시스템 다이내믹스 시뮬레이션 기법을 활용한 원전 안전문화 요소간 영향관계 분석 (An Analysis of Safety Cultural Elements Relationships in Nuclear Power Plant by System Dynamic Simulation)

  • This study analyses the inter-relationships between Safety Cultural Elements by System Dynamics approach. Base Frame for Safety Culture, which is originated from IAEA, NRC and INPO's Safety Culture Documents, helps to elaborate the Causal Loop Diagram of Safety Culture in Nuclear Power Plant(NPP). Also, the simulation results show that ownership of employees is degraded continually and adherence of technical standards is violated because workloads of the employees cannot be minimized and stress and time pressure maintains a high level in NPP.

  • Objective: This study proposes a systematic process to present the analysis methods and solutions of organizational root causes to human errors on the railroad. Background: In fact, organizational root cause such as organizational culture is an important factor in the safety concerns on human errors in the nuclear power plant, railroad and aircraft. Method: The proposed process is as follows: 1) define analysis boundary 2) select human error taxonomy 3) perform accident analysis 4) draw root causes with FGI 5) review root causes analysis with survey 6) chart analysis of root causes, and 7) propose alternatives and solutions. Results: As a result, root causes of the organizations like railroad and nuclear power plant came from the educational problems, violations, payoff system, safety culture and so forth. Conclusion: The proposed process does predict potential railroad accident through retrospect error analysis by building new human error taxonomies and problem solution. Application: This study would contribute to examination of the relationship between human error-based accidents and organizational root causes.

의과대학 학생들의 환자안전에 대한 인식 (Medical Students' Perceptions and Intentions Regarding Patient Safety)

  • Background: The purpose of this study was to examine undergraduate medical students' perceptions and intentions regarding patient safety during clinical clerkships. Methods: Cross-sectional and self-administered questionnaire survey was conducted on 34 students from one medical school using a modified version of the Medical Student Safety Attitudes and Professionalism Survey (MSSAPS). We assessed $4^{rd}-year$ medical students' perceptions of the cultures ('safety', 'teamwork', and 'error disclosure'), 'behavioural intentions' concerning patient safety issues and 'overall patient safety'. The overall response rate was 66.4%. Results: Among safety domains, "teamwork culture" was rated highest. "Error disclosure culture" received the lowest ratings. Regarding the error disclosure domain, only 10% of respondents reported that they have received education or training on how to disclose medical error to patients. Independent of survey domains, when students were asked "Overall, do you think your hospital is safe based on your clinical rotation?", 61.8% reported that the hospital was safe. Conclusions: Assessing students' perceptions of safety culture can provide clerkship directors and educators with information that enhances the educational environment and promotes patient safety. Discussions of medical errors, patient safety, and how best to incorporate an analysis of these issues into the existing curriculum are needed.

종합병원 간호사의 환자안전문화에 대한 인식이 안전간호활동에 미치는 영향 (The impact of Perceived Patient Safety Culture on Safety Care Activities in General Hospital Nurse's)

  • 본 연구는 종합병원 간호사를 대상으로 환자안전문화에 대한 인식과 안전간호활동에 미치는 영향을 파악하고 변인을 규명하기 위함이다. D시와 G시에 위치한 종합병원의 간호사 178명을 대상으로 하였다. 자료는2015년 6월 20일부터 7월 30일까지 수집하였으며, Kim 등이 수정 보완 한 환자안전문화에 대한 인식과 Lee가 개발한 안전간호활동을 연구도구로 사용하였다. 수집된 자료는 t-test, One-way ANOVA, Pearson correlation coefficients와 단계적 중다회귀분석하였다. 환자안전문화에 대한 인식은 환자안전간호활동과 양의 상관관계가 있었다(r=.407, p<.001) 본 연구 결과 대상자의 병원 근무 환경은 안전간호활동에 43.3%의 영향력이 있는 것으로 나타났으며, 병원간호사의 안전간호활동에 병원근무환경이 가장 중요한 변인으로 볼수 있다. 안전간호의 질 향상을 위하여 환자안전문화에 대한 인식을 높일 수 있는 구체적인 방안과 안전간호 지침이 필요하다.

환자안전에 관한 문헌 연구: 국내연구를 중심으로 (A literature review of patient safety in Korea: focused on domestic studies)

  • Objectives: The purpose of this study is to provide basic data on the continuous management and institutional measures in the future by understanding the research trends of patient safety in healthcare field. Methods:The data were extracted from 2011-2016 KoreaMed, KMBase, KISS, NDSL and KISTI. Data were analysis by frequency analysis using the SPSS 14.0 program. Results: 87.0% of the studies were quantitative studies. As for the method of sampling, 'No use' was the highest at 56.5%. Most of the participants in the study were 'nurses' (50.7%). 19 hospitals (35.8%) were the most common. The subjects of the study consisted of 35 (51.5%) patients' safety culture (awareness) and 20 (29.4%) 'safety nursing activities'. Conclusions: Patient safety and patient safety should be maintained. Further, a mature patient safety culture should be settled through cooperation management among medical staff.

  • Purpose: This study aims to compare and analysis job stress, patient safety culture, and patient safety management activities of nurses in comprehensive nursing care service wards and general wards. Methods: Through an online survey conducted on nurses with more than three months of working experience at five general hospitals in the metropolitan area, 156 participants's data were used for 𝜒2-test, t-test, ANOVA using SPSS ver. 26.0. Result: There were no significant differences in the nurses' job stress and patient safety management activities between the comprehensive nursing care service wards and general wards. However, the patient safety culture of nurses in the comprehensive nursing care service ward was significantly lower than that of general ward. Conclusion: Patient safety culture should be promoted through continuous regular patient safety education, training, and organizational support to increase patient safety management activities. Additionally, institutional programs should be prepared to reduce job stress of nurses at the frontline of patient safety.

안전의식 수준 확보를 통한 항공 안전문화 증진 방안 연구 (A Research on Enhancing Aviation Safety Culture by Building the Level of Safety Awareness)

  • Demand for air travel, which shrank sharply following the outbreak of the COVID-19, has recently recovered up to 80% before the spreading pandemic. As a result, the number of passengers are rapidly increasing again. However, financial difficulties and lack of skilled workers in the aftermath of a long-term recession and pandemic lead to a decrease in aviation safety awareness which is fundamental for safety culture. Consequently, this hazard can act as a potential risk factor for the global aviation industry preparing for a new leap forward in the post-pandemic era. Therefore, this study analyzed and defined the four key elements of aviation safety awareness; attitude, belief, perception, and value. Based on this analysis, this study combined the four key elements into the P-D-C-A cycle to derive what top management, middle managers, and workers should each do to continuously improve the positive safety culture.

조직문화가 서비스품질에 미치는 효과 및 재직기간의 조절효과 (The relationship between organizational culture and service quality, and the moderating effect of tenure)

  • This paper reviewed the relationship between organizational culture and service quality, and the moderating effect of tenure in service business. Based on the responses from 586 employees, the results of multiple regression analysis showed that rational culture and affiliation culture have positive relationships with all service quality factors. The results of moderating analysis showed that rational culture has more positive relationships with tangibleness, reliability, responsiveness, and empathy in which tenure is longer. Also progressive culture has more positive relationship with empathy in which tenure is longer.

Analysis of School Safety Education Utilization with Educational Game Elements

  • In order to create and utilize experiential equipment that can be effectively used in school safety education, this paper uses the Korean Safety Education Association's CPR simulator to utilize the elements of educational games: goals, rules, competition, challenge, fantasy, safety, and fun. When the content that combines game elements with general educational equipment was utilized in school education sites, significant results were obtained on the effectiveness of education with active participation of students.

비행 스트레스가 직무 만족도와 안전문화에 미치는 영향 : 항공사 및 직급별 조절된 매개효과 (The Effect of Flight Stress on Job Satisfaction and Safety Culture: Moderator Effect by Airlines and Rank)

  • 본 연구는 국내 민간 항공사 조종사 193명을 대상으로 비행 스트레스가 직무 만족도 및 안전문화에 미치는 영향을 분석하였다. 분석 결과, 비행 스트레스는 직무 만족도에 부정적 영향을 미쳤으며, 직무 만족도가 낮은 조종사는 안전문화 인식도 낮게 나타났다. 특히, 항공사와 조종사의 직급에 따라 비행 스트레스가 직무 만족도와 안전문화에 미치는 영향에 차이를 보였다. 이는 항공사와 조종사 직급별로 맞춤형 스트레스 관리 프로그램 개발의 필요성을 시사한다. 연구는 항공 산업의 안전 문화 강화를 위해 조종사의 스트레스 관리와 직무 만족도 향상이 중요함을 강조한다. 또한, 항공사는 조종사의 스트레스를 감소시키고, 직무 만족도를 높여 안전문화를 강화하는 전략을 개발해야 한다. 본 연구 결과는 항공산업에서 조종사들의 스트레스 관리와 안전문화 강화를 위한 방안 모색에 유용한 기초 자료로 활용하기 위함이다.
