The Reliability of the Transconjunctival Approach for Orbital Exposure: Measurement of Positional Changes in the Lower Eyelid

  • Background: Lower eyelid incisions are widely used for the orbital approach in periorbital trauma and aesthetic surgery. In general, the subciliary approach is known to cause disposition of the lower eyelid by scarring the anterior lamella in some cases. On the other hand, many surgeons believe that a transconjunctival approach usually does not result in such complications and is a reliable method. We measured positional changes in the lower eyelid in blowout fracture repair since entropion is one of the most serious complications of the transconjunctival orbital approach. Methods: To measure the positional changes in the lower eyelids, we analyzed preoperative and postoperative photographs over various time intervals. In the analysis of the photographs, marginal reflex distance 2 ($MRD_2$) and eyelash angle were used as an index of eyelid position. Statistical analyses were performed to identify the significance in the positional changes. All patients underwent orbital reconstruction through a transconjunctival incision by a single plastic surgeon. Results: In 42 blowout fracture patients, there was no statistical significant difference in the MRD2 and eyelash angle. Furthermore, there were no clinical complications, such as infection, hematoma, bleeding, or implant protrusion, during the follow-up periods. Conclusion: The advantages of the transconjunctival approach for orbital access include minimal scarring and a lower risk of eyelid displacement compared with other approaches. Based on these results, we recommend the transconjunctival approach for orbital exposure as a safe and reliable method.

소방.가스안전용 헬멧의 강도안전성에 관한 연구 (A Study on the Strength Analysis of the Helmets for Fire and Gas Safety)

  • 본 본문에서는 유한요소법을 사용하여 여러 가지 헬멧 모델의 강도안전에 관한 응력과 변형률을 해석하였다. 차세대 헬멧은 작업자의 안전성과 작업성을 높일 수 있도록 제작되어야 하고, 오랫동안 착용해도 불편함이 없으면서 머리를 보호할 수 있어야 한다. 결국 헬멧은 안전하고, 착용성이 좋아야 하며, 헬멧의 모체 구조물을 가볍게 제작해야 한다. 따라서 헬멧은 화재나 가스폭발과 같은 가혹한 작업조건에 잘 견딜 수 있어야 한다. FEM 해석결과에 의하면, 외부의 충격력이 헬멧모체 구조물의 정상부 지역에 가해졌을 경우 최대응력과 변형률은 하중이 작용되는 지점부위에서 발생하였다. 따라서 헬멧의 모체 구조물에는 보강뼈대를 설치하고 두께를 늘려서 강도안전성을 확보하는 것이 중요하다. 반면에 헬멧의 일반적인 균일한 두게는 줄여서 헬멧의 경량화를 추구하는 것도 중요하다.

노인에서 경막외강에 3회 분할 투여된 Triamcinolone 60 mg이 혈중 포도당, ACTH와 Cortisol에 미치는 영향 (The Effect of Three 20 mg Caudal Epidural Injections of Triamcinolon on the Blood Glucose, ACTH and Cortisol Concentrations in the Elderly Women)

  • Background: Epidural steroid injections (ESI) have been used widely for the treatment of back and radiating extremity pain. Although its effects on the metabolic and endocrine system have been studied, the effects following repeated injections remain to be determined. We studied the effects of three repeated caudal epidural injections of low dose triamcinolone. Methods: the subject were 10 elderly women with spinal stenosis. Caudal epidural injections were performed biweekly. Triamcinolone (20 mg), mixed with 15 ml of 0.25% lidocaine, was used as the ESI injectate. The procedures were performed with the patient in the prone position. Blood sampling was performed just before the first injection, and used as the baseline, and then just before each injection on the same day of the 2nd and 4th weeks, with the last samples taken 2 weeks after the third injection. Results: The blood glucose concentrations showed no significant changes. The blood cortisol and ACTH concentrations were significantly decreased after the first injection, but there were no further decreases after each of the subsequent injections. The cortisol concentrations were maintained within the normal range. Conclusion: Caudal epidural injections, with low dose triamcinolone, suppressed the hypothalamus-pituitary-adrenal (HPA) axis, but no further suppression followed the subsequent repeated injections. Three consecutive caudal injections at 2 week intervals seems to be a safe procedure.

  • 열차위치 검지시스템에는 일반적으로 궤도회로장치가 거의 100여년을 안정적으로 사용되어 왔으나, 일반철도 구간의 역 구내, 측선, 해안선 구간 및 열차운행 빈도가 적은 지선 구간의 건널목 등에서는 녹 발생으로 궤도회로 단락불능에 따라 해소방안이 필요하다. 국내에서는 고속철도의 차축온도검지시스템과 통신기반열차제어시스템 분기부 일부에 차축카운터시스템이 사용되고 있으나, 해외의 경우 많은 국가에서 차축카운터 시스템이 없는 간선 및 지방 철도를 상상하는 것은 이제 불가능하고 지하철, 전차, 그리고 산업철도에서 상당한 증가가 확인되었다. 본 논문에서는 일반철도 구간의 역 구내 측선의 본선 및 분기부에 시험적용을 위해 기존 설치되어 있는 궤도회로와 병렬로 설치하여 로그분석을 하여 차축카운터시스템의 동작상태를 확인하였다. 그리고 오봉기지역 및 청주역 운행선 상 설치하여 연동장치와의 인터페이스 시험 및 이를 실용화하기 위한 시험결과를 분석하고, 시스템 안전측 설계, 제작 및 시스템 요구조건 등을 정립하였다.

화물자동차 보조 후미등화장치 설치에 따른 운전자 시인성 변화 (Legibility Change of Commercial Vehicles Equipped with the Rear Lighting System)

  • PURPOSES : The purpose of this system (Rear Lighting System) is to provide illumination for the driver to operate the commercial vehicle safely after dark in highway, to increase the conspicuity of the vehicle, and especially be suggesting the finest observable improvement method, depending on color and pattern of rear lighting system of truck for midnight highway traffic. METHODS : Rear lightning system as an improving way for forward commercial vehicles lighting the securing sight from human factors and the surrounding environment in midnight driving. For this one, basic materials were collected from the data analysis about many types of problems, and filed investigation for establishing Driving Simulator. also taking statistic test to human volunteers after finding recognizable distance of them. RESULTS : As a result, color with the highest visuality is amber followed by green-red-blue as in order for all road types. Especially almost no difference is found between red and green, also when the light is turn off, recognizable distances is wide difference compared to turn on the light. One more thing about study per pattern, upper and entire lighting have similar recognizable distances, but under lighting shows short distance with difficulty securing sight from medians. And straight section shows similar recognizable distances. By finding visuality improvement method depending on color and pattern of supplement taillight, it is expected to suggest quantitative judgement standard for introducing regulation and improvement of supplement taillight. CONCLUSIONS : Night time vehicle conspicuity to the rear is provided by rear position lamps. this study is showed that the color of light ramp is not important to be safe driving, most important is to turn on the light, recognizable distances is big different compared to turn off the rear light, so when the drivng dark in highway, have to turn on the light for reducing risk.

외상성 내안각격리증 환자에 있어 Hiraga 절개법을 이용한 내안각 고정술 (Medial Canthopexy using Modified Hiraga's Incision for Correction of Traumatic Telecanthus)

  • Purpose: Traumatic telecanthus can result from nasoethmoid-orbital fractures. Repair of the medial canthal tendon (MCT) using transnasal wiring is regarded as a choice of method to treat telecanthus, however, is often complicated by incomplete anchoring and drift of canthus, extrusion of wire, in-fracture of orbital bone, and eye damage. The authors introduced oblique transnasal wiring method through the Hiraga's epicanthopalsty incision instead of well-known classical bicoronal approach. Methods: Five patients with traumatic telecanthus were treated with this method. Though the Hiraga's epicanthoplasty incision, we could approach the operative field; the medial orbital wall and detached MCT. Oblique transnasal wiring was performed as following steps. After slit skin incision on the contralateral nasal recession area, drill holes were made from this point to the superior and posterior point of lacrimal sac of deformed eye. A 2-0 wire was double-passed through the holes and MCT. Traction was applied to ensure pulling the MCT and the wires were twisted in the contralateral nose, securing the MCT in the correct position. Results: All patients except 1 person showed improvement and rapid recovery. On average each canthus was moved 5.6 mm medially. In all cases, there were no eyelashes disappear, lacrimal canaliculitis, lacrimal duct injury, or infections. Conclusion: The Hiraga's epicanthoplasty incision could give sufficient operative field to reattach the MCT in traumatic telecanthus patients. And the oblique transnasal wiring technique is effective for the Asians who have flat nose and exophthalmic eye. The authors conclude that this technique could be a simple, safe and scarless method to correct traumatic telecanthus.

극저온 지하저장고 주변 ice ring 생성 모델링을 위한 열-수리 해석 (Simulation of Ice Ring Formation around Cryogenic Underground Storage Cavern using Hydro-Thermal Coupling Method)

  • LNG지하저장 핵심기술 중 하나인 ice ring 형성을 열-수리 역학 해석을 통해 분석하였다. ice ring은 극저온물질의 누출에 대한 이차방벽 역할과 지하수의 저장공동 침투에 대한 일차방벽역할을 수행한다. 따라서 ice ring의 두께와 위치는 콘크리트와 PU foam 및 멤브레인으로 구성된 일차방호재의 무결성 유지에 있어서 핵심인자가 된다. 수치해석을 통해 이러한 ice ring의 위치와 두께를 추정하였으며 온도와 지하수위 결과를 계측치와 비교하였다. 수치해석결과는 PU foam 열전도도의 온도의존성과 지하수의 상변화를 고려한 경우 파일럿 공동에서 측정한 계측치와 잘 일치하였다. 본 논문에서 제시한 수치해석 기법은 실규모 저장시설의 설계에 있어서 ice ring 형성을 모델링하는 데에 바로 사용될 수 있을 것으로 판단된다.

Clinical and Computed Tomography Evaluation of Plate and Screw on the Cervical Lateral Mass : A Modified Magerl's Technique

  • Objective : To determine the clinical and radiological safety of 15 consecutive patients managed with plate and screw fixation systems applied to the cervical lateral mass. Methods : 15 patients who underwent posterior cervical and T1 arthrodesis were reviewed from Jan 2002 to Dec 2004. Posterior cervical screw and plate fixation was applied on the lateral mass of the cervical spine. The authors have tried lateral mass screw fixation using a modified Magerl's technique [$20^{\circ}$ lateral and $20{\sim}30^{\circ}$ rostral screw trajectory] under preliminary radiological study. The average patient age was $39.73^{\circ}{\pm}11.00\;years$, and the average follow-up period was $9.73^{\circ}{\pm}6.77\;months$. Computed tomography scans taken after surgery were reviewed to confirm the attempted screw trajectory correct and safety. Results : Three of 93 lateral mass screws were malpositioned but clinical damage was not noted. Two of 8 pedicle screws on the T1 vertebrae were not placed on the correct pedicle area. Screw and plate loosening was observed in one case but was not subjected to an additional procedure because of maintained screw position observed during follow-up periods. Conclusion : The results of this study indicate that lateral mass screw fixation using the Modified Magerl's technique on the cervical lateral mass may provide safe and effective application on the patients. In addition, the chance of incorrectly placed screws was higher in T1 pedicle screw fixation than in lateral mass screw fixation of the cervical area.

Orbital wall restoring surgery with resorbable mesh plate

  • Background: Orbital resorbable mesh plates are adequate to use for isolated floor and medial wall fractures with an intact bony buttress, but are not recommended to use for large orbital wall fractures that need load bearing support. The author previously reported an orbital wall restoring surgery that restored the orbital floor to its prior position through the transnasal approach and maintained temporary extraorbital support with a balloon in the maxillary sinus. Extraorbital support could reduce the load applied on the orbital implants in orbital wall restoring surgery and the use of resorbable implants was considered appropriate for the author's orbital wall restoring technique. Methods: A retrospective review was conducted of 31 patients with pure unilateral orbital floor fractures between May 2014 and May 2018. The patients underwent transnasal restoration of the orbital floor through insertion of a resorbable mesh plate and maintenance of temporary balloon support. The surgical results were evaluated by the Hertel scale and a comparison of preoperative and postoperative orbital volume ratio (OVR) values. Results: The OVR decreased significantly, by an average of 6.01% (p<0.05) and the preoperative and postoperative Hertel scale measurements decreased by an average of 0.34 mm with statistical significance (p<0.05). No complications such as buckling or sagging of the implant occurred among the 31 patients. Conclusion: The use of resorbable mesh plate in orbital floor restoration surgery is an effective and safe technique that can reduce implant deformation or complications deriving from the residual permanent implant.

Elastoplastic FEM analysis of earthquake response for the field-bolt joints of a tower-crane mast

  • Safety measures for tower cranes are extremely important among the seismic countermeasures at high-rise building construction sites. In particular, the collapse of a tower crane from a high position is a very serious catastrophe. An example of such an accident due to an earthquake is the case of the Taipei 101 Building (the author was the project director), which occurred on March 31, 2002. Failure of the bolted joints of the tower-crane mast was the direct cause of the collapse. Therefore, it is necessary to design for this eventuality and to take the necessary measures on construction sites. This can only be done by understanding the precise dynamic behavior of mast joints during an earthquake. Consequently, we created a new hybrid-element model (using beam, shell, and solid elements) that not only expressed the detailed behavior of the site joints of a tower-crane mast during an earthquake but also suppressed any increase in the total calculation time and revealed its behavior through computer simulations. Using the proposed structural model and simulation method, effective information for designing safe joints during earthquakes can be provided by considering workability (control of the bolt pretension axial force and other factors) and less construction cost. Notably, this analysis showed that the joint behavior of the initial pretension axial force of a bolt is considerably reduced after the axial force of the bolt exceeds the yield strength. A maximum decrease of 50% in the initial pretension axial force under the El Centro N-S Wave ($v_{max}=100cm/s$) was observed. Furthermore, this method can be applied to analyze the seismic responses of general temporary structures in construction sites.
