상세불명의 비대상성 간경변증 환자 치험 1례 (A Case Report of a Patient Diagnosed with Decompensated Liver Cirrhosis due to Unspecified Causes)

  • Objectives: This study aimed to describe whether Saenggangunbi-tang supports clinical management of a patient with decompensated liver cirrhosis without unfavorable side effects. Methods: A 78-year-old woman diagnosed with unspecified liver cirrhosis in 2014, who had undergone abdominal paracentesis twice until 2014 and variceal ligation twice until 2017, took Saenggangunbi-tang from July 25, 2019 to March 23, 2020. We observed clinical changes, such as fatigue, leg edema, and gingival bleeding, as well as laboratory findings. Results: After taking Saenggangunbi-tang for about eight months, the patient's symptoms and serum levels of liver enzymes were improved in comparison to her symptoms and serum levels at the first visit. Moreover, there was no occurrence of any complications, such as ascites and gastroesophageal variceal bleeding by portal hypertension. Conclusions: This study suggests that Saenggangunbi-tang might be effective in the treatment of decompensated liver cirrhosis.

  • Objective: This study identified the effects of Korean medicine treatment on a patient with metabolic dysfunction-associated fatty liver disease (MAFLD). Methods: A 43-year-old man with MAFLD was treated with Saenggangunbi-tang with regular exercise from August 13, 2022, to December 24, 2022, to reduce fatigue and dyspepsia and to improve laboratory findings, such as liver enzymes and lipid profiles. We observed changes in symptoms and laboratory findings during the approximately four-month treatment. Results: Treatment with Saenggangunbi-tang resulted in decreased serum levels of liver enzymes, triglycerides, hepatic steatosis index scores, and clinical symptoms. During the treatment, the patient performed regular exercise; however, there was no significant change in body weight until the end of the study. Conclusion: This study suggests the availability of Saenggangunbi-tang as a therapeutic option for managing MAFLD patients.

  • Objectives: This study identified the effect of Korean medicine treatment on a patient with iron overload who discontinued iron chelation therapy. Methods: A 64-year-old woman with iron overload was treated with Saenggangunbi-tang from November 14, 2022, to March 15, 2023, to reduce fatigue and improve laboratory findings. We observed changes in the symptoms and improvement of laboratory findings during the four-month treatment. Results: The approximately four-month treatment with Saenggangunbi-tang showed considerable improvement in laboratory findings and fatigue. In addition, no adverse effects, such as liver injury, were observed during Korean medicine treatment. Conclusion: This study suggests the availability of Saenggangunbi-tang as a therapeutic option for managing patients with iron overload.

비만과 이상지질혈증을 동반한 비알코올 지방간질환 환자 치험 1례 (A Case Report of Nonalcoholic Fatty Liver Disease with Obesity and Dyslipidemia)

  • Objectives: This study aims to report the clinical efficacy of Korean medical treatment of a patient with nonalcoholic fatty liver disease accompanied by obesity and dyslipidemia. Methods: A 52-year-old man with nonalcoholic fatty liver disease, obesity, and dyslipidemia was treated with Saenggangunbi-tang extract from November 3, 2021 to January 8, 2022. During the treatment period, the patient also undertook exercise and maintained a hypocaloric diet to reduce body weight. Changes were observed in the patient's symptoms, laboratory findings, such as liver enzymes and lipid profiles, and bioelectrical impedance analyzer results. Results: After taking Saenggangunbi-tang extract for approximately two months, with concomitant adjustments to exercise and diet, the serum levels of liver enzymes and triglyceride were decreased and the patient's body weight, body mass index, hepatic steatosis index, and visceral fat area were also reduced. In addition, symptoms including fatigue and dyspepsia improved. Conclusion: This study suggests that Saenggangunbi-tang extract with exercise and a regulated diet could be a helpful treatment strategy for managing nonalcoholic fatty liver disease with metabolic disorders.

  • Objective: The purpose of this case report is to describe the efficacy of treatment with Korean medicine of a patient with nonalcoholic steatohepatitis (NASH) and metabolic syndrome. Methods: The patient was treated with Korean medicine (Saenggangunbi-tang) from April 27, 2016 to October 22, 2016. Results: Post-treatment, the patient's AST and ALT levels were significantly decreased. Triglyceride levels were within the normal range during the treatment. Abdominal ultrasonography revealed a significant improvement in hepatic steatosis. Conclusions: The results suggest that Saenggangunbi-tang could be an effective treatment for NASH with metabolic syndrome.

  • Objectives: This study aimed to identify whether herbal medicine improves the clinical symptoms and abnormal blood tests of a patient with fatty liver accompanied by hypertriglyceridemia. Methods: A 44-year-old man with fatty liver and hypertriglyceridemia was prescribed Saenggangunbi-tang from July 20, 2020, to November 11, 2020, to reduce fatigue and to improve laboratory findings, such as liver enzymes and the lipid profile. We observed changes in symptoms, serum levels of liver enzymes, and the lipid profile during about 4-month treatment. We also recorded changes in the bioelectrical impedance analyzer findings during that time. Results: In this case study, an approximately 4-month treatment with Saenggangunbi-tang led to considerable improvement in laboratory findings and visceral fat area. In particular, the patient experienced a noticeable decrease in triglyceride levels compared with the baseline parameters of the first visit. In addition, no side effects-including weight gain and liver enzyme increases-were observed during treatment. Conclusion: This study suggests that appropriately prescribed herbal medicine is a therapeutic option to manage fatty liver combined with hypertriglyceridemia.

알코올로 인한 Wernicke disease의 치험 1례 (One Case Treated Wernicke Disease by Alcohol)

  • Wernicke Disease is well known of nutriotional disorders Wernicke Disease is charaterized by ataxia, confusion and oculomotor disturbance. We diagnosis one patient, who has ataxia, confusion and oculomotor disturbance as Jusang(酒傷), and gave saenggangunbi-tang, bojungchiseub-tang and samchulgunbi-tang. We observed that clinical symptom and Liver function were improved by these herbal medicine Therefore We consider Wernicke Disease as Jusang(酒傷) and further reports with many case, however, will be needed.
