspiration are scarcely developed. The secondary change of tissue by fine needle aspiration can make the diagnostic and subsequent therapeutic difficulties for clinicians and require a careful approach. Fine needle aspiration can cause variety of changes in Warthin tumor, and it is presumed that those changes are mainly caused by the infarction of the mass due to vascular injury and following inflammatory reactions. We would introduce our experience of granulomatous and necrotic change after fine needle aspiration on parotid Warthin tumor with consequent difficulties in diagnostic and surgical approach to the patient.

  • Extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) is a low-grade B-cell lymphoma that typically follows an indolent clinical course. It occurs in a variety of mucosal linings extranodal tissue, most commonly in the stomach. Other commonly involved sites include other parts of the gastrointestinal tract, thyroid, salivary gland, lung, lacrimal gland, synovium, dura mater, breast, skin, and eyes. It occurs very rarely in the buccal mucosa. A 50-year-old man came to the clinic while presenting a 5-month history of right-sided buccal mass. The incisional biopsy did not confirm the diagnosis of the lesion. He underwent complete excision of buccal mass for the diagnosis and treatment. The final pathology confirmed MALT lymphoma immunohistochemically. After surgery, he received radiotherapy with 30.6 Gy. There is no recurrence for 8 months after treatment. Herein we report a rare case of buccal MALT lymphoma with a review of the literature.
