천장관절가동술 후 수정된 벽 스쿼트 운동이 정상성인의 균형에 미치는 영향 (The Effects of Modified Wall Squat Exercise after Sacro­Iliac Joint Mobilization on Balance in Normal Adults)

  • 본 연구는 정상 성인에게 있어 수정된 벽 스쿼트 운동과 천장관절가동술이 정상성인의 균형능력에 미치는 영향을 연구하고자 실시하였다. 34명의 대상자를 선발하여 실험군 17명(여자 10명, 남자 7명) 또는 대조군 17명(여자 10명, 남자 7명)으로 무작위 배치하였으며, 실험군은 6주간 주 3회, 회당 3세트의 수정된 벽 스쿼트 운동과 천장관절가동술을 받았다. 실험군과 대조군의 실험 전과 실험 후 그룹 내 균형을 비교한 결과 실험군은 체중분포지수와 우측 발 뒷부분과 발 앞부분(힘 판 CD), 좌, 우 발 뒷부분(힘 판 AC)과 좌측 발 뒷부분과 우측 발 앞부분(힘판 AD)에서 통계적 유의성이 있었고, 대조군은 전체 항목에서 통계적 유의성이 없었다. 본 연구에서 수정된 벽 스쿼트 운동과 천장관절가동술이 정상성인의 균형능력에 미치는 영향을 확인하였다.

  • Background: To determine effects of mobilization with movements (MWM) application on stroke patient with hypomobility on sacroiliac joint. Methods: The subject was 47years old male who have left hemiplegia because of right intra-cerebral hemorrhage. The subject was have hypomobility on sacroiliac joint. MWM using during 4weeks and using Berg balance scale (BBS), Timed up and go (TUG), 10meter walking (10MW) test for evaluation. Results: The results of this study were summarized below; BBS score for evaluating balance ability was increased, and TUG time, 10MW test time was decrease. Conclusion: We consider that MWM application on stroke patient with hypomobility on sacroiliac joint is not only regain mobility on sacroiliac joint but also increase in balance ability and walking speed.

척추관절통증증후군 (Spinal Joint Pain Syndrome)

  • Spinal joint pain syndrome is composed of atlanto-occipital, atlanto-axial, facet, and sacro-iliac joints pain. The syndrome is characterized as referred pain which is originated from deep somatic tissues, which is quietly different from radicular pain with dermatomal distribution originated from nerve root ganglion. The prevalence of facet joint pain in patients with chronic spinal pain of cervical, thoracic, and lumbar regions has been known 56%, 42%, and 31% as in order. It is generally accepted in clinical practice that diagnostic blocks are the most reliable means for diagnosing spinal joints as pain generators. The sacroiliac joint has been shown to be a source of 10% to 27% of suspected cases with chronic low back pain utilizing controlled comparative local anesthetic blocks. The treatment of spinal joints ideally consists of a multimodal approach comprising conservative therapy, medical management, procedural interventions, and if indicated.

Free Hand Insertion Technique of S2 Sacral Alar-Iliac Screws for Spino-Pelvic Fixation : Technical Note, Acadaveric Study

  • A rigid spino-pelvic fixation to anchor long constructs is crucial to maintain the stability of long fusion in spinal deformity surgery. Besides obtaining immediate stability and proper biomechanical strength of constructs, the S2 alar-iliac (S2AI) screws have some more advantages. Four Korean fresh-frozen human cadavers were procured. Free hand S2AI screw placement is performed using anatomic landmarks. The starting point of the S2AI screw is located at the midpoint between the S1 and S2 foramen and 2 mm medial to the lateral sacral crest. Gearshift was advanced from the desired starting point toward the sacro-iliac joint directing approximately $20^{\circ}$ angulation caudally in sagittal plane and $30^{\circ}$ angulation horizontally in the coronal plane connecting the posterior superior iliac spine (PSIS). We made a S2AI screw trajectory through the cancellous channel using the gearshift. We measured caudal angle in the sagittal plane and horizontal angle in the coronal plane. A total of eight S2AI screws were inserted in four cadavers. All screws inserted into the iliac crest were evaluated by C-arm and naked eye examination by two spine surgeons. Among 8 S2AI screws, all screws were accurately placed (100%). The average caudal angle in the sagittal plane was $17.3{\pm}5.4^{\circ}$. The average horizontal angle in the coronal plane connecting the PSIS was $32.0{\pm}1.8^{\circ}$. The placement of S2AI screws using the free hand technique without any radiographic guidance appears to an acceptable method of insertion without more radiation or time consuming.

천장관절 기능이상과 요통증 (Low back pain due to Sacro iliac joint Dysfunction)

  • The hub of weight bearing in the human body for both static and dynamic activities is the lumbopelvic region. It is a key region of extraordinary stability, since the trunk and ground forces converage in this region. The two sacroiliac joints form an integral part of this lumbopelvic unit. Considerable effort has been expended to study and quantify the normal range of movement of the sacroiliac joints Mitchell suggests that the ilium rotates in a posterior direction at heel strike and progresses in an anterior direction as the individual passes through the stance phase. The overall key appears to lie in determining the weight-bearing pattern of the sacroiliac (lumbopelvic) region from above and below that results in the familiar pain of sacroiliac dysfunction, assessing the status of the injured tissues, and intervening with the proper treatment protocols that maximize the body's healing processes. The purpose of this chapter is to provide a comprehensive overview of the sacroiliac joint's tissues and biomechanics, as well as concepts of evaluation and treatment. This overview is aimed at assisting the clinician in identifying the forces that are potentially destructive to the lumbopelvic tissues.

X-ray 엉치엉덩관절 촬영법을 통한 류마티스 및 퇴행성관절염 진단 (The Diagnosis of Rheumatologic and Degenerative Arthritis by X-ray Sacroiliac Joint Projection)

  • 본 연구는 엉치엉덩관절의 골절, 탈구, 엉치엉덩 퇴행성관절염 진단을 위한 가장 좋은 영상을 얻기 위해 골반 팬톰과 엑스선관을 각도 변화를 주어 엉치엉덩관절의 Joint space 가장 잘 나타내는 영상을 얻어 방사선사에게 주관적 평가를 받아 보았다. 평가 결과 결론으로 엉치엉덩관절염과 퇴행성 관절염 발견을 위한 단순촬영에서는 엎드린상태에서 촬영은 검사반대쪽 엉덩이를 $25^{\circ}{\sim}30^{\circ}$ 들어올리고 엑스선관을 앞엉덩뼈가 시에서 2.5 cm 안쪽을 지나는 시상면에 대해 수직촬영 하고 누운자세에서는 검사쪽 엉덩이를 $25^{\circ}{\sim}30^{\circ}$ 들어 올리고 위앞엉덩뼈가시가 중앙에서 위앞엉덩뼈가시가 중앙에 대해 엑스선관 각도를 발쪽으로 $5^{\circ}$ 촬영 하며 반드시 양쪽 엉치엉덩관절후전사방향(RAO,LAO) 및 전후사방향(RPO, LPO) 모두 촬영을 하게 되면 관절염 진단을 내리는데 있어 많은 도움이 되리라 본다.
