A Study on the Development of Assessment Index for Catastrophic Incident Warning Sign at Refinery and Pertrochemical Plants (정유 및 석유화학플랜트 중대사고 전조신호 평가지표 개발에 관한 연구)

  • In the event of a major accident such as an explosion in a refinery or a petrochemical plant, it has caused a serious loss of life and property and has had a great impact on the insurance market. In the case of catastrophic incidents occurring in process industries such as refinery and petrochemical plants, only the proximate causes of loss have been drawn and studied from inspectors or claims adjustors responsible for claims of property insurers, incident cause investigators, and national forensic service workers. However, it has not been done well for conducting root cause analysis (RCA) and identifying the factors that contributed to the failure and establishing preventive measures before leading to chemical plant's catastrophic incidents. In this study, the criteria of warning signs on CCPS catastrophic incident waning sign self-assessment tool which was derived through the RCA method and the contribution factor analysis method using the swiss cheese model principle has been reviewed first. Secondly, in order to determine the major incident warning signs in an actual chemical plant, 614 recommendations which have been issued during last the 17 years by loss control engineers of global reinsurers were analyzed. Finally, in order to facilitate the assessment index for catastrophic incident warning signs, the criteria for the catastrophic incident warning sign index at chemical plants were grouped by type and classified into upper category and lower category. Then, a catastrophic incident warning sign index for a chemical plant was developed using the weighted values of each category derived by applying the analytic hierarchy process (pairwise comparison method) through a questionnaire answered by relevant experts of the chemical plant. It is expected that the final 'assessment index for catastrophic incident warning signs' can be utilized by the refinery and petrochemical plant's internal as well as external auditors to assess vulnerability levels related to incident warning signs, and identify the elements of incident warning signs that need to be tracked and managed to prevent the occurrence of serious incidents in the future.

Oil Fluorescence Spectrum Analysis for the Design of Fluorimeter (형광 광도계 설계인자 도출을 위한 기름의 형광 스펙트럼 분석)

  • To evaluate the degree of contamination caused by oil spill accident in the sea, the in-situ sensors which are based on the scientific method are needed in the real site. The sensors which are based on the fluorescence detection theory can provide the useful data, such as the concentration of oil. However these kinds of sensors commonly are composed of the ultraviolet (UV) light source such as UV mercury lamp, the multiple excitation/emission filters and the optical sensor which is mainly photomultiplier tube (PMT) type. Therefore, the size of the total sensing platform is large not suitable to be handled in the oil spill field and also the total price of it is extremely expensive. To overcome these drawbacks, we designed the fluorimeter for the oil spill detection which has compact size and cost effectiveness. Before the detail design process, we conducted the experiments to measure the excitation and emission spectrum of oils using five different kinds of crude oils and three different kinds of processed oils. And the fluorescence spectrometer were used to analyze the excitation and emission spectrum of oil samples. We have compared the spectrum results and drawn the each common spectrum regions of excitation and emission. In the experiments, we can see that the average gap between maximum excitation and emission peak wavelengths is near 50 nm for the every case. In the experiment which were fixed by the excitation wavelength of 365 nm and 405 nm, we can find out that the intensity of emission was weaker than that of 280 nm and 325 nm. So, if the light sources having the wavelength of 365 nm or 405 nm are used in the design process of fluorimeter, the optical sensor needs to have the sensitivity which can cover the weak light intensity. Through the results which were derived by the experiment, we can define the important factors which can be useful to select the effective wavelengths of light source, photo detector and filters.

  • Rapidly increasing automobile supply rate according to improved economic level of life makes lack of parking space of apartments. Even though the initial design of parking space compiled with old regulations, it may not observe either new laws or requirement of inhabitants. Even if old apartments have no structural durability problem, outworn facilities and insufficient parking area may be a main reason for reconstruction. It causes waste of national resources and makes recycling issues. Additionally, irregularly parked cars make traffic obstruction to a fire engine and result in many fire accident victims. Parking problems of apartments are not only inconvenience but also serious safety issues. From these points of view, remodeling only for parking area expansion is necessary to avoid overall reconstruction of apartments. The purpose of this study is to suggest a retaining wall selection method for apartments underground parking lots expansion without evacuation of resident people. Effect factors to select retaining wall system are analyzed and weight values are calculated by applying AHP. One selection method of retaining wall is proposed by evaluating applicability and its sensitivity analysis is executed. This selection method is expected to help decision-making of retaining wall system selection.

A Proposition on Reasonable Termination Procedure in Remaining-oil Recovery Work from a Sunken Tanker in Korean Territory (국내 침몰유조선 잔존유 회수작업의 합리적 종료절차 제안)

  • In case of sunken tankers, remaining-oil recovery operation should be conducted due to possible oil spill accident. However, the deep sea operation make difficulties in checking the completion of remaining oil recovery process, therefore the work termination procedure is very important. In this paper, a reasonable work termination procedure through the comparison and analysis of two cases(Youil No.1 and Osung No.3, Kyung-Shin) which were performed in different method, using disparate equipment. By investigating previously applied methods and techniques, work speed, safety and expenses were compared. The proposed ending procedure of the remaining-oil recovery project is to recover the remaining oil from each cargo tanks and to clean up such tanks whilst an independent surveyor proceeds to a confirmation procedure whereby said surveyor checks out whether any remaining oil exists by putting a stick in each cleaned up tanks and opening up the hatch cover of the tanks or the top place of the tanks to confirm the cleanness of oil. Such procedure shall be done through discussion by the ordering party, contractor and the independent surveyor all together with a flexible application.

A Study on Preparing Measures for Reducing Aggressive Driving and Road Rage by Analysing Mechanism of How the Driving Behavior Determinants and Dangerous Driving Behavior Factors Affect Aggressive Driving and Road Rage: Targeting Traffic Law Violator and Assaulter of a Traffic Accident (운전행동 결정요인과 위험운전 행동요인이 난폭운전과 보복운전에 미치는 메커니즘 분석을 통한 대책마련 연구: 교통법규위반자 및 교통사고야기자를 대상으로)

  • The purpose of this study is to prepare countermeasures for aggressive driving and road rage which have recently become a hot issue by analysing mechanism of how the driving behavior determinants(personal anger and aggression) and dangerous driving behavior factors(aggressive driving behavior and over-speeding driving behavior, drunk driving behavior, inattentive driving behavior, and inexperience driving behavior) affect aggressive driving and road rage. From the survey conducted by seven branches of the Road Traffic Authority with 351 people who were traffic offenders and drivers who caused car accidents, this study obtained three results as follows. First, seriousness of aggressive driving and road rage and requirements as types of customized educations, proper length of time for education, and contents of education can be understood. Second, specific relation and mechanism between the driving behavior determinants and dangerous driving behavior factors with respect to aggressive driving and road rage can be clearly identified, which helps to set order of priority and weighting of measures for reducing aggressive driving and road rage. Third, countermeasures can be categorized as corporate measures or customized measures through mechanism analysis model of aggressive driving and road rage.

  • In a loss-of-coolantaccident, considerable debris may be generated and transported to the recirculation sump. The accumulation of debris will reduce the netpositivesuctionhead and threaten the safety of thenuclear power plant. Both NEI 04-07 and USNRC SER suggesteda CFD methodology. However, additional investigation is needed to consider the unique characteristics of nuclear power plants. The transport of the generated debris is strongly influenced by the break location and the plant characteristics, including the configuration.In this paper, a CFD methodology for blow-down transport evaluation is proposed and applied to an OPR1000 nuclear power plant. The results show that the percentage of small debris transported to the upper containment is 32%, which is 7% larger than the valuegiven in the NEI 04-07 baseline analysis. This result may be used as a point of reference in future analytical studies.

  • Reduced crew performance is frequently cited as a major causal factor in maritime accident causation. Although considerable research has been conducted on the hours of work undertaken by seafarers through interviews and the analysis of records, experimental studies to observe the effects of factors such as high workload, shift patterns, stress, sleep deprivation and disturbance on the cognitive performance of mariners have been limited. Other safety-critical transport industries, such as aviation and rail, have developed fatigue management tools to help manage the work patterns of their operators. Such a tool for mariners would assist shipboard crew, marine pilots and shore management in planning and improving work schedules. The overall aim of this paper is to determine a fatigue factor, which can be applied to human performance data, as part of a software program that calculates total cognitive performance. This program enables us to establish the levels of cognitive performance of a group of marine pilots to test a decision-making task based on radar information. This paper addresses one of the factors that may contribute to the determination of various fatigue factors: the effect of different work patterns on the cognitive performance of a marine pilot.

A study on the improvement of the air exhaust system at the PSD installed subway station (도시철도 지하역사 PSD 설치에 따른 배기시스템 개선 연구)

  • Platform screen door (PSD) installed at underground subway station has reduced the safety accident, but it may cause poor air ventilation condition due to the isolated exhaust duct in the subway tunnel area. In this study, the additional ventilation system was suggested, which can be installed at a void space (i.e., storage room under stairs) of platform in order to improve efficiency of air ventilation rate. Exhausted air from platform was directed to underneath of platform and joined with existing ventilation duct of train exhaust system (TES). One subway station in Seoul city was selected to predict the effectiveness of the suggested lower exhaust system by using the computational fluid dynamics (CFD) analysis. The predicted mean age of air was decreased by 16.5% which proves the improvement of air ventilation efficiency when the suggested lower exhaust system was applied.

  • A spent fuel storage cask is required to prove the safety of a canister under a hypothetical accidental drop condition. A hypothetical accidental drop condition means that a canister is assumed to be a lee drop on to a pad of the storage cask during loading it into a storage cask. A pad of the storage cask absorbs shock to maintain the structural integrities of a canister under a hypothetical accidental drop condition. In this paper a finite element analysis for various pad structures was carried out to improve the structural integrity of a canister under a hypothetical accidental drop condition. A pad of a storage cask was designed a steel structure with concrete. The 1/4 height of a pad was modified with a structure composed of a steel and a polyurethane foam as a impact limiter. The effect of a shape of a steel structure was studied. The effects of the thickness of a steel structure and the density of a polyurethane foam was also studied.

  • Background : This study was to reduce incidence of falls by analyzing actual problem and drawing out improvement plan applicable to the clinical practice through operation of the staff nurses-centered fall peer review group. Method : The fall peer review group was composed of 8 nurses having patient nursing experience for over 5 years, and each of fall cases was reviewed and the root cause was analyzed. As a result, it was found that the patients and their families did not fully understandthe content of the education, and the staff nurses did not completely inspect the risk factors of falls and perform immediate intervention when patient's condition changed. Based on the above-mentioned results, improvement activity was conducted for the purposes of consolidating patients education method and supplementing computerized system to support nurses' decision making as well as devices and facilities. Result : As a result of conducting improvement activity in the aspects of education for patients, support of nurse's decision-making, and devices and facilities through operation of the staff nurses-centered fall peer review group, falls decreased by 9.5% compared to before improvement activity. Conclusion : It is concluded that operation of the clinical nurses-centered fall peer review group played a role of promoter to draw out practical and applicable improvement plan to the clinical practice and apply directions of the field-centered, and increased nurses' interest in falls and ultimately, reduced incidence of falls. Therefore the Center will continue to operate the staff nurses-centered peer review group, and recommends participation of nurses who actually take the charge of nursing patients in further analysis of patients' safety accidents.
