Three Cases of Chronic Adult Atopic Dermatitis (만성 성인형 아토피 피부염의 한방 치험 3례)

  • Introduction: Atopic dermatitis is chronic eczematous dermatosis which is related strongly to a family history of atopic disease and hereditary disposition. Its threshold of itching is low so that severe itching occurs and causes secondary eczema. Adult atopic dermatitis has mostly severe symptoms and progresses to chronic relapsing. Objective: The purpose of this study was to estimate the efficacy of oriental medical treatment and management on chronic adult atopic dermatitis. Subject: We studied 3 patients who visited Dongguk University Gangnam Oriental Hospital Dept. of Dermatol. with chronic adult atopic dermatitis from August 2001 to April 2002 and were observed for over 4 months. Methods: We treated them with herb-medication, acupuncture, negative therapy, aroma therapy, full spectrum visible ray therapy(by carbon arc) and external application(Jaun-go). We used the SCORAD index to assess the severity of atopic dermatitis. Results: There were 3 patients with chronic adult atopic dermatitis we could check for over 4 months by clinical scoring using the SCORAD system. 1. The grades of the severity of atopic dermatitis were lower only by the oriental medical treatments and management. 2. Intensities of relapse cases were lower then before the oriental medical treatments. 3. The secondary treatment term for relapse cases was shorter then the first treatment term after oriental remedies were used. Conclusion: Considering the above results, more study is needed on the chronic relapse cases with adult atopic dermatitis, the factors of increasing seventy of adult atopic dermatitis, the prognostic factors of adult atopic dermatitis, and the clinical interchange of chronic adult atopic dermatitis between oriental medicine and western medicine.

Retrospective Study about the Effectiveness of Korean Medicine Treatment on 89 Severe Atopic Dermatitis Patients (중증 아토피피부염 환자 89명의 한의학적 치료 효과에 대한 후향적 연구)

  • Objectives : The purpose of this study is to report the effectiveness of a Korean Medicine Treatment on 89 severe atopic dermatitis patients all through 12 months. Methods : The degree of atopic dermatitis was measured in Objective SCORAD Index (OSI) with photos of patients. Then, the OSI score was estimated according to gender, age and intensity. Results : 1. The ratio of male to female was 41.57 : 58.43. The 24.72% of patients were in 0~11 year-old group, 16.85% were in 12~18 year-old group and 58.43% were in the group above 19 years old. 2. The initial OSI score was 64.43 and the final was 37.03 after 12 months. After 3, 6, 9, 12 months, the OSI score was significantly lower than the initial visit. 3. There was no significant difference between male and female patients on age, OSI scores and OSI improvements. 4. The patients who treated acupuncture were significantly older then who not treated. The OSI improvement had no significant difference whether treated with acupuncture or not. 5. There was no significant difference among 3 different age groups on initial OSI scores. The OSI improvement of 0~11 year-old group was significantly higher than the other groups after 12 month treatment. 6. The OSI score of 97.75% was decreased after 12 month treatment compared to initial visit. 7. After 12 month treatment, the distribution of severe atopic dermatitis was decreased from 100% to 37.08%. 8. There was no significant difference betweent steroid group and non-steroid group on OSI scores at initial visit and after 12 month treatment. Conclusions : The OSI Score of severe atopic dermatitis patients was decreased on the significant rate with Korean medicine treatment for 12 months.

Retrospective Study about the Effectiveness of a Korean Medicine Treatment on 45 Facial Atopic Dermatitis Patients (얼굴아토피 피부염환자 45명의 한의학적 치료효과에 대한 후향적 연구)

  • Objective : The purpose of this study is to report the effectiveness of a Korean Medicine Treatment on 45 facial atopic dermatitis patients. Methods : Total 45 facial atopic dermatitis patients, who has visited korean medical clinic in year 2011 were analyzed by Objective SCORAD Index(OSI) and Investigator's Global Assessment(IGA). Subanalysis of OSI and IGA were done according to topical ointment user/non-user, treatment period and change in IGA distribution. Results : 1. Male and female percent was 20%:80%. 17.8% were teens, 55.6% were twenties, 13.3% were thirties, 13.3% were above age forty. 64.4% were on topical ointment treatment of corticosteroid or calcineurin inhibitor, and 35.6% were not at the initial visit. Patients with family history were 44.4% and 62.2% had atopic dermatitis past history. 2. OSI and IGA were significantly lower after 1~3, 3~6, 6~9 month of treatment. Average post-treatment score was lower in longer-treated group. 3. IGA distribution has changed from average 3.42 at the first visit to 1.76 at final visit. 91.1% of total patients reported decrease in IGA at the final visit. 4. OSI and IGA improvement rate were significantly higher in non-topical ointment user than the user. Age, treatment period, initial OSI and IGA score difference were not significantly different. Conclusion : A significant percent of 45 facial atopic dermatitis patients who were treated with Korean Medicine Treatment reported decrease in OSI and IGA. The difference increased with the treatment period. Non-topical ointment users' improvement was significantly higher than topical ointment user.

Anti-oxidant Activities and Anti-atopic Dermatitis Effect of Combined Extract of Phellinus linteus and Glycyrrhiza uralensis (상황버섯, 감초 복합추출물의 항산화 및 인체적용 아토피 개선 효능 연구)

  • Objectives : This study investigated the anti-oxidant activities and improving effect of Phellinus linteus and Glycyrrhiza uralensis Extract (PGE) on Atopic Dermatitis. Methods : 2,2′-diphenyl-1-picrylhydrazyl (DPPH) radical, 2,2′-azino-bis 3-ethylbenzothiazoline-6-sulphonic acid (ABTS) radical, Hydrogen peroxides scavenging activities and Superoxide dismutase (SOD)-like activities were used for the measurement of anti-oxidant ability. Cytotoxicity of PGE in Raw 264.7 cell was evaluated by MTT assay. To evaluate the anti-atopic dermatitis effect of PGE, a total of 33 patients with atopic dermatitis were observed trans epidermal water loss, skin moisture content, modified SCORAD index of atopic dermatitis and pruritic degree after applying the PGE for 4 weeks. Results : PGE scavenged DPPH ($IC_{50}=25ppm$) effectively, ABTS and Hydrogenperoxides scavenged similar to BHA. As for the SOD-like activity, it had lower effect than ascorbic acid, but it comparable activities in 500ppm. There was no cytotoxicity at PGE at concentrations of 10,000ppm. In clinical research about PGE on patients with atopic dermatitis, skin condition was improved. After 4 weeks, the application of PGE increased skin moisture content from 19.43 to 31.22. Moreover, it reduced the skin temperature (from 32.5 to 31.9), skin pH (from 5.39 to 5.22), trans epidermal water loss (from 39.03 to 24.46) and pruritus score (from 6.07 to 3.87). In addition, the Modified SCORAD index decreased from 31.28 to 20.3. Conclusions : In conclusion, PGE possesses anti-oxidant and anti-atopic dermatitis activities, thus it could be potentially valuable as anti-atopic dermatitis material.

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment on a patient with atopic eruption of pregnancyMethods: Patient who diagnosed by atopic eruption of pregnancy was enrolled in this study. She received Korean medicine therapies such as herbal medicine, acupuncture and external application. Sopungchungyoung-tangs and Seungmahwangryeon-tangs were prescribed according to the patient’s constitution which was pungyeol. The severity of symptom was evaluated by the severity SCORing for Atopic Dermatitis index (SCORAD index).Results: Subjective symptom such as Systemic pruritus, sleep loss, and Dermatitis lesions was reduced by Korean medicine therapies.Conclusions: The present study suggests that Korean medicine therapies have a effect on atopic eruption of pregnancy.

  • We observed and treated a 19 year-old female who had atopic dermatitis. Even though she had been treated with western medicine, she still complained of severe itching, erythema, lichenification, scaling, dryness, oozing and insomnia especially on her face and neck. We treated her with Sasang Constitutional Therapies(Kupoong-san, Teumchoui-tang, Kalkunsungki-tang), apcupuncture, herbal irrigation and moisture emulsion. Having been treated for 24 days in hospital admitted, her symptoms were reduced and changed from severe to mild grade in SCORAD(SCORing Atopic Dermatitis)

  • 환경오염으로 인해 아토피 등의 피부질환이 증가하고 있으며 이에 국내에서는 인터넷을 통한 아토피 진단 서비스는 많다. 하지만 단순한 설문조사 형식으로 진단에 대한 객관성을 유지하기 어려워 신뢰도가 낮다. 이에 본 논문은 컴퓨터 비전 인식 기술을 활용하여, 컴퓨터에서 처리가능한 아토피 환자의 환부 영상을 수집 및 진단하는 시스템을 개발한다. 아토피 진단 시스템은 환부 영상을 사용자로부터 입력받아 색상값에 대한 히스토그램을 만들어 정규화한다. 다음으로 정규화된 히스토그램의 확률적 분포를 이용해 역투영한 결과 영상을 만들어 내게 되며 이 역투영 영상을 바탕으로 피부색을 제외한 부분을 아토피 침범 영역이라 판단하게 된다. 최종적으로는 입력 영상으로부터 얻어진 침범 영역, 문진을 이용한 점수를 이용하여 SCORAD 지수 스코어링 하게 된다.

Retrospective Study about the Effectiveness of a long-term Korean Medicine Treatment on 511 Atopic Dermatitis Patients (아토피피부염 환자 511명의 장기간의 한의학적 치료효과에 대한 후향적 연구)

  • Objective : The purpose of this study is to report the effectiveness of a long-term Korean Medicine Treatment on 511 atopic dermatitis patients. Methods : Total 511 atopic dermatitis patients, who has visited oriental medical clinics from 2011.1.9 to 2012.1.31 and continued treatment 3 month to 9 month were analyzed by Objective SCORAD Index(OSI). Subanalysis of OSI were done according to gender, age, treatment period, and intensity. Results : 1. Male and female percent was 42.5:57.5. Age percent was 0 to 11:12 to 18:above 19=30:16.6:53 and 41.9% was steroid users. 2. OSI Improvement rate was significantly higher in women than men after 3 month of treatment, but not significant after 6, 9 month of treatment. 3. There was no improvement difference of OSI among the age. 4. OSI was significantly lower after 3, 6, 9 month of treatment. Average post-treatment score was lowest in 9 month treatment group. 5. 86.7% of total patients reported decrease in OSI score at the final visit. 6. 85.9% was severe, 13.5% was moderate, and 0.6% was mild by OSI intensity assessment at initial visit. The percent changed at final visit to 37% severe, 54.2% moderate, and 8.8% mild. 7. Patient diagnosed as OSI intensity severe at first visit, their average treatment period was 6.58 months and final improvement rate was 36.27%. Conclusion : A significant percent of Atopic dermatitis patients who were treated with Korean Medicine Treatment reported decrease in OSI. There was significant decrease in OSI before and after 3, 6, 9 month treatment. The difference increased with the treatment period.

Clinical Research of Atopic Dermatitis Treated by Hwangryeonhaedok-Tang in Cosmetics (아토피 피부염 환자에서 황연해독탕(黃連解毒湯)이 함유된 한방화장품에 대한 임상 연구)

  • This clinical research was conducted to test patients with Atopic Dermatitis by external application with Hwangryeonhaedok-Tang in cosmetics. We gave scores to 31 patients who visited the Dept. of Oriental Medical Opthalmology & Otolaryngology & Dermatology of Semyung University Jecheon Oriental Medical Hospital from May 15th, 2008 to June 26th. Fifteen patients were treated with the ointment that contain Hwangryeonhaedok-Tang (experimental group) and sixteen patients were treated with normal ointment that doesn't have Hwangryeonhaedok-Tang (control group) for 4 weeks. We observed change of total IgE, eosinophil count, Skin Temperature, Transepidermal Water Loss(TEWL), Skin Hydration and Skin pH. Also Clinical Index of Atopic Dermatitis(SCORAD Index) and global assesment of efficacy were used to evaluate the effects of Hwangryeonhaedok-Tang. Statistical analysis was performed by using frequency analysis and descriptive analysis. Statistical significance was achieved if the probability was less than 5%(p<0.05). After 4 weeks of external application treatment, SCORAD Index in experimental group was significant statistically decreased compared with control group. After 4 weeks of external application treatment, total IgE of both groups were decreased and eosinophil count in control group was decreased but experimental group was unstatistically decreased. Unstatistically, both groups didn't showed significant effect on Skin Temperature. Transepidermal Water Loss(TEWL) in both groups were increased but experimental group was significant decreased compared with control group. Skin Hydration in experimental group was significant statistically increased compared with control group. Statistically, both groups didn't showed significant effect on Skin pH. Only experimental group showed little increase. After 4 weeks of external application treatment, experimental group showed significant effect on global assesment of efficacy. Considering the above results, we can speculate that cosmetics with Hwangryeonhaedok-Tang has some therapeutical effects in mitigating the symptoms of Atopic Dermatitis.

Retrospective Study about the Effectiveness of a Korean Medicine Treatment on 69 Infant and Young Child Atopic Dermatitis Patients (영유아 아토피피부염 환자 69명의 한의학적 치료효과에 대한 후향적 연구)

  • Objectives The purpose of this study is to report the effectiveness of Korean medicine treatment on 69 infant and young child atopic dermatitis patients. Methods 69 infants and young child atopic dermatitis patients who had visited oriental medical clinics from 2011.1.9 to 2012.1.31 were studied. All of them were continually treated for 3 to 9 months, and were analyzed by Objective SCORAD Index (OSI). A detailed analysis of OSI was done according to gender, age, using of topical steroid ointment, family history of atopic disease, treatment period, and intensity. Results 1. Male and female percentage of the study group was 56.5%: 43.5%. The average period of treatment was 6.09 months. 71.0% used topical steroid ointment, and 65.2% had family history of atopic disease. 2. The average period of treatment was significantly longer in patients who used topical steroid ointment, or who had family history of atopic disease. 3. 95.7% of total patients reported decrease in OSI score at the final visit. OSI was significantly lowered after 3, 6, and 9 months of treatment. The longer period of treatment, the lower average post-treatment OSI. 4. 85.5% of the study group had severe dermatitis, and 14.5% was moderate case based on the OSI intensity assessment at initial visit. These percentages got changed at the final visit as 34.8% of severe, 58.0% of moderate, and 7.2% of mild cases. The average treatment period of patients who were diagnosed as severe at the initial visit was 6.18 months, and their final OSI improvement rate was 39.58%. 58% of them were improved to be mild or moderate at the final visit. Conclusion The Korean medicine treatment is effective in treating infant and young child atopic dermatitis patients. There was significant decrease in OSI score after 3, 6, and 9 months of treatment. The difference increase with the treatment period.
