인체 골격의 좌표형 임상용어체계 표준 개발 : SNOMED CT 기반의 융복합 연구 (Development of Clinical Terminology System for Human Body : Convergence Research of SNOMED CT)

  • 본 연구는 인체 골격의 좌표형 임상용어체계 표준을 개발하기 위한 방법론적 연구이다. 문헌고찰과 자료 수집을 통해 연구 계획을 수립하여 예비 표준(안)을 만들고 전문가 세미나와 자문을 통해 수정 표준(안)을 만든 후 내용타당도 검증 후 최종(안)을 제시하는 4단계를 거쳐 표준을 개발하였다. 좌표형 임상용어체계 표준은 인체 이미지를 2D는 x, y축, 3D는 x,y,z축으로 좌표화하고, 좌표의 개념과 정의는 SNOMED CT의 FSN, Synonym, Preferred name으로 선조합하고 후조합은 개발 된 18개의 Relationship을 통해 정의되고, 개발된 Relationship 표준의 내용타당도 지수는 평균 4.01점이었다. 본 연구를 통해 인체 골격 이외의 뇌, 장기, 조직 등의 인체의 다른 부분에 대한 후속 표준 개발을 제언하고 임상에서 활용성을 높이기 위한 방법 연구를 제언한다.

Application of Standard Terminologies for the Development of a Customized Healthcare Service based on a PHR Platform

  • The personal health record platform can store and manage medical records, health-monitoring data such as blood pressure and blood sugar, and life logs generated from various wearable devices. It provides services such as international standard-based medical document management, data pattern analysis and an intelligent inference engine, and disease prediction and domain contents. This study aims to construct a foundation for the transmission of international standard-based medical documents by mapping the diagnosis items of a general health examination, special health examination, life logs, health data, and life habits with the international standard terminology systems. The results of mapping with international standard terminology systems show a high mapping rate of 95.6%, with 78.8% for LOINC, 10.3% for SNOMED, and 6.5% when mapped with both LOINC and SNOMED.

  • It is difficult to settle the well-designed local terminology for imaging report in the hospital information system (HIS). One of the major reasons is the local terminology with poor contents have been used in the hospital. Thus, we mapped the locally used terms in nuclear medicine imaging report to the SNOMED-CT, which had been widely used in the electronic medical record system, for implementation of hospital information system. Preliminary construction of terminology dictionary was done by mapping of local terms to SNOMED-CT and LexCare Suite. Further study may be warranted.

  • Purpose: This study was performed to propose an ontology methodology based on standardized nursing process as framework in obstetric and gynecologic nursing practice. Methods: The instrument used in this study was based on the nursing diagnosis classification established by North American Nursing Diagnosis Association (NANDA) (2009-2011), fifth edition of the Nursing Interventions Classification (NIC) (2008), forth edition of the Nursing Outcomes Classification (NOC) (2008) developed by Iowa State University and systematized nomenclature of medicine clinical terms (SNOMED CT). The nursing records data were collected from electronic medical records of one hospital from August to October 2010. Results: One hundred and forty-one nursing diagnosis statements used in obstetric and gynecologic nursing unit were linked standardized nursing classifications and constructed nursing diagnosis ontology including interoperability. Conclusion: Not only will this result be helpful to complete nurse's lack of knowledge and experience, it will also help to determine nursing diagnosis logically by using standardized nursing process. It will be utilized as the method to construct ontology including interoperability in other nursing units. It will be presented nursing interventions according to nursing diagnosis and thus will be easier to establish nursing planning. This can provide immediate feedback of the nursing process application.

온톨로지에 기반한 간호진단 지식모델의 설계 (Design of Knowledge Model of Nursing Diagnosis based on Ontology)

  • 간호사는 NANDA, NIC, NOC과 같은 간호과정의 표준 가이드라인에 따라 간호 실무를 수행하고, 간호과정에 대한 정보를 전자의무기록 시스템에 기록하고 있다. 특히, NANDA는 간호진단 분류체계로써 간호진단의 추상적인 개념을 나타내고 있어, 상세한 간호진단 내용의 표현에 어려움이 있다. 그로 인해, 국내 병원에서는 자체적으로 간호진단 목록을 정의하여 사용하고 있으나, 이들은 표준이 적용되지 않아 간호기록의 전산화가 어려운 문제점이 있다. 따라서 본 논문에서는 NANDA와 SNOMED-CT와 같은 표준 용어체계를 참조하여 간호진단 개념을 표현하기 위한 온톨로지로 구축 방법론을 제시한다. 제안한 방법은 각 병원 및 분야에서 주로 사용하는 간호진단 목록을 체계적으로 구축함으로써 의료정보 시스템 간의 상호운용이 가능하고 지식의 확장이 용이하도록 한다. 제안한 방법에 따라 경북대학교병원의 여성건강 간호기록 진술문을 분석하고, 간호진단 정보의 추출 및 정련을 통해 112개의 간호진단 용어를 생성하였다. 그리고 이 용어를 이용하여 여성건강 간호진단 온톨로지를 구축하였고, 전문가 평가 및 실험을 통해 개발한 온토롤지의 타당도와 실용성을 확인하였다.

Computer-based clinical coding activity analysis for neurosurgical terms

  • Background: It is not possible to measure how much activity is required to understand and code a medical data. We introduce an assessment method in clinical coding, and applied this method to neurosurgical terms. Methods: Coding activity consists of two stages. At first, the coders need to understand a presented medical term (informational activity). The second coding stage is about a navigating terminology browser to find a code that matches the concept (code-matching activity). Systematized Nomenclature of Medicine - Clinical Terms (SNOMED CT) was used for the coding system. A new computer application to record the trajectory of the computer mouse and record the usage time was programmed. Using this application, we measured the time that was spent. A senior neurosurgeon who has studied SNOMED CT has analyzed the accuracy of the input coding. This method was tested by five neurosurgical residents (NSRs) and five medical record administrators (MRAs), and 20 neurosurgical terms were used. Results: The mean accuracy of the NSR group was 89.33%, and the mean accuracy of the MRA group was 80% (p=0.024). The mean duration for total coding of the NSR group was 158.47 seconds, and the mean duration for total coding of the MRA group was 271.75 seconds (p=0.003). Conclusion: We proposed a method to analyze the clinical coding process. Through this method, it was possible to accurately calculate the time required for the coding. In neurosurgical terms, NSRs had shorter time to complete the coding and higher accuracy than MRAs.

  • 최근 방대해진 의료 정보 관리와 통합을 위해 다양한 전자기록 시스템이 개발되어왔다. 이 중에 EMR(Electronic Medical Record)은 병원 내의 의료 정보를 전산으로 처리하는 것이다. 이때 많은 의학 용어가 사용되는데 이것을 체계적으로 지원하기 위해 SNOMED(Systematized Nomenclature of Medicine)와 같은 용어 체계가 필요하다. 본 논문에서는 이러한 용어 체계를 위해 의학 용어를 자동으로 구축하는 시스템을 제안하고자 한다. 제안한 시스템은 웹 마이닝을 통해 자동으로 웹에서 데이터를 수집하고 분류해서 의학 용어 데이터를 구축하게 된다.

Study on the Categorical Structure Standardization for Representation of 3D Human Body Position System

  • This study presents the categorical structure for ther epresentation of a 3D human body position system in the WD stage after NP approval by the International Organization for Standardization (ISO), analyzes the needs of electronic medical record users and establishes future implementation plans for expanding its use in Korea. Research was conducted on the needs of doctors, nurses, health and medical information managers, and radiology departments, which are the main stakeholders of electronic medical records. The overall requirements for electronic medical records were derived from the results, and the requirements for each stage of use of electronic medical records were analyzed. Based on the results of the study, the study proposes plans to expand the use of the categorical structure for the representation of the 3D human body position system, and also aims to establish a standard system for health and medical terminology in Korea and contribute to the development of health and medical information standards through international standardization.

  • A controlled medical vocabulary is a vital component of medical information management because it enables computers to use information meaningfully and different institutions to share the medical data. There are currently many standard medical vocabularies - SNOMED-CT, ICD-10, UMLS, GALEN, MED, etc, but none is universally accepted as an optimal controlled medical vocabulary for application to medical information system. Moreover, it is difficult to settle the well-designed local data dictionary consisting of controlled medical vocabularies for the individual hospital information system (HIS). One of the major reasons is the local terminology with poor contents have been used in the hospital. Thus, as a trial, the local controlled vocabulary referencing system has being constructed in a limited medical field - nuclear medicine. We selected practical nuclear medicine terms from interpretation reports and electronic medical records, and removed ambiguity and redundancy, mapping the selected terms to standard medical vocabularies. Relationship and hierarchy structure between terms have being made, referring to standard medical vocabularies. Further studies may be warranted.

  • 자동코딩 방식을 이용함으로써 업무 수행능력이 배가되고 용이하게 호출이 가능하며 디스켓에 따로 저장하는 보완장치가 있으므로 상당히 안전하리라 생각된다. 대형컴퓨터에 비해 그 소요경비가 무척 절감되었으며 언제든지 프로그램 보완이 가능하여 앞으로의 이용이 주목된다 하겠다.
