A Long-term Mortality Prediction Model for Patient with ST-segment Elevation Myocardial Infarction using Decision Tree (의사결정트리를 이용한 ST분절상승 급성심근경색증 환자를 위한 장기 사망 예측 모형)

  • 이 논문에서는 한국인 급성심근경색증 환자에 대한 KAMIR 데이터를 기반으로 ST분절상승 심근경색이 처음 발병한 환자의 사망에 영향을 미치는 위험요소들을 찾고, 이를 기반으로 ST분절상승 급성심근경색환자의 1년 이내 사망을 예측하는 모델을 제시한다. 총 22개의 속성 중에서 속성 선택 알고리즘을 적용한 결과 나이, 심장박출계수, 크레아티닌, 고감도 C-반응성 단백질 등 4개의 속성이 선택되었고, 이 속성들을 이용하여 더욱 정확한 예측 모델을 구축할 수 있었다. 제시된 모델을 통해서 고위험군 환자의 위험성을 평가하고 예후를 추정할 수 있을 것으로 기대한다.

  • A traumatic sternal segment dislocation in children is very rare. Of the few cases reported, some recommended surgical treatment, while others reported good results by conservative treatment. Therefore, the treatment remains unclear. This paper reports a case of completely remodeled traumatic sternal segment dislocation in a 13-year-old boy treated by the restriction of activity and periodic observations.

Effect of a 12-Week Combined Exercise Program on Cardiovascular Function and Electrocardiogram Based ST Segment, QRS in Obese Middle-Aged Women (12주 복합운동 프로그램이 비만 중년여성의 심혈관계 기능과 심전도 기반의 ST 분절, QRS군에 미치는 영향)

  • This study randomly assigned 24 middle-aged women to determine the effect of a 12-week combined exercise program on cardiovascular function and electrocardiogram-based ST segment and QRS in obese middle-aged women. Analysis of the results was conducted to verify differences within groups and between periods and groups. In terms of cardiovascular function, myocardial oxygen consumption showed a significant decrease in the experimental group during the resting phase and the highest intensity phase during exercise, and an interaction between group and period at rest was also observed. The heart rate recovery rate showed a significant decrease in the experimental group at 2 minutes of recovery, while in the control group, on the contrary, a significant increase was observed. The experimental group showed a significant decrease in systolic blood pressure, and an interaction between group and period was also observed. There was no change in diastolic blood pressure in both the experimental and control groups, and there was a statistically significant decrease in pulse pressure only in the experimental group at the 3-minute recovery stage. In the case of the electrocardiogram, the QRS height in the experimental group showed a significant increase in most cases at rest and at 3, 6, and 9 minutes during the recovery period, while there was no significant difference in the QRS period in both the experimental and control groups, and the ST segment height was significantly increased at rest in the experimental group. appeared to have increased. Therefore, summarizing the results of this study, it is believed that the 12-week combined exercise program has a positive effect in improving cardiovascular function, ventricular contractility in electrocardiograms, and myocardial ischemia in obese middle-aged women.

  • Acute coronary syndrome involves three types of coronary artery disease associated with sudden rupture of coronary artery plaque, and has a clinical presentation ranging from ST-segment elevation myocardial infarction (STEMI) to non-ST-segment elevation myocardial infarction (NSTEMI) or unstable angina. Cardiac CT can help quantify and characterize atherosclerotic plaques. According to a previous study, low-attenuation plaque, napkin ring sign, positive remodeling, spotty calcification, and increased perivascular fat attenuation are associated with plaque ruptures on cardiac CT. Therefore, coronary artery stenosis, as well as acute coronary artery syndrome, can be diagnosed using cardiac CT.

  • In this study, we propose a novel method to detect and enhance the feature of ST-segment which offers the crucial information for the diagnosis of myocardial infarction and ischemia. With this aim, PQRST features of Electrocardiogram initially are detected and subsequently ST-segment are estimated. And Dynamic Time Warping(DTW) transformation is applied recursively to minimize the difference in time between ST-segments and calculate the minimum cumulative distance that decides the degree of similarity among ST-segments. As of the results, the inherent characteristic of ST-segment can be emphasized in terms of time parameter and thus the diagnostic features of a ST-segment can be revealed further.

  • Recently, the cardiovascular disease has increased by causes such as westernization dietary life, smoking, and obesity. In particular, the acute myocardial infarction (AMI) occupies 50% death rate in cardiovascular disease. Following this trend, the AMI has been carried out a research for discovery of risk factors based on national data. However, there is a lack of diagnosis minor suitable for Korean. The objective of this paper is to develop a classifier for short-term relapse mortality prediction of cardiovascular disease patient based on prognosis data which is supported by KAMIR(Korea Acute Myocardial Infarction). Through this study, we came to a conclusion that ANN is the most suitable method for predicting the short-term relapse mortality of patients who have ST-segment elevation myocardial infarction. Also, data set obtained by logistic regression analysis performed highly efficient performance than existing data set. So, it is expect to contribute to prognosis estimation through proper classification of high-risk patients.

  • 본 논문에서는 심근허혈 질환을 효율적으로 분류하기 위한 신경망을 설계하였다. European ST-T DB의 심전도로부터 ST 분절의 특징을 추출하여 입력노드를 결정하고 10개의 학습률과 학습 횟수에 따른 신경망의 MES를 계산하였다. 실험 결과 특징 파라미터의 조합을 ST0, ST80, Slope, Area로 하였을 때 MSE를 가장 작았다. 이러한 특징 파라미터를 이용하여 신경망의 입력으로 학습시킨 경우 학습 횟수의 증가에 따라 MSE가 지수합수적으로 감소하였으며 1,000회 이상에서는 둔하게 감소하였다. 또한 학습 횟수가 5,000회, 10,000회, 15,000회 각각의 경우에 대하여 학습률을 0.01부터 0.7까지 증가시키면서 MSE를 계산한 결과 학습 횟수가 증가할수록 MSE를 최소로 하는 최적학습률이 0.1부터 0.04까지 감소하였다.

Anterior Cervical interbody Fusion with Cervical Spine Locking Plate (경추 물림 금속판을 이용한 경추 전방추체간 유합술)

  • Objectives: Anterior approach to achieve arthrodesis of the cervical spine has become a widely accepted and often-used approach since its earliest reports by Bailey and Badgley, Smith and Robinson and Cloward. However, anterior interbody fusion in the presence of the posterior instability may be complicated by the bone graft dislodgement, kyphotic defomity or nonunion. As an attemp to prevent this undesirable complication, additional methods such as skeletal traction, halo appratus or even posterior fusion has been utilized. Therefore, The cervical spine locking plate(CSLP) with the anterior intervertebral body bone grafting provide immediate cervical stabilization and widely successful in achieving fusion. Material and methods: This study analysed 14 patients who underwent a single anterior procedure and application of CSLP for the treatment of the cervical spinal disorder. Eleven patients were disc herniations and three patients were traumatic lesion. The average age of the patient was 47 years and the mean follow up periods was 20 months ranging from 13 to 27 months. Results: Ambulation was started 2nd day after the operation with the aid of the Philadelpia orthoses. Bone union was observed 13 cases on average 12 weeks after operation. The one case was nonunion with plate breakage without clinical symptom. Conclusion: Anterior fusion with CSLP are thought to be a safe and valuable method for treating cervical spine disorder.

Effects of 12 Week Regular Aerobic Exercise on ST-segment and QTc Interval in Type 2 Diabetes Mellitus Patients (12주 규칙적인 유산소 운동이 제 2형 당뇨환자의 ST 분절과 QTc 연장에 미치는 영향)

  • The purpose of the present study was to examine effect of 12 week regular aerobic exercise on ST-segment and QTc interval in middle age type 2 diabetes mellitus (T2DM) patients. The subjects consist of 13 type 2 diabetes mellitus (T2DM) patients in middle age men and all of them had no other complications. Subjects participated in aerobic exercise training for 12 weeks. They started to exercise for $20{\sim}60$ min at $60{\sim}80%$ of $HR_{max}$, (exercise intensity has been increased gradually) per day, $3{\sim}5$ times a week. The results were compared before and after. Weight and BMI, % body fat, fasting glucose, HOMA-IR, $_{peak}DBP$ were significantly decreased and $_{peak}HR$, $_{peak}VO_2$, exercisre time were significantly increased after 12 week aerobic exercise. Also, QTc interval and ST-segment were significantly decreased during at rest, peak exercise after 12 week aerobic exercise. Conclusionally, 12 week aerobic exercise may be improvement in decreased cardiovascular mortality factors (ST-segment) and abnormal autonomic dysfunction (QTc interval) and potentially increased exercise capacity.
