  • This paper describes a development of efficient stress ECG signal analysis algorithm. The algorithm consists of wavelet adaptive filter(WAF), QRS detector and ST segment detector. The WAF consists of a wavelet transform and an adaptive filter. The wavelet transform decomposed the ECG signal into seven levels using wavelet function for each high frequency bank and low frequency bank. The adaptive filter used the signal of the seventh lowest frequency band among the wavelet transformed signals as primary input. For detection of QRS complex, we made summed signals that are composed of high frequency bands including frequency component of QRS complex and applied the adaptive threshold method changing the amplitude of threshold according to RR interval. For evaluation of the performance of the WAF, we used two baseline wandering elimination filters including a standard filter and a general adaptive filter. WAF showed a better performance than compared filters in the noise elimination characteristics and signal distortion. For evaluation of WAF showed a better performance than compared filters in the noise elimination characteristics and signal distortion. For evaluation of results of QRS complex detection, we compared our algorithm with existing algorithms using MIT/BIH database. Our algorithm using summed signals showed the accuracy of 99.67% and the higher performance of QRS detection than existing algorithms. Also, we used European ST-T database and patient data to evaluate measurement of the ST segment and could measure the ST segment adaptively according to change of heart rate.

  • Objective : The purpose of this study was to investigate the correlations among various radiological parameters used to determine cervical alignment from cervical spine radiographs (X-CS) and cervical spine computed tomography (CT-CS), both within and between modalities. Methods : This study included 168 patients (≤60 years old) without a definite whole spine deformity who underwent CT-CS and X-CS. We measured occipital slope (O-s), C1 slope, C2 slope, C7 slope, sella turcica - C7 sagittal vertical axis (StC7-SVA), spino-cranial angle, T1 slope, and C27-SVA. We calculated the O-C2 angle, O-C7 angle, and C2-7 angle from the measured parameters and conducted correlation analyses among multiple parameters. Results : The intrinsic correlation features among multiple cervical parameters were very similar for both X-CS and CT-CS. The two SVA parameters (C27-SVA and StC7-SVA) were mainly influenced by the upper cervical slope parameters (r=|0.13-0.74|) rather than the lower slope cervical parameters (r=|0.08-0.13|). The correlation between X-CS and CT-CS for each radiological parameter was statistically significant (r=0.26-0.44) except for O-s (r=0.10) and StC7-SVA (r=0.11). Conclusion : The correlation patterns within X-CS and CT-CS were very similar in this study. The correlation between X-ray and CT was statistically significant for most radiological parameters, and the correlation score increased when the horizontal gaze was consistently maintained. The lower cervical parameters were not statistically associated with translation-related parameters (C2-7 SVA and StC7-SVA). Therefore, the upper cervical segment may be a better predictor for determining head and neck translation.

  • Purpose: A retrospective analysis was performed to evaluate the incidence of radiation induced lung damage after the radiation therapy for the patients with carcinoma of the lung. Method and Materials: Sixty-six patients with lung cancer (squamous cell carcinoma 27, adenocarcinoma 14, large cell carcinoma 2, small cell carcinoma 13, unknown 10) were treated with definitive, postoperative or palliative radiation therapy with or without chemotherapy between July 1987 and December 1991. There were 50 males and 16 females with median age of 63 years (range: 33~80 years). Total lung doses ranged from 500 to 6,660 cGy (median 3960 cGy) given in 2 to 38 fractions (median 20) over a range or 2 to 150 days (median in days) using 6 MV or 15 MV linear accelerator. To represent different fractionation schedules of equivalent biological effect, the estimated single dose (ED) model, $ED=D{\dot}N^{-0.377}{\dot}T^{-0.058}$ was used in which D was the lung dose in cGy, N was the number of fractions, and T was the overall treatment time in days. The range of ED was 370 to 1357. The endpoint was a visible increase in lung density within the irradiated volume on chest X-ray as observed independently by three diagnostic radiologists. Patients were grouped according to ED, treatment duration, treatment modality and age, and the percent incidence of pulmonary damage for each group was determined. Result: In 40 of 66 patients, radiation induced change was seen on chest radiographs between 11 days and 314 days after initiation of radiation therapy. The incidence of radiation pneumonitis was increased according to increased ED, which was statistically significant (p=0.001). Roentgenographic changes consistent with radiation pneumonitis were seen in $100\%$ of patients receiving radiotherapy after lobectomy or pneumonectomy, which was not statistically significant. In 32 patients who also received chemotherapy, there was no difference in the incidence of radiation induced change between the group with radiation alone and the group with radiation and chemotherapy, among the sequence of chemotherapy No correlation was seen between incidence of radiation pneumonitis and age or sex. Conclusions: The occurrence of radiation pneumonitis varies. The incidence of radiation pneumonitis depends on radiation total dose, nature of fractionation, duration of therapy, and modifying factors such as lobectomy or pneumonectomy.

  • Objectives : In the study on the waveform analysis of radial artery pulse diagnosis, we need to establish fundamentals of contemporary pulse diagnosis research. To achieve certain experimental basis on traditional pulse diagnosis, we have to research the differences of pulse waveform according to pressure (in 5 level) and position (Chon, Gwan, Cheok in radial artery). As a first step, in present thesis, we tried to find the differences of pulse waveform according to pressure. Methods and Results : In this research, we analyzed seven parameters of the waveform at five levels of pressure. The parameters are E(so called 'energy', a representative of pulse strength), h1(height of percussion wave) and h2(height of subincisura). The results were as follows. 1. When we analyzed the change of the waveform according to pressure in the left Gwan, E, hl, h2 and A differed between 1st and 2nd grade, between 1st and 3rd grade, between 1st and 4th grade, between 1st and 5th grade, between 2nd and 3rd grade, between 2nd and 5th grade, between 3rd and 5th grade and between 4th and 5th grade in 95% confidence interval. 2. • The development of a general hospital has a close connection with the trend of the society in general and it flows to the ways that satisfies the demanded value of the time. Therefore, various internal changes have occurred since a hospital has built. The changes is appearing through the time not only in the quantitative changes, but also in functional changes and correlations between each department. This study focused on these changes, and observed the St. Mary's Hospital - one of the major hospitals - which is constructed in 1980's. And then, it was divided into the D/T, O.P.D and ward as function that focused on the time of hospital's establishment (1980), the present (2006), and new hospital (2009) to observe the changing aspect of the function and department. As a result, according as age passes, this study shows function of each department is mixed. Among the D/T, O.P.D and ward, mixing functions of O.P.D is most notable. The mixing function brings changes in hospital's arrangement type. In the past, verticality style hospital was evaded, but recently verticality style hospital is appearing. Through this study, it is expected to forecast the new aspects of hospital and apply it to the new project.

  • The purpose of this study was to identify the effects of foot massage on the stress of student nurses in clinical practice. The research design was a nonequivalent control group pretest-posttest design. The subjects of this study were 52 student nurses from three junior colleges and one university. The subjects were assigned to one of two groups : the experimental group(26 students), and the control group(26 students). The study was conducted from October to November 1999. For the experimental group the foot massage was performed once a day for 3 days, starting from the first day of clinical practice. Subjects' mood, fatigue, blood pressure and pulse rate were measured as dependent variables. The Instrument for mood and fatigue state was a 100mm visual analogue scale. For the experimental group pre and post tests were done just before and after every 3 foot massages and on the 7th clinical practice day. For the control group pretest was done on 1st clinical day and posttest was done on clinical practice 7th day. The data was analyzed by the t-test and paired t-test and the results are summarized as follows: 1. The mood state scores were significantly higher for the experimental group as compared to the control group after the foot massage(t=13.73, p<.001). 2. The fatigue state scores were significantly lower for the experimental group as compared to the control group after the foot massage(t=8.69, p<.001). 3. The systolic blood pressure was significantly different between the experimental group and the control group after the foot massage(t=3.0073, p<.01). 4. The diastolic blood pressure was not significantly different between the experiment group and the control group after the foot massage. 5. The pulse rate was not significantly different between the experiment group and the control group after foot massage. 6. The mood state scores on the 1st, 2nd, 3rd massage day was significantly improved right after the foot massage. 7. The fatigue state scores on the 1st, 2nd, 3rd massage day were significantly lower right after the foot massage. 8. Systolic blood pressure on the 1st and 2nd massage day was significantly decreased right after the foot massage. Diastolic blood pressure and pulse rate were decreased right after massage but the change was not statistically significant. On the basis of the above findings, this study suggests that foot massage can be an effective stress relieving method for student nurses in clinical practice.
