Phase II Study on Safety and Efficacy of Yadanzi® (Javanica oil emulsion injection) Combined with Chemotherapy for Patients with Gastric Cancer

  • Objective: To investigate the efficacy and safety of Yadanzi$^{(R)}$ (Javanica oil emulsion injection) combined with chemotherapy for treatment of patients with advanced gastric cancer. Methods: From January 2011 to December 2012, we recruited 75 patients with advanced gastric cancer, who received javanica oil emulsion injection together with chemotherapy. After two cycles of treatment, efficacy and safety of the combined therapies were evaluated. Results: Overall response rate of 75 patients after treatment was 85.3% (CR+PR+SD). Treatment related side effects were recorded. No treatment related death occurred. Conclusions: Javanica oil emulsion injection combined with chemotherapy could be considered as a safe and effective regimen in treating patients with advanced gastric cancer. Further randomized clinical trials should be conducted to confirm whether the addition of Yadanzi$^{(R)}$ to chemotheraphy could be associated with reduced toxicity, enhanced tolerability and improved quality of life for patients with advanced gastric cancer.

  • Objective The objective was to evaluate changes in swallow safety and dietary status after the transcutaneous injection of botulinum toxin into the upper esophageal sphincter in a series of outpatients with dysphagia. Methods Patients who were at risk for aspiration and who had an unsuccessful trial of swallowing therapy were admitted to the study. All patients showed significant pooling of fluids in the pyriform sinus. All patients were treated in the office; none had previous esophageal dilatation. The upper border of the cricoid cartilage was identified using standard electromyogram procedures and botulinum toxin was injected. Outcomes were assessed using the penetration-aspiration scale, NIH swallowiwng safety score, patients' short-term and long-term subjective impressions of their ability to swallow, and change in dietary status. Results Ten patients underwent an instrumental evaluation of swallowing function. Of the 10 patients, 9 showed an overall improvement in their ability to take an oral diet safely. The penetration-aspiration scale, NIH swallowiwng safety score, patients' short-term and long-term subjective impressions of their ability to swallow, and change in dietary status were significantly improved after office-based botox injection. Conclusion Office-based EMG guided botox injection to the cricopharyngeus muscle is a simple, safe, and effective tool for dysphagia patients. Injection of Botox in the office should be considered when the dysphagia pattern is aspiration after swallow.

Modeling and analysis of selected organization for economic cooperation and development PKL-3 station blackout experiments using TRACE

  • A series of tests dedicated to station blackout (SBO) accident scenarios have been recently performed at the $Prim{\ddot{a}}rkreislauf-Versuchsanlage$ (primary coolant loop test facility; PKL) facility in the framework of the OECD/NEA PKL-3 project. These investigations address current safety issues related to beyond design basis accident transients with significant core heat up. This work presents a detailed analysis using the best estimate thermal-hydraulic code TRACE (v5.0 Patch4) of different SBO scenarios conducted at the PKL facility; failures of high- and low-pressure safety injection systems together with steam generator (SG) feedwater supply are considered, thus calling for adequate accident management actions and timely implementation of alternative emergency cooling procedures to prevent core meltdown. The presented analysis evaluates the capability of the applied TRACE model of the PKL facility to correctly capture the sequences of events in the different SBO scenarios, namely the SBO tests H2.1, H2.2 run 1 and H2.2 run 2, including symmetric or asymmetric secondary side depressurization, primary side depressurization, accumulator (ACC) injection in the cold legs and secondary side feeding with mobile pump and/or primary side emergency core coolant injection from the fuel pool cooling pump. This study is focused specifically on the prediction of the core exit temperature, which drives the execution of the most relevant accident management actions. This work presents, in particular, the key improvements made to the TRACE model that helped to improve the code predictions, including the modeling of dynamical heat losses, the nodalization of SGs' heat exchanger tubes and the ACCs. Another relevant aspect of this work is to evaluate how well the model simulations of the three different scenarios qualitatively and quantitatively capture the trends and results exhibited by the actual experiments. For instance, how the number of SGs considered for secondary side depressurization affects the heat transfer from primary side; how the discharge capacity of the pressurizer relief valve affects the dynamics of the transient; how ACC initial pressure and nitrogen release affect the grace time between ACC injection and subsequent core heat up; and how well the alternative feeding modes of the secondary and/or primary side with mobile injection pumps affect core quenching and ensure stable long-term core cooling under controlled boiling conditions.

  • A burning principle in gasoline engine is the one of being burned, by which a mixer in air and gasoline enters a combustion chamber and causes a spark in the proper timing. This is formed, by which ECU controls the fuel-injection volume and the fuel-injection timing, and determines the performance of engine. The purpose of this study is to test the characteristics on knocking in gasoline engine with the knocking-sensor equipment and to research into the characteristics in knocking while directly controling the optimal igniting timing and the fuel-injection timing through engine ECU. Given controlling ECU by grasping the characteristics in knocking, which becomes the most problem in the engine tuning market, the tuning in a true sense will be formed in gasoline engine.

  • In this paper, an electronic control unit is developed using 16bit microcomputer for automobile engine. This system incorporate AFS(Air Flow Sensor) of Hot Wire type, DIS(Direct Ignition System), ISC(Idle Speed Control) system, CAS(Cranke Angle Sensor) and other peripheral device. This system includes hardware and software to facilitate precision control of both fuel injection and ignition timing. Especially, this controller consists of position signal(180 teeth) and 4 REF signals. Present system has maximum $720^{\circ}CA$ delay. But this system has maximum $180^{\circ}CA$. Thus, this system is able to precision control both fuel injection and ignition timing.

  • Recently, industrial business environments have rapidly changed and face severe competitive challenges. The effective inventory system enables to product and deliver the products quickly for meeting due date of customer's order in this environment. This study have developed a web-based inventory system framework using RFID for a plastic injection molding industry. The system analysis inventory problem issues such as inventory planning, warehouse assignment. In this study, web-based inventory system using Java language is proposed and implemented. As the result of implementation of the system, we expected that it manages to inventory planning continually and systematically.

물분무노즐의 분사각이 화재실 내부의 온도장에 미치는 영향 (Effect of Spray Angle of Water Mist Nozzle on Temperature Field of Compartment Fire)

  • The present study investigates the effect of injection angle of water mist on fire suppression characteristics by numerical simulation. In order to validate the temperature field by numerical simulation, the predicted results are compared with experimental data. It shows that the temperature difference between measurements and predictions are within $10^{\circ}C$. Numerical simulations of fire suppression are performed for 4 different injection angle($60^{\circ}$, $90^{\circ}$, $^120{\circ}$, and $180^{\circ}$). The global mean temperature over the fire compartment decrease with increasing of spray angle. The result shows that the heat transfer between droplets and gas phase are enhanced with the increasing of spray angle. Near the fire source, temperature field by the wide spray angle is slightly higher than that of narrow injection angle because of direct cooling of fire source.

새로운 임시적 성대 주입물 : Radiesse Lite에 대한 예비 임상실험 (Preliminary Results of Clinical Application with a New Temporary Vocal Fold Injection Material : Radiesse Lite)

  • Background and Objectives : Temporary vocal fold injection is not only performed to restore the laryngeal function in temporary unilateral vocal fold paralysis or paresis, but also performed as a test injection prior to the permanent injection. Gelfoam has been safely used for the past several decades for the temporary vocal fold augmentation. Recently, we found a new material, called Radieses Lite, may be used as a temporary injection material. Material and Methods : We report preliminary results of 11 patients who had Radiesse Lite injection with a variety of pathologies. Results : Based on the results from the 6 patients who completed follow-up, the duration of effectiveness of Radiesse Lite seems to be from 2 to 3 months depending on the injection amount. Radiesse Lite satisfies several requirements of the ideal temporary vocal fold injection material in terms of injectability, convenience, duration of effectiveness and safety. Conclusion : The authors concluded that Radiesse Lite might be a good option for temporary vocal fold injection especially in the office setting.

Comparisons between fresh and cryopreserved fat injections in facial lipofilling

  • Background: Autologous fat is considered an ideal filler material, and the use of cryopreserved fat grafts is promising in terms of flexibility and efficiency. Therefore, cryopreserved fat grafts have become more common in recent years; however, their complications require further consideration. Methods: We evaluated 53 patients who underwent facial lipofilling at our institution to confirm the clinical usefulness and safety of cryopreserved fat. Fresh fat injections with or without cryopreserved fat were administered. At one or more sites, 22 patients had a single fresh fat injection, four patients had two or more fresh fat injections, 16 patients had one fresh fat injection followed by one cryopreserved fat injection, six patients had one fresh fat injection followed by two cryopreserved fat injections, and five patients had two fresh fat injections and one or more cryopreserved fat injections. Results: In total, 281 sets of injection procedures were performed at various sites, of which 170 involved one fresh fat injection, 89 involved one fresh fat injection and one cryopreserved fat injection, and 11 involved one fresh fat injection and two cryopreserved fat injections. One patient experienced self-resolving inflammation as a complication after the second injection in the right cheek. No statistically significant differences were found between the fresh and cryopreserved fat injections. Conclusion: We suggest that cryopreserved fat is a useful and safe resource for multiple fat injections, with advantages including aseptic fat handling and the delicacy of the technique.
